Abstract

The prevention of hospital-acquired pressure injuries (HAPIs) in children undergoing long-duration surgical procedures is of critical importance due to the potential for catastrophic sequelae of these generally preventable injuries for the child and their family. Long-duration surgical procedures in children have the potential to result in high rates of HAPI due to physiological factors and the difficulty or impossibility of repositioning these patients intraoperatively. We developed and implemented a multi-modal, multi-disciplinary translational HAPI prevention quality improvement program at a large European Paediatric University Teaching Hospital. The intervention comprised the establishment of wound prevention teams, modified HAPI risk assessment tools, specific education, and the use of prophylactic dressings and fluidized positioners during long-duration surgical procedures. As part of the evaluation of the effectiveness of the program in reducing intraoperative HAPI, we conducted a prospective cohort study of 200 children undergoing long-duration surgical procedures and compared their outcomes with a matched historical cohort of 200 children who had undergone similar surgery the previous year. The findings demonstrated a reduction in HAPI in the intervention cohort of 80% (p < 0.01) compared to the comparator group when controlling for age, pathology, comorbidity, and surgical duration. We believe that the findings demonstrate that it is possible to significantly decrease HAPI incidence in these highly vulnerable children by using an evidence-based, multi-modal, multidisciplinary HAPI prevention strategy.

Abstract

Objectives The early detection and stratification of asphyxiated infants at higher risk for impaired neurodevelopment is challenging. S100B protein is a well-established biomarker of brain damage, but lacks conclusive validation according to the "gold standard" methodology for hypoxic-ischemic encephalopathy (HIE) prognostication, i.e. brain MRI. The aim of the present study was to investigate the predictive role of urinary S100B concentrations, assessed in a cohort of HIE infants receiving therapeutic hypothermia (TH), compared to brain MRI. Methods Assessment of urine S100B concentrations was performed by immunoluminometric assay at first void and at 4, 8, 12, 16, 20, 24, 48, 72, 96, 108 and 120-h after birth. Neurologic evaluation, routine laboratory parameters, amplitude-integrated electroencephalography, and cerebral ultrasound were performed according to standard protocols. Brain MRI was performed at 7-10 days of life. Results Overall, 74 HIE neonates receiving TH were included in the study. S100B correlated, already at first void, with the MRI patterns with higher concentrations in infants with the most severe MRI lesions. Conclusions High S100B urine levels soon after birth constitute trustable predictors of brain injury as confirmed by MRI. Results support the reliability of S100B in clinical daily practice and open the way to its inclusion in the panel of parameters used for the selection of cases suitable for TH treatment.

Abstract

MR imaging provides means for discriminating different patterns of Hypoxic-ischemic encephalopathy (HIE) and may distinguish most severe cases from less severe but is unable to predict long-term outcome. Diffusion tensor imaging (DTI) offers information for a more complete characterization of HIE. The purpose of this study is to compare the modifications of DTI parameters in newborns one week and six months following total-body cooling to healthy controls. Forty-seven cooled newborns were studied with MRI, 20 underwent follow-up at 6 months. 12 healthy newborns and nine children at 6 months were enrolled as control groups (HC). Inferior Longitudinal Fasciculus (ILF), Corpus Callosum Fasciculus (CCF), Corticospinal Tract (CST), Optical Tract (OT), Optic Radiation (OR) were generated in all subjects. DTI parameters were evaluated in basal ganglia (BG), thalamus (TH) and tracks. Statistical analysis was performed with MANOVA. In newborns HIE versus HC, there were significantly lower fractional anisotropy (FA) on OR and CST and higher axial diffusivity (AD), apparent diffusion coefficient (ADC) and radial diffusivity (RD) values on CST, BG and TH in HIE-N. At 6 months there were no significant grouping effects. The analysis showed a significant increase of FA, decrease of ADC, AD, RD after 6 months for HIE and HC. We observed modifications of parameter values in HIE newborns vs HC; however normalization of values at 6 months suggests that changes of parameters cannot be considered early biomarkers for evaluation of therapeutic hypothermia in newborns with moderate HIE and normal conventional MRI. Abstract

The aim of this work is to establish inclusive guidelines on electroencephalography (EEG) applicable to all neonatal intensive care units (NICUs). Guidelines on ideal EEG monitoring for neonates are available, but there are significant barriers to their implementation in many centres around the world. These includebarriers due to limited resources regarding the availability of equipment and technical and interpretive round-the-clock personnel. On the other hand, despite its limitations, amplitude-integrated EEG (aEEG) (previously called Cerebral Function Monitor [CFM]) is a common alternative used in NICUs.The Italian Neonatal Seizure Collaborative Network (INNESCO), working with all national scientific societies interested in the field of neonatal clinical neurophysiology, performed a systematic literature review and promoted interdisciplinary discussions among experts (neonatologists, paediatric neurologists, neurophysiologists, technicians) between 2017 and 2020 with the aim of elaborating shared recommendations.A consensus statement on videoEEG (vEEG) and aEEG for the principal neonatal indications was established. The authors propose a flexible frame of recommendations based on the complementary use of vEEG and aEEG applicable to the various neonatal units with different levels of complexity according to local resources and specific patient features. Suggestions for promoting cooperation between neonatologists, paediatric neurologists, and neurophysiologists, organisational restructuring, and teleneurophysiology implementation are provided.(c) 2021 International Federation of Clinical Neurophysiology. Abstract

The occurrence of multiple primary melanomas(MPM) is quite rare with an estimated incidence ranging from 0.2% to 8.6%(Buljan M et al.2010). However, the risk of developing a primary melanoma is increased in patients with a history of a previous melanoma compared with the general population (De Giorgi et al.,2012). Between 6 and 14% of MPM cases occur in individuals with a family history of disease (Ang CG et al.1998), suggesting a genetic component in the pathogenesis of MPM. Germline mutations in the CDKN2A gene have been widely reported as the most common cause of inherited susceptibility to melanoma (Palmieri G et al., 2009). In the literature,CDKN2A germline mutations have been identified in less than 2% of single primary melanoma cases, 8 to 12% of sporadic MPM cases and 47% of MPM patients with familial melanoma (De Giorgi V et al.,2014;Puig S et al.,2005). Abstract

Introduzione: Le differenze geografiche nell'incidenza del melanoma e nella penetranza delle mutazioni di CDKN2A, principale gene di suscettibilità al melanoma, sono tali che non esistono criteri per indirizzare un paziente al test genetico applicabili in modo uniforme a livello internazionale. Leachman et al. hanno proposto che due eventi oncologici (melanoma e/o adenocarcinoma pancreatico, associato anch'esso a mutazioni di CDKN2A) in un singolo paziente o in un famiglia siano considerati criterio sufficiente per il test genetico nelle aree a bassa incidenza di melanoma(1). L'Italia è inclusa fra queste aree sulla base dei dati liguri in cui mutazioni da effetto fondatore sono state riscontrate anche nel 40% dei casi familiari di melanoma. Lo studio basato sulle raccomandazioni SIGU per l'ammissione al test per il melanoma familiare ha riscontrato mutazioni di CDKN2A nel 25% dei casi familiari con due soli affetti (2), che aumentavano in presenza di casi di melanoma multiplo (MPM). Altri geni di predisposizione al melanoma sono stati identificati di recente ma la penetranza delle nuove mutazioni non è ancora stata chiarita. Scopo principale del nostro studio è stato di valutare su base nazionale la prevalenza di mutazione in CDKN2A, CDK4 e MITF in casi di MPM per stabilire se anche in assenza di familiarità lo sviluppo di MPM possa essere considerato criterio sufficiente di avvio al test genetico. Materiali e metodi: 587 casi di melanoma multiplo e un numero uguale di casi di melanoma singolo e controlli sono stati consecutivamente reclutati nei centri partecipanti e analizzati per CDKN2A, CDK4 e MITF. Risultati Mutazioni di CDKN2A sono state riscontrate nel 19% dei casi di melanoma multiplo e nel 4.4% dei casi di melanoma singolo, compresi melanomi in situ. Nei casi multipli familiari il rate di mutazione varia dal 36.6% al 58.8% mentre nei casi sporadici dall'8.2% al 17.6% nei casi con due o con 3 o più melanomi, rispettivamente. La mutazione MITF E318K è stata riscontrata nel 3% dei casi di melanoma multiplo. Conclusioni I pazienti italiani con due melanomi, anche in situ, e senza familiarità dovrebbero essere indirizzati alla valutazione genetica. Bibliografia 1.Leachman, S. A. et al. Abstract

Summary: Background: Recent studies have shown an increasing incidence of cutaneous adnexal carcinomas (CACs). Objectives: The aim of our study was to evaluate incidence and survival for cases of CACs and investigate their association with other skin neoplasms. Methods: We conducted a population‐based study. Data on incident cases of CACs were obtained from the Tuscany Cancer Registry between 1985 and 2010. In order to determine whether the occurrence of squamous cell carcinoma (SCC) among patients with CAC is higher or lower than expected in the general population, the standardized incidence ratio (SIR) was calculated. Results: A total of 242 patients with CAC were observed; the age‐standardized incidence rate was 3·8 cases per million person‐years. From 1997 to 2010 crude incidence rates increased by 159%. Age‐specific incidence was higher in men over 80 years old than in women of the same age and younger individuals. Carcinomas of sweat gland origin prevailed; the most common histotype was porocarcinoma and the most frequently affected site was the head/neck. Overall, 88% of CACs were diagnosed at a localized stage. The 5‐year overall survival and disease‐specific survival rates were 59% [95% confidence interval (CI) 53–65] and 94% (95% CI 91–98), respectively. In the observation cohort, the number of SCCs was significantly higher than expected as the SIR was calculated to be 33·7 (P < 0·001). Conclusions: Increasing incidence warrants awareness and early diagnosis of CACs. Increased SCC incidence among patients with these tumours highlights the relevance of careful skin examination and follow‐up. What's already known about this topic? Cutaneous adnexal carcinomas (CACs) are rare tumours, but their incidence is increasing.The rarity of these tumours has made it difficult to gain sufficient clinicopathological experience regarding the diagnosis and treatment of CACs.CACs can be a diagnostic challenge owing to mostly nonspecific clinical presentation and histopathological variety that often requires specific expertise for a definite diagnosis. Abstract

Objective: The aim of this study was to assess visual function in 13 infants with evidence of prenatal post haemorrhagic ventricular dilatation. Design: Infants were assessed at 5, 12 and 24 months using a battery of tests specifically designed to assess various aspects of visual function in infancy. Visual findings were correlated with several variables, including extent of the lesion and presence of epilepsy. Results and conclusions: Abnormalities of visual function were frequent (over 60%) in our cohort at age 2 years, ranging from isolated abnormal ocular movements to severe abnormalities of all the aspects of visual function assessed. The most severe and persistent abnormalities of visual function were found in infants with grade IV intraventricular haemorrhage and shunted hydrocephalus who also had epilepsy in the first year.

Abstract

Although total drug levels were measured one hour after administration of the wrong dose (far from the steady state), our patient was already under treatment with digoxin and amiodarone. Dear editor, Digoxin is a cardioactive drug with a narrow therapeutic window. Moreover, considering also a possible rebound toxicity, due to concomitant amiodarone administration, we measured both free and total digoxin fractions to monitor the quantity of free active drug fraction.
