Abstract

The Batalin-Vilkovisky data for Polyakov string theory on a manifold with (non-null) boundary is shown to induce compatible Batalin--Fradkin--Vilkovisky data, thus allowing BV-quantisation on manifolds with boundary. On the other hand, the analogous formulation of Nambu--Goto string theory fails to satisfy the needed regularity requirements. Abstract

After a review of the pure state case, we discuss from a geometrical point of view the meaning of the quantum Fisher metric in the case of mixed states for a two-level system, i.e. for a q-bit, by examining the structure of the fiber bundle of states, whose base space can be identified with a co-adjoint orbit of the unitary group. We show that the Fisher Information metric coincides with the one induced by the metric of the manifold of SU(2), i.e. the 3-dimensional sphere $S^3$, when the mixing coefficients are varied. We define the notion of Fisher Tensor and show that its anti-symmetric part is intrinsically related to the Kostant Kirillov Souriau symplectic form that is naturally defined on co-adjoint orbits, while the symmetric part is nontrivially again represented by the Fubini Study metric on the space of mixed states, weighted by the mixing coefficients., Comment: 20 pages; Abstract and Introduction modified, references added. Abstract

An extension of the notion of classical equivalence of equivalence in the Batalin–Vilkovisky (BV) and Batalin–Fradkin–Vilkovisky (BFV) frameworks for local Lagrangian field theory on manifolds possibly with boundary is discussed. Equivalence is phrased in both a strict and a lax sense, distinguished by the compatibility between the BV data for a field theory and its boundary BFV data, necessary for quantisation. In this context, the first- and second-order formulations of nonabelian Yang–Mills and of classical mechanics on curved backgrounds, all of which admit a strict BV–BFV description, are shown to be pairwise equivalent as strict BV–BFV theories. This in particular implies that their BV complexes are quasi-isomorphic. Furthermore, Jacobi theory and one-dimensional gravity coupled with scalar matter are compared as classically equivalent reparametrisation-invariant versions of classical mechanics, but such that only the latter admits a strict BV–BFV formulation. They are shown to be equivalent as lax BV–BFV theories and to have isomorphic BV cohomologies. This shows that strict BV–BFV equivalence is a strictly finer notion of equivalence of theories.

Abstract

Natural and human-made disasters affect and disrupt the lives and livelihoods of people in different ways. This is at the core of the Sendai Framework for Disaster Risk Reduction. The expected outcome of the framework is a ‘substantial reduction of disaster risk and losses in lives, livelihoods and health and in the economic, physical, social, cultural and environmental assets of persons, businesses, communities and countries’ (UNISDR, 2015, p. 12). In order to monitor this, the first two targets of the framework address the people killed or affected, because people are the most important asset to protect from risk and disaster. People are the ones we ultimately want to protect from the impact of disasters. As the Sendai Framework states, there are not only the direct and immediate impacts of hazards on people such as death or injury. There are also a number of indirect and long-term impacts on people that may not injure or kill directly, but instead may cause long-term impacts on individuals or entire societies. The impacts of disasters may deprive people of their homes, their livelihoods and impede the functioning of society as a whole. Consequently, this subchapter will address the population at risk and the potential impacts of disasters on populations by analysing the different dimensions, from the individual to society as a whole. Starting from the individual, Section 3.2.1 analyses how hazards threaten human lives in Europe, and the wider impacts of disasters on people’s health and well-being, considering how different hazards interplay with the effects beyond immediate impacts. This section addresses the relations between the disaster management cycle and hazards causing death, injury or health damage across Europe. The temporal dimension plays a central role here; the speed of hazard onset (quick/slow onset) has to be linked to the duration of effects on humans (short/long term). The section proposes new approaches such as biomonitoring methods and biomarkers for improved assessment of exposure and human health risk; it also addresses the importance of human behaviour and measures of self-protection as factors influencing impacts, illustrating this with the example of the heatwave in Europe in 2003. Section 3.2.2 enlarges the view from the individual to the immediate habitat of the people – their homes and immediate neighbourhood. It takes the housing/habitat as the asset at risk and analyses the impacts of three different disasters: the Toll Bar flood 2007 in the United Kingdom, the Grenfell Tower fire in 2017 in the United Kingdom and a series of earthquakes in Italy. The primary measure of impact analysed in this section is relocation or displacement of population from their homes. This takes into account the different spatio-temporal scales of displacement. The analysis highlights the importance of hindsight analysis and implementation of the lessons learned. Section 3.2.3 finally expands the analysis of the impact of disasters to the entire society. By ‘society’, we refer to all the people that live together, have a common history and cooperate to carry on their lives and pursue fundamental interests. This can be a local society, but also parts of a nation or the entire population of a country or region. Society has a complex structure with uncounted social and economic relationships. The structure of society is dynamic, with many external and internal factors that are constantly changing and developing it. When disasters strike, they also affect societies and may lead to disruption of the way societies function. This section explores the Van earthquake in Turkey (2011) and a toxic cloud after a technical accident in Zevekote (Belgium) in 2017 to review the impacts of different types of disasters at the community/society level based on the results of case studies, to show social reactions to disasters and to better illustrate social patterns and vulnerable groups in society.

Abstract

PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks.

Abstract

The Batalin–Vilkovisky data for Polyakov string theory on a manifold with (non-null) boundary are shown to induce compatible Batalin–Fradkin–Vilkovisky data, thus allowing BV-quantisation on manifolds with boundary. On the other hand, the analogous formulation of Nambu–Goto string theory fails to satisfy the needed regularity requirements. Abstract

© 2020 This paper presents a novel method to delineate metropolitan areas – or functional urban areas (FUAs) – in the entire world and assesses their population trends. According to the definition developed by the OECD and the European Union, FUAs are composed of high-density urban centres with at least 50 thousand people plus their surrounding commuting zones. The latter represent the urban centres’ areas of influence in terms of labour market flows. The proposed method combines a functional and a morphological approach to overcome the dependency on travel-to-work data to define commuting zones and allow a global delineation. It relies on a probabilistic approach and the use of population and travel impedance gridded data across the globe. Results show that around 3.9 billion people, making up 53% of the world population, live in 8,790 FUAs, out of which 17% live in their commuting zones. Between 2000 and 2015, population growth was higher in larger FUAs, highlighting a general trend toward higher concentration of the metropolitan population. Commuting zones grew faster than urban centres, though with heterogeneous patterns across world regions, income levels and metropolitan size.

Abstract

Background: To report health-related quality of life outcomes as assessed by validated patient-reported outcome measures (PROMs) after radical prostatectomy (RP). ­Methods: This study analyzed patients treated with RP within The PROState cancer monitoring in Italy, from the National Research Council (Pros-IT CNR). Italian versions of Short-Form Heath Survey and university of California los Angeles-prostate cancer index questionnaires were administered. PROMs were physical composite scores, mental composite scores and urinary, bowel, sexual functions and bothers (UF/B, BF/B, SF/B). Baseline unbalances were controlled with propensity scores and stabilized inverse weights; differences in PROMs between different RP approaches were estimated by mixed models. Results: Of 541 patients treated with RP, 115 (21%) received open RP (ORP), 90 (17%) laparoscopic RP (LRP) and 336 (61%) robot-assisted RP (RARP). At head-to-head ­comparisons, RARP showed higher 12-month UF vs. LRP (interaction treatment * time p = 0.03) and 6-month SF vs. ORP (p < 0.001). At 12-month from surgery, 67, 73 and 79% of patients used no pad for urinary loss in ORP, LRP and RARP respectively (no differences for each comparison). Conversely, 16, 27 and 40% of patients declared erections firm enough for sexual intercourse in ORP, LRP and RARP respectively (only significant difference for ORP vs. RARP, p = 0.0004). Conclusions: Different RP approaches lead to significant variations in urinary and sexual PROMs, with a general trend in favour of RARP. However, their clinical significance seems limited.

The triadic description of general relativity in three dimensions is known to be a BF theory. Diffeomorphisms, as symmetries, are easily recovered on-shell from the symmetries of BF theory. This note describes an explicit off-shell BV symplectomorphism between the BV versions of the two theories, each endowed with their natural symmetries.

Abstract

Summary Background Patients with chronic obstructive pulmonary disease (COPD) have few options for treatment. The efficacy and safety of the phosphodiesterase-4 inhibitor roflumilast have been investigated in studies of patients with moderate-to-severe COPD, but not in those concomitantly treated with longacting inhaled bronchodilators. The effect of roflumilast on lung function in patients with COPD that is moderate to severe who are already being treated with salmeterol or tiotropium was investigated. Methods In two double-blind, multicentre studies done in an outpatient setting, after a 4-week run-in, patients older than 40 years with moderate-to-severe COPD were randomly assigned to oral roflumilast 500 μg or placebo once a day for 24 weeks, in addition to salmeterol (M2-127 study) or tiotropium (M2-128 study). The primary endpoint was change in prebronchodilator forced expiratory volume in 1 s (FEV 1 ). Analysis was by intention to treat. The studies are registered with ClinicalTrials.gov, number NCT00313209 for M2-127, and NCT00424268 for M2-128. Findings In the salmeterol plus roflumilast trial, 466 patients were assigned to and treated with roflumilast and 467 with placebo; in the tiotropium plus roflumilast trial, 371 patients were assigned to and treated with roflumilast and 372 with placebo. Compared with placebo, roflumilast consistently improved mean prebronchodilator FEV 1 by 49 mL (p 1 was noted in both groups. Furthermore, roflumilast had beneficial effects on other lung function measurements and on selected patient-reported outcomes in both groups. Nausea, diarrhoea, weight loss, and, to a lesser extent, headache were more frequent in patients in the roflumilast groups. These adverse events were associated with increased patient withdrawal. Interpretation Roflumilast improves lung function in patients with COPD treated with salmeterol or tiotropium, and could become an important treatment for these patients. Funding Nycomed.

Abstract

Geo-information on settlements from Earth Observation offers a base for objective and scalable monitoring of the evolution of cities and settlements, including their location, extent and other attributes. In this work, we deploy the best available global knowledge on the presence of human settlements and built-up structures derived from Earth Observation to advance the understanding of the human presence on Earth. We start from a concept of Generalised Settlement Area to identify the Earth surface within which any built-up structure is present. We further characterise the resulted map by using an agreement map among the state of the art of remote sensing products mapping built-up areas or other strictly related semantic abstractions as urban areas or artificial surfaces. The agreement map is formed by a grid of 1 km2, where each cell is classified according to the number of EO-derived products reporting any positive occurrence of the abstractions related to the presence of built-up structures. The paper describes the characteristics of the Generalised Settlement Area, the differences in the agreement map across geographic regions of the world, and outlines the implications for potential users of the EO-derived products used in this study. Abstract

Pulmonary alveolar proteinosis (PAP), lymphangioleyomiomatosis (LAM) and amyloidosis are three unrelated diseases of rare occurrence, with characteristic histopathological features. A pattern of alveolar filling with granular pink material accumulation is characteristic of PAP. This material can be recognized in lung biopsies, but also in bronchial lavage fluid. PAP is clinically related to the abnormal clearance of alveolar surfactant, most commonly due to the disruption of the granulocyte macrophage-colony stimulating factor signalling pathway. Whole lung lavage is the treatment of choice. LAM is characterized by cystic lung degeneration and interstitial proliferation of LAM cells, which express both melanocyte and smooth muscle cell markers, has a typical cystic pattern on CT scan, can be associated clinically with abdominal angiomyolipomas and limphangioleiomyomas, and occurs in female patients, either in isolation or as a manifestation of tuberous sclerosis. Sex hormone manipulation is the therapy of choice in this otherwise progressive disease. Diffuse interstitial or perivascular amyloid deposits in the lung can form in the context of systemic amyloidosis, usually associated with myeloma or monoclonal gammopathy, and less often with chronic inflammatory diseases. Nodular amyloid deposits, in contrast, are not associated with systemic lung disease, and present instrumentally as a coin lesion or lung mass. Isolated tracheobronchial amyloidosis is another rare form that is not related to systemic disease. In all conditions, amyloid has a typical waxy, amorphous, slightly eosinophilic stain, stains red with Congo red and presents a characteristic apple-green birefringence under polarized light, which is essential for diagnosis.

Abstract

Lymphangioleiomyomatosis (LAM) is a rare lung disease, which occurs sporadically or in association with the genetic disease tuberous sclerosis complex (TSC) 1, 2. Sporadic LAM affects ∼1 in 400,000 adult females; in TSC, LAM occurs in 30–40% of adult females 3, 4 and exceptionally in males and children 5, 6. Patients with LAM usually develop progressive dyspnoea and recurrent pneumothorax, chylous collections and occasional haemoptysis 1. Extra pulmonary lymphadenopathy and cystic masses of the axial lymphatics termed lymphangioleiomyomas can result in abdominal and pelvic lymphatic obstruction 7. LAM is often associated with angiomyolipoma in the kidneys 8, and an increased frequency of meningioma 9. LAM varies in clinical features and rate of progression: this together with an absence of clear prognostic factors results in patients being given conflicting information about prognosis. Diagnosis is made by tissue biopsy (generally from the lung but occasionally from lymph nodes or lymphangioleiomyomas) and/or a combination of history and high-resolution computed tomography scanning (HRCT). Pathological diagnosis relies on characteristic LAM cell morphology and positive immunoreactivity to smooth muscle actin and HMB-45 antibodies. Increasingly HRCT is used to diagnose LAM without resorting to lung biopsy; however a number of conditions with multiple pulmonary cysts can mimic LAM. As LAM is rare, there have been no controlled trials of its management. Supportive treatment includes management of airflow obstruction and hypoxaemia with bronchodilators and oxygen respectively, specific treatment for surgical or pleural complications including pneumo- and chylothorax, and interventional treatment of renal lesions 10, 11. As LAM is a disease of females and is thought to be accelerated by oestrogen, oophorectomy, tamoxifen, progesterone and gonadotropin-releasing hormone (GnRH) analogues have been used without evidence that they are effective. Abstract

A case of Ondine's curse associated with hypothalamic dysfunction in an 8-year-old boy is described. The neuropathological examination revealed a viral encephalitis affecting the hypothalamus and the brainstem. In the medulla the inflammatory process involved the reticular formation and the nuclei considered to control automatic respiration such as the dorsal motor nucleus of the vagus, the nucleus tractus solitarii and the nucleus ambiguous. Although Ondine's curse following viral infection of the central nervous system has been previously reported this represents the first case of viral encephalitis to be pathologically documentated.
