Background: Holmium Laser Enucleation of the Prostate (HoLEP) was established 20 years ago as an alternative to Transurethral Resection of the Prostate (TUR-P) based on improved morbidity in patients with benign prostate syndrome (BPS). HoLEP can be applied independently to almost all sizes of prostate glands and is recommended in national and international guidelines. Although the HoLEP procedure has a good reputation, many patients still prefer conservative treatment due to fears of side-effects such as pain and urinary incontinence. The aim of this study was to identify patients' feelings, fears and perception of their HoLEP treatment based on Patient-reported Outcome Measures (PROMs) via questionnaires., Material Und Methods: During the time period from June to December 2020, 152 consecutive patients were treated by HoLEP due to BPS and were interviewed based on questionnaires right after their surgical treatment and 3 months later concerning their satisfaction as well as micturition and continence. Based on a written informed consent, 112 patients were included in the study, and a complete 3-month follow-up was available for 88 patients., Results: The mean volume of enucleated prostate tissue was 62.1 [g] and the mean prostate volume estimated pre-operatively (trans-rectal ultrasound) was 83.1 [cm3]. Overall patient-reported satisfaction with the hospital stay including surgical treatment was 94.6%; after 3 months it was 91.8%. Concerning micturition, 76.5% of the patients reported satisfaction after surgery and 80.4% were satisfied after 3 months. Urinary incontinence was reported in 8.3% initially and in 9.1% after 3 months. Positive answers to questions concerning the quality of life index [L] were obtained in 62.1% initially and their number increased to 85.7% after 3 months. The share of negative answers decreased from 11.7% after surgery to 3.4% within the 3-month follow-up., Discussion: HoLEP is a well-established treatment of BPS with lower side-effects compared with TUR-P according to the literature. To address doubts and misgivings, it is important to perform surveys on subjective patient satisfaction after the surgical procedure and over time. Language comprehension requires that single words be grouped into syntactic phrases, as words in sentences are too many to memorize individually. In speech, acoustic and syntactic grouping patterns mostly align. However, when ambiguous sentences allow for alternative grouping patterns, comprehenders may form phrases that contradict speech prosody. While delta-band oscillations are known to track prosody, we hypothesized that linguistic grouping bias can modulate the interpretational impact of speech prosody in ambiguous situations, which should surface in delta-band oscillations when grouping patterns chosen by comprehenders differ from those indicated by prosody. In our auditory electroencephalography study, the interpretation of ambiguous sentences depended on whether an identical word was either followed by a prosodic boundary or not, thereby signaling the ending or continuation of the current phrase. Delta-band oscillatory phase at the critical word should reflect whether participants terminate a phrase despite a lack of acoustic boundary cues. Crossing speech prosody with participants' grouping choice, we observed a main effect of grouping choice-independent of prosody. An internal linguistic bias for grouping words into phrases can thus modulate the interpretational impact of speech prosody via delta-band oscillatory phase., (© The Author 2016. Successful working-memory retrieval requires that items be retained as distinct units. At the neural level, it has been shown that theta-band oscillatory power increases with the number of to-be-distinguished items during working-memory retrieval. Here we hypothesized that during sentence comprehension, verbal-working-memory retrieval demands lead to increased theta power over frontal cortex, supposedly supporting the distinction amongst stored items during verbal-working-memory retrieval. Also, synchronicity may increase between the frontal cortex and the posterior cortex, with the latter supposedly supporting item retention. We operationalized retrieval by using pronouns, which refer to and trigger the retrieval of antecedent nouns from a preceding sentence part. Retrieval demand was systematically varied by changing the pronoun antecedent: Either, it was non-embedded in the preceding main clause, and thus easy-to-retrieve across a single clause boundary, or embedded in the preceding subordinate clause, and thus hard-to-retrieve across a double clause boundary. We combined electroencephalography (EEG), scalp-level time-frequency analysis, source localization, and source-level coherence analysis, observing a frontal-midline and broad left-hemispheric theta-power increase for embedded-antecedent compared to non-embedded-antecedent retrieval. Sources were localized to left-frontal, left-parietal, and bilateral-inferior-temporal cortices. Coherence analyses suggested synchronicity between left-frontal and left-parietal and between left-frontal and right-inferior-temporal cortices. Activity of an array of left-frontal, left-parietal, and bilateral-inferior-temporal cortices may thus assist retrieval during sentence comprehension, potentially indexing the orchestration of item distinction, verbal working memory, and long-term memory. The Kv1.1 potassium channel subunit, encoded by the Kcna1 gene, is heavily expressed in the auditory brainstem and is thought to have a critical role in producing the high temporal precision of action potentials characteristic of the auditory system. Our intent was to determine whether temporal acuity was reduced in Kcna1 null-mutant (-/-) mice, compared to wild-type (+/+) and heterozygotic mice (+/-), as measured by the encoding of gaps in the inferior colliculus by near-field auditory evoked potentials (NFAEP) or behavioral gap detection (BGD) using a prepulse inhibition paradigm. NFAEPs were collected at 40, 60 and 80 dB SPL with gap durations from 0.5 to 64 ms. BGD data were collected using silent gaps in 70 dB noise from 1 to 15 ms in duration. There were no systematic effects of Kcna1 genotype on NFAEP recovery functions, NFAEP latencies, or the time constant for BGD, but there was a small reduction in asymptotic prepulse inhibition for the longest gap stimuli in -/- mice. Gap thresholds were approximately 1-2 ms across genotypes, stimulus conditions, and paradigms. These data suggest that the neural pathways encoding behaviorally relevant, rapid auditory temporal fluctuations are not limited by the absence of Kv1.1 expression.

pH induced changes of bacteriostatic activity of nalidixic, pipemidic and oxolinic acids have been studied in vitro by the determination of minimal inhibitory concentrations (MIC), on 9 different species and 172 strains, at pH 5, 6, 7 and 8 and with increasing antibiotic concentrations. The bacteriostatic activity of nalidixic acid is higher at acid pH and conversely the bacteriostatic effects of pipemidic and oxolinic acids are enhanced at basic pH. The relationship between pH and MIC seems to be more dependent on the type of the micro-organism than on the antibiotic which is tested. A study of the bactericidal effect of quinolones on E. Coli and Pr. Morganii has shown a sharp increase of bactericidal activity at basic pH. These results support the idea that alkalinization is a useful addition to the treatment of severe urinary infections by quinolones.
