Schizophrenia is a neurodevelopmental brain disease and a severe psychiatric disorder which is seen approximately in 1% of the society. About the half of the individuals diagnosed with schizophrenia have substance abuse story, and this rate is higher than the general population. Alcohol dependence, smoking and substance abuse has been reported very often in schizophrenia. Especially smoking rate is very high in patients with schizophrenia. The aim of this paper is to briefly review the literature related to the relationship between schizophrenia and nicotine-alcohol dependency.

Creutzfeldt Jakob Disease is an incurable and invariably fatal degenerative brain disease. Sporadic and variant forms of the disease can be observed among patients. We report on a patient for calling attention to clinical features and laboratory findings of Creutzfeldt- Jakob Disease and the psychiatric prodromal symptoms. This case demonstrates that psychiatric symptoms may also be a presenting symptom of Creutzfeldt-Jakob Disease and this diagnosis should be considered when rapid deterioration in cognition is observed with presence of psychiatric and neurological symptoms.

Trichotillomania is a psychiatric disorder that involves repetitive hair pulling to the point of apparent loss. No approved treatment algorithm is available for trichotillomania. We present a case report of a 28-year-old female diagnosed as trichotillomania, with complaints of recurrent hair pulling resulting in noticeable hair loss. She was treated with Habit Reversal Training and a selective serotonin reuptake inhibitor (sertraline) over a period of 6 weeks. Habit reversal training includes self-monitoring, awareness training, competing response training, and homework assignments. The aim of this case report is to provide a brief description of habit reversal training, which is unfamiliar to many professionals.

Objective: Few studies comparing smoking and non-smoking obsessive compulsive disorder (OCD) patients have found results contradicting research on other anxiety disorders. OCD is a disorder with compulsive and impulsive features. Investigating the relationships among addiction, compulsivity and impulsiveness might contribute to the understanding of the interaction of those concepts. Method: This study compared impulsiveness in smoking and non-smoking OCD patients. Then, we measured the correlations among the severities of impulsiveness, OCD and addiction. Depressive symptoms and anxiety were also included in the analysis since they are frequently accompanied by OCD, addictive disorders and pathological conditions characterized by high impulsiveness. Patients with OCD (n=121) were given Yale-Brown Obsessive Compulsive Scale (Y-BOCS), Barratt Impulsiveness Scale-Version 11 (BIS-11), Fagerström Test for Nicotine Dependence (FTND), Hamilton Depression Rating Scale-17 Item (HDRS-17) and Beck Anxiety Inventory (BAI). Results: The prevalence of smoking was 42.14 %. The current and never-smokers did not show any statistically significant differences regarding the scores of the Y-BOCS, HDRS-17, and BAI. The BIS-11 total and non-planning subscales were significantly higher in the current smokers. The Y-BOCS scores showed a positive correlation with the BIS total, cognitive, and non-planning scores only in the current smokers. Only in the smokers, the compulsion scores showed a positive correlation with the BIS-11 total and cognitive impulsiveness subscale. The FTND scores were correlated with the scores of the BIS-11 total and of the subscales of behavioral impulsiveness and non-planning. The FTND scores were also correlated with the scores of the Y-BOCS and BAI but not with the HDRS-17. Conclusion: More severe OCD in more impulsive smokers is related to higher compulsivity rather than obsessiveness, supporting the relationship among compulsivity, impulsiveness, and addiction.

Amaç: Alkolizm beyinde serebral hacim kaybının ve nöropsikolojik bozulmaların eşlik ettiği beyaz ve gri madde hasarına yol açar. Ancak alkol kesilmesi ve ayıklık (sobriety) döneminin ilk günlerinden itibaren serebral hacim kaybı, metabolik ve nöropsikolojik bozukluklar kısmen de olsa geri dönüşümlüdür. Yöntem: Bu gözden geçirme yazısında alkol bağımlılığında ayıklık döneminde beyinde izlenen yapısal ve fonksiyonel düzelmeler ele alınmış ve konuyla ilgili çalışmalar özetlenmiştir. Bulgular: Nörogenezde artış, kolin düzeyinde yükselme, gri ve beyaz cevherde büyüme, beyin devrelerinde reorganizasyon, yeni nöronların oluşumu ve ventriküllerde küçülmenin yanı sıra nörogenezin artmasına paralel olarak yürütücü işlevlerde, bellek ve dikkatte düzelme, davranışsal kontrol ve duygudurumda iyileşme de izlenir. Araştırmalar ayıklık döneminde düzenli fiziksel egzersiz programı uygulanması ile nörogenezin hızlanabileceğini düşündürmektedir. Sosyal içicilik ya da yasal sınırlar içindeki düşük miktarlarda alkol kullanımının da nörogenez üzerine olumsuz etkiler gösterdiğini bildiren çalışmalar, alkol bağımlılığı sonrasında beyin işlevleri zarar görmüş kişilerin ayıklık döneminde alkolden tamamen uzak durmalarının önemini göstermektedir. Sonuç: Ayıklık döneminde izlenen nöronal düzelmenin mekanizmalarının anlaşılması ile beyin rejenerasyonuna ait bir model geliştirilebilecek ve gelecekte diğer nörodejeneratif hastalıkların tedavisinde yol gösterici olabilecektir.

Özellikle tedavisel aralığı dar olan ilaçlar için tedavi etkinliğini sağlayabilmek, yan etkileri en aza indirebilmek için farmakolojik tedavinin kiĢiye özel uygulanması gereklidir. Sitokrom P450 enzimlerinin fenotiplemesi hem değerli bir araĢtırma yöntemi, hem de metabolik kapasitesinin genetik temelinin değerlendirilmesi için bir araçtır. Teknik, bir prob ilaç (ölçüm maddesi) kullanarak metabolik kapasitenin tahmin edilmesine dayanır. Fenotipleme ilaç etkileĢimleri, genetik polimorfizmler, hepatik hastalıklar ve ilacın farmakokinetiğini etkileyebilecek diğer etkenlerin toplam etkisini gösterir. Bunun için fenotiplemede her bir sitokrom enzim sistemine özgü substratların kullanımı gereklidir. Son dönemde birden fazla sitokrom enzim sisteminin in vivo değerlendirmesine olanak tanıyan ölçüm maddeleri kokteylleri ile fenotipleme kullanılmaya baĢlanmıĢtır

Transcranial direct current stimulation (TDCS) is a promising neuromodulatory treatment option in neuropsychiatry. However, such a novel neuromodulation application also brings challenges regarding safety. In this paper, we aimed to provide a retrospective evaluation of adverse effects related to TDCS implementation in a group of psychiatric patients. Twenty-nine patients who were on TDCS with different psychiatric diagnosis were evaluated retrospectively. Occurrence and severity of adverse events monitored with a questionnaire. No subjects experienced seizures or other adverse effects requiring medical intervention. During TDCS an itching sensation was the most common reported adverse effect (48 %, n=12) whereas after TDCS fatigue occurred in 32% of cases (n=8). Also one patient reported mild nausea during the first 3 sessions and one patient reported numbness in the electrode area. Transient redness at the site of stimulation was reported by 3 (12%) of patients. None of the reported side effects were marked as severe.Results suggest that TDCS applied according to the present TDCS safety guidelines, is associated with relatively minor adverse effects in psychiatric population.Our findings encourage further prospective studies to explore the effect of TDCS in psychiatric diseases

Amaç: Valproat ve elektrokonvülsif tedavinin bir arada kullanımı tartışmalı bir konudur. Duygudurum düzenleyici olarak kullanılan antikonvulzanların EKT sırasında nöbetin ortaya çıkışını olumsuz etkilemesinden endişe edilir. Bu araştırmada bipolar bozukluk manik epizot hastalarında elektrokonvülzif tedavi ile eşzamanlı kullanılan tam doz ya da azaltılmış dozdaki valproatın klinik etkileri incelenmiştir. Yöntem: Nisan 2010-Temmuz 2013 ayları arasında bir psikiyari hastanesinde bipolar bozukluk manik epizot tanısı ile elektrokonvülzif tedavi alan, epileptik olmayan, araştırma kriterlerine uygun ardışık 75 hastanın kayıtları incelenmiştir. Nöbet için gerekli enerji düzeyi, nöbet süresi, uygulanan elektrokonvülsif tedavi seans sayısı ve young Mani Ölçeği skorlarındaki değişim, EKT esnasında tam dozda valproat kullanan, % 40 azaltılmış dozda valproat kullanan ve valproat ya da başka bir antikonvülzan kullanmayan hasta grupları arasında karşılaştırılmıştır. Bulgular: Her üç grup karşılaştırıldığında geçirilen nöbet süreleri açısından kontrol grubu ile valproat doz ayarlaması yapılan grup arasında nöbet süreleri açısından fark saptanmamışken (p=0.078), kontrol grubu ile valproat doz ayarlaması yapılmayan grup arasında nöbet süreleri açısından fark saptanmıştır (p=0.038). Remisyona ulaşmak için gerek duyulan EKT seans sayısı tam doz valproat kullanan grupta anlamlı biçimde daha yüksek bulunmuştur. Valproat ile EKT’nin eşzamanlı kullanımı iyi tolere edilmiş, tam doz ya da azaltılmış doz valproat kullanan hastalarda ciddi bir yan etki izlenmemiştir. Tüm olgularda Young Mani skorunda anlamlı düşme ve klinik remisyon izlenmiştir. Tartışma: Valproatın EKT ile eşzamanlı kullanımı rutin EKT uygulamasına engel oluşturmamış ve koruyucu duygudurum düzenleyici tedavisine geçişi hızlandırmıştır. Bu bulguların doğrulanması ve kognitif yan etkilerin gruplar arasında kıyaslanabilmesi için ileriye dönük araştırmalara ihtiyaç bulunmaktadır.

Bupropion is largely used as an antidepressant and smoking cessation therapy. The aim of this work was to evaluate effects of increased bupropion plasma levels on blood pressure and heart rate in patients with depression. A total of sixty patients with a diagnosis of depression completed this naturalistic study. Bupropion was administered in 150 mg/day dose. Bupropion plasma levels were measured on the 7th and 14th day of treatment. Heart rate, systolic and diastolic blood pressure were measured for each patient on the 7th and 14th days of treatment. The results of this study did not show any statistically significant blood pressure or heart rate changes with elevating bupropion plasma levels. No significant difference in blood pressure or heart rate was observed by reaching the therapeutic plasma levels of bupropion. Although our results did not suggest any effect of bupropion plasma levels on heart rate and blood pressure, therapeutic benefit of bupropion to treat depression in hypertensive patients should be carefully weighed.

The psychotic symptoms related to Parkinson’s Disease (PD) are often resistant to antipsychotics, and management of psychotic features is one of the most critical issues in the treatment of PD. We reported a 42-year-old man with PD and medically intractable PD associated psychosis who was significantly benefitted from electroconvulsive therapy

Objective: Research has been done for the purpose of determining the effect of interpersonal addictions of school of health students to substance use cases. Method: 376 students who are registered in a school of health’s departments of nursing, midwife, feeding and dietetic in 2012 - 2013 school years, formed the sample group of research planned in sectional type. To make the research, written approval from the institution and spoken approval from students were gotten. A poll form which includes socio-demographic characteristics and Interpersonal Addiction Criterion were used as data collection means. Results: 7.9 percent of participants were men and 14.8 percent of participants were students of nursing department, average age of participants was 21.04 ±1.56. Women compared to men, feel more emotional confidence and less social self-confidence. It was determined that the ones who say ‘’ Yes ‘’ to the question “Have you ever used any substance?” feel less lack of social self-confidence, compared to the ones who say ‘’ No ‘’. Most of the students think that there is no connection between substance use and communication manner. It was determined that lack of social self-confidence is felt minimum among students who study in Departments of Feeding and Dietetic and a statistical meaningful difference has been established. Conclusion: According to the results of the research students are interpersonal dependent and autonomic so they have the risk of making substance use decisions. For this reason it is suggested that especially university students who will take charge in health discipline, should be evaluated and supported by dependence units. Amaç: Araştırma sağlık yüksek-okulu öğrencilerinin kişilerarası bağımlılıklarının madde kullanma durumlarına etkisini saptamak amacıyla yapılmıştır. Yöntem: Kesitsel tipte planlanan araştırmanın örnek grubunu, 2012-2013 öğretim yılında bir sağlık yüksekokulu Hemşirelik, Ebelik ve Beslenme ve Diyetetik bölümlerine kayıtlı 376 öğrenci oluşturmuştur. Araştırmayı gerçekleştirebilmek için kurumdan yazılı, öğrencilerden sözlü onam alınmıştır. Veri toplama aracı olarak; sosyo-demografik özellikleri içeren anket formu ve Kişiler Arası Bağımlılık Ölçeği kullanılmıştır. Bulgular: Katılımcıların %7.9’u erkek, %14.8’i hemşirelik bölümü öğrencisi olup, yaş ortalaması 21.04 ±1.56’dır. Kadınlar erkeklere göre daha fazla duygusal güven, daha az sosyal özgüven yaşamaktadır. “Daha önce herhangi bir madde kullandınız mı?” sorusuna ‘evet’ diyenlerin, ‘hayır’ diyenlere göre sosyal özgüven eksikliğini daha az yaşadığı belirlenmiştir. Öğrencilerin çoğunluğu madde kullanma ile iletişim tarzı arasında ilişkinin olmadığını düşünmektedir. Sosyal özgüven eksikliğini en az Beslenme ve Diyetetik bölümünde okuyan öğrencilerin yaşadığı belirlenmiş, istatistiksel olarak anlamlı bir fark saptanmıştır. Sonuç: Araştırmanın sonuçlarına göre öğrenciler kişilerarası otonomik ve bağımlılar, buna bağlı madde kullanım kararlarını alma riskine de sahipler. Bu nedenle özellikle sağlık disiplininde görev alacak üniversite öğrencilerinin bağımlılık birimleri tarafından değerlendirilmesi ve desteklenmesi önerilmektedir.

Bir çok elektrokonvülsif tedavi kılavuzundai elektrokonvülsif tedavi uygulamasını tercih edilen tedavi olarak gösterildiği durumlar olarak psikotik özellikli şiddetli depresyon, maink deliryum ve katatoni gösterilir. Elektrokonvülsif tedavi uygulaması kararı her bir hasta için, psikiyatrik tanı, belirtilerin tipi ve şiddetş, geçmiş tedavi öyküsüve tedavi yanıtları, alternatif tedavi seçeneklerin belirlenmesi ve hasta tercihi hgibi maddeler göz önüne alınarak fayda-zarar değerlendirmesi yapılarak verilir. Bu gözden geçirmede elektrikonfülzif tedavinin etki mekanizması nörükimyasal etkileri ve yan etkileri literatür ve klinik deneyimler ışığında tartışılacaktır.

Transkraniyal manyetik uyarım (TMU) tekniği, gerek serebral korteks işlevlerinin lokalizasyonunu belirlemeyi amaçlayan nörofizyolojik araştırmalarda, gerekse sinir sistemi hastalıklarının tedavisini hedefleyen klinik çalışma-larda kullanılır. TMU beyin dokusunun uyarımını sağlayan, invazif olmayan bir tekniktir. Sinir sisteminin belli bir bölgesine ayarlanabilir bir şiddette ve frekansta, bir defa veya çok sayıda manyetik atım verilmesinden oluşur. TMU'nun klinik uygulamaya girdiği 1985 yılından bu yana en fazla çalışma depresyon hastaları üzerine yapılmış-tır. Klinik çalışmaların çoğu, TMU'nun depresyon tedavisinde etkili olduğunu göstermektedir. TMU farmakolojik tedavilere yanıt vermeyen dirençli depresyonda da uygulanmış ve yalancı (sham) TMU'ya üstün bulunmuştur. Beynin her iki hemisferinin çeşitli bölgeleri farklı parametreler kullanılarak uyarılmıştır, ama araştırmalara katılan depresyon hastalarının %90'ına sol dorsolateral prefrontal kortekse yüksek frekanslı ve tekrarlayan uyarımlar verilmiştir. Yüksek frekanslı ve tekrarlayan TMU'nun en ciddi yan etkisinin, oldukça nadir görülmekle birlikte nöbet olduğu bildirilmektedir. Bu yazıda tekrarlayan TMU (tTMU) uygulaması sırasında jeneralize tonik-klonik nöbet geçiren bir olgu sunulmakta ve TMU ile indüklenen nöbet üzerine literatür gözden geçirilmektedir.

Introduction: Hypotension is a known effect of atypical antipsychotics. However, there is little information on acute hypertension resulting from antipsychotic drugs. Aripiprazole is a potent partial dopamine D2 agonist, a serotonin 5-HT1A agonist and a 5- HT2A antagonist. It also displays high affinity for the α-1B, -2A, -2C and β1, -2 adrenergic receptors. Many studies underline the role of α-1A adrenergic receptors in malignant hypertension. Method: Here we present a case of a 38-years old male patient with psychotic depression. Discussion: Aripiprazole was initiated at 10 mg/d and 36 hours after the first dose the patient complained of headache and palpitation. His physical examination revealed that he developed a hypertensive crisis with arterial hypertension (170/110 mmHg) and tachycardia (118 beats/minute). His blood pressure returned to normal a week after the interruption of aripiprazole.

Numerous studies have showed that the cerebellum has a role in processing higher brain functions besides motor coordination. Various types of congenital lesions of cerebellum are associated with neuropsychological impairments and behavioral changes. In Chiari malformation, the cerebellar structures have been stuck in posterior fossa. In this paper a case of Chiari malformation, presenting with affective instability and behavioral disinhibition was described., Serebellumun, motor hareketlerin koordinasyonunun yanı sıra, yüksek beyin fonksiyonlarının gerçekleşmesinde rolü olduğu birçok çalışmada gösterilmiştir. Serebellumun konjenital lezyonları çeşitli nöropsikolojik bozukluklar ve davranış değişiklikleri ile ilişkilendirilmiştir. Chiari malformasyonunda, serebellar yapılar posterior fossa içinde sıkışmış halde bulunurlar. Bu yazıda duygusal instabilite ve davranışsal disinhibisyon belirtileri ile giden bir Chiari malformasyonu olgusu sunulmuştur.

Objective: Repetitive transcranial magnetic stimulation (rTMS) can enhance the excitement of the brain through adjusting the biological activities of the cerebral cortex and has wide biological effects, making it one basic mechanism of therapy for depression. In the treatment of unipolar depressive disorder, almost in every treatment method, hypomanic and manic shifts can be observed. There is still a lack of data regarding manic and hypomanic symptoms triggered by rTMS applications. Method: We describe four cases with unipolar depression in which high-frequency rTMS over the left dorsolateral prefrontal cortex applied as an add-on antidepressive strategy may have induced a hypomanic episode. Results: In these cases, 25 Hz rTMS combined with antidepressants may have contributed to the occurrence of hypomanic symptoms. Conclusion: Using an intensive methodology of rTMS may induce hypomanic or manic symptoms.

Introduction A diagnosis of frontotemporal dementia may be delayed or missed because early symptoms may develop gradually and can mimic symptoms of a variety of disorders or conditions. We aimed to write a report on a patient with frontotemporal dementia with bipolar disorder. Case report This is the case of a 63-year-old man with frontotemporal dementia whose presentation was consistent with bipolar affective disorder. With brain imaging and neurocognitive testing, frontotemporal dementia was diagnosed. Conclusion A differential diagnosis between bipolar disorder and frontotemporal dementia is difficult to establish. Frontotemporal dementia is a heterogeneous disease with a large variety of cognitive dysfunctions.

Olfactory Reference Syndrome (ORS) is a rare psychiatric condition in which the affected person has a certain belief that his/her body odor is foul or unpleasant and his/her occupation on this issue leads to a loss of functionality. ORS patients blame themselves because of the smell they emit and avoidance behaviors are evident. Major depressive disorder comorbidity rate is very high. In our case, we have an ORS patient with major depressive disorder (recurrent) and panic disorder comorbidity who suffers from avoidance behavior and loss of functionality due to the thoughts of emitting sweat and foot odor since 23 years. Sertraline- Olanzapine combination therapy and behavioral therapy has been applied; there was a significant improvement in the symptoms.

Frontotemporal demansın klinik gidişatında izlenen kişilik ve davranış değişimleri, apati, tekrarlayıcı kompulsiyon benzeri davranışlar, disnihibisyon, içgörü yokluğu özellikle erken evrelerde tanıda sorun oluşturur. Frontotemporal demans olgularına yanlış biçimde psikotik bozukluk, depresyon, geç başlangıçlı bipolar afektif bozukluk tanıları konabilir. Duygu durumu bozukluğu olan hastaların ilerleyen yaşlarda kognitif disfonksiyon ve demans gelişimi için risk taşıdığı da bildirilmiştir. Bu olgu serisinde uzun süre bipolar afektif bozukluk tanısı ile izlendikten sonra ileri yaşlarda frontotemporal demans tanısı alan 4 olgu bipolar afektif bozukluk ile frontotemporal demans arasındaki ilişkinin literatür ışığında incelenmesi amacıyla sunulmuştur.

Introduction In pregnant women with depression, there are a few case reports and studies showing the efficacy and reliability of transcranial magnetic stimulation. This report presents the acute and maintenance treatment results of transcranial magnetic stimulation in a pregnant woman who meets the diagnostic criteria for a major depressive episode. Case report A 24-year-old, 17-week pregnant housewife was admitted to ER with 10-week ongoing crying, distress, loss of appetite, weight loss, insomnia, sadness, loss of daily functioning, request of pregnancy termination and suicidal thoughts. Conclusion Transcranial magnetic stimulation is promising as an effective alternative in pregnant patients

Evliliğin her derde deva olmasını, yaşayacakları aşkla samanlıkların seyran olacağını bekleyen insanlar, çoğu zaman kendilerini mutsuz edecek koşullarla ve medeniyetin gerçekleriyle evlendiklerinin farkına varmazlar. Toplumsal hayatın gelişimi içinde, evlilik ilişkileri ve aile yaşantılarında bugün gelinen nokta, hem kadınların hem de erkeklerin psikolojik ve biyolojik dengelerini zorlar niteliktedir. İlişkileri sadece iki insan arasında olanlar değil, tabiat, sosyal yaşam, kişisel anlayışlar, evrim bilim, ekonomi, medeniyet etkiler. Bu sebeple psikoterapi uygulamalarında her evlilik ve aileye özel anlayış geliştirmemizi kolaylaştıracak zengin bir kültürel birikime ihtiyacımız vardır. Bu kitapta evlilik ve aile terapilerinin gelişimi, başlıca terapi modellerinin ana hatları, evlilik terapilerinin parçası olan cinsel terapi uygulamalarını özetledik. Ancak bir psikoterapist, evliliğe ve aileye yüzyıl öncesinde olduğu gibi geleneksel anlayışlar, inanışlar ve eldeki kuramlar ile yaklaşma kolaycılığına sahip değildir. Toplumda yaşanan değişim ve dönüşümün etkilerini dikkate alarak var olan aile ve eş terapisi kuramlarını yeniden yorumlama konusunda bazı önerilerimizi de bu kitapta sunduk.

TMU farmakolojik tedavilere yanıt vermeyen dirençli depresyonda da uygulanmış ve yalancı (sham) TMU'ya üstün bulunmuştur. Beynin her iki hemisferinin çeşitli bölgeleri farklı parametreler kullanılarak uyarılmıştır, ama araştırmalara katılan depresyon hastalarının %90'ına sol dorsolateral prefrontal kortekse yüksek frekanslı ve tekrarlayan uyarımlar verilmiştir. Yüksek frekanslı ve tekrarlayan TMU'nun en ciddi yan etkisinin, oldukça nadir görülmekle birlikte nöbet olduğu bildirilmektedir. Bu yazıda tekrarlayan TMU (tTMU) uygulaması sırasında jeneralize tonik-klonik nöbet geçiren bir olgu sunulmakta ve TMU ile indüklenen nöbet üzerine literatür gözden geçirilmektedir.

Despite the significance of major depressive disorder, objective procedures for selecting optimal treatments are lacking, there is a need for reliable and objective measures capable of differentiating between those who may or may not respond to specific treatments. Studies using neuroimaging, neurocognitive, and electrophysiologic measures have found that pre-treatment differences among depressed patients are related to subsequent clinical response to antidepressant drugs. Besides some clinical features and biological markers, the modern methods of brain imaging and quantitative electroencephalogram might be useful in prediction of treatment response.

Bu yazıda insan beyninin duyusal deneyimleri ile ilişkili bir gelişimsel teori sunulmaktadır. Bu teoriye göre, duyusal kapasite yaşam boyunca gelişir ve dürtü, duygu, hareket, algı gibi kendi duyusal ürünlerini yaratır.
