Abstract

New York State is experiencing teacher shortages in specific subject areas. One way to address these shortages is through the certification and placement of new teachers. This study explored the pathways through which new teachers between 2015/16 and 2017/18 earned certificates, their certification areas, and their subsequent placement and retention in districts across the state, particularly high-need districts. While the majority of new teachers earned certificates through the traditional in-state pathway, this varied somewhat by certification area. The proportion of teachers who earned certificates through the individual evaluation pathway was higher for the shortage certification area of career and technical education than for other certification areas. The most frequent certification area was the shortage certification area of special education, while the shortage certification areas of career and technical education and bilingual special education were among the least frequent. New York City district schools employed new teachers who earned certificates through the alternative in-state pathway at a higher rate than other types of high-need districts (rural, large city--not New York City, and other urban/suburban) as well as average- and low-need districts. New teachers employed in high-need districts had higher rates of retention in the same district for a second year than new teachers employed in average- and low-need districts. Abstract

Study question: The effect of HPV infection on semen characteristics was investigated using a new method that allow to evaluate virus localization in the different semen components. Summary answer: Our data indicate a correlation between HPV infection of exfoliated epithelial cells (from spermatic ducts and accessory glands) and reduction of sperm cell motility. What is known already: HPVs are agents of the most common sexually transmitted diseases. Both in men and in women, HPVs cause a variety of clinical symptoms ranging from warts to cancer. It has been shown (IARC, 2009) that infection with 12 oncogenic HPVs is the major cause of development of cervical cancer in women and it is also related to penile cancers in men. HPV infects epithelial cells and can bind other cell types. Its presence in semen is widely known, but the effects on fertility and reproductive function are still controversial. Study design, size, duration: From January 2015 to December 2015, twenty men were enrolled. From each subject seminal parameters were evaluated according to WHO guidelines. HPV was detected and genotyped both in total semen and in swim up-fraction. HPV detection was performed also in separated semen fractions: sperm cells from total semen and from swim-up and epithelial cells. Semen parameters of positive samples were compared with parameters of negative samples. Moreover, HPV genome wholeness in sperm cells was evaluated. Participants/materials, setting, methods: Partners of women with high-grade squamous intraepithelial lesions. DNA was purified after differential lysis. HPV genotyping was performed by a conventional reverse hybridization and HPV detection in the different semen components was performed by nested PCR with the use of universal primers (MY09 / 11, GP5+ / 6+). HPV genome wholeness was evaluated by Real Time-PCR using six different target genes. Main results and the role of chance: Samples were distributed into HPV positive and negative based on the conventional genotyping (total DNA extraction and reverse hybridization method) or on the new method (differential lysis, DNA extraction from separate fractions and nested PCR method) for every different fraction. In any case, we did not find any statistically significant difference on semen volume, sperm concentration, morphology. On the contrary, only when we compared HPV-positive epithelial cell samples to negative samples in the same fraction, we found a difference in sperm motility. The average percentage of sperm linear progressive motility amounted to 10% ± 11.5 in the positive samples and 54.4% ± 23.3 in the negative samples (two-tailed P value=0.0003). Moreover the HPV genome wholeness was tested by RT-PCR. Results performed on DNA extracted from sperm cells of a sample showed that the copy number of E1 and E2 genes was lower than LCR (Long Control Region) and E6 copy numbers (copy number ratio: 0.4 ± 0.05). These data indicate that in this sample more than half viral genomes are defective, suggesting that a recombination event could be occurred into the sperm heads. Limitations, reasons for caution: Low number of samples because of the sporadic frequency of infection with a single high-risk HPV genotype. Wider implications of the findings: The data support the proposed role of HPV in decreased male and couple fertility. The presence of defective viral genomes rises the idea that these defective forms could be integrated in the sperm genome. In turn, this risk prompts new possible consequences of the viral infection in semen.

Abstract

INTRODUCTION: Hypertension is considered to be among the most important risk factors for cardiovascular and cerebrovascular diseases. In recent years, several investigators have reported that high plasma levels of total homocysteine (t-hcy) has a key role in the development of hypertension, and the deficiency of B complex vitamins could increase the risk of hypertension. The purpose of this study was to investigate the relationship between plasma homocysteine, folate and vitamin B12 in hypertensive patients. MATERIALS AND METHODS: In 116 patients with hypertension and 81 healthy subjects, total plasma homocysteine, vitamin B12 and folate levels were measured. RESULTS AND DISCUSSION: Homocysteine was significantly higher in patients than in control subjects (22.9±3.5 versus 9.0±2.3 μmol/L respectively, p0.05). Our results confirmed that, as previously observed, elevated t-hcy levels and low folate levels, but not vitamin B12 levels, are significantly associated with hypertension.

21. Small interfering RNA targeted to IGF-IR delays tumor growth and induces proinflammatory cytokines in a mouse breast cancer model

Abstract

Insulin-like growth factor I (IGF-I) and its type I receptor (IGF-IR) play significant roles in tumorigenesis and in immune response. Here, we wanted to know whether an RNA interference approach targeted to IGF-IR could be used for specific antitumor immunostimulation in a breast cancer model. For that, we evaluated short interfering RNA (siRNAs) for inhibition of in vivo tumor growth and immunological stimulation in immunocompetent mice. We designed 2'-O-methyl-modified siRNAs to inhibit expression of IGF-IR in two murine breast cancer cell lines (EMT6, C4HD). Cell transfection of IGF-IR siRNAs decreased proliferation, diminished phosphorylation of downstream signaling pathway proteins, AKT and ERK, and caused a G0/G1 cell cycle block. The IGF-IR silencing also induced secretion of two proinflammatory cytokines, TNF-alpha and IFN-gamma. When we transfected C4HD cells with siRNAs targeting IGF-IR, mammary tumor growth was strongly delayed in syngenic mice. Histology of developing tumors in mice grafted with IGF-IR siRNA treated C4HD cells revealed a low mitotic index, and infiltration of lymphocytes and polymorphonuclear neutrophils, suggesting activation of an antitumor immune response. When we used C4HD cells treated with siRNA as an immunogen, we observed an increase in delayed-type hypersensitivity and the presence of cytotoxic splenocytes against wild-type C4HD cells, indicative of evolving immune response. Our findings show that silencing IGF-IR using synthetic siRNA bearing 2'-O-methyl nucleotides may offer a new clinical approach for treatment of mammary tumors expressing IGF-IR. Interestingly, our work also suggests that crosstalk between IGF-I axis and antitumor immune response can mobilize proinflammatory cytokines.

Abstract

Role of insulin-like growth factor (IGF-I) and its type I receptor (IGF-IR) in tumorigenesis has been well documented as well as various approaches to counteract their action in cells and animals either for basic research or for therapeutic purpose. Few of these strategies like RNA or oligonucleotide antisense-based down- regulation of IGF-I or IGF-IR have shown in addition to in vivo tumor growth inhibition, an interesting triggering of antitumoral immune response. In our work, IGF-I and IGF-IR blockade through RNA interference and their potential immunological stimulation induced by IGFs inhibition are addressed in syngenic models using 2’Omethyl modified short interfering RNA. After screening various sequences of siRNAs using luciferase-based reporter constructs, real-time PCR and western blot analysis, we find some sequences, which are able to downregulate efficiently IGF-I and IGF-IR mRNA and proteins in murine hepatocarcinoma and breast cancer cells. Moreover, silencing of IGF-IR diminished the phosphorylation of downstream signaling pathway proteins AKT and ERK1/2, inhibited cell proliferation and induced a G0/G1 cell cycle block. When transfected with anti IGF-I or anti IGF-IR siRNA, C4HD cells, which belongs to a murine progestin-dependent mammary tumor model, significantly decreased their tumorigenicity in BALB/c mice. Histological analyses of such tumors revealed a low mitotic grade and the presence of lymphocyte infiltration, suggesting the involvement of an antitumoral immunological response. In another set of preliminary experiments, IGF-IR down-regulated C4HD cells were used as a cellular vaccine to induce a protective immune response. A moderate immune response was therefore observed against parental tumor challenge. Abstract

We have compared the immunosuppressive effect of delta HOP and glucocorticoids on lymphocyte proliferation and IL-1 secretion. The new synthetic steroid only inhibited proliferation of phytohaemagglutinin (PHA)-stimulated human lymphocyte at high concentrations and the effect did not persist after washing out the steroid. In contrast, glucocorticoids produced the classical dose-response inhibition and the effect persisted when they were removed from the cultured medium. Although both steroids decreased IL-1 secretion from human monocytes stimulated with lipopolysaccharide (LPS), they exert the same effect through a different mechanism. The experiments we report suggest that the decrease of IL-1 synthesis produced by delta HOP could be caused by inhibition of LPS phagocytosis. These results support our hypothesis that delta HOP exerts its immunosuppressive and anti-inflammatory effect by a non-genomic mechanism.

41. Analysis of persistence of human papillomavirus infection in men evaluated by sampling multiple genital sites.

Abstract

OBJECTIVE: Although human papillomavirus (HPV) infection has been studied extensively in women, data on male infection are limited. The purpose of this study was to investigate persistence of HPV infection at multiple genital sites in men and to define potential associations with socio-behavioural characteristics. PATIENTS AND METHODS: Penile, urethral and seminal specimens were tested by the INNOLiPA HPV system (Innogenetics) and a PCR assay. Persistence was defined as the detection of the same HPV type at = 2 consecutive visits. The Kaplan-Meier method and the log-rank test were applied to estimate the likelihood of persistence. RESULTS: A total of 50 men (median age: 33 years) were fol lowed for a median of 14.7 months. Altogether, 49%, 36%, 26% and 11% of baseline HPV-positive men had 6-, 12-, 18- and 24-month persistent infection with any HPV type, respectively. The 6-, 12- and 18- month persistence was more common for oncogenic HPV infections; 24-month persistence was similar. The median duration of persistence was 21.7 months for any HPV. The median duration of persistence for any HPV type was significantly longer in the penile sample (22.5 months, 95% CI: 18.3-26.7) than the semen sample (15.3 months, 95% CI: 14.5-16.1). CONCLUSIONS: Over a third of type-specific HPV infections in men remained persistent over a 24-month period. The median duration of HPV infection was longer in penile samples compared to seminal samples. With increasing attention on HPV vaccination as a potential preventive approach for men, it is imperative to obtain additional insight on natural history of HPV infection in men, particularly as far as incidence and duration are concerned. Abstract

Asynchrony between embryo and uterine environment is one of the major limits in human in-vitro fertilization (IVF). A culture system which could prolong culture time and increase embryonic cleavage rate and viability would improve success rates. Using Vero cells, an in-vitro co-culture system was developed to investigate and promote human embryo development. Vero cells provide good support for human early embryos up to the blastocyst stage. When fertilized embryos were co-cultured, 68% of them reached the blastocyst stage. Pregnancy rate was 50% per transfer in patients with several previous failures of implantation. A significant increase in clinical pregnancy rate was also demonstrated when zygotes were maintained on Vero cell monolayer for only 24 h. The beneficial effect of the feeder layer may be through the release of embryotrophic factors and the detoxification of the culture medium by the cells. Abstract

AbstractObjective: To investigate the kinetics of insulin-like growth factor-1 receptor (IGF-1R) expression in PHA-stimulated T lymphocytes. Methods: IGF-1R protein and mRNA were detected by flow cytometry and RT-PCR respectively, between 0 and 48 h after cell activation. Results: Few minutes after T lymphocytes were activated, internalization of the IGF-1R from the cell membrane was observed, achieving the lower level between 1 and 6 h and was accompanied by a reduction in its mRNA. This was followed by re-expression of IGF-1R on the cell surface and an increase in IGF-1R mRNA levels in the cytoplasm, reaching levels higher than those recorded initially after 48 h activation. Conclusion: This down- and up-regulation suggests that restoration of IGF-1R would be the result of receptor recycling and de novo synthesis and highlights its importance for T lymphocyte proliferation.

Abstract

Human papilloma virus infection (HPV) is the most common sexually transmitted disease. Little is known about male infection. Nonavalent vaccine against types 6/11/16/18/31/33/45/52/58 was approved and neutral gender immunization programs have been proposed. This study evaluates the potential impact of nonavalent vaccine compared to quadrivalent in male living in Sicily (Italy). 58.7% of samples were HPV positive and forty-four types of HPV were identified. A significant higher estimated coverage of nonavalent vaccine than quadrivalent was observed (64.3% vs. 45.8%), with absolute and relative additional impact of 20.1% and 47.2%, respectively. Low impact of the vaccine were calculated as the empirical probability of HPV genotypes 6/11/16/18/31/33/45/52/58 alone or in combination; the high impact as empirical probability of HPV6/11/16/18/31/33/45/52/58 genotypes alone or in association with other genotypes. The potential impact of the nonavalent vaccine vs quadrivalent was significant for low and high impact (29.7% > 18:8%; 34:6% > 26.6%, respectively). Particularly, in men with lesions and risky sexual contact was significant only for low impact (35.5% > 29.7%; 31.4% > 19.7%, respectively). In partners with positive females was significant for low impact (26.3% > 15.1%) and high impact (33.7% > 23.2%). Nonavalent vaccine offers broader protection in men with HPV positive partners, who would have a potential role in the transmission of the infection.
