Abstract

Even though neurosurgeons exercise these enormous and versatile skills, the COVID-19 pandemic has shaken the fabrics of the global neurosurgical family, jeopardizing human lives, and forcing the entire world to be locked down. We stand on the shoulders of the giants and will not forget their examples and their teachings. We will work to the best of our ability to honor their memory. Professor Harvey Cushing said: "When to take great risks; when to withdraw in the face of unexpected difficulties; whether to force an attempted enucleation of a pathologically favorable tumor to its completion with the prospect of an operative fatality, or to abandon the procedure short of completeness with the certainty that after months or years even greater risks may have to be faced at a subsequent session-all these require surgical judgment which is a matter of long experience." It is up to us, therefore, to keep on the noble path that we have decided to undertake, to accumulate the surgical experience that these icons have shown us, the fruit of sacrifice and obstinacy. Our tribute goes to them; we will always remember their excellent work and their brilliant careers that will continue to enlighten all of us.

Abstract

© 2017 Elsevier Ltd Background Rindopepimut (also known as CDX-110), a vaccine targeting the EGFR deletion mutation EGFRvIII, consists of an EGFRvIII-specific peptide conjugated to keyhole limpet haemocyanin. In the ACT IV study, we aimed to assess whether or not the addition of rindopepimut to standard chemotherapy is able to improve survival in patients with EGFRvIII-positive glioblastoma. Methods In this randomised, double-blind, phase 3 trial, we recruited patients aged 18 years and older with glioblastoma from 165 hospitals in 22 countries. Eligible patients had newly diagnosed glioblastoma confirmed to express EGFRvIII by central analysis, and had undergone maximal surgical resection and completion of standard chemoradiation without progression. Patients were stratified by European Organisation for Research and Treatment of Cancer recursive partitioning analysis class, MGMT promoter methylation, and geographical region, and randomly assigned (1:1) with a prespecified randomisation sequence (block size of four) to receive rindopepimut (500 μg admixed with 150 μg GM-CSF) or control (100 μg keyhole limpet haemocyanin) via monthly intradermal injection until progression or intolerance, concurrent with standard oral temozolomide (150–200 mg/m2for 5 of 28 days) for 6–12 cycles or longer. Abstract

Background: Intraoperative imaging must supply data that can be used for accurate stereotactic navigation. This information should be at least as accurate as that acquired from diagnostic imagers. Objectives: The aim of this study was to compare the stereotactic accuracy of an updated compact intraoperative MRI (iMRI) device based on a 0.15-T magnet to standard surgical navigation on a 1.5-T diagnostic scan MRI and to navigation with an earlier model of the same system. Methods: The accuracy of each system was assessed using a water-filled phantom model of the brain. Data collected with the new system were compared to those obtained in a previous study assessing the older system. The accuracy of the new iMRI was measured against standard surgical navigation on a 1.5-T MRI using T1-weighted (W) images. Results: The mean error with the iMRI using T1W images was lower than that based on images from the 1.5-T scan (1.24 vs. 2.43 mm). T2W images from the newer iMRI yielded a lower navigation error than those acquired with the prior model (1.28 vs. 3.15 mm). Conclusions: Improvements in magnet design can yield progressive increases in accuracy, validating the concept of compact, low-field iMRI. Avoiding the need for registration between image and surgical space increases navigation accuracy.

Abstract

Gliomatosis cerebri (GC) is a rare astrocytic tumor that typically affects multiple lobes of the brain. Due to its insidious nature and atypical symptom presentation, diagnosis is often made late in the course of disease process. To date, no definitive treatment guidelines have been established, and due to its infiltrating pattern, neurosurgical resection of the lesion is generally not feasible. We report a case of a 69-year-old male who initially presented with symptoms similar to normal pressure hydrocephalus. The patient underwent CSF diversion, with an inconclusive tissue biopsy. His symptoms were relieved for more than two years thereafter until he demonstrated worsening of symptoms, with a MRI clearly demonstrating GC. Patients presenting with hydrocephalus and a "full posterior fossa" should be followed carefully for evidence of GC progression.

47. Modeling of intraoperative brain shift for registration

Abstract

We present preliminary results of our research on intraoperative brain shift modeling. This is a part of our wider research interest on intraoperative brain MR registration. We try to increase the accuracy of preoperative to intraoperative MR image registration. We are particularly interested in intraoperative images obtained after a tumour resection had been performed. The soft brain tissue deforms as the body of the tumour is being removed. The topic of this paper is modeling of gravity-induced brain shift appearing due to tumour resection, in order to guide a preoperative to intraoperative brain registration algorithm. The idea is to segment the brain from the intraoperative images and to construct an elastic model of the brain based on segmentation results. Elastic parameters of the brain model are obtained from in-vivo experiments. In order to model the effect of gravity force we apply the force equal to gravitation but with opposite direction. Such model of the brain without tumour body is then deformed using Finite Elements Method (FEM). In this research we used National Library of Medicine Insight Segmentation and Registration Toolkit (ITK).
