To compare scapular kinematics and muscle activity among various scapular muscle exercises. A cross-sectional study. A university research laboratory. Eighteen healthy men participated in this study. Three-dimensional scapular kinematics was measured with an electromagnetic motion capture system. Activities of the upper trapezius (UT), lower trapezius (LT), and serratus anterior (SA) were measured by using surface electromyography (EMG). In addition, the ratio of LT to the UT (LT/UT) and SA to the UT (SA/UT) was calculated. A repeated one-way analysis of variance and Shaffer's post-hoc analysis were used to detect the differences in each outcome during five exercises. The scapula was rotated upwardly during all exercises except push-up plus. The LT/UT ratio during side-lying external rotation and side-lying flexion were significantly higher than that for scapular plane elevation (P <.05), although the highest activity of the lower trapezius was produced during scapular plane elevation. The activities of the serratus anterior and SA/UT ratio during horizontal adducted elevation and push-up plus were significantly higher than that during scapular plane elevation (P <.05). Introduction: In symptomatic patients with rotator cuff tear, MRI and radiographic studies have ascribed the pain symptom to insufficient humeral head depression during arm elevations. The arm adductors such as the teres major and pectoralis major may contribute to depression of the humerus head during arm elevations. Researchers have demonstrated that neuromuscular electrical stimulation (NMES) of the serratus anterior and lower trapezius can control scapular motions and improve acromiohumeral distance. It is unknown, however, if adductor neuromuscular training could help patients with rotator cuff tear. Materials and methods: A cross-sectional study of NMES of the teres major and pectoralis major was conducted on 30 symptomatic subjects with rotator cuff tear. We measured the acromiohumeral distance by ultrasonography and scapular kinematics during arm elevation with a three-dimensional motion tracking system. Results: The acromiohumeral distance significantly increased during NMES of the teres major (0.73 mm, p < 0.001). However, the distance significantly decreased with NMES of the pectoralis major (0.78 mm, p < 0.001). Additionally, scapular upward rotation was greater during NMES of the teres major than during NMES of the pectoralis major (3.4°, p < 0.001). Scapular external rotation decreased significantly more during NMES of the pectoralis major than during NMES of the teres major (1.6°, p = 0.003). Conclusions: NMES of the teres major can increase acromiohumeral distance and scapular upward rotation during arm elevation. A skin marker-based motion capture model providing measures of scapular rotations was recently developed. The aim of this study was to investigate the concurrent validity and the interrater reliability of the model. Shoulder range of motion (RoM) and activities of daily living (ADL) were tested in healthy volunteers with reflective markers on the scapula and thorax. To investigate the validity, the model was compared to simultaneous data collection from markers on a scapular intracortical pin. The interrater reliability was tested by comparing the skin marker-based protocol performed by two investigators. The mean root mean square error (RMSE) and the intraclass correlation coefficient (ICC(2,1)) were calculated to determine the validity and the interrater reliability, respectively. Eight subjects were included in the validity test: female/male = 2/6, mean (SD) age 35.0 (3.0) and BMI 23.4 (3.3). The mean RMSE of all scapular rotations ranged 2.3–6.7° during shoulder RoM and 2.4–7.6° during ADL. The highest errors were seen during sagittal and scapular plane flexions, hair combing and eating. The reliability test included twenty subjects: female/male = 8/12, mean (SD) age 31.4 (4.9) and BMI 22.9 (1.7). The ICC(2,1) for measuring protraction ranged 0.07–0.60 during RoM and 0.27–0.69 for ADL, for upward rotation the corresponding ICC(2,1) ranged 0.01–0.64 and 0.38–0.60, and anterior tilt 0.25–0.83 and 0.25–0.62. The validity and interrater reliability of the model are task dependent, and interpretation should be made with caution. The model provides quantitative measurements for objective assessment of scapular movements and can potentially supplement the clinical examination in certain motion tasks.

The aim of this systematic literature review is to report and describe the existing methods to estimate 3D scapular movements through wearable systems integrating M-IMUs. A comprehensive search of PubMed, IEEE Xplore, and Web of Science was performed, and results were included up to May 2023. A total of 14 articles was included. The results showed high heterogeneity among studies regarding calibration procedures, tasks executed, and the population. Two different techniques were described, i.e., with the x-axis aligned with the cranial edge of the scapular spine or positioned on the flat surface of the acromion with the x-axis perpendicular to the scapular spine. Sensor placement affected the scapular motion and, also, the kinematic output. Further studies should be conducted to establish a universal protocol that reduces the variability among studies. Establishing a protocol that can be carried out without difficulty or pain by patients with shoulder musculoskeletal disorders could be of great clinical relevance for patients and clinicians to monitor 3D scapular kinematics in unstructured settings or during common clinical practice. Understanding scapular motion during everyday tasks is essential for adequate return-to-work and intervention programming, yet most scapular assessments involve restricted motion analysis within a laboratory setting. Inertial measurement units (IMUs) have been used to track scapular motion, but their validity compared to "gold standard" optical motion capture is not well defined. Further, it is unclear how different IMU sensor placements could affect scapular kinematic outcomes during a functional task protocol. To assess the reliability of scapular motion measurements with the use of the "gold standard" optical motion capture and IMUs, and to compare scapular IMU placement to assess which location (acromion or spine) was best for reliability of scapular motion, participants completed two testing sessions. During each lab visit, participant torso, humeri, and scapulae motion was tracked during 3 trials of 8 dynamic tasks and two elevation movements. Scapular angles were extracted during each task. To assess intra-session reliability, intra-class correlation coefficients (ICCs), and root mean square errors (RMSEs) were calculated. Results showed ICCs and RMSEs were acceptable. Although there appeared offsets between the two motion capture system scapular kinematics outcomes based on the plotted waveforms, the movement patterns appeared consistent between both motion capture methods. Data also showed that acromion IMU placement produced slightly more reliable outcomes than placement on the spine. Two placements of scapular tracking IMUs were tested with identical procedures. Shoulder pain is one the most common musculoskeletal complaints. The most common pathological causes of shoulder pain in the general population are subacromial impingement syndrome and adhesive capsulitis, commonly referred to as "frozen shoulder". The purpose of this study was to evaluate the role of the scapulo-thoracic complex, particularly in scapular kinematic functions, in rehabilitative interventions for shoulder pain in patients suffering from these two common conditions. This systematic review was performed using the scientific search engines PubMed, PEDro and Cochrane Library, considering only randomized controlled clinical trials. Selected articles were evaluated according to the level of evidence and methodological quality. Thirteen randomized clinical trials were selected. Interventions have been divided into three macro-categories: (1) manual therapy in patients with subacromial impingement, (2) therapeutic exercise programs including interventions on the scapulothoracic complex in patients with subacromial impingement syndrome, and (3) therapeutic exercise programs including interventions on the scapulothoracic complex in patients with frozen shoulder. Following this, a qualitative analysis was performed according to outcomes such as pain, shoulder function, and scapular kinematics. Physiotherapy exercise programs that included scapular motor control training and scapular mobilizations, in particular, those of the scapulo-thoracic complex in scapular kinematic function, represent valid alternatives in the management of patients with subacromial impingement syndrome. Background: It is often assumed that body posture, standing vs. supine, changes shoulder muscle activation and range of motion, but these altered shoulder mechanics have not been objectively assessed. We expected the supine posture might facilitate scapular rotation and change subacromial pressure. The purpose of this study is to evaluate the influence of body posture on shoulder kinematics during arm elevation. Methods: Ten males and eight females with a mean age of 33 years participated in this study. Shoulder kinematics were assessed during scapular plane elevation in the standing and supine postures by using single-plane fluoroscopic images. Kinematics were measured using 3-dimensional to 2-dimensional model-image registration techniques: matching the 3-dimensional bone model derived from computed tomography onto each fluoroscopic image. Purpose: The aim of this study was to compare the effects of scapular stabilization exercise with and without cognitive functional therapy (CFT) on disability and scapular kinematics in people suffering from chronic neck pain. Methods: A total of 72 patients with chronic non-specific neck pain were randomized into scapular stabilization exercise alone, n = 24, combined (scapular stabilization exercise + CFT), n = 24, and a control group, n = 24. Scapular kinematic and disability were measured at baseline and after the intervention. Results: Statistically significant differences in neck pain and disability scale (NPAD) were found when the multimodal physiotherapy group including a cognitive functional approach was compared with stabilization exercises group at 6 weeks (effect size (95%CI) = –1.63 (–2.55, –.71); P = 0.019)). Regarding the neck disability index (NDI), a significant between-group difference was observed at six-week (effect size (95%CI) = –2.69 (–3.80, –1.58); P = 0.007), with the superiority of effect in multimodal physiotherapy group. A significant between-group difference was observed in the scapular upward rotation and scapular osterior tilt at 30°, 60°, 90° and 120° of shoulder adduction. Conclusions: A group-based multimodal rehabilitation program including scapular stabilization exercise plus cognitive functional therapy was superior to group-based stabilization exercises alone for decreasing disability and, improving scapular kinematic in patients with chronic neck pain. Background: the pathogenesis of frozen shoulder (FS) is thought to be one of inflammation and fibrosis possibly influenced by hyperglycemia. Biomechanical changes of the shoulder joint in terms of muscle strength, scapular kinematics and proprioception might occur in FS., Objectives: to compare muscle strength, scapular kinematics, proprioception, and blood glucose levels within patients with FS and to asymptomatic individuals., Design: cross-sectional study., Method: Thirty-five patients with FS and 35 asymptomatic age and gender-matched individuals underwent physical assessment to determine muscle strength (abduction, external and internal rotation), scapular kinematics (both visually and with a plurimeter), proprioception (joint position sense), and blood glucose level., Results: Patients with FS showed a decrease in muscle strength in their affected shoulder compared to both the unaffected shoulder and asymptomatic individuals. Significant differences were found between the affected and unaffected shoulder in the FS group and between groups (FS versus controls) in scapular upward rotation (plurimeter) at 30° and 60° abduction. Postural asymmetry due to the alterations in scapular posture is typically considered to be associated with injury. Pectoralis minor (PM) has played a crucial role in scapular posture, but limited knowledge exists that shows the possible effect of PM length on static scapular positioning in adolescents. The aim of this study was to investigate the association between static scapular positioning and two different PM length measurements in adolescents. Pearson rank test demonstrated that direct measurements of the PM length significantly and strongly inversely correlated to scapular anterior tilting (p =.01; r = −.592)and significantly and moderately inversely correlated to scapular internal rotation (p =.013; r = −.465) and scapular downward rotation (p =.028; r = −.416). However, indirect measurement of the PM length was not related to scapular posture. No significant correlation was found between direct and indirect PM length measurements. To characterize the scapular pitching biomechanics in symptomatic GIRD pitchers (SG) compared to asymptomatic GIRD (ASG) and healthy pitchers. The scapular kinematics and associated muscle activities during pitching were recorded in 33 high school pitchers. Compared to healthy, GIRD pitchers had less scapular posterior tilt in each pitching event (average difference, AD=14.4°, p<0.01) and ASG demonstrated less scapular upward rotation at ball release (AD=12.8°, p<0.01) and greater muscle activity in the triceps brachii in the early-cocking phase (AD=9.9%, p=0.015) and in the serratus anterior in the late-cocking phase (AD=30.8%, p<0.01). Additionally, SG had less muscular activity on triceps brachii in the acceleration phase and serratus anterior in the cocking phase (AD=37.8%, p=0.016; AD=15.5%, p<0.01, respectively) compared to ASG. GIRD pitchers exhibited less scapular posterior tilt during pitching, which may cause impingement. Since tightness of the anterior shoulder is a common cause of inadequacy of posterior tilt during arm elevation, stretching exercise of the anterior shoulder is recommended. Background: Frozen shoulder (FS) is characterized by restricted active and passive shoulder mobility and pain., Purpose: Compare the effect of muscle-biased manual therapy (MM) and regular physical therapy (RPT) in patients with FS., Study Design: Pretest-post-test control group study design., Methods: We recruited 34 patients with FS and compared the effect of 12-session MM and RPT. The outcome measures were scapular kinematics and muscle activation, scapular alignment, shoulder range of motion, and pain intensity. Two-way analysis of variance was used to examine the intervention effect with α = 0.05., Results: Both programs resulted in similar improvements in pain and shoulder function. Compared to the RPT, MM resulted in increased posterior tilt (MM: 7.04°-16.09°, RPT: -2.50° to -4.37°; p = 0.002; ES = 0.261) and lower trapezius activation (MM: 260.61%-470.90%, RPT: 322.64%-313.33%; p = 0.033; ES = 0.134) during scaption, and increased posterior tilt (MM: 0.70°-15.16°, RPT: -9.66° to -6.44°; p = 0.007; ES = 0.205) during the hand-to-neck task. The MM group also showed increased GH backward elevation (MM: 37.18°-42.79°, RPT: 43.64°-40.83°; p = 0.004, ES = 0.237) and scapular downward rotation (MM: -2.48° to 6.80°, RPT: 1.93°-1.44°; p < 0.001; ES = 0.404) during the thumb-to-waist task, enhanced shoulder abduction (MM: 84.6°-102.3°, RPT: 85.1°-92.9°; p = 0.02; ES = 0.153), and improved scapular alignment (MM: 10.4-9.65 cm, RPT: 9.41-9.56 cm; p = 0.02; ES = 0.114)., Conclusions: MM was superior to the RPT regarding scapular neuromuscular performance. Videogrammetry is commonly used to record upper limb motions. However, it cannot track the glenohumeral joint center (GH). GH is required to reconstruct upper limb motions. Therefore, it is often estimated by separately measuring scapular motions using scapular kinematics measurements devices (SKMD). Applications of SKMD are neither straightforward nor always noninvasive. Therefore, this work investigates the feasibility of an alternative method to estimate GH from videogrammetry using a CT/MRI image of subject's glenohumeral joint and without requiring SKMD. In order to evaluate the method's accuracy, its GH estimations were compared to reference GH trajectories. The method was also applied to estimate scapular configurations and reconstruct an abduction motion measured by videogrammetry. Background and Objectives: One of the abnormal forms of scapular malalignment is the scapular downward rotation defect, which includes depression, protraction, and anterior scapula tilt, which potentially reduces the upper rotation and posterior tilting of the scapula. Therefore, the aim of this study was to determine the effect of six weeks of corrective exercises on the kinematics of the scapula in males with scapular downward rotation defect. Materials and Methods: This randomized clinical trial was performed in the Faculty of Physical Education of Kharazmi University in 2019, Iran. Twenty-four eligible patients were selected and randomly divided into two equal groups of 12, corrective exercises and control group. The control group did not receive any specific treatment during the study. The scapular upward rotation and scapular posterior tilt were measured in the same way before and after the corrective exercises in the intervention group by Noraaxon's MyoMotion 3D device. Data were analyzed using twoway repeated measures ANOVA and paired t-test. Results: A significant increase was observed in scapular upward rotation in the angles of 30º (p=0.038), 60º (p<0.001), 90º (p<0.001) and 120º (p<0.001), and in scapular posterior tilt in the angles of 30º (p<0.001), 60º (p< 0.001), 90º (p<0.001) and 120º (p<0.001) after applying the exercise intervention in the intervention group compared to the control group. Conclusion: The results showed that the intervention of scapular correction exercises improves the angles of scapular upward rotation and scapular posterior tilt in people with scapular downward rotation defect. Rounded shoulder posture (RSP) is a common postural condition which can alter scapular position. Although, there is no consensus on the relationship between posture and musculoskeletal dysfunction, some evidence suggests a significant relationship between RSP and shoulder dysfunction. Therefore, treatment of this postural condition is important. Various treatment methods are used to correct RSP. However, the effectiveness of scapular mobilization, (SM) as a method which can alter scapular kinematics, has not been investigated. To evaluate the effects of SM on scapular resting position in individuals with RSP, and to compare this technique to pectoralis minor self-stretching (PMS), and combined SM + PMS. 52 healthy students (18 men and 34 women; mean age 23.67 ± 6.73 years) with RSP were randomly assigned to four groups (SM, PMS, combined SM + PMS, control). The mobilization group received SM, the stretching group performed self-PMS, and the combined group received SM + PMS. The control group received no treatment. Kinematics data to measure scapular protraction (cm), anterior tilt (°), internal rotation (°), and downward rotation (°) were captured with a motion analysis system before and after 5 sessions of group intervention. All variables decreased significantly post-intervention compared to baseline values (P < 0.05). Internal rotation and downward rotation decreased significantly in the intervention groups compared to the control group (P < 0.05). No significant differences were observed between the intervention groups. SM appears to be an effective technique to change scapular resting position in individuals with RSP. This is a systematic review. Scapular taping is widely used in the management of scapular dysfunction. However, its effects on the scapular kinematics and the electromyography (EMG) activity of the periscapular muscles are not clear. The purpose of the study was to systematically review the current literature to examine whether scapular corrective taping alters the EMG activity of the periscapular muscles and the 3-dimensional scapular kinematics. MEDLINE and Web of Science databases were searched using specific mesh terms up to April 2018. A hand search was also conducted on the reference list of the included articles. A total of 157 studies were identified, and they were further analyzed for the eligibility to the systematic review. Studies that investigated the effects of scapular corrective taping on the EMG activity of the periscapular muscles and on the 3-dimensional scapular kinematics on patients with shoulder problems or asymptomatic subjects were eligible for the systematic review. The Cochrane Effective Practice and Organization of Care criteria were modified and used for the risk-of-bias assessment. Eleven articles met the inclusion criteria and were included in the systematic review. Five studies investigated the effects of corrective taping on the scapular kinematics and 8 studies reported the effects of corrective taping on the EMG activity of the periscapular muscles. There was an agreement among the studies that scapular upward rotation is increased with the corrective taping, while there are inconsistent results concerning the scapular external rotation and posterior tilt. In addition, studies mostly reported that corrective taping decreases the activity of the upper trapezius, while it has conflicting effects on the activity patterns of other periscapular muscles. Scapular corrective taping was found to increase the scapular upward rotation; however, its effects on scapular external rotation and posterior tilt are controversial. It was also found that corrective taping might decrease the EMG activity of the upper trapezius, while it had no effects on the activity of lower trapezius, and its effects on other periscapular muscles were controversial. The results of the studies indicated that scapular corrective taping might alter the 3-dimensional scapular kinematics, while there are controversies about the effects of corrective taping on the EMG activity of the periscapular muscles. Background: During nonoperative or postoperative rehabilitation after sports injuries, exercise selection is often based on minimal load on the injured/repaired glenohumeral structures, while optimally activating scapulothoracic muscles. Previous research explored scapular muscle activity during rehabilitation exercises using surface electromyography (EMG). However, limited information exists about the deeper lying muscle activity, measured with fine-wire electrodes, even more in combination with 3-dimensional scapular kinematics. Purpose: To report scapular kinematics synchronously with surface and fine-wire EMG during specific shoulder exercises for early rehabilitation. Study Design: Descriptive laboratory study. Methods: A total of 23 healthy male patients were recruited. Three-dimensional scapular kinematics were measured combined with EMG recording of 8 muscles during 4 commonly used shoulder exercises (inferior glide, low row, lawnmower, and robbery). Upper (UT), middle (MT), and lower (LT) trapezius and serratus anterior muscle activities were measured with bipolar surface electrodes. Intramuscular electrodes were placed in the levator scapulae (LS), rhomboid major (RM), pectoralis minor (Pm), and infraspinatus (IS) muscles. All data were normalized as a percentage of maximal voluntary isometric contraction (%MVIC). A linear mixed model with Bonferroni correction was applied for statistical analysis. Results: Scapular kinematics revealed an anterior tilt position during the inferior glide, low row, and robbery (P <.05). An upward rotation position between 20° and 30° was reached in all exercises except low row. Inferior glide (31°) and low row (42°) represented a significantly increased internal rotation position compared with lawnmower and robbery. Lawnmower and robbery showed significantly (P <.05) more MT (lawnmower, 36% MVIC; robbery, 39% MVIC) and RM (lawnmower, 59% MVIC; robbery, 66% MVIC) activation compared with inferior glide and low row. Lawnmower and robbery showed significantly (P <.05) less Pm activation (9.5%-12% MVIC). LS was significantly more active during robbery (58% MVIC) compared with inferior glide and low row (27%-36% MVIC) (P <.05). IS showed moderate activity (24%-37% MVIC) for all exercises, except low row (13% MVIC). Conclusion/Clinical Relevance: This study provides new insights about scapular positions and activation of the deeper layer muscles during 4 commonly used shoulder rehabilitation exercises. The lawnmower showed a favorable position of the scapula with less Pm activity in contrast to the low row. Context: Subacromial impingement syndrome (SIS) is associated with scapular dyskinesis, or imbalanced scapular muscle activity. Evidence has shown that feedback can improve scapular control in patients with SIS. However, it is unknown whether real-time video feedback or electromyography (EMG) biofeedback is optimal for improving scapular kinematics and muscle activity during a functional task. Objective: To compare the effects of video and EMG feedback sessions on absolute muscle activity (upper trapezius [UT], lower trapezius [LT], serratus anterior), muscle balance ratios (UT/LT, UT/serratus anterior), and scapular kinematics (anterior-posterior tilt, external-internal rotation, upward rotation) in SIS participants during arm elevation and lowering. Design: Randomized controlled clinical trial. Setting: Research laboratory. Patients or Other Participants: Overhead athletes who were diagnosed with SIS and who also exhibited scapular dyskinesis (N = 41). Main Outcome Measure(s): Three-dimensional kinematics and EMG were recorded before and after feedback training. Results: Lower trapezius muscle activity increased (4.2%- 18%, P, .011) and UT/LT decreased (0.56-1.17, P, .013) in the EMG biofeedback training group as compared with those in the video feedback training group. Objective: To determine the effectiveness of a scapular exercise programme in addition to a physical therapy treatment in patients with distal radius fracture. Design: A single-blinded randomized controlled trial was conducted. Setting: Clinical Hospital San Borja Arriaran, Santiago, Chile. Participants: A total of 102 patients above 60 years of age with extra-articular distal radius fracture were randomly divided into two groups. Interventions: The control group (n = 51) received a six-week physical therapy treatment; the intervention group (n = 51) received the same treatment plus a scapular exercise programme. Outcome measures: The two groups were assessed at baseline and after the six-week treatment. The arm function was assessed with the disabilities of the arm, shoulder and hand (DASH) questionnaire; secondary outcomes were measured by the patient-rated wrist evaluation (PRWE) questionnaire and visual analogue scale (VAS). Results: A total of 102 patients, 51 in the control group (40 women; mean age of 65.3 ± 4.8 years) and 51 in the intervention group (42 women; mean age of 67.2 ± 5.4 years), were analysed. At the end of the treatment, the difference between groups for the DASH was 16.7 points (P < 0.001), 1.5 points (P = 0.541) for the PRWE, 0.2 cm (P = 0.484) for the VAS at rest, and 1.7 cm (P < 0.001) for the VAS at movement. All differences were in favour of the intervention group. Conclusion: In the short term, adding a scapular exercise programme provides a significant clinical benefit in arm function and pain relief with movement in patients above 60 years of age with extra-articular distal radius fracture treated conservatively. The aim of this study is to investigate the effects of scapular taping on scapular kinematics by three-dimensional electromagnetic system during shoulder elevation in facioscapulohumeral muscular dystrophy patients. A total of 11 patients with facioscapulohumeral muscular dystrophy were included in the study. Scapular anterior-posterior tilt, upward-downward rotation, and internal-external rotations were evaluated using the three-dimensional electromagnetic system during the elevation of the upper limbs in the scapular plane before and after kinesio taping. For maximum humerothoracic elevation, there were no differences between the patients before and after taping on both dominant (p = 0.72) and non-dominant sides (p = 0.64). For scapular internal rotation, upward rotation, and posterior tilt, there were no differences between patients before and after taping during humerothoracic elevation on both dominant and non-dominant sides (p > 0.05). These results showed us that the excessive and abnormal movements of the scapula observed during the humeral elevation in facioscapulohumeral muscular dystrophy patients cannot be supported with flexible methods like kinesio taping. Therefore, we recommend to evaluate the scapula position by applying flexible and rigid taping to the patients who can reach over 90o in humerothoracic elevation in future studies. Abstract Background Function loss caused by rotator cuff tears alters the scapular orientation, however, few prior studies have reported on scapular movements after rotator cuff repair. The purpose was to determine the scapular orientations before and after rotator cuff repair. Methods We recruited 14 healthy controls, 10 small and six massive rotator cuff tear in patients. The scapular upward rotation during arm elevation was analyzed using fluoroscopic imaging. Findings Before surgery, both rotator cuff groups demonstrated greater scapular upward rotation compared to healthy controls. Two months postoperation, the analyses showed significant differences between the patients with small rotator cuff tears and healthy controls at arm elevations of 90°, and between patients with both rotator cuff tear groups and healthy controls at arm elevations of 120°. At five months post-operation, significant differences still existed between the healthy controls and both rotator cuff groups. In regard to the temporal effects in the patients with small rotator cuff tears, the scapular upward rotation decreased significantly over time (2–5 months postoperation) at arm elevations of 120°. We did not identify a main effect owing to time in the patients with massive rotator cuff tears. Interpretation In patients with small rotator cuff tears, scapular upward rotation was reduced over the period of 2–5 months postoperation, however, the patients with massive rotator cuff tears showed greater scapular upward rotation throughout the experimental period. The results suggested that the execution of the rehabilitation program should consider that the tear size could affect scapular motion. Purpose: Acromioclavicular-coracoclavicular ligament injury occurs frequently, and the clavicle hook plate technique is an easy-to-use treatment method. However, complications such as subacromial impingement syndrome, synovitis, erosion, osteolysis, post-operative pain, and post-operative limitations in range of motion have been reported. We aimed to evaluate the use of the clavicle hook plate in the shoulder joints and to compare in vivo three-dimensional (3D) scapular kinematics and scapulohumeral rhythm between the shoulders with a clavicle hook plate and contralateral normal shoulder joints.Methods: Ten male patients (aged 40.5 ± 14.4 years) who underwent clavicle hook plate fixation for an acromioclavicular-coracoclavicular ligament injury were selected. Computed tomography and fluoroscopy were conducted on both the shoulder joints, and 3D models were created. Using a 3D-2D model-image registration technique, we determined the 3D coordinates of the scapula, and we measured the scapular kinematics and scapulohumeral rhythm.Results: The values for upward rotation, posterior tilt, and external rotation in the two groups increased in proportion with humeral elevation, showing significant differences between the two groups (p < 0.05). Overall, the value in the clavicle hook plate group (group H) was smaller than that in the control group (group C) by 23.5% (6.7°) of upward rotation and 64.8% (18.9°) of posterior tilt. However, the external rotation in group H was greater than that in group C by 32.3% (2.3°). In overall value, there was a significant difference not in upward rotation and external rotation, but in posterior tilt. During humeral elevation, the overall changes in scapulohumeral rhythm were 4.65 ± 2.45 in group H and 3.8 ± 0.8 in group C, and statistical differences were not detected between the two groups.Conclusions: Clavicle hook plate fixation changes the scapular kinematics and scapulohumeral rhythm; thus, when clavicle hook plate fixation is complete, the implant should be promptly removed. Background: The scapula is a critical link utilized in the kinetic chain to achieve efficient overhead movement and transfer energy from the lower extremity to the upper extremity. Additionally, daily activities such as sitting at a computer or driving in a car may negatively influence an individual's ability to maintain proper body posture and therefore compromise those movements. To reduce these negative influences, posture garments have been designed to cue the individual in maintaining and improving posture and alignment, specifically targeting scapular positioning. Purpose: The purpose of this study was to compare scapular positioning between an IntelliSkin™ posture-cueing compression garment and a generic performance garment on scapular kinematics during static standing. Study Design: Case control. Methods: Forty active females (1.68 ± 0.07 m; 67.29 ± 11.25 kg) stood in a natural standing position while wearing two different garments: IntelliSkin™ posture-cueing compression garment and a generic performance garment. Kinematic data were collected at 100 Hz using an electromagnetic tracking system (trakSTAR™, Ascension Technologies, Inc., Burlington, VT, USA) synced with The MotionMonitor® (Innovative Sports Training, Chicago, IL., USA). Results: Repeated measures ANOVAs revealed a statistically significant Shirt by Side interaction for scapular protraction/retraction (F(1,39) = 52.91, p < 0.05) and main-effect of Shirt for scapula anterior/posterior tilt (F(1,39) = 96.45, p < 0.05). Individuals showed increased retraction and posterior tilt while wearing the IntelliSkin™ posture-cueing compression garment. Conclusion: The results of the current study indicate that the IntelliSkin™ posture-cueing compression garment improved scapular positioning during static standing posture. Background: Scapular substitution is an alteration of scapulohumeral kinematics that may occur when patients have shoulder pain or dysfunction. These abnormal scapular kinematic patterns have been recognized in patients with rotator cuff tears. It remains unknown if 1) normal scapular kinematics can be restored with rehabilitation after rotator cuff repair surgery and 2) abnormal scapular kinematics are associated with inferior patient-determined outcome scores, range of motion, or strength. Purpose: The purpose of this study was to determine 1) if scapular substitution can be decreased or improved with rehabilitation after rotator cuff repair surgery and 2) if the presence or amount of scapular substitution was correlated with patient-determined outcome scores, range of motion, or strength after rotator cuff repair surgery. Study Design: Retrospective review of prospectively collected data (LOE IV) Methods: Forty-eight patients who underwent post-operative rehabilitation after an arthroscopic rotator cuff repair were reviewed for this study. The outcomes measures of interest included: patient-determined outcome scores (WORC, Simple Shoulder Test, the ASES Score, the Shoulder Activity Score, and the SANE rating), identification and quantification of scapular substitution, active range of motion, and strength. Outcomes were prospectively collected up to 12 months after surgery and assessed retrospectively. Results: As patients progress through their first year of rehabilitation from a rotator cuff repair, the amount of scapular substitution decreases but remains statistically significantly greater than the contralateral, asymptomatic side. At all post-operative time points, patients with scapular substitution, (determined subjectively by a physical therapist), had 1) inferior WORC, ASES, SANE, and SST scores, 2) inferior flexion, abduction, and external rotation range of motion, and 3) inferior scaption strength compared to those patients without subjective scapular substitution. Conclusions: Rehabilitation decreases but does not normalize the amount of scapular substitution up to one year after rotator cuff repair. Background Altered scapular kinematics have been associated with shoulder pain and functional limitations. To understand kinematics in persons with spinal cord injury during manual handrim wheelchair propulsion, a description of normal scapular behaviour in able-bodied persons during this specific task is a prerequisite for accurate interpretation. The primary aim of this study is to describe scapular kinematics in able-bodied persons during manual wheelchair propulsion. Methods Sixteen able-bodied, novice wheelchair users without shoulder complaints participated in the study. Kinematic and kinetic data were collected during a standardized pose in the anatomic posture, frontal-plane arm elevation and low-intensity steady-state handrim wheelchair propulsion and upper-body Euler angles were calculated. Findings Scapulothoracic joint orientations in a static position were 36.7° (SD 5.4°), 6.4° (SD 9.1°) and 9.1° (SD 5.7°) for respectively protraction, lateral rotation and anterior tilt. At 80° of arm elevation in the frontal plane, the respective values of 33.4° (SD 8.0°), 23.9° (SD 5.4°) and 4.1° (SD 11.3°) were found. During the push phase of manual wheelchair propulsion, the mean scapular rotations were respectively 32.7° (SD 7.1°), 7.1° (SD 9.2°) and 9.8° (SD 8.3°). Interpretation The orientation of the scapula in a static pose, during arm elevation and in manual wheelchair propulsion in able-bodied participants showed similar patterns to a previous study in persons with para- and tetraplegia. The purpose of this study is to evaluate scapular movements by the three-dimensional electromagnetic system during shoulder elevation in FSHMD patients, and to compare the results with healthy individuals. 10 patients with FSHMD and 10 healthy individuals were included in the study. Scapular anterior-posterior tilt, upward-downward rotation and internal-external rotation at 30°, 60° and 90° were evaluated using the three-dimensional electromagnetic system during the elevation of the upper limbs in the scapular plane. Humerothoracic elevation levels on the dominant and non-dominant sides were found to be lower in the patients than healthy individuals (p < .001). Both scapula were rotated downwards at 30° (dominant/non-dominant p < .001) and 60° (dominant p = .009, non-dominant p = .04) of humerothoracic elevation, the scapula was rotated internally at 30° of humerothoracic elevation on the non-dominant side (p = .03), and the scapula was tilted posteriorly at 90° of humerothoracic elevation on the non-dominant side (p = .009) in patients. These existing abnormal patterns of the scapula in the patients increase the risk of impairment, pain, impingement and instability especially in the activities that require arm elevation. It is thought that physiotherapy approaches should first be emphasized to improve scapular stabilization and strengthening exercises should then be performed for the shoulder girdle muscles. Objectives A history of shoulder pain is common in swimmers and may influence scapular position, possibly increasing the risk of shoulder pain recurring. The aim of this study was to establish and compare bilateral static measures of scapular upward rotation in swimmers (14–20 years), some with a history of shoulder pain but all currently pain free, in two different elevated positions of shoulder abduction. Design Cross-sectional, observational study. Participants Eighty-five swimmers without current shoulder pain. Methods Scapular upward rotation position was measured on both shoulders using a digital inclinometer in 90° and 140° shoulder abduction. Descriptive statistics were calculated for degrees of scapular upward rotation in both shoulder positions. Differences between shoulders (dominant, non-dominant, history and no history of pain) were explored using one-way ANOVA and paired t tests. Results A large range of values for scapular upward rotation was found at both positions of shoulder abduction but there were no significant differences between the shoulders: with and without a history of shoulder pain for the dominant and non-dominant sides. Conclusions A history of shoulder pain and arm dominance did not influence scapular upward rotation position when measured in shoulder abduction in swimmers without current shoulder pain. • Most functional impairments the patients experience improve in the short-term. • Shoulder rotation at 90° abduction continues to improve in the long-term. • Proprioception does not change in the clinical course of frozen shoulder. • Direct longitudinal correlations between functional impairments were established. Contradictory evidence exists regarding the clinical course of frozen shoulder (FS). To explore the clinical course of FS regarding disabilities, pain, range of motion (ROM), muscle strength, scapular upward rotation, and proprioception and to establish longitudinal correlations between these variables. Patients with FS were prospectively followed for 9 months at 3-month intervals. Assessment included the Disabilities of the Arm, Shoulder and Hand questionnaire; visual analogue scale for pain; an inclinometer for shoulder external rotation (ER), internal rotation (IR), flexion, and abduction ROM, and scapular upward rotation and proprioception, as well as handheld dynamometry for muscle strength in shoulder abduction, ER, and IR. Initially, 149 patients (98 females; mean (SD) age 53 (9) years) were included, with 88 completing all follow-up assessments. Most variables showed early improvement in the clinical course of FS, particularly ER and IR at 90° abduction, which continued to improve from 6 to 9 months of follow-up. Associations were observed between disabilities and pain (r = 0.61), disabilities/pain and ROM (r =-0.62 to -0.59 and r =-0.47 to -0.39, respectively), disabilities/pain and muscle strength (r =-0.24 to -0.35 and r =-0.36 to -0.17, respectively), and between disabilities/pain and scapular upward rotation below shoulder level (r = 0.23 to 0.38 and r = 0.24 to 0.30, respectively). ROM correlated with muscle strength (r = 0.14 to 0.44), while both ROM and ER muscle strength correlated with scapular upward rotation below shoulder level (r =-0.37 to -0.23 and r =-0.17 to -0.12, respectively). Muscle strength correlated with scapular upward rotation above shoulder level (r = 0.28 to 0.38) and lift-off muscle strength correlated with joint repositioning (r =-0.17 to -0.15). Upper limb motion can be challenging to measure and analyze during work or daily life tasks. Further, humeral angle calculation method substantially influences angle outcomes. Therefore, the purpose of this study was to assess the repeatability of scapular and humeral kinematics and compare thoracohumeral angle calculation during a work-related and functional task (WRAFT) protocol. Thirty healthy young adults completed the WRAFT protocol (Comb Hair, Wash Axilla, Tie Apron, Overhead Reach, Side Reach, Forward Transfer, Floor Lift, and Overhead Lift) on two separate occasions. Peak humeral angles and select scapular angles were extracted for each task. Intra-class correlation coefficients (ICCs), standard error of measurement, and minimal detectable change (MDC) were examined. Humeral angles were compared using the XZY and ZXY rotation sequences and "true" axial rotation for incidence of gimbal lock and amplitude coherence. Results showed that for scapular kinematics, elevation-based WRAFTs produced overall better ICC scores (0.23–0.90) compared to those tasks primarily driven by lateral humeral motion (0.02–0.84). MDCs ranged from 7°–78°, suggesting some tasks demonstrated good repeatability (Comb Hair, Overhead Reach, Floor Lift), while others had very high variability (Side Reach, Tie Apron). Amplitude coherence for thoracohumeral angles was best for ZXY for all tasks except the Comb Hair and Tie Apron, for which XZY is recommended. "True" axial rotation demonstrated good coherence for all but Tie Apron. Objective: Athletes have the high potential to positional and kinematics alternations in scapula. Various factors such as body mass index and sports experience can affect the scapular position and kinematics in athletes. Therefore, we discussed, in this research, the effect of body mass index and sports experience on scapular kinematics in basketball players. Materials and Methods: Thirty healthy male basketball players aged between 20 to 31 years old were simple non-randomly tested. One examiner measured the minimum distance between inferior angle of scapula and spinous process of T7 (scapular index) using digital caliper in neutral, 90 degrees of abduction, 90 degrees of scaption and 180 degrees of elevation of shoulder. The Pearson correlation coefficient was used to determine the relationship of body mass index and sports experience with scapular kinematics. Results: BMI had a significant correlation with the index of scapula in neutral position (r=0.38, p=0.04 in right side and r=0.39, p=0.03 in left side) and 180 degrees of elevation (r=0.48, p=0.007 in right side and r=0.45, p=0.01 in left side). Basketball sports experience also showed a significant correlation with scapular index only in180 degrees of elevation of shoulder. (r=0.49, p=0.006 in right side and r=0.38, p=0.03 in left side) Of course, the correlation was not seen in 90 degrees of abduction and scaption. Conclusion: The positive relationship of scapular kinematics with body mass index and sports experience can be know as a compensatory positional strategy over time in basketball players.

Athletes with rotator cuff (RC) tendinopathy demonstrate an aberrant pattern of scapular motion which might relate to deficits in the scapular muscles. This study aimed to determine whether alteration in scapular kinematics is associated with deficits in the activity onset of scapular muscles. Forty-three male volleyball players (17 asymptomatic and 26 with RC tendinopathy) joined the study. Three-dimensional scapular kinematics was quantified using an acromial marker cluster method. The activity onset of the upper (UT), middle (MT), and lower trapezius (LT), and serratus anterior (SA) during arm abduction was assessed with electromyography. Athletes with RC tendinopathy demonstrated less scapular upward rotation (6.6±2.3 vs. 8.2±1.1°, p=0.021) in the early phase of shoulder abduction from 0° to 30° when compared to asymptomatic athletes. The tendinopathy group had delayed activity onset of LT (14.1±31.4ms vs. 74.4±45.1ms, p<0.001) and SA (-44.9±26.0ms vs. 23.0±25.2ms, p<0.001) relative to UT when compared to the asymptomatic group. In asymptomatic athletes, earlier activity onset of MT and LT relative to UT was associated with more scapular upward rotation during 0-30° of abduction (r=0.665, p=0.021) and 30-60° of abduction (r=0.680, p=0.015), respectively. Electromyography (EMG) activity onset of upper trapezius (UT), middle trapezius (MT), lower trapezius (LT) and serratus anterior (SA) and the three-dimensional scapular kinematics quantified by using an acromial marker cluster method were compared with three scapular taping protocols, namely, no taping, therapeutic taping, and placebo taping.Results: The MT, LT and SA activated significantly earlier in both therapeutic taping (all p<0.005) and placebo taping conditions than no taping conditions (all p<0.002). There was a small increase in the scapular upward rotation when therapeutic taping and no taping conditions were compared (p=0.007).Conclusions: Scapular taping may enhance the neuromotor control of the scapular muscles. Whether it provides adequate support for normal scapular kinematics during arm movement in athletes with RC tendinopathy await for further studies. [ABSTRACT FROM AUTHOR]

Summary This study aimed to identify the role of the scapula in shoulder musculoskeletal disorders (SMDs) and provided a systematic review of available studies in the field of scapular three-dimensional kinematics. We systematically searched 5 international databases, including Scopus, EMBASE, PubMed, CINAHL, PEDro, and Cochrane Library from June to September 2015. Twenty studies met the inclusion criteria and were retrieved in full paper. The selected studies were critically appraised independently by two researchers. The patients with shoulder impingement syndrome (SIS) and shoulder instability had an increased protraction, lesser upward rotation (UR), and increased internal rotation (IR) during scapular plane elevation, whereas the patients with frozen shoulders had lesser protraction. Moreover, the patients with SIS had a greater scapular posterior tilt (PT) and external rotation during shoulder abduction. Increased scapular UR and PT with decreased scapular IR was seen in patients with stiffness of Latissimus Dorsi and fibromyalgia without any changes in the scapular IR. The results of this systematic review help the clinicians to have an insight about scapular kinematics as a predictive index for SMDs. [ABSTRACT FROM AUTHOR]

Objective To examine the effect of kinesiology tape (KT) on shoulder strength, proprioception, and scapular kinematics in healthy and Subacromial Impingement Syndrome (SAIS) subjects. Design Placebo-controlled quasi-experimental study. Setting Research laboratory. Participants A total of 30 physically active subjects participated. Ten healthy subjects with no previous history of shoulder pathology received KT on the dominant shoulder. Twenty subjects with shoulder pain for a minimum of two weeks and presenting with clinical signs of impingement were allotted to receive KT (n = 10) or placebo taping (PT, n = 10) on the involved shoulder. Main outcome measures All participants were tested pre- and post-application. Shoulder internal/external rotation (IR/ER) strength was assessed with isokinetic dynamometry (average peak torque/body weight). Shoulder IR/ER proprioception was assessed through threshold to detect passive motion (mean absolute error in degrees). Scapular position at 90° and 120° of shoulder abduction during arm raising/lowering were assessed using a 3D motion analysis system. Results No significant within group or between group differences were demonstrated for any measure. Conclusions Taping does not appear to aid/impair shoulder strength, shoulder proprioception, or scapular kinematics. Background: Rigid and kinesio tapings are commonly used in the rehabilitation of subacromial impingement syndrome (SIS). Yet; the effect of postural correction with the two taping materials in SIS has not been extensively studied. The purpose of the study is to examine the effect of postural correction with two different taping materials on scapular kinematics and electromyography of scapular upward rotators in patients with SIS. Methods: Twenty female patients with SIS participated in this study. Their age ranged from 30-60 years. Participants were randomly assigned into: Group I (Kinesio tape, n=10) and Group II (rigid tape, n=10). Thoracic and scapular taping with posture correction was applied to both groups. Scapular upward rotation at 0˚, 60˚, 90˚ and 120˚ of shoulder elevation and the activity level of the upper fibers of trapezius (UT), lower fibers of trapezius (LT) and serratus anterior (SA) muscles were measured before and immediately after taping application. Results: Both taping materials significantly increased scapular upward rotation at 60°, 90° and 120° angles (P =.004,.002 and .047 respectively) after the application of tape as compared to the before. In addition, significantly greater muscle activity of the LT and SA muscles (P =.027 and 0.05 respectively) were demonstrated by the kinesio-taping group as compared to rigid taping group during real taping condition. Conclusion: Both taping materials are effective in restoring scapular kinematics. Furthermore, kinesio taping has a facilitatory effect on the LT and SA muscles. Kinesio taping may be considered an alternative to rigid taping in patients with SIS.
