Abstract

High magnetic fields up to 105 T have been utilized in deriving the upper critical field $B_{\rm c2}$ of LaFeAsO$_{1-x}$H$_x$ throughout whole temperatures below $T_{\rm c}$. Resistivity measurements demonstrate that $B_{\rm c2}$ behaves differently in samples with $x = 0.12$ (SC1) from those with 0.32 (SC2). In SC1, the two-band model assuming the $s$-wave pairing gives a good fitting with repulsive intraband interaction and dominant interband coupling. In SC2, we have to assume attractive intraband interaction with weak interband coupling, which in fact suggests a non-$s$-wave pairing in view of the strong Coulomb repulsion. These results support the possibility that SC1 and SC2 have different pairing symmetries.

Abstract

Monitoring is indispensable to check status, activities, or resource usage of IT services. A combination of Kibana and Elasticsearch is used for monitoring in many places such as KEK, CC-IN2P3, CERN, and also non-HEP communities. Kibana provides a web interface for rich visualization, and Elasticsearch is a scalable distributed search engine. However, these tools do not support authentication and authorization features by default. In the case of single Kibana and Elasticsearch services shared among many users, any user who can access Kibana can retrieve other's information from Elasticsearch. In multi-user environment, in order to protect own data from others or share part of data among a group, fine-grained access control is necessary. The CERN cloud service group had provided cloud utilization dashboard to each user by Elasticsearch and Kibana. They had deployed a homemade Elasticsearch plugin to restrict data access based on a user authenticated by the CERN Single Sign On system. It enabled each user to have a separated Kibana dashboard for cloud usage, and the user could not access to other's one. Based on the solution, we propose an alternative one which enables user/group based Elasticsearch access control and Kibana objects separation. It is more flexible and can be applied to not only the cloud service but also the other various situations. We confirmed our solution works fine in CC-IN2P3. Moreover, a pre-production platform for CC-IN2P3 has been under construction. We will describe our solution for the secure use of Kibana and Elasticsearch including integration of Kerberos authentication, development of a Kibana plugin which allows Kibana objects to be separated based on user/group, and contribution to Search Guard which is an Elasticsearch plugin enabling user/group based access control. Abstract

Summary: Background and Aims: The prognostic impact of previous‐HBV‐infection (pHBV) in non‐HBV‐related hepatocellular carcinoma (non‐HBV‐related‐HCC) and the prevalence, characteristics and significance of recently developed high‐sensitivity HBs antigen positivity (hHBsAg+) in these patients remain unclear. We aimed to close these gaps. Methods: We retrospectively screened patients with newly diagnosed non‐HBV‐related‐HCC (standard HBsAg‐test negative) at Hokkaido University. Patients with complete clinical information and preserved serum for hHBsAg+ were included. We evaluated the prevalence, characteristics and prognostic impact of pHBV and hHBsAg+ in non‐HBV‐related‐HCC. Results: A total of 401 non‐HBV‐related‐HCC patients were included (288 with pHBV/113 without pHBV). In non‐HBV‐related‐HCC, pHBV did not affect overall survival (OS). Among non‐HBV‐related‐HCC patients with pHBV, 11.8% (34/288) were hHBsAg+ and had more advanced stages of HCC, higher AFP levels, higher vascular invasion rates, and significantly shorter OS than others (OS: 19.3 vs. 61.4 months, p = 0.012). Comparison of OS among non‐HBV‐related‐HCC patients without pHBV (group 1), those with pHBV and without hHBsAg+ (group 2), and those with pHBV and hHBsAg+ (group 3) revealed significantly shorter OS in group 3 (19.3, 56.6 and 66.4 months in groups 1, 2 and 3, respectively; p = 0.036). Multivariate Cox regression indicated that compared with group 1, only group 3 was significantly and independently associated with shorter OS (HR: 2.044, p = 0.011). Subgroup analysis revealed that this association was particularly evident in non‐HBV‐related‐HCC patients with non‐B‐non‐C aetiology and advanced HCC. Conclusions: In non‐HBV‐related‐HCC patients, hHBsAg+, not pHBV, is significantly and independently associated with poor prognosis. [ABSTRACT FROM AUTHOR]

45. Determinants of polydoctoring among multimorbid older adults; a cross‐sectional study in an urban area of Japan.

Abstract

Background: Multimorbidity increases with age, leading to various adverse outcomes, including higher mortality, care dependency, hospitalizations, and healthcare costs. Polydoctoring, managing a patient with multimorbidity by multiple healthcare providers, can be a risk of fragmented care and increased healthcare expenditures. This study aims to identify patient‐related factors contributing to polydoctoring in older adults with multimorbidity. Methods: This study is a cross‐sectional study using baseline data from the Kawasaki Aging and Wellbeing Project. Participants were residents of Kawasaki City aged 85–89 years, without disability in basic activities of daily living, and being able to visit study site. The regularly visited facilities (RVF) index was employed to quantify polydoctoring. Polydoctoring was defined as having two or more RVFs. Poisson regression analysis was conducted to assess the association between polydoctoring and patient demographics, including types of chronic conditions and socioeconomic factors. Results: A total of, 968 participants with multimorbidity were analyzed. Increased RVF was significantly associated with eye diseases (rate ratio [RR] 1.27, 95% confidence interval [CI] 1.12–1.44), osteoporosis (RR 1.22, 95% CI 1.08–1.38), prostate diseases (RR 1.22, 95% CI 1.07–1.40), and osteoarthritis (RR 1.16, 95% CI 1.05–1.27). No significant correlation was found with educational status or financial hardship. Conclusion: The study indicated that certain chronic conditions are linked to increased polydoctoring among multimorbid older adults in Japan. However, most of those conditions are considered to be within a scope of family medicine/general practice. Training general practitioners to manage these conditions could reduce healthcare costs and the treatment burden, indicating a direction for future healthcare policy and medical education.
