A Community Screening for Abdominal Aortic Aneurysm - Abdominal aortic aneurysm is an asymptomatic degenerative disease of the aortic wall especially affecting old males, leading to progressive expansion and rupture with profuse bleeding and death. Early diagnosis by Echography is simple and allows elective surgical or endovascular correction with contained mortality. We screened the population of a Social Community over 70 years of age, with results quite similar to those referred in literature for studies on larger numbers. A huge previously unknown aneurysm was corrected by surgery and several minor aortic dilations are on follow-up. Screening per Aneurisma dell’Aorta Addominale presso un’Associazione di Società Cooperative-L’aneurisma dell’aorta addominale è una patologia degenerativa dilatante e progressiva, che colpisce l’età avanzata ed in prevalenza la popolazione maschile. L’evoluzione dell’aneurisma, che spesso è ignorato poiché asintomatico, è verso la rottura, il più delle volte mortale. La diagnosi, attraverso una semplice ecografia, consente la sua correzione in chirurgia “aperta” tradizionale od endovascolare in elezione, con una mortalità assai ridotta. Abbiamo eseguito uno screening per aneurismi su una comunità ristretta di associati ad una Società cooperativa, con risultati sovrapponibili a quelli riferiti in letteratura per studi più vasti. I nostri dati hanno confermato la validità di uno screening di prevenzione. Un voluminoso aneurisma a rischio di rottura, sconosciuto al paziente, ha potuto essere evidenziato e corretto chirurgicamente con successo. Altre dilatazioni aortiche di minor entità sono state poste in periodico controllo.

Classical anti-ischemic drugs are the first-line form of treatment in patients with microvascular angina (MVA), but they often fail to achieve a satisfactory control of angina symptoms. It is unknown whether there is any relation between improvement of angina status and changes in microvascular function induced by classical anti-ischemic drugs in MVA patients.To assess whether, in MVA patients, the effects of classical anti-ischemic drugs on symptoms and quality of life (QoL) are related to changes in coronary microvascular function.We studied 51 patients (59±10 years; 15 men) with MVA. Coronary blood flow (CBF) response to adenosine (ADO) and to cold pressor test (CPT), Seattle Angina Questionnaire (SAQ) and EuroQoL scale were assessed at baseline, in pharmacological washout, and after 12 months under anti-ischemic therapy. Patients were divided into 2 groups: (1) Group 1 included patients with no improvement of QoL (EuroQoL score change10 points); (2) Group 2 included patients with QoL improvement (increase in EuroQoL score ≥ 10 points).At baseline, the 2 groups were similar in age, gender, cardiovascular risk factors, CBF response to ADO and to CPT, SAQ and EuroQoL scores. At follow-up the 2 groups differed only for beta blockers use (27% vs. 88% in group 1 and 2, respectively; p0.001). A significant improvement in SAQ score was observed only in group 2. CBF response to both ADO and CPT showed a similar improvement in the 2 groups. No relation was found between changes in coronary microvascular function and in angina status.In MVA patients beta-blockers are more effective than other anti-ischemic drugs in improving angina symptoms. The improvement of angina status does not seem to be mediated by changes in coronary microvascular function.

The aim of our study was to evaluate the accuracy of MDCT in the study of gastrointestinal stromal tumour (GIST), and to compare CT results with histological findings.MDCT exams of 18 patients with 19 lesions, with histological proven GISTs diagnosis, were retrospectively evaluated in order to assess the localization, the size, the contours, as well as the CT pattern and enhancement of the lesions. All the tumors were recorded with Fletcher and Miettinen classification, which evaluate the risk assessment in the gastrointestinal stromal tumours. CT findings were correlated with histological results after surgery.MDCT properly identified the localization and the size in all cases. CT features essentially agreed with histological features.The immunopositivity to c-KIT (CD117) is the key to making a diagnosis of GIST. CT is the modality of choice to study these neoplasms, evaluating the tumour's site and size with high accuracy. In our experience MDCT proved to be a valid diagnostic tool, highly correlated with histological features.

El objetivo de este trabajo es mostrar la aplicabilidad operacional de un modelo propuesto recientemente (Sánchez et al., 2008) junto con imágenes de satélite para hacer un seguimiento de los flujos de energía de la superficie a escala regional. En concreto, nos centraremos en la obtención de la evapotranspiración diaria por la especial significación que tiene este flujo en la generación de precipitación o en la gestión de las fuentes de agua para usos agrícolas. Presentamos una metodología detallada para aplicar el Método Simplificado de Balance Energético de Dos Fuentes a las imágenes Landsat. Los distintos rasgos de las tierras de cultivo de superficie vienen caracterizados por los mapas del proyecto CORINE Land Cover y las variables meteorológicas necesarias se obtienen interpolando los datos de una red de estaciones agrometeorológicas distribuidas por la región de interés. Finalmente, mostramos la aplicación de la metodología a tres imágenes Landsat distintas que cubren toda la región de Basilicata (en el sur de Italia). Se realizan mapas de los distintos flujos incluyendo la evapotranspiración diaria. Se comparan los resultados con algunas medidas del suelo y se hace un análisis tomando la clasificación de uso del suelo como base. Se obtiene una precisión cercana a 30 W m-2. Juan.M.Sanchez@uv.es
