Abstract

The WSG as a German interdisciplinary group initiated 05/94 a large multicentre phase III trial to evaluate adjuvant high-dose chemotherapy in high-risk breast cancer. About 100 centres all over Germany participated. The second trial in metastatic breast cancer (MBC) was initiated in 4/97 in co-operation with the German Intergroup. The main characteristics of the two trials are listed in Table ​Table11. Table 1 The first trial tests high-dose Tandem E90C3000T400 + PBPC versus dose-dense conventional chemotherapy with G-CSF support. All patients received irradiation of the chest wall and the supraclavicular lymph nodes, and tamoxifen in case of ER+ tumors. The trial will probably be closed by the end of the year. The first interim analysis was done in 1/99, and the second one is planned in 1/02. Interim data will be presented. The trial in MBC randomizes chemosensitive, ER-negative patients to 1× versus 2× STAMPV. The first interim analysis was done in 12/00. Data will be presented.

Abstract

Background: Needle stick injuries are associated with a risk of infection. The aim of this study was to collate the reasons for the failure to carry out prophylactic measures from the perspective of those affected., Methods: An anonymous internet questionnaire was designed to record the experiences of health care workers at the University Hospital Rostock with secondary infection prophylaxis after needle stick injuries., Results: During the investigation period 106 questionnaires were returned. There were deficiencies in the acceptance of prophylactic measures due to job-associated lack of time and social pressure., Conclusion: The study suggests reorganization of work-flows and additional educational measures about the necessity of prophylactic procedures after needle stick injuries.

Abstract

The proto-oncogene c-kit is known to be expressed in poorly differentiated breast cancer. In this study, we retrospectively evaluated the prognostic and predictive impact of c-kit in a high risk subgroup of breast cancer patients (>9 axillary node metastases) who received high-dose (HDCT) or dose-dense (DDCT) conventional chemotherapy and correlated these findings with the expression of the basal-type markers CK5 and CK 17, estrogen (ER) and progesterone (PR) receptor, Her-2/neu and MIB 1. C-kit, CK5, CK17, ER, PR, Her-2/neu and MIBI expression was evaluated immunohistochemically using tissue microarrays containing breast cancer samples from 236 patients who were randomized to the WSG AM01 trial (median follow-up of 60 months). There was a significant overall survival (OS) benefit for patients receiving HDCT compared to DDCT (p = 0.027). C-KIT expression was found in 12 % of all breast cancers and correlated with a poorer OS in multivariate analysis (p = 0.051). Furthermore, c-kit correlated with high grade (p = 0.019), CK5- and CK17-positivity (p <0.0001 and p = 0.001, respectively) and ER- and PR-negativity (p = 0.04 and p = 0.008, respectively). In contrast to CK5 and CK17, patients with c-kit positive breast cancers revealed no benefit from high-dose chemotherapy. These findings underline that c-kit expression represents an independent negative prognostic marker in high-risk breast cancer. Correlation with CK5 +/CK17+ and ER-/PR-suggests that c-kit positive carcinomas are at least partly of basal-type.
