Abstract

Evidence against the use of prophylactic drain after gastrectomy are increasing and ERAS guidelines suggest the benefit of drain avoidance. Nevertheless, it is unclear whether this practice is still widespread. We conducted a survey among Italian surgeons through the Italian Gastric Cancer Research Group and the Polispecialistic Society of Young Surgeons, aiming to understand the current use of prophylactic drain. A 28-item questionnaire-based survey was developed to analyze the current practice and the individual opinion about the use of prophylactic drain after gastrectomy. Groups based on age, experience and unit volume were separately analyzed. Response of 104 surgeons from 73 surgical units were collected. A standardized ERAS protocol for gastrectomy was applied by 42% of the respondents. Most of the surgeons, regardless of age, experience, or unit volume, declared to routinely place one or more drain after gastrectomy. Only 2 (1.9%) and 7 surgeons (6.7%) belonging to high volume units, do not routinely place drains after total and subtotal gastrectomy, respectively. More than 60% of the participants remove the drain on postoperative day 4–6 after performing an assessment of the anastomosis integrity. Interestingly, less than half of the surgeons believe that drain is the main tool for leak management, and this percentage further drops among younger surgeons. On the other hand, drain’s role seems to be more defined for duodenal stump leak treatment, with almost 50% of the surgeons recognizing its importance. Routine use of prophylactic drain after gastrectomy is still a widespread practice even if younger surgeons are more persuaded that it could not be advantageous.

Abstract

Introduction The concept of “weekend effect”, that is, substandard healthcare during weekends, has never been fully demonstrated, and the different outcomes of emergency surgical patients admitted during weekends may be due to different conditions at admission and/or different therapeutic approaches. Aim of this international audit was to identify any change of pattern of emergency surgical admissions and treatments during weekends. Furthermore, we aimed at investigating the impact of the COVID-19 pandemic on the alleged “weekend effect”. Methods The database of the CovidICE-International Study was interrogated, and 6263 patients were selected for analysis. Non-trauma, 18+ yo patients admitted to 45 emergency surgery units in Europe in the months of March–April 2019 and March–April 2020 were included. Demographic and clinical data were anonymised by the referring centre and centrally collected and analysed with a statistical package. This study was endorsed by the Association of Italian Hospital Surgeons (ACOI) and the World Society of Emergency Surgery (WSES). Results Three-quarters of patients have been admitted during workdays and only 25.7% during weekends. There was no difference in the distribution of gender, age, ASA class and diagnosis during weekends with respect to workdays. The first wave of the COVID pandemic caused a one-third reduction of emergency surgical admission both during workdays and weekends but did not change the relation between workdays and weekends. The treatment was more often surgical for patients admitted during weekends, with no difference between 2019 and 2020, and procedures were more often performed by open surgery. However, patients admitted during weekends had a threefold increased risk of laparoscopy-to-laparotomy conversion (1% vs. 3.4%). Hospital stay was longer in patients admitted during weekends, but those patients had a lower risk of readmission. There was no difference of the rate of rescue surgery between weekends and workdays. Subgroup analysis revealed that interventional procedures for hot gallbladder were less frequently performed on patients admitted during weekends. Conclusions Our analysis revealed that demographic and clinical profiles of patients admitted during weekends do not differ significantly from workdays, but the therapeutic strategy may be different probably due to lack of availability of services and skillsets during weekends. Abstract

This study reports the concentration level of volatile organic compounds (VOCs) including siloxanes in the biogas produced from sewage sludge in the municipality of Milan, Northern Italy. The obtained biogas was upgraded to biomethane in a pilot scale vacuum swing adsorption (VSA) plant based on activated carbon and zeolite 13X. The produced biomethane was also characterised in order to assess its suitability as automotive fuel. While critical levels were observed in the biogas for of H2S (1570 mg m-3) and HCl (26.8) were observed in biogas, halogenated VOCs and mercaptans were relatively low concentrated. High concentration of total volatile silicon (TVS) was confirmed in sewage sludge biogas, with a major contribution of siloxanes D5 and D4, while trimethylsilanol was below quantitation limit. The suitability of siloxane D5 as an indicator of total siloxanes is discussed. Abstract

Variation of gene content and gene expression in terms of relative quantitative expression and tissue/organ specificity is a substantial factor affecting phenotypic diversity. In crops, particularly in cereals, the pantran scriptome and pan - genome concepts raised soon after the reference genomes were made available. Characterizing the gene expression presence - absence variation (ePAV) of tetraploid durum wheat ( Triticum turgidum ssp. durum) enables to investigate the associat ion between the genotypic and phenotypic variation at an unprecedented level of precision. The current study presents the transcriptome analysis for 13 elite varieties from worldwide germplasm spanning from 1969 up to 2005. We aim to describe the gene expr ession variation in relation to a high - quality reference genome sequence assembly of durum wheat cv. Svevo (c/o International Durum Wheat Genome Sequencing Consortium). cDNA libraries for the 13 varieties were produced from roots and leaves at the seedling stage, and developing grains. In order to study the gene expression pattern, RNA - seq libraries of 13 varieties were aligned to the durum wheat genome using HISAT2. The transcript abundance was calculated using StringTie and Ballgown. Expression matrix was then normalized using R package DESeq2 for further clustering and variance analysis. The annotation of the Svevo assembled genome resulted in 66,559 high confidence gene models. Overall, 75.0% (48,007 genes), 70.5% (45,142) and 74.5% (47,702) of genes wer e expressed in grain, leaf and roots, respectively. Considering the cultivars and the tissue/organ libraries overall, the percentage of genes mapped to the Svevo genome reference varied from 48.0% (Altar84, Capeiti 8, Claudio, Saragolla leaves) to a maximu m of 61.0% (Meridiano, Strongfield roots and grains). Principal Component Analysis (PCA) analysis showed a clear gene expression clustering lead by organs (leaves, grains and roots accounting for 33.0 % variance). Hierarchical clustering based on the stron gest PC1 – PC2 scores clearly differentiated up - and down - regulated gene clusters based on tissues and varieties. Variance expression analysis projected on the Svevo assembly allowed us to identify the chromosome regions that drove the major expression varia tion patterns. Presence/absence of expression polymorphism could also be observed for several of the genes sorted by PCA. Interestingly, by clustering the gene expression profiles and the cultivar’s expression profiles several gene expression patterns rela ted to the ancestry relationship among cultivars were evidenced, particularly for the grains. The functional annotation of these gene clusters is ongoing. Towards assembly of a pan - transcriptome in durum, the cultivar - specific reads that could not be mappe d on the Svevo genome (4 - 30% referred to the Svevo Illumina sequencing data) are being de novo assembled. This expression pattern database could also be useful to identify genes regulated by eQTLs and to elucidate the function of candidate genes

Abstract

[EN] Single primer enrichment technology (SPET) is a new, robust, and customizable solution for targeted genotyping. Unlike genotyping by sequencing (GBS), and like DNA chips, SPET is a targeted genotyping technology, relying on the sequencing of a region flanking a primer. Its reliance on single primers, rather than on primer pairs, greatly simplifies panel design, and allows higher levels of multiplexing than PCR-based genotyping. Thanks to the sequencing of the regions surrounding the target SNP, SPET allows the discovery of thousands of closely linked, novel SNPs. In order to assess the potential of SPET for high-throughput genotyping in plants, a panel comprising 5k target SNPs, designed both on coding regions and introns/UTRs, was developed for tomato and eggplant. Genotyping of two panels composed of 400 tomato and 422 eggplant accessions, comprising both domesticated material and wild relatives, generated a total of 12,002 and 30,731 high confidence SNPs, respectively, which comprised both target and novel SNPs in an approximate ratio of 1:1.6, and 1:5.5 in tomato and eggplant, respectively. The vast majority of the markers was transferrable to related species that diverged up to 3.4 million years ago (Solanum pennellii for tomato and S. macrocarpon for eggplant). Maximum Likelihood phylogenetic trees and PCA outputs obtained from the whole dataset highlighted genetic relationships among accessions and species which were congruent with what was previously reported in literature. Better discrimination among domesticated accessions was achieved by using the target SNPs, while better discrimination among wild species was achieved using the whole SNP dataset. Our results reveal that SPET genotyping is a robust, high-throughput technology for genetic fingerprinting, with a high degree of cross-transferability between crops and their cultivated and wild relatives, and allows identification of duplicates and mislabeled accessions in genebanks.

25. Digestate treatment with algae-bacteria consortia: A field pilot-scale experimentation in a sub-optimal climate area

Abstract

This paper addresses the efficiency of a microalgae-based agricultural digestate treatment at pilot-scale in an outdoor raceway pond (880 L, pH-dependent CO2 dosage) and in a bubble column (74.5 L, air-bubbling). Specifically, nitrogen removal, evolution of the algae-bacteria consortium, and the actual process applicability in the Po Valley climate are discussed. The performance of the two reactors varied seasonally. The average algal productivity in the raceway was 32.4 ± 33.1 mg TSS·L−1·d−1 (8.2 ± 8.5 g TSS·m−2·d−1) while in the PBR it was 25.6 ± 26.8 mg TSS·L−1·d−1; the average nitrogen removal was 20 ± 29% (maximum 78%) and 22 ± 29% (maximum 71%) in the raceway and in the column, respectively. Nevertheless, nitrification had a key role as 61 ± 24% and 52 ± 32% of the nitrogen load was oxidized in the raceway and in the column, respectively.

Abstract

The liquid fraction of digestate produced by agricultural biogas plants is rich in macro and micronutrients that are valuable for the culturing of microalgae. Nonetheless, the high ammonium concentration may cause toxicity and the high optical density may reduce light penetration, negatively affecting the biomass production rate. Dilution with fresh water has been frequently suggested as a mean for improving the digestate characteristics in view of microalgal culturing. In this paper, the feasibility of culturing microalgae on undiluted raw digestate or on digestate after pretreatment by stripping and adsorption was investigated. First, adsorption tests were performed using commercial activated carbon from wood in order to identify appropriate conditions for optical density (OD) reduction. Up to 88% reduction was obtained by dosing 40 g L− 1 after 24 h of contact time. Then, culturing tests were performed on a microalgal inoculum including mainly Chlorella spp. and Scenedesmus spp. under controlled temperature and light conditions during 6–14 weeks. Raw, stripped, and stripped and adsorbed digestate samples were tested. The biomass production rate increased from 27 ± 13 mg TSS L− 1 d− 1 on raw digestate, to 82 ± 18 mg TSS L− 1 d− 1 by using stripped digestate, and to 220 ± 78 mg TSS L− 1 d− 1 by using the stripped and adsorbed digestate. Moreover, nitrification was constantly suppressed when using the stripped and adsorbed digestate, while relevant nitrite built-up was observed when using raw and stripped digestate. These results suggest that microalgae are able to grow on the raw digestate, provided that long hydraulic retention times are applied. Abstract

Background A randomized clinical trial was performed to compare D1 and D2 gastrectomy in specialized Western centres. This paper reports short-term results. Method A total of 267 patients with gastric cancer were randomly assigned to either a D1 or a D2 procedure in five specialized centres. Based on the findings of the phase II trial and published phase III trials, a prespecified non-inferiority boundary at 12 per cent difference between groups was set regarding total morbidity. Results In the intention-to-treat analysis, the overall morbidity rate after D2 and D1 dissections was 17·9 and 12·0 per cent respectively (P = 0·178), with a 95 per cent confidence interval of the difference of 0 to 13·0 per cent, slightly exceeding the prespecified non-inferiority limit. There was a single duodenal stump leak in the D2 arm (0·7 per cent). The postoperative 30-day mortality rate was 3·0 per cent after D1 and 2·2 per cent after D2 gastrectomy (P = 0·722). Conclusion In specialized centres the rate of complications following D2 dissection is much lower than in published randomized Western trials. D2 dissection, in an appropriate setting, can therefore be considered a safe option for the radical management of gastric cancer in Western patients.
