2. Acute leukemia with KMT2A rearrangement: A master of disguise

Abstract

Mixed-phenotype acute leukemia (MPAL) is a rare form of leukemia with ambiguous lineage, and there are challenges in accurately diagnosing this entity according to formal criteria. Here we report a case which was initially diagnosed as “AML” based on atypical peripheral blood flow cytometry that was subsequently determined to be B-ALL with KMT2A rearrangement based on marrow results. Although KMT2A rearrangements represent a defining genetic abnormality for acute leukemia of ambiguous lineage, this case did not meet the criteria for MPAL based on WHO 2022 criteria. This case highlights the diagnostic challenges of MPAL and the potential limitations of the current classification. We discuss the most appropriate workup and management of these patients and identify areas for future study.

Abstract

We describe the generation of sequences of random bits from the parity of photon counts produced by polarization measurements on a polarization-entangled state. The resulting sequences are bias free, pass the applicable tests in the NIST battery of statistical randomness tests, and are shown to be Borel normal, without the need for experimental calibration stages or postprocessing of the output. Because the photon counts are produced in the course of a measurement of the violation of the Clauser-Horne-Shimony-Holt inequality, we are able to concurrently verify the nonclassical nature of the photon statistics and estimate a lower bound on the min-entropy of the bit-generating source. Abstract

Randomness is a valuable resource in science, cryptography, engineering, and information technology. Quantum-mechanical sources of randomness are attractive because of the indeterminism of individual quantum processes. Here we consider the production of random bits from polarization measurements on photons. We first present a pedagogical discussion of how the quantum randomness inherent in such measurements is connected to quantum coherence, and how it can be quantified in terms of the quantum state and an associated entropy value known as min-entropy. We then explore these concepts by performing a series of single-photon experiments that are suitable for the undergraduate laboratory. We prepare photons in different nonentangled and entangled states, and measure these states tomographically. We use the information about the quantum state to determine, in terms of the min-entropy, the minimum amount of randomness produced from a given photon state by different bit-generating measurements. 9. UNLOCKING THE WEST: A CRITIQUE OF PREVIOUS STATUTORY ATTEMPTS AND A PROPOSED STATUTORY SOLUTION TO ALLOW ACCESS TO CORNER-LOCKED PUBLIC LAND

Abstract

Across the American West, 9.52 million acres of federal and state public lands are inaccessible to the public. The legality of accessing much of those public lands through corner-crossing is unclear in the law because, in the infinitesimal space where four corners of land meet in a checkerboard pattern, there is ambiguity; neither private landowners nor the public can assert an exclusive right or absolute control without infringing upon the rights of the other. This paper discusses the origins of landlocked public land, common law doctrines that inform legal actors in this space, and analyzes federal statute and state legislative attempts to clarify corner-crossing. Informed by principles of statutory interpretation, the paper systematically examines seven previous legislative attempts to legalize corner crossing in Wyoming, Nevada, Montana, and Colorado. It proposes a solution that progresses the Mountain West’s outdoor recreation heritage while respecting private property rights through a thoughtful addition to state code.

11. Assessment of high-dose acetylcysteine in acute high-risk paracetamol (acetaminophen) ingestion.

Abstract

Background: Prompt acetylcysteine treatment with standard doses (300 mg/kg over 21 h in divided doses) is almost universally effective in preventing hepatotoxicity after paracetamol (acetaminophen) overdose. However, hepatotoxicity is reported despite early treatment when paracetamol concentrations exceed 300 mg/L (1,985 μmol/L) at 4 h. Prior studies evaluating high-dose acetylcysteine to treat high-risk ingestions have shown mixed results. We compared outcomes in patients with high-risk ingestions receiving standard or high-dose acetylcysteine. Methods: Records from a single poison center were reviewed from 1 January 2017 to 31 December 2022. We included cases of acute paracetamol ingestion treated with intravenous acetylcysteine with an initial paracetamol concentration above the "300 mg/L" (1,985 μmol/L) line on the Rumack-Matthew nomogram. We compared standard and high-dose acetylcysteine groups by odds ratios and multivariable logistic regression. We defined hepatotoxicity as aminotransferase activity >1,000 U/L. Results: We included 190 cases. Fifty-six percent received standard-dose acetylcysteine while 44% received high-dose acetylcysteine. Treatment within 8 h yielded no difference in hepatotoxicity between groups (odds ratio 1.67, 95% CI 0.067–42.3). Among patients treated after 8 h, hepatoxicity was more common in the high-dose group (odds ratio 3.39, 95% CI 1.25–9.2) though odds of liver failure were similar (odds ratio 2.78, 95% CI 0.89–8.69). Eighty-eight percent of patients with hepatotoxicity had elevated aminotransferase activity at presentation. No patient died or received a liver transplant. Discussion: Rates of hepatotoxicity were low in patients treated within 8 h regardless of acetylcysteine dose. Unexpectedly, high-dose acetylcysteine treatment was associated with an increased odds of hepatoxicity in those treated after 8 h, but most had abnormal aminotransferase activities at presentation and there was no difference in rates of liver failure. Limitations include the use of retrospective, voluntarily reported poison center data. Conclusions: Prompt treatment with acetylcysteine, regardless of dose, prevented hepatotoxicity in high-risk paracetamol ingestion. [ABSTRACT FROM AUTHOR]

We investigate the concept of effective resistance in connection graphs, expanding its traditional application from undirected graphs. We propose a robust definition of effective resistance in connection graphs by focusing on the duality of Dirichlet-type and Poisson-type problems on connection graphs. Additionally, we delve into random walks, taking into account both node transitions and vector rotations. This approach introduces novel concepts of effective conductance and resistance matrices for connection graphs, capturing mean rotation matrices corresponding to random walk transitions. 14. To Get-to-Know-You or Not to Get-to-Know-You: A Two Phase Study of Initial Engagement Activities

Abstract

In the pedagogy of classroom engagement, most instructors have become vastly familiar with first day of class Get-to-Know-You exercises. While entertaining, the empirical value of these exercises is not well established. The present analysis provides a data driven study of the utility of initial engagement activities, including a generalized Get-to-Know-You exercise as well as, specifically, the on-going "Photo Roster" activity by evaluating cross sectional data. An analysis of Time One and Time Two data from 140 students builds upon previous investigations while correcting for methodological issues of past studies. We establish that students' use of the Photo Roster Get-to-Know-You procedure results in higher levels of classmate liking, instructor liking, and classmate name recall as compared with students in the traditional Get-to-Know-You exercise group at its initial time or over time. No difference was found between groups for elements of anxiety reduction, student empowerment, or group immediacy. (Contains 2 tables.)

30. Discerning the concentration and bi-directional flux of ammonia in an urban estuary using the relaxed eddy accumulation method

Abstract

Narragansett Bay, the largest estuary in the northeastern U.S., is a heavily urbanized watershed impacted by deposition and runoff. Nutrient budgets and local policy rely on deposition data from a 1990 study that did not include any direct observations of dry deposition of gaseous ammonia (NH3) and particulate ammonium (NH4+) due to uncertainties in their flux direction and measurement difficulty. Recent precipitation measurements show that wet deposition of NH4+ to the Bay has increased by a factor of 6 over the past three decades, leading to a 2.5-fold increase in wet nitrogen (N) deposition. The documented increase in wet deposition of NH4+ concurrent with managed nutrient reductions in urbanized estuaries has potentially increased the impacts of atmospheric N deposition, including the dry deposition of NH3 and NH4+. However, a scarcity of measurements hinders our interpretation of this important N source. For the first time over Narragansett Bay, and to our knowledge over open water, dry (particulate and gas phase) deposition of total NHx (NHx = NH3 + NH4+) and bidirectional NH3 flux was quantified using a relaxed eddy accumulation sampling technique. We find that total N entering the Bay from the atmosphere has doubled since 1990. Dry deposition of NHx comprises 9.6% of total N deposition. During the fall season, the dominant flux direction for NH3 is upward, which also has implications for urban air quality. We estimate that NH3 emitted from the Bay to the atmosphere makes up to 10% of the local NH3 emission budget.

Abstract

Anthropogenic changes to nitrogen cycling have dramatically altered ecosystems across the globe, particularly in coastal estuaries where nitrogen limits primary production. The impacts of nitrogen pollution are especially evident in Narragansett Bay, RI, where eutrophication in the urbanized Providence River Estuary persists despite a > 50% decrease in wastewater nitrogen loading over the past two decades. Precipitation-derived nitrogen tends to be understudied and may be underestimated in contemporary budgets. Here, the magnitude and ecological impacts of precipitation-derived nitrogen in the Providence River Estuary were assessed by the following: relationships between precipitation, river discharge, and chlorophyll abundance; inorganic nitrogen concentration and isotopic composition of precipitation, surface runoff, and stormwater; and changes in stable nitrogen (δ15N) and carbon (δ13C) isotopes of macroalgae (Ulva spp.) after large precipitation events. Precipitation and chlorophyll concentration measured 5 days after the rain event exhibited a linear relationship. Furthermore, Ulva spp. experienced decreases in δ15N of 0.3–1‰ and increases in δ13C of 1–3‰ that peaked 5 days after precipitation events. These results are indicative of a direct impact of precipitation-derived nitrogen on nutrient dynamics, including summer phytoplankton blooms. However, the inorganic nitrogen concentrations of stormwater were approximately 240% higher than surface runoff, and the isotopic composition of stormwater discharge indicates that most precipitation-derived nitrate stems from terrestrial nitrogen and sewage rather than direct atmospheric deposition. The magnitude and spatial heterogeneity of precipitation-derived nitrogen remains uncertain, and further work is needed to understand how this nitrogen source will change in a wetter climate of the future.

39. Seasonal Skin Colour Changes in a Sample Teenage Population Measured by Reflection Spectrophotometry

Abstract

As part of a classroom-based research project, reflectance spectra from the skin of a group of teenage school students were recorded over a four-month period, from early spring to mid-summer. The relative changes in skin colour during the course of the study were quantified by integrating over the full wavelength range of the normalized reflectance spectra. Measurements made upon the inner forearm and the back of the hand produce results which indicate a decrease in total reflectance (increase in absorption) corresponding to different levels of tanning for limited and extended exposure to ambient sunlight, respectively. The rate of change of skin colour qualitatively matches that of the solar illuminance. (Contains 1 table and 6 figures.)

Abstract

We report a case of myasthenia gravis, myocarditis, and myositis following the treatment of melanoma with nivolumab. The patient was a 68-year-old Caucasian male with stage 3 melanoma status after two doses of nivolumab with shortness of breath, intermittent palpitations, dizziness, and nausea. During his initial evaluation, he was found to have atrial fibrillation with rapid ventricular response along with new-onset proximal muscle weakness, double vision, dysphagia, and ptosis of the right eye. Further diagnostic workup of the pleural effusion with CT of the chest showed large right pleural effusion with adjacent atelectasis. Thoracentesis was completed without complications and resulted in an exudative effusion with negative cytology and cultures. Serologic studies showed elevated troponin and serum creatine kinase, negative acetylcholine receptor antibody, and negative modulating antibody. Despite negative antibody tests, the patient's symptoms suggested a clinical diagnosis of myasthenia gravis. The ice pack test was performed, which showed temporary improvement of the patient's ptosis. Given the suspicion for myasthenia gravis and positive ice pack test, he was treated with corticosteroids, intravenous immunoglobulin (IVIG), and pyridostigmine. He completed a total of three doses of IVIG with improvement in diplopia. Despite steroids and respiratory support with BiPAP (bilevel positive airway pressure), on the 14th day of hospitalization, the patient had multiple organ failure along with worsening respiratory failure. The patient discussed the situation with his family, and they decided on hospice care. The patient was discharged to hospice on admission day 14.
