PURPOSE:Patients may transfer of hospital for clinical reasons but this may delay time to treatment. The purpose ofthis study is to provide insight in the extent of hospital transfer in breast cancer care; which type of patients transfer and what is the impact on time to treatment.METHODS:We included 41,413 breast cancer patients registered in the Netherlands Cancer Registry between 2014and 2016. We investigated transfer of hospital between diagnosis and first treatment being surgery or neoadjuvant chemotherapy (NAC). Co-variate adjusted characteristics predictive for hospital transfer were determined. To adjust for possible treatment by indication bias we used propensity score matching (PSM). Time to treatment in patients with and without hospital transfer was compared.RESULTS:Among 41,413 patients, 8.5% of all patients transferred to another hospital between diagnosis and firsttreatment; 4.9% before primary surgery and 24.8% before NAC. Especially young (aged CONCLUSIONS:While almost 5% of Dutch patients treated with primary surgery transfer hospital after diagnosis andup to 25% for patients treated with NAC, our findings suggest that especially those treated with primary surgery are at risk for additional treatment delay by hospital transfer.

De COVID-19-epidemie heeft tijdens de eerste golf grote impact gehad op de zorg, vooral in het voorjaar van 2020. Afspraken, behandelingen en operaties voor niet-COVID-patiënten zijn afgezegd of uitgesteld. Ook meden patiënten zelf de zorg, bijvoorbeeld uit angst om te worden besmet of om de zorg te ontlasten. Een deel van de afspraken is vervangen door zorg op afstand, bijvoorbeeld telefonisch of via beeldbellen. Aan het begin van de zomer herstelden veel sectoren in de zorg grotendeels, maar het lukte niet de achterstand in te halen. Sommige vormen, zoals de paramedische zorg, dagbesteding en groepsbehandelingen, herstelden moeizamer. Door de coronamaatregelen, zoals 1,5 meter afstand houden en de extra hygiënemaatregelen, was het vaak niet mogelijk om evenveel zorg als voorheen te leveren. Voor dit onderzoek is gekeken naar de behandelingen die binnen de 12 grootste specialismen in ziekenhuizen het meest worden uitgevoerd (in totaal 48). Tijdens de eerste coronagolf is gemiddeld 23 procent van deze behandelingen niet doorgegaan. De gezondheidswinst die behandelingen normaal gesproken opleveren is daardoor niet bereikt. Dit wordt uitgedrukt in 'verloren gezonde levensjaren', een eenheid die effecten op sterfte en kwaliteit van leven aangeeft. Door de uitgevallen behandelingen zijn er naar schatting 34.000 tot 50.000 minder gezonde levensjaren bereikt. Dit zijn vooral effecten op kwaliteit van leven, en in mindere mate op overlijden. Een relatief groot deel van de verloren gezonde levensjaren zijn het gevolg van weggevallen behandelingen binnen de specialismen oogheelkunde en orthopedie, zoals staar-, knie- en heupoperaties. De schattingen over de gevolgen voor kankerpatiënten vallen buiten de berekeningen van dit onderzoek. Deze schattingen zijn ingewikkelder. Als eerste aanzet daarvoor zijn de gevolgen voor melanoom uitgewerkt, de agressiefste vorm van huidkanker. Naar schatting zijn 1.600 tot 2.800 gezonde levensjaren verloren gegaan door deze uitgevallen zorg. De onderzochte behandelingen vormen 28 procent van de medisch-specialistische zorg. Deze behandelingen leveren in verhouding veel gezondheidswinst op, en dus ook relatief veel verlies als de zorg niet doorgaat. Het totale gezondheidsverlies zal zeker groter zijn dan de genoemde aantallen, maar niet drie tot vier keer zo groot. Een deel van het gezondheidsverlies gaat niet definitief verloren als de komende jaren extra behandelingen kunnen worden uitgevoerd.

Background: General health in the working population is thought to depend on working conditions. Objective: This survey studied job demands and health complaints in working white and blue collar employees. We expect physical and psychological job demands to be differentially distributed among white and blue collar workers. Do they report health complaints consistent with their working conditions? Method: Cross-sectional study of 323 white and 383 blue collar workers. They completed the Basic Occupational Health Questionnaire, a valid and reliable self-report questionnaire about health, work and working conditions. The results were analysed using Chi-square and logistic regression analysis, controlling for educational level as a proxy of socioeconomic status. Results: The questionnaires of 280 white and 251 blue collar workers were suitable for analysis. White collar workers reported higher psychological job demands, and blue collar workers reported higher physical demands. In both occupational groups, low back pain, fatigue and upper respiratory complaints were most common. The rates of low back pain and pain in the lower extremity were higher in blue collar workers, as were regular headaches, pain in the cardiac region and feeling sleepy. However, these relationships substantially weakened when the educational level was adjusted for. Conclusions: Despite the differential distribution of job demands, white and blue collarworkers reported similar health complaints. Health in the working population depended predominantly on socioeconomic status. Interventions to improve general health of employees should be directed at their socioeconomic position instead of working conditions.

A quantitative analysis of 278 distinct FNV bargaining agendas shows that, contrary to expectations, nei-ther the share of women nor the share of young parents, covered by the collective labour agreement, are related to trade union agenda setting with respect to childcare issues. Rather, central recommenda-tions and history dependence play important roles, along with the concept of communities of practice within FNV unions. h3. Nederlandse samenvatting Uit eerder onderzoek bleek dat er geen direct verband bestaat tussen vraag naar arbeid-en-zorgregelingen in CAO’s en het aanbod ervan – deze regelingen komen voornamelijk in CAO’s voor wanneer de werkgever daar de financiële ruimte voor heeft. Betekent dit dat nu ook dat vakbonden niet daar waar dat nodig is inspelen op de vraag naar arbeid-en-zorgregelingen? Met andere woorden, komt de vraag van de achterban naar dit soort regelingen terug op de onderhandelingsagenda voor de desbetreffende CAO? Onderzoek in 556 CAO’s (278 recente CAO’s en van elk de direct er aan voorafgaande CAO) laat zien dat, met uitzondering van het CAO-jaar 2001, alleen de mate waarin arbeid-en-zorgregelingen voorko-men in de vorige CAO bepalend zijn voor de onderhandelingsagenda. En in 2001, het jaar waarin de Wet arbeid & zorg tot stand kwam, is nog iets meer aan de hand. Hoe kunnen we het schijnbare gebrek aan relatie tussen vraag en aanbod van arbeid-en-zorgregelingen verklaren? Ligt hier gebrekkige communicatie tussen de sociale partners, of tussen vakbonden en hun achterban aan ten grondslag? Of moeten we de oorzaak in een andere hoek zoeken?

It is also apparent that there is a rapid development of childcare and other work-family balance provisions in collective labour agreements (CLAs): either these provisions are being included in collective agreements, or existing provisions are being expanded. The reasons for this are obvious: from the employee’s point of view, childcare provisions make it interesting for women to join or stay in the labour market; from the employer’s point of view, although it may be a costly issue, these provisions may help attract and retain female employees. The main question is the following: to what extent can the share of female employees covered by a CLA explain the presence of work-family provisions in CLAs? Furthermore, does the gender of the union negotiator have a significant additional affect? The DUCADAM dataset, a digital database on collective labour agreements in the Netherlands, is used to tackle these questions. The findings show that the hypothesised relationships do not exist. Appar-ently, the supply of work-family provisions in CLAs is not a response to female employees’ demands, nor is it affected by the negotiator’s gender. Rather, economical factors seem to underlie work-family developments in CLAs, as indicated by the positive correlation between work-family provisions and yearly wage increase levels.

Nederlandse samenvatting Het lijdt geen twijfel dat het percentage vrouwen op de Nederlandse arbeidsmarkt de afgelopen jaren is toegenomen. Het is ook bekend dat steeds meer kinderopvang– en andere werk-zorgregelingen in col-lectieve arbeidsovereenkomsten (CAO’s) worden opgenomen. De redenen hiervoor liggen voor de hand. Uit het oogpunt van de werknemer bezien maken kinderopvangregelingen het interessant voor de vrouwelijke werknemer om te gaan of blijven werken; uit het oogpunt van de werkgever bezien kunnen deze regelingen, hoewel kostbaar, helpen om vrouwelijke werknemers aan te trekken en te behouden. De kernvraag is: in hoeverre kan de aanwezigheid van werk-zorgregelingen in CAO’s verklaard worden door het aandeel vrouwen dat onder de CAO valt? Is verder het geslacht van de CAO-onderhandelaar van significant belang? Deze vragen worden beantwoord met behulp van de DUCADAM dataset, een digitale CAO-databank waarin Nederlandse CAO’s zijn opgeslagen. De resultaten van het onderzoek laten zien dat de veronderstelde verbanden niet bestaan. Blijkbaar zijn de werk-zorgontwikkelingen in CAO’s niet het antwoord op de vraag daarnaar door vrouwelijke werknemers, noch op het geslacht van de CAO-onderhandelaar. De verklaring lijkt gezocht te moeten worden in economische motieven, gezien de positieve samenhang tussen werk-zorgregelingen in de CAO en jaarlijkse structurele loonsverhoging.

The ‘FNV cao-databank’ is a digital database that stores all collective labour agreements negotiated by FNV trade unions in the Netherlands since 1995. Every agreement has been coded with respect to 650 different characteristics. The DUCADAM dataset is both a scientific translation of this database and an extension of it. The extension consists of newly created meta-variables, which are not actually included in CLAs but are closely tied to them. In the DUCADAM dataset codebook, the complete dataset is described, with a special focus on the design of the meta-variables. Specific problems that social scientists may encounter when doing research on collective labour agreements are clarified and solved. The reader also learns about other CLA databases in the Netherlands and about the project background regarding the construction of the DUCADAM dataset. In the final chapter, several research projects that make use of the FNV cao-databank and/or the DUCADAM dataset are described. In the appendices, one can find extensive tables with information about all collective agreements and about the variables themselves. This codebook is a useful source of information for social scientists, journalists, policy-makers and others who are concerned with industrial relations and collective agreements.
