Background: It has been more than 17 years since the introduction of free ART in India. At this point, it would be prudent to look at the factors associated with the survival of persons living with human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) (PLHA) who are already enrolled in the ART program. Methods: PLHAs enrolled from antiretroviral therapy (ART) centers located in three different cities in India – Delhi, Pune and Kolkata, and were followed up at six monthly intervals monitoring the WHO stage, CD4 counts, complete blood counts, and liver and kidney function tests, for a duration of three years. objective: Monitoring the WHO stage, CD4 counts, complete blood counts and liver and kidney function tests, for a duration of three years Results and Discussion: The incidence of mortality among HIV/AIDS patients on ART was 5·0 per 1000 patient-years (21/1410, 1.4%). Age at initiation of ART, being above 35 years, was the only significant predictor of mortality (log-rank p = 0·018). Multivariable analysis showed a significant association of an unfavourable outcome (defined as mortality or development of opportunistic infection during follow-up) with male gender (adjusted odds ratio (AOR) = 5.26, p = Conclusion: The survival of PLHA with good adherence to ART is independent of the WHO stage or CD4 counts at the initiation of ART. Initiation of ART after 35 years of age was a significant predictor of mortality. conclusion: The survival of PLHA with good adherence to ART is independent of the WHO stage or CD4 counts at initiation of ART.

Internet of Things (IoT) is becoming more and more pervasive in all applications. It has greater capabilities like remote monitoring and control. Different available APIs make IoT devices and applications easy to develop and deploy. The data generated by IoT devices is smaller in size and needs light weight protocols like MQTT to carry it over the network. Risk Mitigation in IoT data is very crucial and to do that traditional security and risk mitigation algorithms like RSA, SHA-512 and all cannot be used as IoT devices have smaller data. Applying traditional security and Risk Mitigation techniques to IoT data by traditional algorithms will cause the computation overhead in IoT applications. Different Light Weight Encryptions schemes for risk mitigation are suggested like PRESENT, HUMMINGBIRD et al. in this paper, different light weight encryption algorithms used in IoT risk mitigation are studied and understood. Their problems are noted down and possible improvements are suggested to make them more efficient

The present investigation was conducted to examine the 20 Bread Wheat genotypes to study the genetic parameters, correlation and genetic diversity. The experiment was carried out in main experimental station of Agricultural Research Farm, Rama University (U.P), Mandhana, Kanpur during Rabi Season, 2020-21 in Randomized Block Design (RBD) with three replications. Analysis of variance showed highly significant differences among 20 Bread Wheat for 11 characters studied. Genetic divergence was estimated among 20 genotypes by using Mahalanobis’s D2 statistic. The genotypes were grouped into 5 clusters. Cluster 1 comprises maximum genotypes which is 15 in numbers namely (HPST-16-17-07, BHU 25, BHU 31, ZINCO1, ANKUR, PBW Zn 1, WB 02, HPAN 101, HPAN 147, HPAN 164, HPAN 57, HPAN 65, HPAN 111, HPAN 127, HD 2967) followed by cluster 2 comprises 1 genotypes ((HPAN 42), cluster 3 comprises 1 genotype (HPST 16 -17-15), cluster 4 comprises 2 genotype (HPST 16-17-16, CRD GHEHU 1), cluster 5 comprises 1 genotype (PBW 677). The maximum Intra-cluster (D2) was registered for cluster 1 (7.93). Inter-cluster distance (D2) was found maximum between cluster 4 and cluster 5. Cluster 4 showed maximum cluster mean value for grain yield per plant. Cluster means indicated that none of the clusters was superior for all the characters studied. Therefore, hybridization between genotypes belonging to different clusters is suggested for development of superior genotypes. Thousand seed weight was the main factor contributing towards genetic diversity accounting for (26.32%) followed by grain per spikelets (25.79%).

In this work, we employ a key access control management scheme which effortlessly transitions any organization-like security policy to state of the art cloud level security. Offering a very flexible, secure, and hierarchical key access mechanism for institutions that deal with mission-critical data. The scheme also minimizes concerns about moving critical data to the public cloud and ensures only the users with sufficient permission from equal or higher privileged members can access the key by the use of topological ordering of a directed graph which includes self-loop. The main overheads such as public and private storage needs are restricted to a level that is tolerable, and the derivation of key is computationally fast and efficient. From a security perspective, the proposed scheme would be resistant to collaboration attacks and would provide key in distinguishability security. Since the key isn’t stored anywhere so, the problem of a data breach is eliminated. This study proposes an innovative design solution based on the design for additive manufacturing (DfAM) and post-process for manufacturing industrial-grade products by reducing additive manufacturing (AM) time and improving production agility. The design of the supportless open cell Sea Urchin lattice structure is analyzed using DfAM for material extrusion (MEX) process to print support free in any direction. The open cell is converted into a global closed cell to entrap secondary foam material. The lattice structure is 3D printed with Polyethylene terephthalate glycol (PETG) material and is filled with foam using the Hybrid MEX process. Foam-filling improves the lattice structure’s energy absorption and crash force efficiency when tested at different strain rates. An industrial case study demonstrates the importance and application of this lightweight and tough design to meet the challenging current and future mass customization market. A consumer-based industrial scenario is chosen wherein an innovative 3D-printed universal puck accommodates different shapes of products across the supply line. The pucks are prone to collisions on the supply line, generating shock loads and hazardous noise. The results show that support-free global closed-cell lattice structures filled with foam improve energy absorption at a high strain rate and enhance the functional requirement of noise reduction during the collision.

Ankylosing spondylitis (AS) is a chronic, systemic, inflammatory disease that affects primarily sacroiliac joints and the spine. It is a gradually progressive condition over several years until structural damage manifests clinical- ly as sacroiliitis, loss of spinal mobility, extra-articular symptoms, peripheral arthritis and reduced quality of life. The onset is typically between the ages of 20 to 30, with a male preponderance of about 3:1. Modern science has very limited options to treat Ankylosing spondylitis. So, the necessity of management through Ayurveda is very much essential. Various Panchakarma procedures and internal Ayurvedic medicines have been proved beneficial in the management of Ankylosing spondylitis. The present report deals with a case of ‘Ankylosing spondylitis came to our hospital for Ayurvedic treatment. The patient was diagnosed as having 'Asthi-Majja Gata Vata’ ac- cording to Ayurveda and treated with various Panchakarma procedures and internal medicines for 60 days. A cri- terion of assessment was based on the scoring of 'Bath Ankylosing Spondylitis Disease Activity Index (BASDAI)'. Total two assessments were carried out before and after 60 days of treatment. The patient has shown good improvement on BASDAI (80% relief). Improvement was found in signs and symptoms like fa- tigue/tiredness, back/hip pain, tenderness and intensity as well as the duration of morning stiffness. Ayurvedic treatment seems to be promising in the management of Ankylosing spondylitis without causing any adverse effects. Background Chordoma is a rare, slow growing and locally aggressive mesenchymal neoplasm with uncommon distant metastases. It is a chemo-resistant disease with surgery and radiotherapy being the mainstay in treatment of localized disease. In advanced disease imatinib has a role. We report a case of metastatic sacral chordoma with symptomatic and radiological response to erlotinib post-progression on imatinib. Case presentation A 48-year-old male with a sacral chordoma underwent partial sacrectomy followed by post-operative radiotherapy. Upon recurrence he received palliative radiotherapy to hemipelvis and was offered therapy with imatinib. However, the disease was refractory to imatinib and he was started on treatment with erlotinib—showing a partial response on imaging at two months. He is currently doing well at 13 months since start of erlotinib. Conclusions As seen in previously reported cases, erlotinib is a therapeutic option in advanced chordoma, even in imatinib refractory cases and thus warrants exploration of its therapeutic role in prospective clinical trials.

Non-invasive tests like handwriting using spirals patterns have been used over the years to study writing speed and strength of patients compromised by Parkinson’s. Handwriting tests can be administered at various times during a patient’s treatment to understand the severity of the disease, disease progression, and effect of treatment. In this paper, we study handwriting traces and analyze them using machine learning methods to understand the subject.

There are sparse data on neoadjuvant systemic chemotherapy (NACT) in eyelid sebaceous gland carcinoma (SGC). The aim of this study was to evaluate efficacy and outcomes with NACT in eyelid SGC. We retrospectively analyzed 8 patients who received platinum-based NACT. The median number of cycles per patient was 4 (range, 3–5). The mean percentage reduction of tumor diameter after NACT was 71% (range, 30–100%). Two patients had a radiological complete response (CR). After NACT, surgical treatment for residual tumor was performed in 5 cases. One patient had a pathological CR and is recurrence free for 11 years. After a mean follow-up period of 44.5 months (range, 9–109), tumor recurrence occurred in 4 cases. Among these 4 cases, 3 were rechallenged with the same regimen and all responded. Systemic NACT has a role in eyelid SGC, downstages the tumor, and allows less aggressive and organ-sparing surgeries, warranting a prospective study.

Though allogeneic hematopoietic stem cell transplant (HSCT) is standard for relapsed pediatric acute myeloid leukemia, often it may not be accessible due to donor unavailability and resource limitations. We retrospectively analyzed outcomes of 26 children (mean age: 10.9±4.5 y) who underwent autologous HSCT in complete remission 2. Three-year event-free survival and overall survival were 50.1±10.7% and 63.3±9%, respectively; with 1 treatment-related death and 46% relapse rate. Notwithstanding the retrospective study design and somewhat favorable patient characteristics, the outcomes are comparable with those of other series and our own allogeneic HSCT data. Autologous HSCT is a potential alternative to allogeneic HSCT, especially in countries with poorly maintained donor registries.

Although a certain fraction of the world suffers from speech and hearing disabilities, sign language is not a widespread language across the world. In today's oral world, a gesture-based language is not particularly popular with the general masses. However, sign language itself is a fully developed language with multiple regional dialects across the globe. Therefore, to aid with a smoother communication between the speaking and non-speaking world, technical developments can be introduced. A considerable amount of work has been done in this direction. The basic need of the hour is for an application that can function in real-time and can facilitate real-time conversations between a person who can sign in sign language and one that cannot. This paper proposes an application that works on this problem statement. In order to construct such an application designed to respond in real-time and aid a live conversation between a speaking and nonspeaking individual, it is necessary to allow for live video inputs to be made to the app which would then be translated to speech. This paper proposes the use of convolutional neural networks (CNN) alongside the use of Text-to-Speech translator. By use of the CNN algorithm, the gestures can be identified by the proposed application and converted to text, which can then be converted into speech.

The diversity of the internet can be fascinating with our everyday devices getting automated. However, such technology of connecting everything to the internet can actually be daunting. This data could easily be attacked by malicious users and hence hangs the usage of such technologies in a precarious condition. To prevent such attacks, we have used AES algorithm to secure data bits and also designed a two-stage encryption and decryption algorithm. This algorithm was successfully tested by transmitting data bits through a channel between two wireless sensor nodes for five test cases. The model is tested in such a way that it can be universally applicable to all IoT networks.

The interfacial connection of metal and reinforced plastic is very critical for lightweight manufacturing in electronics, automotive, and aerospace industry. The design freedom offered by 3D printing has been leveraged in this work to fabricate lattice structures over metal substrate, to be used as mechanical interlocking between the polymer resin and metal during injection molding process. In this study, different lattice structures were first designed in a CAD environment and the strength of the subsequent lattice-resin interlock was predicted using the Finite Element Analysis (FEA) method. Individual stress status of lattice and resin during tensile test were investigated to determine the failure strength of lattice-resin interlock. FEA predicted failure strength was correlated with the experimental result with reasonable deviations.

In this study, a comparison is made between Artificial Neural Network Algorithm and Machine Learning Algorithm of Support Vector Machine by considering pre-release and pre plus post release features of a movie for predicting a movie to a particular success class. These success classes includes Flop/Disaster, Average, Hit, Super-Hit and Block-Buster of a movie. Our best performing model from Artificial Neural Network and Support Vector Machine is able to give better performance in order to predict any particular movie to specific success class of a movie for both the cases of prerelease as well as pre plus post release features of a movie.

Nowadays the amount of data generated is in massive form. Continuously data is generated by the IoT sensors which is in unstructured form and this unstructured data is to be converted into structured form so that knowledge is to be extracted from it. Data generated is useful for a different type of analysis such as forecasting data that can be used for analysis for future prediction. For many organization performing analytics on such vast data become challenge. In this paper, machine learning algorithms i.e. SVM, KNN, Logistic regression, Linear Regression, and Decision tree are applied on such data and which is then converted into a structured form which helps in future prediction and compared the accuracy of each algorithm.

Venous malformations (VMs) are part of the spectrum of vascular malformations composed of ectatic venous channels. VMs of the oral cavity in pediatric patients are relatively rare but reportedly carry a higher rate of rupture than in adults. We describe the management of an 18-month-old boy with VM who presented with a localized dome-shaped swelling measuring approximately 9.4 mm by 7.4 mm in the anterior third of the palate. Management with sclerotherapy using an intralesional injection of three percent sodium tetradecyl sulphate was used to treat the lesion with minimum armamentarium, cost, and complication. 129 Background: Docetaxel is a lipophilic drugs with a high affinity for adipose tissue resulting in a higher volume of distribution.There are contradictory data with regards to the differential effect of docetaxel based on BMI in breast cancer patients. However, there are no such data in patients with prostate cancer. A We performed an exploratory analysis to determine if the benefit of docetaxel in patients with metastatic castration resistant prostate cancer (mCRPC) is modified by BMI. Methods: This is a post hoc analysis of data retrieved from the phase III ENTHUSE M1C study that assessed the efficacy and safety of additional zibotentan in combination with docetaxel in patients with mCRPC (ClinicalTrials.gov identifier: NCT00617669). BMI (kg/m2) was categorized as: 18.5 to < 25, lean; 25 to < 30, overweight; and ≥ 30, obese. Cox regression models were constructed to determine the impact of BMI on progression-free survival (PFS) and overall survival (OS) after adjusting for baseline characteristics. Results: A total of 466 patients were eligible for current analysis, of whom 34%, 46% and 20% were < 65 years, 65-74 years and > 75 years, respectively. The median total and free baseline PSA were 99.5 (interquartile range [IQR], 33.6 to 237.0) ng/mL and 13.9 (IQR, 5.4 to 37.4) ng/mL, respectively. There were 31% (n = 145), 46% (n = 213) and 23% (n = 108) lean, overweight and obese patients. Visceral metastasis was present in 52% patients, while the number of bone metastases were 1-3 in 15%, 4 in 5%, 5-20 in 58% and ≥ 21 in 23%. The median number of cycles of docetaxel administered were 10 (IQR, 6-10). The median PFS was 7.3, 7.7 and 8.4 months (hazard ratio [HR], 0.92; 95% confidence interval [CI], 0.81 to 1.06; P = .26) for lean, overweight and obese patients, respectively. The median OS was 10.3, 10.7 and 12.4 months (HR, 0.75; 95% CI, 0.63 to 0.89; P = .01) for lean, overweight and obese patients, respectively. After adjusting for baseline and tumor related characteristics, there was no association of BMI with PFS (overweight, HR, 0.92; 95% CI, 0.73 to 1.17; P = .50; obese, HR, 0.90; 95% CI, 0.67 to 1.18; P = .42) while overweight (HR, 0.68; 95% CI, 0.52 to 0.91; P = .01) and obese (HR, 0.61; 95% CI, 0.43 to 0.88; P = .01) patients had significantly better OS as compared with lean patients. Conclusions: The differential effect of docetaxel based on BMI was not observed in patients with mCRPC. Interestingly, obese patients had a significantly longer OS, which warrants further investigation.

Mediastinal tumours are not uncommon in paediatric population and often pose a diagnostic challenge. They include a variety of entities including developmental, inflammatory, infectious and neoplastic; most are malignant. These lesions can be classified based on imaging according to the specific compartment (anterior, middle and posterior), generating a focused differential diagnosis. This combined with a rational, clinically oriented approach based on patient's history, focused physical examination, age, gender, symptoms, signs, anatomic localization, imaging characteristics and laboratory investigations including tumor markers paves way to a presumptive diagnosis guiding additional and prudent investigations. For example, a suspicion of lymphoma should be kept in a child presenting with a neck mass and superior vena cava syndrome. Neuroblastoma should be suspected among children younger than 5 years old with a posterior mediastinal mass. Such a structured approach along with histopathology will lead to an exact diagnosis. Surgery remains the mainstay of treatment of most benign and malignant non-lymphoid tumours. For optimal management, a combined modality of treatment incorporating chemotherapy and radiotherapy is often required in malignant tumours and is associated with high survival rates in these patients. In the present article, we review the approach to evaluation of mediastinal masses in childhood from a clinical perspective.

Maintenance chemotherapy (MC) with pemetrexed is a commonly used strategy in nonsquamous non-small cell lung cancer. However, most of the available evidence is from subjects with good performance status (PS), while data on the real-world utility and safety of this strategy in subjects with various categories of PS is limited. We performed a retrospective analysis of multicentric data of 3 centers from India. All patients with advanced nonsquamous non-small cell lung cancer who received MC with pemetrexed after induction chemotherapy were included. Subjects were divided into 2 groups based on baseline Eastern Cooperative Oncology Group score before initiation of induction chemotherapy as good PS (2) and poor PS (≥2). Progression-free survival, overall survival, and toxicity were assessed in the study population. A total of 290 subjects were included in the study, of whom a significant proportion (n = 104, 35.9%) had poor PS. Survival was better in subjects with good PS as compared to those with poor PS (1-year progression-free survival: 43.5% vs 29.8%, P = 0.021; 1-year overall survival: 61.8% vs 48.1%, P = 0.023). Grade 3/4 toxicity was observed in 14.5% of subjects during MC and was not different between both groups (P = 0.287). Renal dysfunction was more common in subjects who received ≥10 cycles of MC (10.7% vs 4.2%, P = 0.040). MC with pemetrexed appeared to be beneficial and safe in the real-world setting regardless of the baseline PS. However, survival benefit was more pronounced in subjects with good PS at baseline. Renal dysfunction was more frequently encountered in subjects who received prolonged MC.

Agriculture is one major and important sector for the growth of economy for any country. As per the current scenarios, various problems are present in agriculture like techniques which are used currently are not efficient, requirement of larger manpower and appropriate time for irrigation and spreading of fertilizer to yield. Internet of Things (IoT) is latest technology for smart farming to enhance efficiency, productivity and resolve various issues present in agriculture. IoT network consist of various sensor node which is used to monitor soil acidity level, temperature, and other variables. In this paper, the steps involved for agriculture are discussed and mainly focus on use of IoT in agriculture i.e. in proposed architecture which leads to growth of agriculture exponentially and the economy.

Ensuring high On-Shelf Availability (OSA) is essential for retailers today. It is a measure of retailer performance. Out-of Stock is a major problem in retailing, as it leads to lost sales and decreased consumer loyalty. The term "Out-of-Stock" is used to describe a situation where a consumer does not find the product on the shelf, at the time he/she wishes to purchase it. The root causes leading to OOS include inventory inaccuracy, unexpected high demand, restock frequency and poor shelf monitoring. Yet, the possibilities for detecting and measuring an out-of shelf situation are limited, mainly involving visual shelf audits. Hence, the existence of an automatic method for detecting the products that are not on the shelf, would be valuable, offering an accurate view of the shelf availability to the store manager. The proposed solution, is a real-time application connected to a camera device that monitors the on- shelf availability of products and sends alerts to the store manager when products go out of shelf or are misplaced. The proposed solution, compared to the existing solutions, is cost effective, easy to implement and easy to use.

This paper is the review of Self organization in future mobile network. As for now self organization is being applied to autonomic computer, or wireless network but presently self organization in mobile network is the emerging field with lot of research direction in various field related to mobile communication. In this paper a brief introduction of the term self organization and SON is presented. Also paper contains the weakness of existing system with increasing complexity. Key objectives for the deployment of self organization in system are to ensure excellent coverage and capacity with reduced interference. Deployment of SON also reduces capital operating expenditure as it eliminates the need of skilled labour. SON is mainly classified in three type’s viz. self configuration, self optimization and self healing which covers almost all parameters and function related to mobile communication. Lastly it concludes with few research directions and the areas which need more research work to be done.

Brain Tumor which is also known as Intracranial Neoplasm is a vital brain disease. This is caused when abnormal cells are formed within the brain. The two essential types of tumor are Malignant or Cancerous tumor and Brain tumor. The patient does not recover when the growth of the abnormal cells is more than the 50% mark of the brain. The describes two different algorithms of image processing. The techniques used are Segmentation method and Symmetric property with region growing approach. The paper explains the algorithm in details and does a comparison between the algorithms for a better algorithm construction in future.

Cantilever-based sensors have emerged as a promising label free detection technique, which have been used for high precision mass detection and biomolecular recognition. By surface functionalization, the cantilever can be modified specific to certain compounds detection. Molecules adsorbed to one side of the cantilever will deflect the cantilever due to changes in surface stress. Alternatively, minute mass changes can be detected by monitoring the resonant frequency change of the cantilever for high-precision mass detections. This work is dedicated to finite element (FE) 3Dstructural modeling of three layers micromechanical sensors in ANSYS 13.0 gives 3D model which are close to reality mathematical models. Material used in cantilever for different layers are silicon-dioxide, poly-silicon and nitride. The emphasis of the analysis is put on tile effects of the angle of inclination of the concentrated force upon the deformed shape, the load-deflection relationship stresses and strain for further analysis with a greater degree of accuracy. The model we made is three different model i.e. single layer microcantilevers, three layers microcantilever with same height and three layers with different height. In three layer the centre layer i.e. second layer, is piezoresistive layer that helps to calculate Characteristics i.e. deflection, deformation, stress and strain in the cantilever for the given applied force that can we used for future analysis for the detection of biomolecules in various biosensing application. Finally the comparison of all the three different model of cantilever according to their characteristics.

The Internet of Things is an emerging technology across the world, which helps to connect sensors, vehicles, hospitals, industries and consumers through internet connectivity. This type of architecture leads to Smart Cities, Smart home, Smart agriculture and Smart World. Architecture of IoT is very complex because of the large number of devices, link layer technology and services that are involved in this system. However, security in IoT is the most important parameter. In this paper, we give an overview of the architecture of IoT with the help of Smart World. In the second phase of this paper, we discuss the security challenges in IoT followed by the security measures in IoT. Finally, these challenges, which are discussed in the paper, could be research direction for future work in security for IoT.

The era of digitization has led computers to become the real face of handling commercial processes across a plethora of industries. As institutions today specifically in the medical domain have resorted to these virtual machines for realizing their goals, more and more medical data is being generated on a continuous basis. This data is being used in many recommendation systems which deliver a personalized individual's health profile model [1]. As a result, a colossal amount of data has been accumulated and is being continuously dumped by the medical organizations. This trend has led to the emergence of big data analytics which is being adopted by these recommendation systems in order to enhance predictive analysis foreseeing its huge upside potential in the healthcare sector. The paper discusses the transitions being observed in the healthcare domain which require the use of big data to make some useful predictions based upon analysing a variety of datasets and comparing their similar features. A detailed comparison amongst the existing prediction based algorithms is given and their key features are highlighted. Finally, the work illustrates a scope of a profile set comprising of various noteworthy implications observed from these models and discusses the benefits of big patient data. The profile set may denote a stepwise analysis of the patients health based upon various parameters, and also give a framework for effective treatment strategies.

Emergence of Microelectromechanical Systems (MEMS) in the last decade has been tremendous and ever-growing. Microcantilever belong to MEMS class of devices and is truly the simplest structure. Merits for employing microcantilevers as sensing mechanism far outnumber the conventional senors. Sensing applications of Microcantilevers is widespread and explored. This work is dedicated to finite element (FE) 3D structural modeling of various micromechanical sensors. All cantilever design are made of silicon, poly and gold. ANSYS 13.0 gives 3D models which are close to reality mathematical models. It gives us stresses, strain and deflections for further analysis with a greater degree of accuracy. Micromechanical sensors made up of different materials have been explored. Characteristics improvement has been observed as a result of comparison.
