Abstract

Aims: \ud Hypertensive disorders occur in women with peripartum cardiomyopathy (PPCM). How often hypertensive disorders co-exist, and to what extent they impact outcomes, is less clear. We describe differences in phenotype and outcomes in women with PPCM with and without hypertensive disorders during pregnancy.\ud \ud Methods: \ud The European Society of Cardiology PPCM Registry enrolled women with PPCM from 2012-2018. Three groups were examined: 1) women without hypertension (‘PPCM-noHTN’); 2) women with hypertension but without pre-eclampsia (‘PPCM-HTN’); 3) women with pre-eclampsia (‘PPCM-PE’). Maternal (6-month) and neonatal outcomes were compared.\ud \ud Results: \ud Of 735 women included, 452 (61.5%) had PPCM-noHTN, 99 (13.5%) had PPCM-HTN and 184 (25.0%) had PPCM-PE. Abstract

Zielsetzung: In dieser Studie haben wir die Einführungsphase eines neuen Mentoring-Programms für Medizinstudierende in der hausärztlichen Grundversorgung evaluiert. Es sollten folgende Aspekte beurteilt werden: die Integration der Studierenden in den Praxisalltag der Grundversorger, die Qualität des Unterrichts, der Nutzen eines frühen Patientenkontakts im Studium und die Zufriedenheit der Teilnehmer mit dem Programm. Methodik: Im ersten Implementierungsjahr haben Studierende des 1. und 3. Studienjahres jeweils acht halbe Tage, Studierende des 4. Studienjahres einen dreiwöchigen Blockunterricht bei einem Lehrarzt (Mentor) in der Hausarztpraxis absolviert. Studierende und Lehrärzte wurden nach einem Jahr Erfahrung mit dem Programm mittels Fragebögen befragt. Quantitative und qualitative Daten wurden analysiert. Ergebnisse: 90% der Studierenden und 98% der Lehrärzte gaben an mit dem Programm zufrieden zu sein. Wichtige Faktoren, die zur grossen Akzeptanz beigetragen haben, waren: die Möglichkeit der flexiblen Terminvereinbarung, die gute Integration der Studierenden in den Praxisalltag, das Lernen der Studierenden am Patienten im realen Berufskontext, die intensive 1:1-Betreuung, das Engagement und die Motivation der Teilnehmer und die Definition klarer Ausbildungsziele. Beide Gruppen stuften den Nutzen der Praktika für Studierende im 3. und 4. Studienjahr höher ein als für diejenigen im 1. Studienjahr. Negative Aspekte der Praktika waren die nicht immer regelmässig durchgeführten Feedbackgespräche und die zeitliche Belastung der Lehrärzte. Schlussfolgerungen: In dieser Studie konnten Faktoren identifiziert werden, die zur Zufriedenheit der Teilnehmer mit dem Unterricht in Hausarztmedizin beigetragen haben. Diese können von Curriculumsplanern herangezogen werden. Wichtige Themen für die Weiterbildung der Lehrärzte und Massnahmen für die Optimierung des Programms werden aufgezeigt. An der Medizinischen Fakultät der Universität Bern wurden im Herbst 2007 neue Ausbildungsmodule in Hausarztmedizin implementiert. Diese obligatorischen Praktika ermöglichen allen Studierenden der Humanmedizin während der ersten vier Studienjahre einen kontinuierlichen Kontakt zu einem Lehrarzt in der Grundversorgung. Die Studierenden absolvieren im Bachelor-Studium acht halbtägige Praktika pro Jahr, im Masterstudium einen dreiwöchigen Praktikumsblock. Die Praktika sollen das Ausbildungsspektrum erweitern, das Interesse der Studierenden an der Grundversorgung wecken und den Aufbau einer Mentorbeziehung zu einem Hausarzt ermöglichen. Mit dem frühen Patientenkontakt soll die Motivation der Studierenden für das Studium und den Beruf gefördert und die professionelle Haltung verbessert werden. Dank dem enormen Engagement der Grundversorger konnten 645 benötigte Praktikumsplätze innert zweier Jahre bereitgestellt werden. 38. Primary care at Swiss universities--current state and perspective.

Abstract

BACKGROUND: There is increasing evidence that a strong primary care is a cornerstone of an efficient health care system. But Switzerland is facing a shortage of primary care physicians (PCPs). This pushed the Federal Council of Switzerland to introduce a multifaceted political programme to strengthen the position of primary care, including its academic role. The aim of this paper is to provide a comprehensive overview of the situation of academic primary care at the five Swiss universities by the end of year 2012. RESULTS: Although primary care teaching activities have a long tradition at the five Swiss universities with activities starting in the beginning of the 1980ies; the academic institutes of primary care were only established in recent years (2005 - 2009). Only one of them has an established chair. Human and financial resources vary substantially. At all universities a broad variety of courses and lectures are offered, including teaching in private primary care practices with 1331 PCPs involved. Regarding research, differences among the institutes are tremendous, mainly caused by entirely different human resources and skills. CONCLUSION: So far, the activities of the existing institutes at the Swiss Universities are mainly focused on teaching. However, for a complete academic institutionalization as well as an increased acceptance and attractiveness, more research activities are needed. In addition to an adequate basic funding of research positions, competitive research grants have to be created to establish a specialty-specific research culture.

Abstract

Despite decades of research, the pathophysiology of bipolar disorder (BD) is still not well understood. Structural brain differences have been associated with BD, but results from neuroimaging studies have been inconsistent. To address this, we performed the largest study to date of cortical gray matter thickness and surface area measures from brain magnetic resonance imaging scans of 6503 individuals including 1837 unrelated adults with BD and 2582 unrelated healthy controls for group differences while also examining the effects of commonly prescribed medications, age of illness onset, history of psychosis, mood state, age and sex differences on cortical regions. In BD, cortical gray matter was thinner in frontal, temporal and parietal regions of both brain hemispheres. BD had the strongest effects on left pars opercularis (Cohen’s d=-0.293; P=1.71 × 10-21), left fusiform gyrus (d=-0.288; P=8.25 × 10-21) and left rostral middle frontal cortex (d=-0.276; P=2.99 × 10-19). Longer duration of illness (after accounting for age at the time of scanning) was associated with reduced cortical thickness in frontal, medial parietal and occipital regions. We found that several commonly prescribed medications, including lithium, antiepileptic and antipsychotic treatment showed significant associations with cortical thickness and surface area, even after accounting for patients who received multiple medications. We found evidence of reduced cortical surface area associated with a history of psychosis but no associations with mood state at the time of scanning. Our analysis revealed previously undetected associations and provides an extensive analysis of potential confounding variables in neuroimaging studies of BD.

43. Intermittent claudication as a predictor of outcome in patients with ischaemic systolic heart failure: Analysis of the controlled rosuvastatin multinational trial in heart failure trial (CORONA)

Abstract

To examine the relationship between baseline intermittent claudication and outcomes in patients enrolled in the Controlled Rosuvastatin Multinational Trial in Heart Failure trial (CORONA). Intermittent claudication is an independent predictor of worse outcome in coronary heart disease, but its prognostic importance in heart failure (HF) is unknown. Patients aged ≥60 years with NYHA class II-IV, low ejection fraction HF of ischaemic aetiology were enrolled in CORONA. Rosuvastatin did not reduce the primary outcome or all-cause mortality. Methods and results: To determine whether intermittent claudication was an independent predictor of clinical outcomes, a three-step multivariable model was built: (i) demographic/clinical variables, (ii) biochemical measures added, (iii) high-sensitivity C-reactive protein and N-terminal pro B-type natriuretic-peptide added. Of the 5011 patients, 637 (12.7) had intermittent claudication at baseline. Patients with intermittent claudication were more likely to be male (83 vs. 75), be a current smoker (19 vs. 9), and have diabetes mellitus (36 vs. 29) relative to those without intermittent claudication. Over a median 33-month follow-up, 2168 patients died or were hospitalized for HF. Patients with intermittent claudication had an increased risk of death (any cause) (adjusted hazard ratio 1.36, 95 CI 1.19-1.56, P < 0.0001), death from worsening HF (1.35, 1.03-1.77, P = 0.028), sudden death (1.24, 1.00-1.54, P = 0.05), and risk of non-fatal or fatal myocardial infarction (time to first event 1.67, 1.24-2.27, P < 0.001). In the full multivariable model, intermittent claudication remained an independent predictor of most outcomes evaluated. Abstract

AIM: This paper is a report of a study to describe patient and nurse perceptions of patient satisfaction with information about the medicines at two heart failure clinics after medication up-titration and information-giving. BACKGROUND: Chronic heart failure is a major cause of hospitalization. Poor adherence to medications increases mortality and heart failure-related hospitalizations. To achieve mutual goal-setting (concordance) regarding medication-taking behaviours, health providers need to understand patient information needs regarding the prescribed medicines. METHODS: A convenience sample of 56 patients with chronic heart failure referred for an up-titration of medicines and information-giving about the condition and treatment completed the Satisfaction about Information about Medicines Scale at their first and last visits. Nurses completed the same questionnaire after each patient's final visit, assessing the patient's need for further information. The data were collected between 2002 and 2004. RESULTS: Patient scores indicated statistically significantly more satisfaction with their information at the final visit compared with the first visit (P = 0.005). However, at the programme conclusion, nurses (n = 7) rated patients to be appropriately informed while patients reported a persistent need for further information (P = 0.011). CONCLUSION: Further research should evaluate more advanced pedagogical strategies such as how to address patients' expectations about the effect of medicines when actual effects of the treatment are related to mortality and morbidity at the population level and may not result directly in symptom relief at an individual level.
