Abstract

Introduction: The American Society of Anesthesiologists (ASA) classification is used to assess the fitness of a patient for surgery. Whether laparoscopic surgery is appropriate for colorectal cancer patients with poor ASA performance status (PS) remains unclear. Methods: Among 4585 patients who underwent colorectal surgery between 2016 and 2023, this study retrospectively reviewed all 458 patients with ASA‐PS ≥3. Patients were divided into two groups: patients treated by open surgery (O group, n = 80); and patients treated by laparoscopic surgery (L group, n = 378). We investigated the impact of surgical approach on postoperative complications in patients with colorectal cancer and ASA‐PS ≥3. Results: Operation time was longer (170 min vs. 233 min, p <.001), blood loss was less (156 mL vs. 23 mL, p <.001), postoperative complications were less frequent (40.0% vs. 25.1%, p =.008), and hospital stay was shorter (23 days vs. 14 days, p <.001) in L group. Univariate analysis revealed rectal cancer, open surgery, longer operation time, and blood loss as factors significantly associated with postoperative complications. Multivariate analysis revealed open surgery (odds ratio [OR] 2.100, 95% confidence interval [CI] 1.164–3.788; p =.013) and longer operation time (OR 1.747, 95% CI 1.098–2.778; p =.018) as independent predictors of postoperative complications. Conclusion: Laparoscopic surgery provides favorable outcomes for colorectal cancer patients with poor ASA‐PS. [ABSTRACT FROM AUTHOR]

Elderly patients in general tend to have comorbidities and decreased organ function that potentially influence the NLR score. The aim of this study was to investigate the relationship between NLR and cancer-specific survival in elderly patients with colon cancer, using a propensity score-matched analysis.Patients and Methods: A total of 203 patients aged over 75 years who underwent curative resection for colon cancer and were diagnosed pathologically with stage II/III disease were eligible for entry to the study. Patients were divided into two groups according to NLR score: NLR-High (NLR≥ 4.5) group (NLR-H, n=60) and NLR-Low (NLR< 4.5) group (NLR-L, n=143). After propensity score matching, 57 patients in each group were matched.Results: Before matching, Charlson comorbidity index was significantly higher in the NLR-H group (4 vs 2, p< 0.001). After matching, all factors were similar between the groups. The median follow-up period was 43 months (range, 1– 160 months). Five-year relapse-free-survival (69.8% vs 87.3%, p=0.030) and cancer-specific survival (83.0% vs 96.0%, p=0.042) were significantly lower in the NLR-H group.Conclusion: NLR appears to be a cancer-specific prognostic marker in elderly patients with colon cancer.Keywords: cancer-specific survival, colon cancer, neutrophil-to-lymphocyte ratio

Abstract

Purpose: The impact of neoadjuvant chemotherapy for locally advanced colorectal cancer has not yet been investigated; thus, this study aimed to examine the safety, feasibility, and oncological effects of neoadjuvant chemotherapy for locally advanced colorectal cancer. Methods: In this multicenter study, we retrospectively reviewed the data of 83 locally advanced colorectal cancer patients (cT3/4 or N1/2) who received neoadjuvant chemotherapy followed by radical resection between April 2016 and September 2020. The NAC regimens were FOLFOX (5-fluorouracil, leucovorin, and oxaliplatin), XELOX (capecitabine and oxaliplatin), or SOX (S-1 and oxaliplatin). We evaluated the pathological responses as well as the short- and long-term outcomes. Results: A pathological complete response was achieved in 4 patients (4.8%). Tumor down-staging and nodal down-staging were achieved in 57 (68.7%) and 49 (59.0%) patients, respectively. One patient (1.2%) experienced progressive disease. Postoperative complications occurred in 21 patients (25.3%). Multivariate analysis revealed that the pathological N positive status (p = 0.015; odds ratio, 4.458; 95% confidence interval, 1.331 to 7.9300) was an independent predictive factor for relapse-free survival. Conclusion: Neoadjuvant chemotherapy for colorectal cancer could achieve good tumor control and down-staging without increasing the rate of complications. Abstract

Robotic surgery is increasingly being applied for rectal cancer and its feasibility and safety have been reported. However, problems associated with advanced robotic surgery such as pelvic exenteration include lengthy operation time and difficulty in controlling unexpected bleeding. A 47-year-old woman had undergone laparoscopic left hemicolectomy for descending colon cancer three years previously (pT3N0M0 pStageII). And had undergone bilateral oophorectomy for ovarian metastases one year previously. Follow-up CT detected a peritoneal metastasis in the pelvic space. After seven courses of systemic chemotherapy, she received robotic anal preserving posterior pelvic exenteration combined with the transanal-vaginal approach. The postoperative course was uneventful. There is no evidence of recurrent disease 8 months after surgery. In conclusion, robotic anal preserving posterior pelvic exenteration combined with the transanal-vaginal approach is a safe and feasible minimally invasive approach for the treatment of advanced rectal malignancies., Acta medica Nagasakiensia, 66(1), pp.35-39; 2022

Abstract

Introduction: Thirty‐day reoperation rate reflects short‐term surgical outcomes following surgery. Laparoscopic surgery for colorectal cancer reportedly has positive effects on postoperative complications. This retrospective study investigated risk factors for 30‐day reoperation rate among patients after laparoscopic colorectal cancer surgery using a multicenter database. Methods: Participants comprised 3037 patients who had undergone laparoscopic resection of colorectal cancer between April 2016 and December 2022 at the Nagasaki University and six affiliated centers, classified into those who had undergone reoperation within 30 days after surgery (RO group; n = 88) and those who had not (NRO group; n = 2949). Clinicopathological characteristics were compared between groups. Results: In the RO group, anastomotic leakage occurred in 57 patients (64.8%), intestinal obstruction in 12 (13.6%), and intraabdominal abscess in 5 (5.7%). Female patients were more frequent, preoperative treatment less frequent, body mass index (BMI) lower, operation time longer, blood loss greater, and hospital stay longer in the RO group (p <.05 each). Multivariate analysis revealed BMI (odds ratio, 0.415; 95% confidence interval, 0.218–0.787; p =.021) and poor performance status (odds ratio, 1.966; 95% confidence interval, 1.106–3.492; p =.021) as independent predictors of reoperation. Conclusion: Perioperative measures are warranted for patients with low BMI and poor performance status undergoing laparoscopic colorectal surgery. [ABSTRACT FROM AUTHOR]

Abstract

Background/Aim: Japanese Gastric Cancer Treatment Guidelines do not recommend adjuvant chemotherapy after radical gastrectomy for pathological stage (p) T1N+ or pT2~3N0 gastric cancer. However, some patients experience disease recurrence. This study aimed to identify the risk factors for recurrence in pT1N+ or pT2-3N0 gastric cancer. Patients and Methods: The study included 157 patients with diagnosed pT1N+ or pT2-3N0 gastric cancer who underwent radical gastrectomy at our institution between January 2001 and December 2020. Clinicopathological data and surgical data were obtained. Independent prognostic factors were analyzed using a Cox proportional hazards regression model. Results: Thirteen patients (8.3%) experienced disease recurrence. Multivariate analysis revealed that the number of examined lymph nodes was an independent prognostic factor for recurrence-free survival (hazard ratio=10.90; 95% confidence interval=1.39-85.86; p=0.023). The group with ≤35 examined lymph nodes had significantly worse recurrence-free survival compared with the group with ≥36 examined lymph nodes (80.7% versus 98.7%; p=0.0005). Conclusion: The number of examined lymph nodes (≤35) was an independent risk factor for recurrence after radical gastrectomy with pT1N+ or pT2-3N0 gastric cancer.
