Abstract

Opioid related deaths are increasing dramatically in recent years, and opioid epidemic is worsening in the United States. Combating opioid epidemic becomes a high priority for both the U.S. government and local governments such as New York State. Analyzing patient level opioid related hospital visits provides a data driven approach to discover both spatial and temporal patterns and identity potential causes of opioid related deaths, which provides essential knowledge for governments on decision making. In this paper, we analyzed opioid poisoning related hospital visits using New York State SPARCS data, which provides diagnoses of patients in hospital visits. We identified all patients with primary diagnosis as opioid poisoning from 2010-2014 for our main studies, and from 2003-2014 for temporal trend studies. We performed demographical based studies, and summarized the historical trends of opioid poisoning. We used frequent item mining to find co-occurrences of diagnoses for possible causes of poisoning or effects from poisoning. We provided zip code level spatial analysis to detect local spatial clusters, and studied potential correlations between opioid poisoning and demographic and social-economic factors.

Abstract

PurposeTo examine the relationship of choroidal thickness with axial length (AL) and myopia in young adult eyes in the ethnically diverse Correction of Myopia Evaluation Trial (COMET) cohort.DesignCross-sectional, multicenter study.MethodsIn addition to measures of myopia by cycloplegic autorefraction and AL by A-scan ultrasonography, participants underwent optical coherence tomography imaging of the choroid in both eyes at their last visit (14 years after baseline). Using digital calipers, 2 independent readers measured choroidal thickness in the right eye (left eye if poor quality; n = 37) at 7 locations: fovea and 750, 1500, and 2250 μm nasal (N) and temporal (T) to the fovea.ResultsChoroidal thickness measurements were available from 294 of 346 (85%) imaged participants (mean age: 24.3 ± 1.4 years; 44.9% male) with mean myopia of -5.3 ± 2.0 diopters and mean AL of 25.5 ± 1.0 mm. Overall, choroidal thickness varied by location (P < .0001) and was thickest at the fovea (273.8 ± 70.9 μm) and thinnest nasally (N2250, 191.5 ± 69.3 μm). Multivariable analyses showed significantly thinner choroids in eyes with more myopia and longer AL at all locations except T2250 (P ≤ .001) and presence of peripapillary crescent at all locations except T1500 and T2250 (P ≤ .0001). Choroidal thickness varied by ethnicity at N2250 (P < .0001), with Asians having the thinnest and African Americans the thickest choroids.ConclusionChoroids are thinner in longer, more myopic young adult eyes. The thinning was most prominent nasally and in eyes with a crescent. In the furthest nasal location, ethnicity was associated with choroidal thickness. The findings suggest that choroidal thickness should be evaluated, especially in the nasal regions where myopic degenerations are most commonly seen clinically.

Abstract

Objectives. We evaluated the impact of a mailed, tailored intervention on skin cancer prevention and skin self-examination behaviors of adults at moderate and high risk for skin cancer. Methods. Adults at moderate and high risk for skin cancer were recruited in primary health care settings in Honolulu, HI, and Long Island, NY. After completing a baseline survey, participants were randomized to 2 groups. The treatment group received tailored materials, including personalized risk feedback, and the control group received general educational materials. Multivariate analyses compared sun protection and skin self-examination between groups, controlling for location, risk level, gender, and age. Results. A total of 596 adults completed the trial. The tailored materials had a significant effect on overall sun-protection habits, the use of hats, the use of sunglasses, and the recency of skin self-examination. Some effects were moderated by location and risk level. Conclusions. Tailored communications including personalized risk feedback can improve sun-protection behaviors and skin self-examination among adults at increased risk for skin cancer. These convenient, low-cost interventions can be implemented in a variety of settings and should be tested further to assess their long-term effectiveness. 39. Caregivers' perceptions of the relationship among weight, health status, and asthma in their children.

Abstract

Asthma and obesity are the two most common childhood illnesses and are physiologically interrelated. Few studies have assessed parental perceptions and beliefs about this relationship to better target education and therapy. This study aimed to determine caregiver beliefs and perceptions regarding weight, health status, and asthma diagnoses. Data from a survey of caregivers to children aged 4–11 years are merged with corresponding anthropometric and medical data from the electronic medical record. Caregivers of children with asthma completed a supplemental questionnaire. Univariable and multivariable logistic regressions were used to evaluate associations between perception of health problem, asthma, and weight status. Increased weight status was ≥ 85th body mass index percentile per Centers for Disease Control classifications. Compared to caregivers of healthy children and those of children with healthy weight and asthma, caregivers of dual diagnosis children were more likely to identify weight as a health problem (OR = 3.89, 95% confidence interval [1.48, 10.21]). Nevertheless, only 31% of caregivers of children with dual diagnosis believed weight contributed to the severity of their child's asthma. Less than one third of caregivers of dual diagnosis children believed that these diagnoses are interrelated. Abstract

Older patients with Hodgkin's disease suffer a significantly poorer prognosis than do younger patients. However, the cause for this is not clear. Determining the true nature of the relationship between age and prognosis requires that confounding factors be properly controlled for. For example, older patients may be more conservatively staged, which would increase their risk. Younger patients might be treated more aggressively and watched more closely, optimizing their chances. In an effort to evaluate the effect of age on the prognosis of Hodgkin's disease, data on 6,345 patients collected by the Patterns of Case Study sponsored by the American College of Surgery were used. These data provide information on 19 clinical and 11 pathological tests, as well as clinical stage and pathologic stage. This permitted the comparison between older and younger patients who were graded and treated equivalently. Although the matching reduced the apparent size of the age effect, there remained nonetheless a significantly poorer prognosis for older people with Hodgkin's disease than younger. While factors such as disease stage correlate with age, age is an independent predictor of poorer prognosis in Hodgkin's disease patients.
