Abstract

Objective: To test the a priori hypothesis that longer duration of ruptured membranes is associated with increased risk of vertical transmission of HIV. Design: The relationship between duration of ruptured membranes and vertical transmission of HIV was evaluated in an individual patient data meta-analysis. Methods: Eligible studies were prospective cohort studies including at least 100 mother-child pairs, from regions where HIV-infected women are counselled not to breastfeed. Analyses were restricted to vaginal deliveries and non-elective Cesarean sections; elective Cesarean section deliveries (those performed before onset of labour and before rupture of membranes) were excluded. Results: The primary analysis included 4721 deliveries with duration of ruptured membranes ≤ 24 h. After adjusting for other factors known to be associated with vertical transmission using logistic regression analysis to assess the strength of the relationship, the risk of vertical HIV transmission increased approximately 2% with an increase of 1 h in the duration of ruptured membranes [adjusted odds ratio, 1.02; 95% confidence interval, 1.01-1.04; for each 1 h increment]. There were no significant interactions of duration of ruptured membranes with study cohort or with any of the covariates, except maternal AIDS. Among women diagnosed with AIDS, the estimated probability of transmission increased from 8% to 31% with duration of ruptured membranes of 2 h and 24 h respectively (P < 0.01). Conclusions: These results support the importance of duration of ruptured membranes as a risk factor for vertical transmission of HIV and suggest that a diagnosis of AIDS in the mother at the time of delivery may potentiate the effect of duration of ruptured membranes. © 2001 Lippincott Williams & Wilkins.

Abstract

Background To evaluate the relation between elective cesarean section and vertical transmission of human immunodeficiency virus type 1 (HIV-1), we performed a meta-analysis using data on individual patients from 15 prospective cohort studies. Methods North American and European studies of at least 100 mother-child pairs were included in the meta-analysis. Uniform definitions of modes of delivery were used. Elective cesarean sections were defined as those performed before onset of labor and rupture of membranes. Multivariate logistic-regression analysis was used to adjust for other factors known to be associated with vertical transmission. Results The primary analysis included data on 8533 mother-child pairs. After adjustment for receipt of antiretroviral therapy, maternal stage of disease, and infant birth weight, the likelihood of vertical transmission of HIV-1 was decreased by approximately 50 percent with elective cesarean section, as compared with other modes of delivery (adjusted odds ratio, 0.43; 95 percent confidence interval, 0.33 to 0.56). The results were similar when the study population was limited to those with rupture of membranes shortly before delivery. The likelihood of transmission was reduced by approximately 87 percent with both elective cesarean section and receipt of antiretroviral therapy during the prenatal, intrapartum, and neonatal periods, as compared with other modes of delivery and the absence of therapy (adjusted odds ratio, 0.13; 95 percent confidence interval, 0.09 to 0.19). Among mother-child pairs receiving antiretroviral therapy during the prenatal, intrapartum, and neonatal periods, rates of vertical transmission were 2.0 percent among the 196 mothers who underwent elective cesarean section and 7.3 percent among the 1255 mothers with other modes of delivery. Conclusions The results of this meta-analysis suggest that elective cesarean section reduces the risk of transmission of HIV-1 from mother to child independently of the effects of treatment with zidovudine.

North American and European studies of at least 100 mother-child pairs were included in the meta-analysis. Uniform definitions of modes of delivery were used. Elective cesarean sections were defined as those performed before onset of labor and rupture of membranes. Multivariate logistic-regression analysis was used to adjust for other factors known to be associated with vertical transmission. The primary analysis included data on 8533 mother-child pairs. After adjustment for receipt of antiretroviral therapy, maternal stage of disease, and infant birth weight, the likelihood of vertical transmission of HIV-1 was decreased by approximately 50 percent with elective cesarean section, as compared with other modes of delivery (adjusted odds ratio, 0.43; 95 percent confidence interval, 0.33 to 0.56). The results were similar when the study population was limited to those with rupture of membranes shortly before delivery. The likelihood of transmission was reduced by approximately 87 percent with both elective cesarean section and receipt of antiretroviral therapy during the prenatal, intrapartum, and neonatal periods, as compared with other modes of delivery and the absence of therapy (adjusted odds ratio, 0.13; 95 percent confidence interval, 0.09 to 0.19). Among mother-child pairs receiving antiretroviral therapy during the prenatal, intrapartum, and neonatal periods, rates of vertical transmission were 2.0 percent among the 196 mothers who underwent elective cesarean section and 7.3 percent among the 1255 mothers with other modes of delivery. The results of this meta-analysis suggest that elective cesarean section reduces the risk of transmission of HIV-1 from mother to child independently of the effects of treatment with zidovudine.

In this review we discuss different approaches to sentinel lymph node processing in an intra operative setting and in the consecutive embedding in paraffin. We propose a method, which uses routine intra operative examination of lymph nodes with stereo microscopy with selected frozen section analysis. We demonstrate preliminary data on a larger patient collective along with data on a control group. We could show in our study that a higher rate of metastates can be achieved avoiding intra operative frozen sections on grossly inconspicuous sentinel lymph nodes.

Die Sentinel-Lymphknotenoperation weist eine wesentlich geringere Morbidität auf und vermeidet bei gegebener Indikation zunehmend die komplette Axilladissektion. Die Sentinel-Lymphonodektomie hat sich als neuer operativer Standard etabliert und sollte allen Patientinnen mit unizentrischen Tumoren unter 3 cm Durchmesser angeboten werden. Die offenen Fragen betreffen vor allem den Injektionsort der Markersubstanz, die Bedeutung des Auffindens und Entfernens extra-axillärer Lymphknoten und die Anwendbarkeit der Methode bei multizentrischem Tumor, nach neoadjuvanter Chemotherapie, vorausgegangener Brustbiopsie, beim DCIS und bei Tumoren von mehr als 3 cm Grösse. Auch das Vorgehen nach Auffinden von Mikrometastasen ist noch Gegenstand wissenschaftlicher Studien.

Unlabelled: The aim of this investigation was to document the personal learning curve of the sentinel lymphonodectomy in breast surgery and to compare it with the experiences of other authors., Results: Between July 1999 and May 2004, the authors performed 218 sentinel lymphonodectomies, 48 of them during the 24-month evaluation period with consecutive full axillary dissection. The overall detection rate was 212/218 or 97.2%, while during the evaluation period it was 38/48 or 89.6% (failures were procedures No. 6, 7, 22, 24 and 47). Among the first 48 cases, 24 had histologically proven metastasis to one or more of the axillary lymph nodes. In 3 of these cases, we were not able to detect a sentinel lymph node, and 4 times the sentinel lymph node was false-negative after rapid section. The false-negative results during the learning period belonged to cases No. 2, 10, 23 and 29 (4/21 or 19%). From September 2001 until May 2004, the detection rate was over 99% (165/166)., Discussion: Our own experience documents the initial difficulties and insecurities with this operative procedure. The common recommendations, i.e. that sentinel lymphonodectomy without axillary clearance should not be offered before having completed one's own learning curve with the aid of experienced surgeons, are to be supported.

Abstract

Complete axillary dissection is still integral component of breast cancer treatment, but significant morbidity is associated with this procedure. Sentinel lymphonodectomy can replace complete axillary dissection in selected cases and can significantly reduce morbidity. Sentinel lymphonodectomy has become a new surgical standard and should be offered to all patients with unicentric tumors of less than 3 cm in diameter. Open questions concern the localization of the market injection, the significance of detecting and extirpation of extra-axillary lymph nodes, the applicability of the method to multicentric tumors, after neoadjuvant chemotherapy, after previous breast biopsy, to DCIS and tumors of more than 3 cm in diameter. Clinical trials have to establish whether axillary dissection after finding a micrometastasis in the sentinel node is necessary or not.

Abstract

Background: Breast abscesses have usually been treated by incision and drainage. During the past 10 years conservative treatment with repeated ultrasound-guided drainage to evacuate the abscess combined with antibiotic treatment has become a valuable alternative., Patients and Methods: From 1991 to 2003 the authors treated 17 patients with puerperal abscess with this method. Under local anesthesia with ultrasound guidance a thick needle (preferably a Venflon) is introduced into the cavity. The procedure is repeated every two or three days until the ultrasound image demonstrates a diameter of the cavity of 1.5 cm or less. The oral antibiotic treatment lasts for 6 to 10 days., Results: In 7 cases only one puncture was needed, 5 cases needed 2 punctures while 4 women needed 3 or more punctures (up to 5). In 16 cases an open drainage could be avoided. One patient wanted to discontinue the conservative treatment after the first puncture and requested the surgical drainage., Conclusions: Conservative therapy with ultrasound-guided drainage of puerperal breast abscesses can therefore be recommended as a standard treatment.

Abstract

We report on a rare peripartal neuropathy of the leg caused by prolonged difficult labour. Immediately after delivery two patients complained of unilateral footdrop and numbness in the leg. The footdrop was most probably due to compression of the lumbosacral trunk exposed to the foetal head. This trunk contains fibres from lumbar roots L4 and L5 and connects the lumbar with the sacral plexus. The outcome was favourable in both patients. If subsequent pregnancies occur, caesarean section may be indicated.

Abstract

The regular self-examination of the breast is one of the possibilities to detect breast cancer early. Our aim was to test the sensitivity of the method on rural, poorly informed patients. Almost one fourth of the 90 cancer patients belonged to the self-examination group. 52% of these women had a stage-I tumor. 58% of the women who didn't examine themselves entered hospital with stage-III or stage-IV tumor. In the self-examination group, in 71% of the lymph nodes were negative. This cohort showed also a better result concerning tumor extension. The average size was 1.9 cm compared to 3.8 cm in the 'non-analyser group'. We summarize that women controlling themselves have a better prognosis. Therefore, self-examination, combined with mammography and clinical control, is one of the best methods for early detection of mammary-cancer.
