Objectives: The consecutive case series accesses the results and experiences of ridge augmentation using an umbrella screw tenting technique. Method and materials: In total, 279 patients were treated between 26 May 2015 and 16 June 2021, including horizontal and vertical ridge defects. Sex, age, smoking behavior, jaw, graft material, soft tissue thickness, extent of horizontal/vertical augmentation, resorption rate, and occurrence of early/late exposure were evaluated. Bone gain was determined by resorption at the screw head. Only cases without premature screw removal were evaluated metrically (n = 201). All other augmentations were evaluated according to whether implantation was possible with or without further augmentation (n = 27). A target performance index was calculated, which should enable evidence-based comparability of different augmentation methods in future. Results: In total, 54 wound dehiscences (39 early, 15 late exposures) occurred, which corresponds to 24.08% of the augmented sites; 42 umbrella screws were removed prematurely. In all cases an implantation was possible at the desired position afterwards. Cases with a vertical augmentation component showed a higher prevalence of exposure (early, P = .000; late, P = .024). The extent of the vertical augmentation was only relevant for early exposure (P = .048). Mean bone gain of 4.23 ± 1.69 mm horizontally and 4.11 ± 1.99 mm vertically could be achieved. Regression analysis showed that there was no limit in horizontal/vertical direction. Mean percentage target performance index was 75.90 ± 20.54 for vertical and 82.25 ± 16.67 for horizontal portions. Objectives: The aim of this in vitro study was to investigate whether and to what extent different scenarios of rotational freedom in different IAC designs affect the vertical dimension of a three‐part fixed partial denture (FPD). At the same time, the experimental setup should simulate all clinical and laboratory steps of the implementation of such an FPD as accurately as possible. Material and Methods: Twenty identical pairs of jaw models were fabricated from aluminum, each lower‐jaw model holding two implants with conical or flat IACs. Three impressions of each model were taken to fabricate stone casts and three‐unit FPDs. Three assembly scenarios were compared for the vertical position stability they offered for these FPDs, differing by how the sequential implant components (impression posts > laboratory analogs > abutments 1 > abutments 2) were aligned with the positional index of the IAC. In this way, a total of 60 stone casts and FPDs were fabricated and statistically analyzed for changes in vertical dimension (p < 0.05). Results: Regardless of whether a conical/flat IAC was used (p > 0.05), significantly greater mean changes in vertical dimension were consistently (all comparisons p < 0.0001) found in a "worst‐case scenario" of component alignment alternating between the left‐ and right‐limit stop of the positional index (0.286/0.350 mm) than in a "random scenario" of 10 dentists and 10 technicians with varying levels of experience freely selecting the alignment (0.003/0.014 mm) or in a "best‐case scenario" of all components being aligned with the right‐limit stop (−0.019/0.005 mm). Conclusions: The likelihood of integrating a superstructure correctly in terms of vertical dimension appears to vary considerably more with assembly strategies than with IAC designs. Specifically, our findings warrant a recommendation that all implant components should be aligned with the right‐limit stop of the positioning index. Summary: What is Known: Tiny vertical deviations can result in substantial clinical problems.Little is known about the impact of implant‐abutment connection (IAC) designs and rotational freedom on the vertical dimension of implant‐supported restorations.Past experimental setups, rather than simulating the entire clinical process from impression‐taking to intraoral delivery, have remained confined to specific steps. The present preference clinical trial compared the long-term outcome of acellular dermal matrix allograft (ADMA) versus autogenous connective tissue graft (CTG) in the treatment of gingival recessions. Method and materials: Thirtynine consecutive patients with 233 Miller Class I and II recessions were treated by one operator (MS) with coronally advanced flaps and in addition either ADMA or CTG harvested from their palate. Clinical parameters were measured by an independent and masked assessor at baseline, 6 months, and 5 years. Results: Thirty-two patients could be recruited for long-term examination (seven dropouts). At 6 months and 5 years, all clinical parameters showed significant improvements in both groups with slightly better but statistically not significant clinical results for CTGs. At 5 years, the CTG group revealed an additional gain of keratinized mucosa width (t6m-0: CTG 1.88 mm, ADMA 1.04 mm, P = .081; and t5y-0: CTG 3.98 mm, ADMA 3.06 mm, P = .01) compared to 6 months, whereas the mean for ADMAs remained stable (intergroup comparison statistically significant, P = .010). In all other parameters in both groups, slight but not statistically significant relapses were detected. Only one minor postoperative complication at one ADMA- treated site occurred. Conclusions: Regarding the longterm results, ADMA could be an alternative treatment option to thicken soft tissue and to cover multiple gingival recessions. If the gain of keratinized mucosa width is considered as a main goal, CTG may have a slight advantage over ADMA. [ABSTRACT FROM AUTHOR]

The objective of this study was to quantitatively determine ridge contour changes after different alveolar ridge preservation techniques. An initial total of 40 patients provided a final total of 35 single-gap extraction sites. After tooth removal, the socket was subjected to one of four treatment modalities: placement of a deproteinized bovine bone mineral (DBBM; Endobon) covered with a soft tissue punch from the palate (Tx1); placement of DBBM without soft tissue punch (Tx2); placement of an adsorbable collagen membrane (Osseoguard) covering the DBBM (Tx3); and no additional treatment (control). Silicone impressions were obtained before and 6 months after tooth extraction for quantitative-volumetric evaluation on stone cast models. Bone quality and need for further bone augmentation were also noted. Tx1 and Tx3 resulted in significantly less bucco-oral tissue loss when compared to Tx2 and the control group. Premolar teeth and teeth extracted for traumatic reasons revealed significantly less tissue loss. Using barrier membranes or soft tissue punches in addition to placement of DBBM seems to be advantageous to limit bucco-oral tissue atrophy. The clinical benefit, however, is still questionable. Purpose: Titanium bases are used frequently in daily practice for bonding to CAD/CAM abutments or crowns. Due to intimate contact between the adhesive gap of the titanium-base abutment and the peri-implant bone, the physical and chemical characteristics of the bonding material, or the gap itself, may affect peri-implant inflammatory reactions. The present study therefore aimed to examine the long-term effects of individualised abutments bonded to titanium bases on peri-implant health. Materials and methods: A total of 24 patients, each with one test and one control abutment, participated in the present prospective, single-blind, randomised controlled clinical trial. The test abutments were CAD/CAM titanium abutments bonded to titanium bases. As the control abutments were individualised, one-piece CAD/CAM titanium abutments were used. Clinical and radiographic parameters were assessed at abutment insertion and then on a yearly basis over the following 5 years. Results: No significant differences in marginal bone level were observed between the titanium-base and one-piece abutments at any of the follow-up time points; however, when intragroup marginal bone levels were compared to the baseline values, significant differences were found at several follow-up time points. Intergroup differences were only found to be significant for pocket depth at the 4- (P = 0.006) and 5-year follow-ups (P = 0.024), favouring titanium-base abutments. Conclusions: Within the limitations of the present study, it appears that the peri-implant tissues of this specific patient cohort responded to titanium-base abutments in a rather similar manner to one-piece abutments over a 5-year period; however, no definitive conclusions can be drawn due to the low power of the present study. The emerging literature has recently reported an alarming increase in peri-implantitis. This disease is typically described as the result of an imbalance between host response and bacterial load, supported by gram-negative anaerobic microflora. The current literature on the prevention and treatment of peri-implantitis does not allow for the extraction of applicable clinical information. In fact, the lack of efficacy of the current treatment methods may be a result of insufficient understanding of the biology. The aim of this position paper was to try to re-evaluate the etiopathogenesis of peri-implantitis, highlighting the principal clinically induced triggering factors of the disease. The consensus conference provided strong evidence to suggest that a different microbiologic flora (slightly different from that collected around teeth affected by periodontitis) could support peri-implantitis. However, the evidence to support a consensus statement regarding clinically triggering factors (surgical, prosthetic, and biomechanical) for peri-implantitis is only of moderate strength (cohort studies or consistent results from long-term, well-populated case series). Expert opinion led the consensus group to support the following: rectifying the number of peri-implant inflammatory situations caused by surgical, restorative, or material complications may lower the number of infections to a more realistic figure and may suggest different and more appropriate treatment plans. At the same time, it can be stated that implant material, shape and surface characteristics, procedures and biomaterials used for bone augmentation, and incorrect prosthetic procedures and biomechanical plans could also be risk factors for the occurrence and progression of peri-implantitis. There is a need for the evaluation of the long-term response to xenogeneic extraction socket grafting. In this case report, two maxillary incisors were extracted, and the sockets were filled with a xenogeneic bone substitute to counteract postextraction volume loss. Three years after tooth extraction and ridge preservation, there was ample bone volume to insert two implants. Histologic and histomorphometric analysis of trephine drill biopsy specimens provided evidence of new bone formation and biologic integration of the bone substitute. The results confirm the benefit of ridge preservation techniques and give insight into the histologic long-term results after socket filling. [ABSTRACT FROM AUTHOR]

Abstract Introduction The aim of this study was to clinically assess the capacity of a novel bovine pericardium based, non-cross linked collagen matrix in root coverage. Methods 62 gingival recessions of Miller class I or II were treated. The matrix was adapted underneath a coronal repositioned split thickness flap. Clinical values were assessed at baseline and after six months. Results The mean recession in each patient was 2.2 mm at baseline. 6 Months after surgery 86.7% of the exposed root surfaces were covered. On average 0,3 mm of recession remained. The clinical attachment level changed from 3.5 ± 1.3 mm to 1,8 ( ± 0,7) mm during the observational time period. No statistically significant difference was found in the difference of probing depth. An increase in the width of gingiva was significant. With a baseline value of 1.5 ± 0.9 mm an improvement of 2.4 ± 0.8 mm after six month could be observed. 40 out of 62 recessions were considered a thin biotype at baseline. After 6 months all 62 sites were assessed thick. Conclusions The results demonstrate the capacity of the bovine pericardium based non-cross linked collagen matrix for successful root coverage. This material was able to enhance gingival thickness and the width of keratinized gingiva. The percentage of root coverage achieved thereby is comparable to existing techniques. This method might contribute to an increase of patient's comfort and an enhanced aesthetical outcome.

This case series evaluated the use of a collagen matrix with a coronally advanced flap procedure for the treatment of multiple recession defects. Fifteen patients with a total of 80 recession defects were included. Root coverage was 85% ± 13% at 6 months and 81% ± 22% at 12 months. Complete root coverage was achieved in 60% of the sites after 6 months and in 56% after 12 months. The percentage of sites with thick gingival morphotype increased significantly. The results indicated that the collagen matrix may be a useful alternative to the connective tissue graft in the treatment of multiple recession defects. [ABSTRACT FROM AUTHOR]

Objectives: Hydrophilic implant surfaces promote faster osseointegration of dental implants with a higher bone-implant contact (BIC) rate. Animal and in vitro studies proved that ultraviolet (UV) irradiation of titanium implants regains hydrophilicity. Clinical impact is still unclear. The objective of this RCT was to assess the removal torque (RT) required to unfix a surface-treated implant (test group) versus the original surface implant (control group) performed at various points in time. The null hypothesis stated that test and control implants will show the same deliberation force at specific time points. Material and methods: One hundred eighty partially edentulous patients were randomly assigned to six groups. In single-stage surgery, each patient received one test and one control implant. In total, 180 test and 180 control implants were placed epicrestally. Test implants received a surface treatment with UV irradiation prior to insertion, in order to reduce carbon and enhance hydrophilicity and thus wettability. Maximum RT values for test and control implants were recorded with a torque measuring device at implant placement (T1), after 1 (group 1), 2 (group 2), 3 (group 3), 4 (group 4), 6 (group 5) (T2), and 8 weeks (group 6) of healing. Subsequently, implants were returned to their original position for the continuation of the healing process. Results: No implant was lost. Age, gender, smoking, implant position, and bone quality could be excluded as confounding factors because of the lack of statistical significance. At T2, RT values were higher for test implants compared with those for control implants, being statistically significant in groups 2, 3, 4, and 6 (p < 0.05). Conclusions: Our data support rejection of the null hypothesis. Clinical relevance: Photo-activation of the surface of titanium implants leads to higher resistance to RT forces compared with that of non-treated implants, indicating improved healing and implant stability especially in the early healing phase. Thin facial soft tissue may lead to visible soft tissue discoloration around implants and therefore to esthetic deficiencies. The aim of this article is to present a surgical approach to peri-implant soft tissue discoloration caused by the shine through effects of restorative materials in the anterior maxilla. A clinical case is used to illustrate the approach. A minimally invasive tunneling approach and connective tissue graft are used. The optical outcome is documented in an objective and standardized manner using a spectrophotometer after a follow-up of 12 months. The presented technique clearly improved the clinical condition and esthetic outcome. Spectrophotometric follow-up revealed a soft tissue color difference between the implant and adjacent tooth (control) that was clearly less than initially presented. In the area 3 mm apical to the soft tissue margin, this difference was beneath the clinically perceptible threshold of 3.7. [ABSTRACT FROM AUTHOR]

Esthetics is a topic that has become much discussed both in medicine and in dentistry. However, it should be questioned how reliable and reproducible the guidelines and protocols for esthetic procedures really are. This article will describe in detail the interactions between the musculoskeletal and masticatory systems. Dentists are frequently misled by their false interpretation of existing reference lines, without recognizing the true problem. The close relationship between skeletal and dental problems and their impact on the masticatory and musculoskeletal systems will be explained in this paper. Esthetics and function - not only of the teeth - form an entity that needs to be viewed and treated as a whole. Many of these interactions are thought of as "alternative medicine" and are therefore hardly considered at all in conventional mainstream medicine. Purpose: To evaluate whether a human dermis graft (HDG) could be used instead of an autogenous connective tissue graft (ACT) to increase gingival thickness and to cover multiple gingival recessions. Materials and methods: Forty-two consecutive patients with multiple gingival recessions were offered the option to have their recessions covered either with ACTs harvested from their palate or with HDGs. Outcome measures were complications, gingival recession changes (REC), gingival biotype changes (BIO), probing attachment level changes (PAL), probing pocket depth changes (PPD) and keratinised mucosa height changes (KER) evaluated by an independent and masked assessor after 6 months. Results: Twenty-one patients with 110 recessions opted for ACTs and 21 patients with 160 recessions opted for HDGs. No patients dropped out. Only one minor complication occurred at one HDG-treated site. Recessions significantly decreased in both groups (2.7 mm for ACT and 2.0 mm for HDG), however, ACTs resulted in a statistically significantly improved root coverage and CAL of 0.5 mm, as well as 18% more root coverage than HDGs. Conclusions: HDGs can be an effective alternative to ACTs for covering exposed roots and increasing soft tissue thickness, especially when many recessions have to be treated in the same mouth, though they may provide slightly less root coverage than ACTs. Covering exposed roots becomes more and more difficult as the gingiva becomes thinner and the vestibule becomes more shallow. Also, the outcome becomes less predictable. In addition, where there is high frenal attachment or muscle pull, such as the mentalis muscle in the mandibular anterior region, secondary retraction of a coronally advanced flap will likely occur. Therefore, a transplanted connective tissue graft may not completely cover the recession. This case series presents a technique where the roots are covered with a combination epithelializedsubepithelial connective tissue graft. The epithelialized portions of the graft arepositioned directly over the exposed roots to aid in resistance to the environment of the mouth, and there is no displacement of the mucogingival junction or flattening of the vestibule. Objectives: To evaluate whether connective tissue grafts performed at implant placement could be effective in augmenting peri-implant soft tissues. Materials and methods: Ten partially edentulous patients requiring at least one single implant in the premolar or molar areas of both sides of the mandible were randomised to have one side augmented at implant placement with a connective soft tissue graft harvested from the palate or no augmentation. After 3 months of submerged healing, abutments were placed and within 1 month definitive crowns were permanently cemented. Outcome measures were implant success, any complications, peri-implant marginal bone level changes, patient satisfaction and preference, thickness of the soft tissues and aesthetics (pink aesthetic score) evaluated by an independent and blinded assessor 1 year after loading. Results: One year after loading, no patients dropped out, no implants failed and no complications occurred. Both groups lost statistically significant amounts of peri-implant bone 1 year after loading (0.8 mm in the grafted group and 0.6 mm in the non-grafted group), but there was no statistically significant difference between groups. Soft tissues at augmented sites were 1.3 mm thicker (P < 0.001) and had a significantly better pink aesthetic score (P < 0.001). Patients were highly satisfied (no statistically significant differences between treatments) though they preferred the aesthetics of the augmented sites (P = 0.031). However, five patients would not undergo the grafting procedure again and two were uncertain. Conclusions: Connective tissue grafts are effective in increasing soft tissue thickness, thus improving aesthetics. Longer follow-ups are needed to evaluate the stability of peri-implant tissues over time. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate aesthetic and patient satisfaction after tooth extraction using a bone substitute (and soft tissue grafting when tissue thickness was lacking) under a pontic to preserve the alveolar ridge for aesthetic purposes. The contralateral natural tooth acted as internal control. Materials and methods: All patients with at least one site under a pontic augmented with Bio-Oss® or Bio-Oss® Collagen with or without a concomitant connective tissue graft with at least a follow-up of 6 months after the ridge preservation procedure were eligible for the present retrospective study. Sites with a damaged buccal wall were excluded. Outcome measures were: aesthetics (pink esthetic score, PES) evaluated by an independent and blinded dental hygienist on the basis of clinical pictures, patient satisfaction, patient preference and complications. Results: Twenty-six patients were consecutively treated, and 23 patients attended the evaluation visit. In seven patients, soft tissue grafts were performed in conjunction with Bio-Oss placement. Eight to 86 months after the ridge augmentation procedure (mean 38 months), there were no statistically significant differences observed in PES between preserved sites and control teeth. Patient satisfaction did not show any statistically significant difference between the two groups either. All patients declared they would undergo the same procedure again. Conclusions: Bio-Oss placement in post-extractive sites with a remaining buccal bone plate lead to a good aesthetic result. Randomised clinical trials with suitable control groups are needed to identify the most effective techniques and/or materials to preserve ridges under pontics. [ABSTRACT FROM AUTHOR]

This case report presents the clinical and technical protocol involved in treating a 32- year-old patient whose teeth and dental appearance were severely compromised as a result of bulimia nervosa. Patients suffering from this disorder have a high incidence of erosion of dentin and enamel, which over time may cause loss of vertical dimension.
