Shock-compression experiments were performed on V-O alloys $({\mathrm{VO}}_{0.10},$ ${\mathrm{VO}}_{0.18},$ ${\mathrm{VO}}_{0.28})$ to 130 GPa using a propellant gun and a two-stage light-gas gun. Approximately linear relations were found for the shock velocity and particle velocity for the three alloys. Comparison of the pressure dependence of the oxygen-induced volume in the V-O alloys with reported data on elemental oxygen leads us to envisage a universal compression curve of oxygen atoms placed in metallic environments.

症例は67歳, 男性. 昭和42年 (40歳時) 慢性糸球体腎炎と診断され, 昭和56年4月29日 (54歳時) より維持血液透析を施行されていた. 昭和62年9月に左内腸骨動脈瘤破裂の既往がある. 平成6年4月7日午後4時, 納屋の2階より落下し, 左第7・8肋骨を骨折し保存的に加療されていた. 受傷時の腹部CTは施行されていない. この間血液透析時の抗凝固薬は, 低分子ヘパリンが使われていた. 血液透析翌日の4月29日, 左背部の激痛が出現し前ショック状態となり緊急入院となった. 腹部CT施行したところ, 肝周囲腹腔内にみられる液体貯留像と脾の腫大および脾内のlow density areaを認め, 脾破裂が疑われ緊急血管造影施行. 出血部位確認後, transcatheter arterial embolization (TAE) 施行し止血を確認した. 塞栓術後, 疼痛の再発, 貧血の進行を認めず, 1年後の腹部CTにても異常所見は認められていない. 本症例は透析患者に発症した遅発性脾破裂の最初の報告であり, TAEが有効で, 脾を温存し得た.

A 43-year-old man presented with gastrointestinal bleeding. A tumor with central ulceration was observed in the jejunum, with the use of a new enteroscopy system called “double-balloon enteroscopy”. Bleeding after biopsy sampling of the tumor was controlled endoscopically by using electrocoagulation. Histological findings of the biopsy specimens were consistent with gastrointestinal stromal tumor, and this was surgically resected. Double-balloon enteroscopy was useful for the diagnosis as well as the control of bleeding in this patient.

Background Deep insertion of an enteroscope by use of a push technique is difficult. A new method of enteroscopy was developed, a double-balloon method, to improve the access to the small intestine. Methods The new method uses 2 balloons, one attached to the tip of the endoscope and another at the distal end of an overtube. By using these balloons to grip the intestinal wall, the endoscope can be inserted further without forming redundant loops in the small intestine. This method was tried with a standard upper endoscope in 3 patients and with a longer enteroscope in 1 patient. Results Despite its short length the upper endoscope was successfully inserted as far as 30 to 50 cm beyond the ligament of Treitz in the 3 patients. In the fourth patient the longer enteroscope was successfully inserted beyond the ileo-cecal valve. Conclusions The double-balloon method facilitates endoscopic access to the small intestine.

Acute massive hematochezia provides one of the greatest diagnostic and therapeutic challenges. Although emergency colonoscopy has been shown to be useful for diagnosis and treatment of acute lower GI bleeding, the presence of massive volumes of blood and clots sometimes prevents satisfactory observation and/or endoscopic intervention, necessitating emergency surgery in the absence of precise diagnosis of the location and causes.1 To improve the diagnostic accuracy of colonoscopy in acute lower GI bleeding, colonoscopic observation was evaluated with a flowing water technique.
