Abstract

Rationale: Primary treatment of obstructive sleep apnea can be accompanied by a persistence of excessive sleepiness despite adherence. Furthermore, effectiveness of sleep apnea treatment is limited by poor adherence. Currently available pharmacologic options for the treatment of sleepiness in this population are limited. Objectives: To evaluate the efficacy and safety of solriamfetol (JZP-110), a selective dopamine and norepinephrine reuptake inhibitor with robust wake-promoting effects, for the treatment of excessive sleepiness in participants with obstructive sleep apnea with current or prior sleep apnea treatment. Methods: This was a double-blind, randomized, placebo-controlled, parallel-group, 12-week trial comparing solriamfetol, 37.5, 75, 150, and 300 mg, with placebo. Abstract

Improvements to urban transportation systems are designed to reduce travel times, but can also facilitate additional productivity gains through agglomeration. These systems, however, are complex, making it difficult to identify and quantify the mechanisms that improve economic productivity. In this paper we propose a quantitative urban spatial equilibrium model with simple urban structure that incorporates endogenous agglomeration and multi-modal congestion to study the transportation policy effects of investment and pricing on economic productivity. The model features a novel traffic flow modelling tool called the multi-modal macroscopic fundamental diagram (MFD). This modelling tool allows us to account for effects of network topology and multi-modal traffic operations in the macroscopic modelling of congestion. Abstract

New York is considering additional emission regulation on top of its obligations under the Regional Greenhouse Gas Initiative (RGGI) to achieve its State Energy Plan targets. The proposed measure is a so-called "carbon adder" on CO2 emissions from the power sector which is set as the difference between the targeted social cost of carbon and the prevailing RGGI price for CO2 emission allowances. We investigate the potential economic and environmental impacts from the imposition of a carbon adder on New York's power sector. While our analysis indicates the risk of excess cost through overlapping regulations, we find that the carbon adder gives the "right" price signal for New York's power generation to turn into a greener one. Market requirements for permit price floors in the RGGI market induces carbon permit retirements across RGGI states leading to small reductions in region- and country-wide emissions levels.

Abstract

The rapid expansion of oil and gas extraction in large parts of the Marcellus fields in the Appalachian Basin and the Bakken fields in the Williston Basin of the Dakotas and Montana has reignited debate over the role of natural resource extraction as a rural economic growth strategy. While the popular media has focused on hydraulic fracturing and natural gas and oil production, there has been significant interest in silicon sand mining in western Wisconsin and southeastern Minnesota. This “frack sand” is used in oil and gas hydraulic fracturing activity. In this study we explore the relationship between non-oil and gas mining (NAICS 212) activities and economic growth for nonmetropolitan U.S. counties for the period 2000 to 2007. We find robust results suggesting that non-oil and gas mining is associated with lower population growth, and a positive impact on per capita income, but has no impact on employment growth.

Abstract

Subsidies and cross-subsidies in the energy sector are common throughout Eastern Europe and Central Asia. In Belarus, revenues from an industrial tariff on electricity are used to cross-subsidize heating for households. Input-output (IO) data and a household consumption survey are used to analyze the distributional impacts of this cross-subsidization. This paper illustrates cost shares and electricity-intensity of different sectors and consumption categories and uses the IO data to obtain first-order estimates of the distributional incidence of policy reform. The paper then analyzes distributional impacts of subsidy reform with a Computable General Equilibrium model. Although poorer households benefit from reduced heating costs, the increase in prices of other consumer goods due to higher electricity prices more than offsets the benefits they receive from the subsidies. The analysis finds that the current cross-subsidies are regressive, and policy reform would be highly progressive.

Abstract

Background: For patients with hormone receptor positive metastatic breast cancer (MBC), both steroidal and non-steroidal aromatase inhibitors (AIs) have demonstrated efficacy as initial and subsequent lines of treatment in trials comparing them with other hormonal agents like tamoxifen and megestrol acetate. Patients with MBC and predominant visceral involvement have a shortened survival compared to those with non-visceral disease. These patients are usually treated with chemotherapy, with under-utilization of endocrine strategies due to fear of rapid progression. In this review, we present the available data for both classes of AIs in patients with visceral predominant MBC and examine efficacy and safety differences between them. Methods: An exhaustive Medline search to retrieve published articles based on pre-defined inclusion criteria was conducted. Data from 13 published studies of randomized comparisons of the two classes of AIs with other hormonal therapies or each other form the basis of the efficacy analyses in this report. Conclusion: Based on our review of the available data, we argue that their manageable toxicity profile with demonstrable clinical benefit supports the use of both classes of AIs, with no significant efficacy differences between the agents, in appropriately selected patients with visceral predominant MBC. Due to the lack of randomized trial data comparing the two classes of AIs in this setting with the exception of one study, the comparisons in this review are mostly indirect and need confirmation in future prospective trials. 32. Tools for Open Source, Subnational CGE Modeling with an Illustrative Analysis of Carbon Leakage.

Abstract

This paper introduces the Wisconsin National Data Consortium (WiNDC) framework for producing self-consistent accounts based on publicly available datasets that can be used in sub-national economic equilibrium analysis in the United States. We describe the process used to generate regional social accounting matrices and a calibrated static multi-regional, multi-sectoral computable general equilibrium model conformal with the constructed dataset. As illustration, we show how the core model can be applied for the analysis of energy-environment issues. We use an energy-economy extension of the core model to assess the effectiveness of several state level greenhouse gas mitigation proposals. Sub-national abatement measures result in carbon leakage - mandated reductions in controlled areas may be vitiated by increased emissions in uncontrolled jurisdictions. Using a WiNDC-based model, we calculate leakage rates and show how these depend on the underlying trade model. Our calculations demonstrate the importance of both data and modeling assumptions for the simulation of policy experiments.
