Objective: The primary goal of the Campania Oncology Network (ROC) was to reduce cancer delay and care fragmentation through the establishment of cancer-specific multidisciplinary oncologic groups (GOMs) and diagnostic and therapeutic assistance paths (PDTAs)., Methods: Five cancer centres of the ROC, with their own cancer specific GOM, were selected. In our analysis, we have focused on four neoplasms: lung, colon, ovarian and prostate cancers. The median time for pre-GOM and GOM Times was calculated for each tumour site. Univariate and multivariate logistic regressions were performed to individuate risk factors for pre-GOM and GOM Time., Results: Significant differences were observed for prostate cancer compared to other patients either for pre-GOM or GOM Times. Significant risks were found for ovarian and prostate cancers in pre-GOM time and for prostate cancer in GOM-Time., Conclusions: This experience will produce knowledge and data to guide decision-making and to manage more effectively the challenges of fighting cancer in Campania region. The Valutazione Percorso Rete Oncologica Campana (ValPeROC) study evaluates, for the first time, the ROC activity, through the analysis of key performance indices. COVID-19 pandemic following the outbreak in China and Western Europe, where it finally lost the momentum, is now devastating North and South America. It has not been identified the reason and the molecular mechanisms of the two different patterns of the pulmonary host responses to the virus from a minimal disease in young subjects to a severe distress syndrome (ARDS) in older subjects, particularly those with previous chronic diseases (including diabetes) and cancer. The Management of the Istituto Nazionale Tumori - IRCCS "Fondazione Pascale" in Naples (INT-Pascale), along with all Health professionals decided not to interrupt the treatment of those hospitalized and to continue, even if after a careful triage in order not to allow SARS-CoV-2 positive subjects to access, to take care of cancer patients with serious conditions. Although very few (n = 3) patients developed a symptomatic COVID-19 and required the transfer to a COVID-19 area of the Institute, no patients died during the hospitalization and completed their oncology treatment. Besides monitoring of the patients, all employees of the Institute (physicians, nurses, researchers, lawyers, accountants, gatekeepers, guardians, janitors) have been tested for a possible exposure. Personnel identified as positive, has been promptly subjected to home quarantine and subdued to health surveillance. One severe case of respiratory distress has been reported in a positive employees and one death of a family member. Further steps to home monitoring of COVID-19 clinical course have been taken with the development of remote Wi-Fi connected digital devices for the detection of early signs of respiratory distress, including heart rate and oxygen saturation.In conclusion cancer care has been performed and continued safely also during COVID-19 pandemic and further remote home strategies are in progress to ensure the appropriate monitoring of cancer patients.

A common complication of tooth replantation after traumatic avulsion is tooth ankylosis causing dental malpositioning, partial loss of function, tipping of adjacent teeth and worsening of aesthetics. The aim of this study is to evaluate the effects of a repositioning procedure of such ankylosed permanent front teeth by a distraction osteogenesis procedure. Five patients (mean age 13.4 years), with an ankylosed permanent front tooth in the anterior area, were enrolled in the present study. After the pre-operatory orthodontic preparation, each selected site was treated with single-tooth dento-alveolar block osteotomy surgery performed with a piezoelectric surgery device with ultrasonic cuts on the buccal side. Subsequently, a custom distraction device, made by a resin splint and a sector expansion screw, was set in such a way it delivered a force with direction and sense towards the planned position of the tooth as well as the osteotomy's incision. After a latency period of 14 days, the distraction of the dento-alveolar block was started with a rate of distraction of 0.8 mm per day. The average shift obtained by the ankylosed teeth was 7.8 ± 0.75 mm (±SD), and the ankylosed tooth was regularly positioned into the occlusion in 20.4 ± 1.85 days (±SD), with a slight relapse (±0.5 mm) observed after 1 year. This study indicates that a therapeutic approach combining piezoelectric surgery and orthodontic therapy may be useful for the treatment of ankylosed permanent teeth in the frontal area with a long-term follow-up over 5 years.

The purpose of this study was to histologically evaluate new bone formation and dimensional soft tissue changes of two different healing protocols (16 weeks and 32 weeks) using deproteinized bovine bone mineral (DBBM) covered with collagen matrix (CM) for alveolar ridge preservation in the anterior esthetic zone prior to dental implant placement. Compared to baseline, both treatments yielded statistically significant differences in several clinical parameters and in the microarchitecture of the native bone and in the newly formed bone in the augmented sites. However, the protocol at 32 weeks determined greater new vital bone formation and fewer dimensional tissue changes.

Aim: The aim of this study was to identify potential predictive factors for cervical disease in women with HIV and to evaluate adherence during follow-up to cervical cancer screening., Methods: In order to identify the independent role of factors associated with the presence of a cervical abnormality, all of the variables showing in univariate analyses a potential association with the outcome variable (presence of cervical abnormalities) were entered into a multivariate logistic regression model, along with age at first visit to our center, and age at diagnosis., Results: A total of 540 HIV-positive women who received screening for cervical cancer during the first year after their first visit to our center were included in the analysis; 423 (78.3%) had normal cytology and 117 (21.7%) had cytological abnormalities, classified as follows: 21 atypical squamous cells of undetermined significance (17.9%); 51 low-grade squamous intraepithelial lesions (43.6%); 41 high-grade squamous intraepithelial lesions (35.0%); and four cervical cancers (3.4%). In our study, women with more than two previous pregnancies were significantly associated with a lower risk of cervical cytological abnormalities compared to the other women. Women with CD4+ levels of 200-499/mm 3 had a higher risk of developing cervical cytological abnormalities compared to those with a CD4+ level > 500/ mm 3 ., Conclusion: In summary, management of HIV-positive women must be modeled on HIV-clinical status, CD4+ cell count, drug regimen, and adherence to follow-up, relying on the cooperation of highly qualified professionals. Objectives: The present two-year prospective cohort study was undertaken to evaluate marginal masticatory mucosa dimensional changes around immediate post-extractive implants positioned transgingivally with a non-submerged healing screw., Material and Methods: Twenty-one immediate post-extractive implants from 21 patients were enrolled, peri-implant gap was filled with bovine bone mineral, and soft tissue was allowed to heal around a non-submerged healing screw. Post-extractive socket dimension was recorded. Intraoperative (T0) vertical distances: bone margin level (BML) from the bone margin to the implant platform and mucosal margin height (MMH) from marginal mucosa to implant platform were taken; MMH measurement was repeated 4 months later (T4). Horizontal mucosal level (HML): from customized stent to marginal mucosa at 0, 4, 12, and 24 months postoperatively (T0, T4, T12, T24) and vertical mucosal level (VML): from the stent to marginal mucosa at T4, T12, T24 were registered., Results: One implant failed at 3 weeks; in the remaining 20 cases the MMH, coronally positioned with respect to the BML ≅2 mm at T0, showed a statistically significant vertical contraction of the mucosa at T4. Other vertical mucosal measurements (VML) did not show further changes over time. HML measures showed a, statistically significant, shrinkage of the mucosa on the transverse plane between T0/T12 and T0/T24 and between T4/T12 and T4/T24., Conclusions: Immediate post-extractive implant inserted transgingivally with a non-submerged healing screw and internal peri-implant gap filled with bovine bone mineral may favor an early and stable peri-implant soft tissue healing over 2 years., (© 2014 John Wiley & Sons A/S. Nevoid basal cell carcinoma syndrome (NBCCS), an autosomal dominant disorder with a high degree of penetrance and variable expressivity, is characterized by basal cell carcinomas, odontogenic keratocysts, palmar and/or plantar pits, and ectopic calcifications of the falx cerebri. More than 100 minor criteria have been described, but 2 major and 1 minor criteria or 1 major and 3 minor criteria are necessary for the diagnosis. In this report we present an 8-year-old girl affected by NBCCS showing a uterus bicornis, a hitherto unreported association. However, further research is needed to confirm the association between NBCCS and mullerian fusion defects and to assess the hypothesis that focuses on chromosome 9 the mutant gene for NBCCS and fusion defects of female genital tract.

Aim: The aim of this study was the clinical and radiographic evaluation of peri-implant tissues healing associated with two-stage implants performed with one-stage surgery and placed into post-extraction sockets of maxillary premolars., Methods: Ten natural tapered submerged titanium implants with double acid-etched surface were installed in post-extraction sockets of maxillary premolars showing no bone dehiscence or fenestrations. Implants were selected according to the greatest dimension compatible to vertical and horizontal diameters of the post-extraction sockets. Peri-implant defects showing more than 2 mm of distance between the marginal palatal bone and the implant margin, were not treated with a regenerative procedure but with a bone swaging technique by means of piezoelectric surgery. At 16 weeks osteointegration of implants was examined with a clinical and standardized radiographic evaluation., Results: All patients showed good clinical healing without any complication. At 16 weeks all implants, either those with reduced marginal defect or those with modified crestal bone profile, were osteointegrated., Conclusions: The use of two-stage implants performed with one-stage surgery in 4 walls post-extraction sockets of maxillary premolars, seems to allow osteointegration both in reduced peri-implant horizontal defects as well as in defects larger than 2 mm. Furthermore, these procedures seem to simplify the management of soft peri-implant tissues.
