Abstract

IntroductionPragmatic trials in comparative effectiveness research assess the effects of different treatment, therapeutic, or healthcare options in clinical practice. They are characterized by broad eligibility criteria and large sample sizes, which can lead to an unmanageable number of participants, increasing the risk of bias and affecting the integrity of the trial. We describe the development of a sampling strategy tool and its use in the PREPARE trial to circumvent the challenge of unmanageable work flow.MethodsGiven the broad eligibility criteria and high fracture volume at participating clinical sites in the PREPARE trial, a pragmatic sampling strategy was needed. Using data from PREPARE, descriptive statistics were used to describe the use of the sampling strategy across clinical sites. A Chi-square test was performed to explore whether use of the sampling strategy was associated with a reduction in the number of missed eligible patients.Results7 of 20 clinical sites (35%) elected to adopt a sampling strategy. There were 1539 patients excluded due to the use of the sampling strategy, which represents 30% of all excluded patients and 20% of all patients screened for participation. 7. Development of a Computer System To Educate Students To Evaluate and Interpret Published Drug Studies.

Abstract

The education of students in the techniques of critical appraisal of drug studies has been identified as a deficiency in many health sciences curricula. Errors in research design and inconsistencies in the reporting of study results persist in professional pharmacy and medical journals. Thus, thorough and accurate review and interpretation of journal studies are essential for assuring that patients receive proper drug therapy. This project developed an interactive, computer instructional program that would teach students to evaluate all aspects of published drug efficacy studies. The content for the program was identified, prepared, and designed for an interactive computer format, and the program was developed using "Authorware Professional." The program, "Evaluation of Clinical Drug Studies," consists of 10 main sections with several subsections. Two consecutive classes of pharmacy students at West Virginia University tested the program, which was found to increase significantly their knowledge of critical drug study evaluation techniques compared to a control group and pretests. Abstract

Globalisation can facilitate improvements in health through the speed and ease of shared information, advances in health care delivery and health policy, and the enhanced pace of discovery through international research collaborations. However, there are associated potential risks to health such as the spread of communicable diseases and antibiotic-resistance. Current international priorities in the development, supply and use of medicines reflect the influence of globalisation. This is relevant to all aspects of contemporary pharmacy, and means that graduates are increasingly working in a globalised profession. The challenge for schools of pharmacy, as for other higher education institutions, is how to best prepare our students so that they develop an appreciation of the importance of global perspectives in health and medicines use, acquire the skills and aspirations to address new challenges, and recognise the career opportunities that this presents. This publication reflects upon the experiences of the "Students as Global Citizens" project from a pharmacy perspective. The project was a collaboration between the University College London (UCL) School of Pharmacy, the Institute of Education (University of London), the UCL Institute for Global Health, the Royal Veterinary College and the London International Development Centre, and was funded by the UK Department for International Development. The main aim of the project was to develop and evaluate methods to integrate teaching and learning about global and development issues into undergraduate curricula in pharmacy, medicine and veterinary medicine.

Abstract

Objective: The Outcomes and Assessment Committee at the Virginia Commonwealth University School of Pharmacy was tasked with refining the school’s key performance indicators (KPIs) to improve programmatic assessment by focusing on the most important measures. Methods: Initially, 56 KPIs were tracked, nine of which were university mandated, divided into 10 modules: admissions, community outreach, continuing education, diversity, faculty experience and success, fundraising, graduate program, research and scholarship, staff experience and success, and student experience and success. Using a three-round Delphi consensus technique, KPIs were reviewed by faculty and staff. Each participant responded whether they considered each KPI to be essential or not essential for school quality assessment and improvement. Consensus for the first, second, and third rounds was defined as ≥90%, ≥80%, and ≥75% agreement, respectively. Results: Of 109 faculty and staff invited, 49 participated in the first round, 51 in the second, and 42 in the third. At the end of the third round, accumulated consensus was achieved for 35 out of 88 (39.8%) KPIs that were considered essential and 3 out of 88 (3.4%) that were considered non-essential. Consensus percentage per module was: 15.4% (2/13) admissions, 28.6% (2/7) community outreach, 33.3% (3/9) continuing education, 27.3% (3/11) diversity, 62.5% (5/8) faculty experience and success, 55.6% (5/9) fundraising, 40% (4/10) graduate program, 33.3% (3/9) research and scholarship, 57.1% (4/7) staff experience and success, and 66.7% (4/6) student experience and success. Conclusions: Ultimately, 35 KPIs achieved consensus as essential to measure achievement of benchmarks for the school, which totals 44 KPIs, including nine university mandated KPIs. The process facilitated faculty and staff involvement in KPI selection and achieved improved focus for programmatic assessment.

Abstract

It is important to understand the stability of the formed gels and the factors that affect the behavior of these agents in a complex system to impart desirable properties to a new product. The purpose of this paper was to evaluate the morphological structure of gelling agents, LM-pectin, kappa-carrageenan and guar gum and to study the effects of these gelling agents in low-calorie fruit jelly model systems. Scanning electron microscopy (SEM) analysis allowed us to recognize the morphology of the gelling agents of interest, and showed that only the powdered samples of LM-pectin and kappa-carrageenan presented structures that collaborate for the process of gelling. Based on all the information gathered through the analysis of low-calorie fruit jelly modeling systems using the gelling agents studied, we suggest that the best combinations among the formulations tested were those containing concentrations higher or equal than 75% LM-pectin.

The proceedings are for two workshops, held in November 1972 and April 1973, in the process of establishing a structured externship in pharmacy. At the first workshop the field instructors were apprised of the goals and techniques of the project, and the policy of the program was developed. At the followup workshop the experiences of the first semester of the program were reviewed, problem areas were discussed, and plans were laid for the future. Appended to the proceedings are: the program guidelines; instruments for evaluating the training site, student performance in community practice, and student performance in hospital practice; checklists of extern experiences in community pharmacy and in hospital pharmacy; a student profile form; field instructor profile forms for community practice and institutional practice; and a summary report of evaluation of field instructors and externship experience. Abstract

Lipoplexes (LDs) have been prepared from DNA and positively charged vesicles composed of the helper lipid, dioleoyl l-α-phosphatidylethanolamine (DOPE) and either a dichain (DC) oxyethylated cationic lipid or their corresponding novel trichain (TC) counterpart. This is the first study using the TC lipids for the preparation of LDs and their application. Here the results of biophysical experiments characterising the LDs have been correlated with the in vitrotransfection activity of the complexes. Photon correlation spectroscopy, zeta potential measurements and transmission electron microscopy studies indicated that, regardless of the presence of a third chain, there were little differences between the size and charge of the TC and DC containing LDs. Small angle neutron scattering studies established however that there was a significant conformational re-arrangement of the lipid bilayer when in the form of a LD complex as opposed to the parent vesicles. This re-arrangement was particularly noticeable in LDs containing TC lipids possessing a third chain of C12or a longer chain. These results suggested that the presence of a third hydrophobic chain had a significant effect on lipid packing in the presence of DNA. Picogreen fluorescence and gel electrophoresis studies showed that the TC lipids containing a third acyl chain of at least C12were most effective at complexing DNA while the TC lipids containing an octanoyl chain and the DC lipids were least effective. The transfection efficacies of the TC lipids in the form of LDs were found to be higher than for the DC analogues, particularly when the third acyl chain was an octanoyl or oleoyl moeity. Little or no increase in transfection efficiency was observed when the third chain was a methyl, acetyl or dodecanoyl group. The large enhancement in transfection performance of the TC lipids can be attributed to their ability to complex their DNA payload. These studies indicate that presence of a medium or long third acyl chain was especially beneficial for transfection.

Abstract

Chlorinated and brominated forms of salicylic acid (SA) have recently been identified as a new class of disinfection byproducts (DBPs) in drinking water. Herein, we report the inherent reactivity of several aqueous halogenating agents toward hydrogen salicylate, the predominant species of salicylic acid under environmental conditions. Using synthetic waters, halogenation rates associated with the formation of 3-chloro, 5-chloro, 3-bromo, and 5-bromosalicylate were measured as a function of pH, [Cl−], [Br−], free chlorine dose, and the initial concentration of SA. Halogenating-agent specific second-order rate constants were determined and decrease in the order: BrCl > BrOCl > Br2> Br2O > Cl2> Cl2O > HOBr > HOCl. Chloride is capable of enhancing rates of bromination and chlorination, ostensibly by promoting the formation of BrCl and Cl2, both of which are several orders of magnitude more inherently reactive than HOBr and HOCl, respectively. Kinetic data also support the participation of salicyloyl hypochlorite as a chlorination intermediate capable of influencing chlorination rates at pH >8. Experiments in which buffer concentrations were varied indicate that phosphate buffers can enhance rates of SA bromination but not chlorination; carbonate and borate buffers did not appreciably influence rates of bromination or chlorination. Under conditions representative of chlorinated drinking water, rates of SA bromination will generally exceed rates of SA chlorination. The results discussed herein demonstrate the importance of considering halogenating agents beyond HOBr and HOCl when developing kinetic models to describe and predict halogenation rates and selectivity in waters containing free chlorine.

The Smiles rearrangement is the intramolecular nucleophilic aromatic substitution reaction incorporating a heteroatom as the nucleophilic component and an activated electrophilic arene. One particular variation—the Truce–Smiles rearrangement—utilises a carbon-based nucleophile and an electrophilic arene which does not require additional activation. Abstract

The use of a confined space in which to carry out reactions has proven popular in recent years, as demonstrated by the large volume of work published on ‘molecular microreactors’ such as zeolites, micelles and nanoparticles. This article looks at reactions in microstructured reactors, also known as microchannelled reactors or microreactors. In general, these consist of a ‘chip’ with narrow channels etched into it. Microstructured reactors have been the subject of several review articles to date, focusing on preparation, types of reactions that may be carried out and on the potential for ‘green’ applications. However, the use of microstructured reactor technology in photochemistry has, until now, not been subject to review. This perspective aims to outline the work done to date in this area and in particular to demonstrate the advantages and future prospectives of this technology in photochemical processes. Photochemistry in microstructured reactors is an emerging area of interest and to date has demonstrated significant potential as a viable alternative to traditional photochemical synthesis. Abstract

We describe the ultra-fast and sensitive detection of the gene encoding the protective antigen of Bacillus anthracis the causative agent of anthrax. Our approach employs a highly novel platform technology, Microwave-Accelerated Metal-Enhanced Fluorescence (MAMEF), which combines the use of Metal-Enhanced Fluorescence to enhance assay sensitivity and focused microwave heating to spatially and kinetically accelerate DNA hybridization. Genomic and exosporium target DNA of Bacillus anthracis spores was detected within a minute in the nanograms per microliter concentration range using low-power focused microwave heating. The MAMEF technology was able to distinguish between B. anthracis and B. cereus, a non-virulent close relative. We believe that this study has set the stage and indeed provides an opportunity for the ultra-fast and specific detection of B. anthracis spores with minimal pre-processing steps using a relatively simple but cost-effective technology that could minimize casualties in the event of another anthrax attack. This collection is minimally processed. It includes administrative records of the WVU School of Pharmacy, and records related to the creation of the Medical Center. Box 1 includes: correspondence regarding the Medical Center (ca. 1951-1958); School of Pharmacy faculty meeting minutes (1937-1961) and Visiting Committee correspondence and reports (January 1952 - August 1967); School of Pharmacy reports to the West Virginia Pharmaceutical Association (ca. 1961-1972); materials from the Medical Center Committee on Policy and Arrangements (1958-1959), Deans' Council (1958-1959), and Executive Committee (1959-1968); and accreditation-related correspondence with the American Council on Pharmaceutical Education (ca. 1950-1964). Box 2 includes: various materials from the 1950s related to the planning, preparation, creation, and administration of the Medical Center, including its library, physical facilities, equipment, hospital materials, alcohol permitting and storage (ca. 1959-1973), and a consultant's report on the Medical Center educational program (1952); material regarding WVU's war casualties (former Dean of Pharmacy J. Lester Hayman was on the Committee on WVU World War II Casualties, to assist in the university's effort to make a complete list of student and alumni casualties in World War II, and to create a memorial for them), including the publications Lewis Countians in World War II (1943); and miscellaneous photographs and clippings. Also includes one ledger of School of Pharmacy annual reports and inventories (ca.1936-1968), and an oversize folder containing three blueprints to the Medical Center (undated).
