Objective The aim of the present study is to investigate the biomechanical stability of different subtrochanteric osteotomy types and graft positions in cases of dysplastic coxarthrosis that require total hip arthroplasty with shortening osteotomy, as well as to find out the most effective osteotomy type and graft position. Method Femur sawbones were used to compare different types of femoral shortening osteotomy (transverse, oblique, and step-cut). Strut grafts, which were prepared at the side of the subtrochanteric shortening osteotomy, were fixed in different positions (anterolateral, mediolateral, and anteroposterior). The fixation of the strut grafts was performed using two steel cables (with 2.0mm of thickness) with the same strength. The failure values of composite femurs were recorded for axial and rotational loadings. Results Biomechanically, there were no statistically significant differences between the types of femoral subtrochanteric shortening osteotomy and the positions of the applied strut graft. Conclusion No superiority was observed between the types of femoral subtrochanteric shortening osteotomy regarding stability. Additionally, against stress, similar results were obtained for different strut graft positions. In conclusion, we believe that using the method in which the surgeon is experienced and that is the easiest to apply would be the best choice. Resumo Objetivo O objetivo do presente estudo é investigar a estabilidade biomecânica de diferentes tipos de osteotomias subtrocantéricas e posições de enxertos em coxartroses displásicas que necessitam de artroplastia total de quadril com osteotomia de encurtamento, e determinar o tipo de osteotomia e a posição do enxerto que são mais eficazes. Método Modelos de fêmur (sawbones) foram usados para comparar os tipos de osteotomia de encurtamento femoral (transversal, oblíqua e emdegrau de escada [step-cut]). Os enxertos em haste, preparados do lado da osteotomia subtrocantérica, foram fixados em diferentes posições (ântero-lateral, medial-lateral e ântero-posterior). A fixação dos enxertos foi feita com2 cabos de aço (de 2,0mmde largura) de mesmaresistência.Os valores de falha dos fêmures compostos para cargas axiais e rotacionais foram registrados. Resultados Do ponto de vista biomecânico, não houve diferenças estatisticamente significativas entre os tipos de osteotomia de encurtamento subtrocantérico femoral e as posições dos enxertos em haste aplicados. Conclusão Não houve superioridade entre os tipos de osteotomia de encurtamento subtrocantérico femoral quanto à estabilidade. Além disso, em relação à resistência ao estresse, os resultados obtidos com diferentes posições das hastes foram similares. Assim, acreditamos que o método mais indicado é aquele em que o cirurgião é experiente e cuja aplicação é mais fácil.

Objectives: Hyaluronic acid (HA) is used in osteoarthritis, especially for the control of pain. In this study, we investigated the effect of intra-articular HA on experimental osteoarthritis of early stage.Methods: Osteoarthritis was induced in both knees of 10 rabbits by transecting the anterior cruciate ligament under intramuscular anesthesia with ketamine and xylazine. Intra-articular HA at a dose of 0.6 ml (15 mg/ml) and physiologic saline solution (0.6 ml) were injected into the right and left knees, respectively, three times with a week interval. Three rabbits died during the study period and were excluded. The remaining rabbits were sacrificed in the 12th week via high dose anesthesia to remove the the distal femora for histological evaluation using the Mankin scale and for measurements of the cartilage area.Results: The mean cartilage areas calculated in HA- and saline-injected knees differed significantly (1.097 mm2 and 0.477 mm2, respectively; p0.05), the mean scores for the structure of the cartilage were significantly different (0.86 versus 4.43; p, Amaç: Hiyalüronik asit (HA), osteoartritli eklemlerde özellikle ağrıyı gidermek için kullanılmaktadır. Bu deneysel çalışmada, eklem içine uygulanan HA’nın erken evredeki osteoartrit üzerine etkileri araştırıldı.Çalışma planı: On adet tavşanın her iki dizinin ön çapraz bağı, ketamin ve ksilazin ile intramusküler anestezi altında kesilerek osteoartrit oluşturuldu. Amaç: Tavşanlar üzerinde yapılan bu deneysel çalışmada düğüm ile tespit edilen açık-kapı laminoplasti ameliyatının spinal kanal çapına aneliyat sonrası erken dönemdeki etkisinin incelenmesi amaçlandı.Çalışma planı: Çalışmaya alınan 20 beyaz Yeni Zelanda tavşanına C4 ile C7 arası seviyelere Hirabayashi açık-kapı laminoplasti uygulandı. Kobayların spinal kanal çapı operasyon öncesi 1. günde ve operasyon sonrası 1. ve 42. günlerde radyolojik olarak değerlendirildi.Bulgular: Ortalama spinal kanal çapı ameliyat öncesi 6.42 mm, ameliyat sonrası 1.gün 8.04 mm ve 42. gün 8.02 mm olarak bulundu. Ameliyat öncesi ortalama değere göre tavşanların spinal kanal çapları ameliyat sonrası 1. günde anlamlı olarak yüksek bulundu (p0.05). Çıkarımlar: Sonuçlarımız tavşan modelinde uygulanan düğüm tespitli açık-kapı laminoplastinin spinal kanal çapını anlamlı olarak arttırdığını ve bu etkinin ameliyat sonrası erken dönemde korunduğunu göstermektedir., Objective: The aim of this experimental study was to evaluate the effect of expansive open-door laminoplasty with simple suture fixation on spinal canal diameter in a rabbit model.Methods: Twenty white New Zealand rabbits were operated on with C4-C7 Hirabayashi open-door laminoplasty. The spinal canal diameter was evaluated radiologically on preoperative day 1 and postoperative days 1 and 42.Results: The mean spinal canal diameter was 6.42 mm preoperatively, 8.04 mm on postoperative day 1 and 8.02 mm at day 42. There was a significant difference between the mean preoperative and postoperative day 1 spinal canal diameter (p0.05).Conclusion: Our results suggest that the open-door laminoplasty with simple suture fixation is an effective method to expand the spinal canal diameter. No recurrent narrowing is expected in short term.

Objectives: We evaluated tendon holding capacities of suture materials that are commonly used in repair of Achilles tendon ruptures.Methods: Achilles tendons of 60 sheep were removed by incisions 2-cm proximal to the calcaneal insertion and 1-cm distal to the musculotendinous junction. The tendos were randomly divided into six groups and sutures were placed with the Kessler technique at the distal end of the tendons using one of the following suture materials: 2 polydioxanone (PDS), 1 PDS, 2 Vicryl, 1 Vicryl, 2 Ethibond, and 1 Prolene. The distal end of the suture material was left free. Each specimen was mounted in an Instron machine, with the tendon being placed proximally and the suture material distally. The system was loaded with a displacement rate of 20 mm/min.Results: Failure of all the specimens was due to pull-out of the suture material through the tendon. There were no failures due to suture breakage. The highest and the lowest tendon holding capacities were found with 2 PDS and 2 Ethibond sutures, respectively. Conclusion: Following Achilles tendon repair, the healing period, in particular the first three weeks, is precarious for pull-out of the suture material through the tendon. Thus, tendon holding capacity of the suture material is an important factor for the strength of the repair. Among the tested suture materials, 2 PDS was found to have the highest tendon holding capacity., Amaç: Bu çalışmada Aşil tendonu tamirinde sık kullanılan dikiş materyallerinin tendonu tutma kapasiteleri değerlendirildi.Çalışma planı: Altmış adet koyun Aşil tendonu, muskülotendinöz bileşkenin 1 cm distalinden ve kalkaneus yapışma yerinin 2 cm proksimalinden kesilerek hazırlandı. Rastgele altı gruba ayrılan tendonların distal ucuna, dikiş materyalinin ucu boşta kalacak şekilde, 2 polidioksanon (PDS), 1 PDS, 2 vikril, 1 vikril, 2 etibond ve 1 prolen dikiş materyallerinden biri kullanılarak Kessler konfigürasyonu yapıldı. Hazırlanan örnekler Instron germe cihazına, tendon üst çeneye, dikiş materyali alt çeneye gelecek şekilde yerleştirildi. Cihaz 20 mm/dk germe hızına ayarlanarak yükleme yapıldı. Sonuçlar: Tüm deneylerde yetersizlik nedeni dikiş materyalinin tendondan sıyrılmasıydı. Dikiş materyali kopmasına bağlı yetersizlik oluşmadı. Tendonu tutma kapasitesi en yüksek dikiş materyali 2 PDS, en düşük dikiş materyali 2 etibond olarak bulundu.Çıkarımlar: Aşil tendonu tamiri sonrasında iyileşmenin özellikle ilk üç haftası, dikiş materyalinin tendondan sıyrılmasıyla tamir dokusunun bozulması açısından riskli bir dönemdir. Bu nedenle, tamirde kullanılan dikiş materyalinin tendonu tutma kapasitesi önemli bir faktördür. Çalışmamızda tendonu tutma kapasitesi en yüksek dikiş materyalinin 2 PDS olduğu görüldü.

VI. ÖZET Çalışmamızın iki ana amacı bulunmaktadır. Bunlar Meniskûste mevcıi olan proprioseptif sinir uçlarından gelen uyanların somatosensoryel kortekse ulaştığım fizyolojik koşullarda göstermek ve Meniskûsteki proprioseptif yolağm semimembranozus kasmda aktif kasılma ile sonlanan bir refleks ark oluşturduğunu göstermektir. Bu çalışma, ACL ve medial menisküsü sağlam diğer eklemiçi patoloji nedeniyle artroskopik cerrahi uygulanan 17 hastada yapıldı. Yaşlan 21 ile 76 arasında değişmekte idi. Menisküsün proprioseptif özelliğini göstermek için medial menisküs elektriksel olarak uy anlarak somatosensoryel kortekste oluşan uyandınlmış potansiyellerin (kortikal SEP) kayıtlaması denenmiştir. Refleks ark oluştuğunu göstermek için medial menisküs elektriksel olarak uyanlarak hamstring kasmda oluşan refleks bileşik kas aksiyon potansyeli kaydedilmiştir. Tüm vakalarda artroskopik işlemden önce H refleksi ve tibial SEP kayıtlan alındı. Klasik H refleks ve SEP çalışması hastanemiz EMG laboratuarında, artroskopik çalışma ise ameliyathane koşullarında gerçekleştirildi. Artroskopik işlem esnasında, sonuçların etkilenmemesi için kortikal SEP yanıtında depresyon etkisi yapan anestetikler kullanılmadı. Bileşik kas aksiyon potansiyeli kaydını etkilememesi için kas gevşetici kullamlmadı. Eklemiçi sıvı olarak elektrolit içermeyen reseksiyon avısı kullanıldı. Artroskopik girişim yapılan 17 olgunun 145ünde medial menisküs tamamen intakttı. Diğer 3 vakada ileri dejeneratif değişiklikler mevcuttu. Medial 37menisküsü intakt olan 14 vakada intraoperatif kayıtlar kolaylıkla elde edildi. Dejeneratif değişiklikleri olan vakalardan birinde uzun latanslı bir refleks kayıtlanabilirken diğerlerinde intraoperatif kayıtlar elde edilemedi. Medial menisküs uyarısı ile elde edilen kortikal SEP ortalama latansı N:44.75, P:53.94 msn idi. Medial menisküs uyanmı ile hamstring kas grubundan elde edilen BKAP ortalama latansı 19.74 msn, amplitüdü 5.90 mV idi. Medial menisküs orta kısmına, kondile ve sıvıya verilen uyanlardan hiçbir yanıt alınamadı. Olguların % 82'sinde yanıt alınmış olup, bu % 82'lik olgunun da % 93 'ünde sonuçların anlamlı değer aralıklarında olduğu görülmüştür. Menisküslerin periferal kısmında az oranda serbest sinir uçlan, bulunmasına karşın posterior ve anterior boynuzlar serbest sinir uçlarından ve üç tipte enkapsüle mekanoreseptörlerden oldukça zengin olması ve medial menisküs posterior boynuzuna hamstring kas grubunun insersiyoya sahip olması bizi menisküslerin kondiller arasına yakalanarak zarar görmesini önleyici bir mekanizmanın varlığını düşünmeye itmiştir. Bunun yanında dizin fleksiyon ve ekstansiyon hareketleri esnasında, menisküslerin femur ve tibia kondillerinin anteroposterior planda yaptıkları hareketin yansı kadar yer değiştirmesi menisküslerin korunması için menisküsteki proprioseptif yolağın semimembranozus kasında aktif kasılma ile sonlanan bir refleks ark oluşturduğunu düşünmeye itmiştir. Sonuç olarak, başta sormuş olduğumuz iki soruya yanıt vermiş bulunuyoruz. Birincil olarak, menisküste proprioseptif sinir ucu varlığını ve bu 38yolağın somatosensoryel kortekse ulaştığını göstermiş bulunuyoruz, ikincil olarak, menisküsteki proprioseptif yolağın semimembranozus kasında aktif kasılma ile sonlanan bir refleks ark oluşturduğunu göstermiş bulunuyoruz. Çalışmamız sonucunda, medyal menisküs posterior boynuz yırtıkları sonrası yapılacak menisektomilerin mümkün olduğunca koruyucu yapılması ve daha önemlisi hamstring kas grubunun ACL rekonstrüksiyonlarmda greft olarak kullanılmaması önerilebilir. 39 47

Background: Valgus foot is a common foot deformity in spina bifida. The most popular operation for the valgus deformity has been the Grice talocalcaneal blocking. It has not been studied primarily in children with spina bifida. We report a prospective series, we present the results of hind foot valgus deformity of children with spina bifida, using Grice talocalcaneal arthrodesis with a tricortical iliac bone graft. Materials and Methods: Between May 2000 and December 2003, 21 patients with bilateral (42 feet) valgus deformity of feet underwent surgery. There were 7 males and 14 females. The mean age of patients was 67.7 months (range 50-108 months). Results: The total number of feet that had nonunion was 11, in 7 of them the grafts were completely reabsorbed and the outcome of all these feet was unsatisfactory. Four feet had partial union of which three had unsatisfactory and one had satisfactory outcome. Sixteen feet had residual valgus deformity at the last followup visit, 10 patients had nonunion, and 6 had inadequate correction . Mean preoperative talocalcaneal and calcaneal pitch angles were 48.5΀ and 31.9΀, respectively, which decreased to 38.5΀ and 29.1΀, respectively, postoperatively. The decrease in talocalcaneal angle and calcaneal pitch was significant between preoperative and postoperative measurements ( P Conclusion: Grice subtalar arthrodesis technique is still a valuable option for valgus foot in patients with spina bifida. In this study, we found more encouraging results in older patients.
