Abstract

Intravenous injection of complete Freund's adjuvant (CFA) in rats has been proposed as an experimental model for pulmonary sarcoidosis, but only some animals develop granulomas. Because the detection of the presence of granulomas and evaluation of the extent of the reaction has required histological evaluation, this model has been of limited use in following the evolution of the granulomatous process.The present study evaluated the ability of lung computed tomography (CT) scanning to identifyin vivopulmonary granulomas induced by CFA in rats. Wistar rats were injected with CFA to induce pulmonary sarcoid-like granulomas, and the presence and extent of pulmonary abnormalities, as detected by spiral CT and histopathological analysis, were compared.Spiral CT had a high sensitivity and specificity for the detection of sarcoid-like granulomas in rats injected with CFA. The extent of the pulmonary granulomatous reaction as assessed by the two techniques strongly correlated (r=0.93, pThus, lung computed tomography appears to be a valuable tool for thein vivoevaluation of the pulmonary granulomatous reaction induced by complete Freund's adjuvant in rats. With the help of computed tomography, this experimental model should be suitable for the sequential study of pulmonary sarcoid-like granulomas, particularly in response to various therapeutic strategies.

Abstract

Mediastinal fibrosis is a rare, benign disorder characterised by proliferation of fibrous tissue within the mediastinum. It can present as diffuse mediastinal infiltration or, more frequently, as a localised soft tissue mass, usually in the right paratracheal or subcarinal areas. 18-fluorodeoxyglucose (18-FDG) positron emission tomography (PET) imaging has only been reported in a few case reports with variable aspects.Herein, we present an unusual pattern of mediastinal fibrosis, presenting as a thick retrosternal mass contiguous with pleural thickening. 18-FDG PET showed intensely increased uptake in the mass. Both the location of fibrosis and PET imaging were confusing with malignancy. Histopathological proof of benign fibrosis was obtained by surgery. The patient had been exposed to asbestos and the possible relationship of this particular pattern of fibrosis with asbestos exposure will be discussed.

Abstract

Purpose: To measure the fractional anisotropy (FA) and the mean diffusivity (MD) values of L4, L5 and S1 nerve roots using diffusion tensor imaging (DTI) and to correlate them with four different clinical patterns. Patients and methods: Fifty-six human participants were prospectively included and divided between four groups: healthy subjects, patients with clinical symptomatic nerve root pain with and without anatomical discoradicular conflict and patients with incidental anatomical discoradicular conflict seen on magnetic resonance imaging (MRI). MRI protocol included anatomical sequences (sagittal T1- and T2-weighted, axial T2-weighted) and a 25 directions DTI sequence. FA and MD values were measured in consensus by two readers and compared between the four groups. Results: Mean FA and MD values were significantly different for patients with clinically symptomatic nerve root pain (n = 27) both with (n = 16) (FA = 0.187 ± 0.015; MD = 510 ± 40) and without (n = 11) (FA = 0.193 ± 0.011; MD = 490 ± 30.5) anatomical discoradicular conflict compared to healthy subjects (n = 29) (FA = 0.221 ± 0.011; MD = 460.9 ± 35.5) including 2 subjects withincidental anatomical discoradicular conflict (FA = 0.211 ± 0.013; MD = 450.8 ± 41.2) on MRI(P = 0.003).Conclusion: Measurement of FA and MD values of L4, L5 and S1 nerve roots using DTI could beuseful in lumbar nerve root pain assessment.
