Abstract

Abstract Background The treatment of Herpes-simplex-virus-encephalitis (HSVE) remains a major unsolved problem in Neurology. Current gold standard for therapy is acyclovir, a drug that inhibits viral replication. Despite antiviral treatment, mortality remains up to 15%, less than 20% of patients are able to go back to work, and the majority of patients suffer from severe disability. This is a discouraging, unsatisfactory situation for treating physicians, the disabled patients and their families, and constitutes an enormous burden to the public health services. The information obtained from experimental animal research and from recent retrospective clinical observations, indicates that a substantial benefit in outcome can be expected in patients with HSVE who are treated with adjuvant dexamethasone. But currently there is no available evidence to support the routine use of adjuvant corticosteroid treatment in HSVE. A randomized multicenter trial is the only useful instrument to address this question. Design GACHE is a multicenter, randomized, double-blind, placebo-controlled, parallel group clinical trial of treatment with acyclovir and adjuvant dexamethasone, as compared with acyclovir and placebo in adults with HSVE. The statistical design will be that of a 3-stage-group sequential trial with potential sample size adaptation in the last stage. Conclusion 372 patients with proven HSVE (positive HSV-DNA-PCR), aged 18 up to 85 years; with focal neurological signs no longer than 5 days prior to admission, and who give informed consent will be recruited from Departments of Neurology of academic medical centers in Germany, Austria and The Netherlands. Abstract

It has become clear that the AIDS virus infects the brain, and many AIDS patients become demented as the disease progresses. Nevertheless, the actual pathologic processes which contribute to this dementia are still uncertain. There is some evidence that AIDS patients are more likely to develop cerebral ischemia (reduced blood flow to the brain) and stroke, and pathological studies have revealed an increased likelihood for AIDS patients to have cerebrovascular lesions. For this reason, a study was undertaken to determine if abnormalities in the function of the cerebral circulation could be detected early in the course of infection with HIV (human immunodeficiency virus). Twenty HIV-infected patients with at most early symptoms were evaluated using single-photon emission tomography (SPECT). Using technetium 99m hexamethyl-propyleneamine oxime, a radioactive tracer which permits the imaging of cerebral blood flow, abnormal blood flow results were observed in 14 patients. Only four of these patients had signs of cerebral atrophy (wasting of the cerebrum) on computed tomography (CT) scan, and seven appeared to be normal on neurological examination. While the observed patterns of abnormalities are not specific for HIV encephalopathy, and resemble those seen in Alzheimer's disease, they indicate that abnormalities in the cerebral circulation can occur in advance of clinically observable neurological symptoms or mental decay. Abstract

Lyme borreliosis, also called Lyme disease, is transmitted by ticks infected with Borrelia burgdorferi. Tick bites are the usual mode of disease transmission. Lyme borreliosis may affect the skin, heart, nervous system, and joints. The disease is usually first noted by a skin lesion (a reddened area that may be flat or raised) at the site of the tick bite. When Lyme disease affects the nervous system it is called Lyme neuroborreliosis, and may lead to meningitis (inflammation of the membranes covering the brain and spinal cord), cranial neuritis (inflammation of the nerves in the head), and painful radiculoneuritis (inflammation of the root of the nerves exiting the spinal cord). Penicillin, one of the first drugs used to treat Lyme disease, is not effective in killing Borrelia burgdorferi. The effectiveness of two cephalosporin antibiotics was assessed in 33 patients with Lyme neuroborreliosis. Over a 10-day period, patients were treated with ceftriaxone (2 grams per day) or cefotaxime (6 grams per day). During the treatment period, neurological symptoms improved or disappeared in 12 of 17 patients treated with ceftriaxone and 14 of 16 patients treated with cefotaxime. Eight months later, symptoms had completely resolved in 17 of 27 patients. Ten patients continued to experience neurological symptoms and one of them remained infected with Borrelia burgdorferi. These results indicate that a 10-day treatment program with ceftriaxone or cefotaxime is effective in reducing clinical symptoms in patients with Lyme neuroborreliosis. Abstract

The Relevant Outcome Scale for Alzheimer's disease (ROSA) is a novel, valid, and reliable instrument for multidimensional assessment of Alzheimer's disease (AD) symptoms across all severity stages. The ROSA and four standard instruments -- the Alzheimer's disease Assessment Scale-cognitive (ADAS-cog), Severe Impairment Battery (SIB), Disability Assessment for Dementia (DAD), and the Neuropsychiatric Inventory (NPI) -- were used in an open-label, multicenter, single-arm clinical study to assess treatment-induced changes in cognitive, functional, and behavioral symptoms in patients with AD at different severity stages.A total of 451 patients were treated with memantine (initiated at 5 mg/day and up-titrated with 5 mg weekly to a final dose of 20 mg/day) for 12 weeks. The study endpoints comprised changes from baseline in the scores of the ROSA, ADAS-cog, SIB, DAD, and NPI as well as global changes on the Clinical Global Impression of Change (CGI-C). Analyses were performed for the overall population and by AD severity stage (early, middle, late).The ROSA scores increased significantly after a 12-week treatment in all study groups except for early stage. Mean changes in the ADAS-cog score indicated a trend towards worsening in early and middle stages. Non-significant changes were shown by the SIB, NPI, and DAD assessments at week 12. The CGI-C demonstrated 'minimal improvement' or 'no change' for most of the patients. Overall, memantine treatment was safe and well tolerated.The results demonstrated the ROSA feasibility in daily practice for assessment of memantine effects over time in patients with moderate and late AD.

Ziel dieser Arbeit war es, Ursache und klinisches Erscheinungsbild einer konsekutiven Gruppe immunkompetenter erwachsener Enzephalitispatienten, deren funktionelles, neuropsychologisches und soziales Outcome im Langzeitverlauf sowie einen möglichen Hirnsubstanzverlust mittels Planimetrie von Magnetresonanztomogrammen zu untersuchen. Die retrospektive Analyse von 111 Patienten erbrachte eine eindeutige ätiologische Zuordnung in nur 28 % der Fälle; am häufigsten waren Varizella-zoster-Virus und Herpes-simplex-Virus-I. An nicht-viralen Erregern sind Mycoplasmen hervorzuheben. Mit der cranialen Magnetresonanztomographie waren bei ca. 50 % parenchymatöse Veränderungen nachweisbar. Die Sensitivität der Elektroenzephalographie betrug über 80 %. Ein Drittel der Patienten mußte intensivmedizinisch behandelt werden. Die Letalität war mit knapp 2 % gering. Die follow-up-Untersuchung von 73 Patienten mit durchschnittlich drei Jahre zurückliegender akuter Enzephalitis ergab bei 86 % der Patienten einen günstigen bis befriedigenden Verlauf mit erhaltener Selbständigkeit. Bei den ungünstig verlaufenden Fällen dominierten kognitive Beeinträchtigungen und pharmakoresistente Epilepsien, hochgradige körperliche Behinderungen persistierten nur bei 4 %. Neuropsychologisch meßbare Defizite persistierten nur bei einer geringen Zahl von Patienten. Im Vergleich zu einer Kontrollgruppe ergaben sich Defizite insbesondere für diejenigen Patienten, die während der akuten Enzephalitis viele epileptische Anfälle erlitten hatten. Diese Gruppe von Patienten hatte auch eine signifikant deutlichere Zunahme der planimetrisch bestimmten ventricle-brain-ratio, also einen stärkeren Verlust an Hirnparenchym, als die anderen Patienten. Auch die subjektive Lebensqualität war bei dieser Gruppe von Patienten am stärksten beeinträchtigt. One third of the patients required intensive care therapy. Mortality was low with less than 2 %. 38. Human Cytomegalovirus (HCMV) Encephalitis in an Immunocompetent Young Person and Diagnostic Reliability of HCMV DNA PCR Using Cerebrospinal Fluid of Nonimmunosuppressed Patients

Abstract

ABSTRACTHuman cytomegalovirus (HCMV) encephalitis in adult nonimmunosuppressed patients has rarely been reported. We have diagnosed HCMV encephalitis in an anti-HCMV immunoglobulin G-negative, nonimmunosuppressed young woman by HCMV DNA PCR and virus isolation from cerebrospinal fluid (CSF). At the same time, HCMV antigen and HCMV DNA could be demonstrated in peripheral blood leukocytes, and the virus was isolated in fibroblast cultures. After 22 days of acute illness, the virus disappeared from the CSF. Remarkably, the patient did not generate detectable anti-HCMV antibodies within 5 months after the beginning of illness. To investigate the significance of HCMV DNA detection in CSF, samples of CSF, blood cells, and serum from 35 nonimmunosuppressed patients with various neurological disorders (but no herpes simplex virus central nervous system [CNS] disease) were tested for HCMV DNA, antigen, and antibodies. Eleven of these patients were found to be positive for virus DNA and/or antigen in peripheral blood leukocytes. Additionally, HCMV DNA was detected in the CSF of two patients with noninflammatory CNS diseases. A causative role of HCMV in the CNS diseases of these two patients was not evident. In summary, HCMV DNA amplification from CSF samples is a very suitable method to verify HCMV-associated encephalitis, but it should be taken into consideration that there are few cases of positive PCR with DNA from CSF without any known clinical correlative.
