Influence of reflections on operation of gyrotrons with axial output is studied both theoretically and experimentally. By way of example the Fukui large orbit gyrotron with a permanent magnet operating in third harmonic at frequency 89 GHz is considered. In the case of strong reflection ([absolute value of R] = 0.6), extreme sensitivity of output power on the reflection phase is found. A qualitative agreement between theory and experiment is observed. Index Terms--Frequency tuneability Rieke diagram, gyrotron, reflections.

In the usual measurements of radionuclides in the environmental samples by g-ray spectrometry, Compton scattering of 1461 keV g-rays from 40K severely interferes with the detection of artificial radionuclides in marine and agricultural products. In order to eliminate the interference of 40K, we have developed a simple and convenient “water leaching treatment' method applicable to seaweed (sargasso) samples. By this treatment, over 98% of the potassium in seaweed samples is removed without notable losses of artificial and/or natural radionuclides. In combination with the low-level g-ray counting in the Ogoya underground laboratory, the detection limit could be improved by ~1 order of magnitude. [ABSTRACT FROM AUTHOR]

Background: Nivolumab plus ipilimumab (NIVO + IPI) is the first-line treatment for patients with metastatic renal cell carcinoma (mRCC). While approximately 40% of patients treated with NIVO + IPI achieve a durable response, 20% develop primary resistance with severe consequences. Therefore, there is a clinical need for criteria to select patients suitable for NIVO + IPI therapy to optimize its therapeutic efficacy. Accordingly, our aim was to evaluate the association between candidate biomarkers measured before treatment initiation and survival., Methods: This was a multi-institutional, retrospective, cohort study of 183 patients with mRCC treated with systematic therapies between August 2015 and July 2023. Of these, 112 received NIVO + IPI as first-line therapy: mean age, 68 years; men, 83.0% (n = 93), and clear cell histology, 80.4% (n = 90). Univariable and multivariable analyses were used to evaluate associations between biomarkers and survival., Results: On univariate analysis, high C-reactive protein and systemic index, a high neutrophil-to-lymphocyte and platelet-to-lymphocyte ratio, and a low lymphocyte-to-monocyte ratio (LMR) were associated with shorter overall survival (OS). On multivariable analysis, a LMR ≤ 3 was retained as an independent factor associated to shorter OS with the highest accuracy (C-index, 0.656; hazard ratio, 7.042; 95% confidence interval, 2.0-25.0; p = 0.002)., Conclusion: A low LMR may identify patients who would be candidate for NIVO + IPI therapy for mRCC., (© 2024. A young patient diagnosed with advanced colon cancer and liver metastasis was found to have familial adenomatous polyposis (FAP) through comprehensive genomic analysis. Whole-genome array comparative genomic hybridization (aCGH) revealed germline deletions at chromosome 5q22.1-22.2 encompassing the entire APC gene. The patient and her son exhibited mild intellectual disability without developmental delay. This case highlights the need for further exploration of the characteristics associated with whole APC deletions. aCGH is a valuable tool for studying FAP and provides a detailed analysis of large deletions., (© 2024. Background: We report a case of diffuse large B-cell lymphoma that progressed rapidly after androgen deprivation therapy for prostate cancer in a patient with a history of IgG4-related disease. Estrogen has been reported to be a possible cause of acute exacerbations of malignant lymphoma only in mouse models. Therefore, its clinical significance has not been clarified., Case Presentation: This case report describes a 75-year-old man with prostate cancer who had IgG4-related disease. Hormone therapy was initiated to treat prostate cancer, but he developed dyspnea and back pain. A diagnosis was made of diffuse large B-cell lymphoma. Immunohistochemistry was positive for estrogen receptor β, which led us to suspect rapid progression of diffuse large B-cell lymphoma due to estrogen suppression by gonadotropin-releasing hormone antagonists. Hormone therapy was discontinued, and the patient received R-CHOP therapy. Subsequently, the lymphoma masses shrunk, and the patient obtained remission., Conclusion: This case is the first report of clinical significance regarding the crucial role of estrogen and estrogen receptor β in malignant lymphoma in a patient with IgG4-related disease. Background: Although nivolumab plus ipilimumab is the standard treatment for metastatic renal cell carcinoma (RCC), its efficacy and safety in older patients remain unclear. Therefore, this study aimed to assess the clinical outcomes of nivolumab plus ipilimumab for metastatic RCC in patients aged ≥ 75 years., Methods: We enrolled 120 patients with metastatic RCC treated with nivolumab plus ipilimumab from August 2015 to January 2023. Objective response rates (ORRs) were compared between patients aged < 75 and ≥ 75 years. Progression-free survival (PFS), overall survival (OS), and adverse events were compared between the groups. Adverse events were evaluated according to the Response Evaluation Criteria in Solid Tumors 1.1., Results: Among the patients, 57 and 63 were classified as intermediate and poor risk, respectively, and one could not be classified. The median follow-up duration after the initiation of treatment was 16 months. The patient characteristics between the groups, except for age, were not significantly different. Intergroup differences in ORR (42% vs. 40%; p = 0.818), PFS (HR: 0.820, 95% CI 0.455-1.479; p = 0.510), and median OS (HR: 1.492, 95% CI 0.737-3.020; p = 0.267) were not significant. The incidence of adverse events (50% vs. 67%; p = 0.111) and nivolumab plus ipilimumab discontinuation due to adverse events was not significantly different between the groups (14% vs. 13%; p = 0.877)., Conclusions: The effectiveness of nivolumab plus ipilimumab was comparable between patients with metastatic RCC aged < 75 and those ≥ 75 years with respect to their ORRs, PFS, OS, and adverse event rates., (© 2023. A recent approach to radiotherapy for prostate cancer is the administration of high doses of radiation to the prostate while minimizing the risk of side effects. Thus, image-guided radiotherapy utilizes advanced imaging techniques and is a feasible strategy for increasing the radiation dose. New radioactive particles are another approach to achieving high doses and safe procedures. Prostate brachytherapy is currently considered as a combination therapy. Spacers are useful to protect adjacent organs, specifically the rectum, from excessive radiation exposure.

Background: Nivolumab plus ipilimumab (NIVO+IPI) is the first-line treatment for patients with metastatic renal cell carcinoma (mRCC). Approximately 40% of patients achieve a durable response; however, 20% develop primary resistant disease (PRD) to NIVO+IPI, about which little is known in patients with mRCC. Therefore, this investigation aimed to evaluate the clinical implication of PRD in patients with mRCC to select better candidates in whom NIVO+IPI can be initiated as first-line therapy., Methods: This multi-institutional retrospective cohort study used data collected between August 2015 and January 2023. In total, 120 patients with mRCC treated with NIVO+IPI were eligible. Associations between immune-related adverse events and progression-free survival, overall survival (OS), and objective response rate were analyzed. The relationship between other clinical factors and outcomes was also evaluated., Results: The median observation period was 16 months (interquartile range, 5-27). The median age at NIVO+IPI initiation was 68 years in the male-dominant population (n = 86, 71.7%), and most patients had clear cell histology (n = 104, 86.7%). PRD was recorded in 26 (23.4%) of 111 investigated patients during NIVO+IPI therapy. Patients who experienced PRD showed worse OS (hazard ratio: 4.525, 95% confidence interval [CI]: 2.315-8.850, p < 0.001). Multivariable analysis showed that lymph node metastasis (LNM) (odds ratio: 4.274, 95% CI: 1.075-16.949, p = 0.039) was an independent risk factor for PRD., Conclusions: PRD was strongly correlated with worse survival rates. LNM was independently associated with PRD in patients with mRCC receiving NIVO+IPI as first-line therapy and might indicate that a candidate will not benefit from NIVO+IPI., (© 2023 The Authors. Single nucleotide polymorphisms (SNPs) reportedly influence the effect of nivolumab in metastatic renal cell carcinoma (mRCC). This study aimed to evaluate the relationship between the clinical outcomes of patients with mRCC and SNPs in programmed cell death protein 1 (PD-1) protein-coding gene ( PDCD1 ) and explore any potential correlation with patient prognosis and incidence of immune-related adverse events (irAEs). In total, 106 patients with mRCC, who were treated with nivolumab alone ( n = 59) or nivolumab and ipilimumab ( n = 47), were enrolled in the study. Three SNPs in the PDCD1 gene, namely PD-1.3 , PD-1.5 , and PD-1.6 , were assessed. Patients harboring the PD-1.6 G allele experienced more severe (odds ratio, 3.390; 95% confidence interval 1.517-7.756; p = 0.003) and multiple (OR, 2.778; 95% CI, 1.020-6.993 p = 0.031) irAEs than those harboring the AA genotype. Thus, the existence of the PDCD1 PD-1.6 polymorphism ( G allele) was associated with the occurrence of severe and multiple irAEs in patients with mRCC. Further evaluation of PDCD1 polymorphisms might help identify patients experiencing irAE by nivolumab treatment.

A 46-year-old woman was referred to our hospital with a left-sided renal tumor pointed out by ultrasonography at the time of a medical checkup．Computed tomography revealed a mass measuring 88×77×68 mm on the upper pole of the left kidney. She was diagnosed with cT2aN0M0 clear cell renal cell carcinoma. Laparoscopic left nephrectomy was performed uneventfully. Histopathological diagnosis was clear cell renal cell carcinoma, G2, v1, pT2. Four months after surgery, lung metastases appeared, and systemic therapy was given sequentially as follows ; sunitinib for 2 months, nivolumab for 8 months, axitinib for 17 months, and pazopanib for 2 months．However, metastases progressed, and a re-administration of nivolumab was planned. The nivolumab re-treatment resulted in a marked reduction in multiple lung metastases despite the previous failure by nivolumab treatment. There are few reports on the therapeutic effect of re-administration of nivolumab. We report a case of successful treatment by re-administration of nivolumab.

Readily available Boc-protected Z-alkenyl aminals could be used as Z-alkenyl and E-alkenyl imine precursors under acidic conditions. In the Mukaiyama-Mannich reaction of Z-alkenyl Boc-aminals, the E/Z geometry of the products was controlled by the catalyst used. The present method was also applied to asymmetric Mukaiyama-Mannich reactions.

Silyl dienol ethers prepared from α,β-unsaturated ketones underwent nickel-catalyzed difluorocyclopropanation of the electron-rich alkene moiety with trimethylsilyl 2,2-difluoro-2-(fluorosulfonyl)acetate. The subsequent vinylcyclopropane-cyclopentene rearrangement afforded silyl 5,5-difluorocyclopent-1-en-1-yl ethers in good yields. The obtained five-membered silyl enol ethers were demonstrated to be versatile intermediates for the synthesis of di- and monofluorinated cyclopentanones and cyclopentenones. A nickel difluorocarbene complex is proposed as a key intermediate in the difluorocyclopropanation.

Background: xCT is a component of the cysteine/glutamate transporter, which plays a key role in glutathione synthesis. The objectives of the present study were to investigate the role of xCT in the regulation of genes involved in cell cycle progression and the clinicopathological significance of its expression in esophageal squamous cell carcinoma (ESCC)., Methods: xCT expression in human ESCC cell lines was analyzed by Western blotting and immunofluorescent staining. Knockdown experiments were conducted with xCT siRNA, and the effect on cell cycle was analyzed. The cells' gene expression profiles were analyzed by microarray analysis. An immunohistochemical analysis of 70 primary tumor samples obtained from ESCC patients that had undergone esophagectomy was performed., Results: xCT was highly expressed in TE13 and KYSE170 cells. In these cells, the knockdown of xCT using siRNA inhibited G1-S phase progression. Microarray analysis identified 1652 genes whose expression levels in TE13 cells were altered by the knockdown of xCT. Pathway analysis showed that the top-ranked canonical pathway was the G1/S checkpoint regulation pathway, which involves TP53INP1, CDKN1A, CyclinD1/cdk4, and E2F5. Immunohistochemical staining showed that xCT is mainly found in the nuclei of carcinoma cells, and that its expression is an independent prognostic factor., Conclusions: These observations suggest that the expression of xCT in ESCC cells might affect the G1/S checkpoint and impact on the prognosis of ESCC patients. As a result, we have a deeper understanding of the role played by xCT as a mediator and/or biomarker in ESCC.

Cepharanthine (CEP), an alkaloid drug that has a cell membrane stabilizing effect and an immunomodulating effect, has been reported to improve symptoms and signs of chronic immune thrombocytopenia (ITP). In this study, we retrospectively assessed the clinical efficacy and adverse events of high-dose CEP for 47 patients with ITP. The response rate (elevation of platelet count>5×10(4)/μl) was 44%, and CEP treatment was judged useful in clinical aspects by their attending doctors in 77% of the cases. Next, we made a comparative analysis between patients who were administered CEP as a single agent (22 patients) and those administered CEP in combination therapy with prednisolone (PSL) (20 patients). There was a marked increase in platelet count in both groups compared to the count before CEP treatment (P<0.01), and no significant difference was seen between the two groups. High-dose CEP was well tolerated, and in some patients single-agent CEP therapy resulted in a significant elevation of platelets, allowing a reduced dosage of PSL.

In the presence of catalytic amounts of PtCl(2) and metal iodides, beta-substituted vinylsilanes reacted with aldehydes at the beta-position to give allyl silyl ethers. The Pt-catalyzed addition to aromatic aldehydes proceeded efficiently in the presence of LiI. The combined use of PtCl(2) and MnI(2) was found to be effective in addition to aliphatic aldehydes.

TCDD is a very toxic environmental contaminant which is known to cause a variety of toxic symptoms in many species. Because of a myriad of biochemical changes TCDD is known to induce in many test animals, it has been difficult to pinpoint the causative event common to all those symptoms in different species. One of the research avenues we have been following is identification of the pattern of TCDD-induced changes in DNA binding characteristics of nuclear transcription factors (NTFs), each of which has the property to trigger a set of coordinated changes in many gene expressions. Since in our previous work we studied animals affected by TCDD in vivo using gel mobility shift assay approached with32P labeled oligonucleotide probes, we examined in the current study the possibility whether we could establish an equivalent in vitro system in NIH-3T3 mouse fibroblast cells so as to be able to learn the similarities and the dissimilarities of TCDD-induced responses of NTFs between in vitro and in vivo. The results indicated that, for a large part, this in vitro test system could reasonably reproduce the pattern of changes occurring in vivo at the early stages of TCDD's action in terms of induced changes in binding of thes NTSs to DNA. The key features were TCDD induced upregulation of NTFs binding to the response elements for AP-1, dioxin (DRE) and T3 (thyroid hormone) and down-regulation of those to response elements (REs) for c-Myc, Sp-1 and retinoic acid receptor (RARE). However, the time course required the changes in DNA binding activity was much shorter in vitro. To study the basic cause for such changes in NTF binding, we studied the effects of exogenously added EGF, forskolin, TPA (12-0-tetradecanoylphorbol acetate) and TNFalpha on the expression of TCDD's action on some of these NTFs. The results showed that these agents indeed greatly influence the outcome. The most influential agents were TNFalpha, forskolin and EGF. These results indicate that this in vitro cell model is useful in simulating the action of TCDD with respect to its basic action on NTFs.

The immunoglobulin superfamily (IgSF) has been indicated as functioning in the development and maintenance of nervous systems through cell-cell recognition and communication in several model invertebrates, Drosophila melanogaster and Caenorhabditis elegans. To further explore the functions of the IgSF in the brain of an invertebrate with more complex behavior, we identified and characterized a novel brain-specific Dscam family member, AbsCAM, from honey bee (Apis mellifera). The level of the AbsCAM protein was high in newly hatched bees and was dramatically reduced with age. The AbsCAM protein level was constant among worker bees of the same age performing different tasks, suggesting that it was primarily determined by age and not task in adult brains. Two different AbsCAM transcripts (AbsCAM-Ig7A and B) were generated by the alternative splicing of exon 11 encoding immunoglobulin domain 7 in an age-dependent manner. AbsCAM was expressed in the major brain neuropils where the synaptic density was high. AbsCAM can mediate the isoform-specific homophilic cell adhesion in vitro, and affected the axonal projections in Drosophila embryonic central nervous system and adult mushroom body by ectopic expression. Furthermore, AbsCAM promoted the neurite outgrowth of cultured neurons isolated from honey bee pupal brains. These results thus suggest that AbsCAM is the first honey bee IgSF implicated as functioning in neuronal wiring during honey bee brain development.

Honeybees have been shown to exhibit cognitive performances that were thought to be specific to some vertebrates. However, the molecular and cellular mechanisms of such cognitive abilities of the bees have not been understood. We have identified a novel gene, Mahya, expressed in the brain of the honeybee, Apis mellifera, and other Hymenoptera. Mahya orthologues are present in Deuterostomes but are absent or highly diverged in nematodes and, intriguingly, in two dipteran insects (fruit fly and mosquito) and Lepidoptera (silk moth). Mahya genes encode novel secretory proteins with a follistatin-like domain (Kazal-type serine/threonine protease inhibitor domain and EF-hand calcium-binding domain), two immunoglobulin domains, and a C-terminal novel domain. Honeybee Mahya is expressed in the mushroom bodies and antennal lobes of the brain. Zebra fish Mahya orthologues are expressed in the olfactory bulb, telencephalon, habenula, optic tectum, and cerebellum of the brain. Mouse Mahya orthologues are expressed in the olfactory bulb, hippocampus, and cerebellum of the brain. These results suggest that Mahya may be involved in learning and memory and in processing of sensory information in Hymenoptera and vertebrates. Furthermore, the limited existence of Mahya in the genomes of Hymenoptera and Deuterostomes supports the hypothesis that the genes typically represented by Mahya were lost or highly diverged during the evolution of the central nervous system of specific Bilaterian branches under the specific selection and subsequent adaptation associated with different ecologies and life histories.

L-Glutamate is a major neurotransmitter at the excitatory synapses in the vertebrate brain. It is also the excitatory neurotransmitter at neuromuscular junctions in insects, however its functions in their brains remain to be established. We identified and characterized two different subtypes (AmGluRA and AmGluRB) of metabotropic glutamate receptors (mGluRs) from an eusocial insect, honeybee. Both AmGluRA and AmGluRB form homodimers independently on disulfide bonds, and bind [3H]glutamate with K(D) values of 156.7 and 80.7 nM, respectively. AmGluRB is specifically expressed in the brain, while AmGluRA is expressed in the brain and other body parts, suggesting that AmGluRA is also present at the neuromuscular junctions. Both mGluRs are expressed in the mushroom bodies and the brain regions of honeybees, where motor neurons are clustered. Their expression in the brain apparently overlaps, suggesting that they may interact with each other to modulate the glutamatergic neurotransmission.

Honeybee (Apis mellifera) worker bees (workers) are known to perform wide variety of tasks depending on their ages. The worker's brains also show the activity and behavior-dependent chemical and structural plasticity. To test if there are any changes of gene expression associated with different ages in the worker brains, we compared the gene expression patterns between the brains of newly emerged bees and old foraging workers (foragers) by macroarray analysis. The expression of genes encoding signal transduction pathway components, ion channels, and neurotransmitter transporters is elevated in the old forager brains, suggesting that the neuronal activities would be enhanced. The mRNA levels of cell adhesion protein, transcription related factors, and plasma membrane associated proteins are also increased in the old forager brains. Meanwhile, the mRNA level of one putative cell adhesion protein is decreased in the old forager brains. These results thus suggest that the dramatic changes of gene expression occur in honeybee brains associated with ages.

The honeybee, Apis mellifera, has been used as a model to study the development of the visual system and adult bee behaviour. However, the regulation of the levels of visual component genes has never been addressed in this organism. We isolated honeybee cDNAs encoding green-sensitive opsin and visual arrestin and then measured their mRNA levels in honeybee workers. Both mRNAs fluctuate on a daily cycle that depends on a pacemaker that functions separately from the pacemaker which controls rhythmic locomotor activity. The cycling-patterns of opsin and arrestin mRNAs are different from each other and are modified by light. Furthermore, light exposure can increase the absolute levels of both mRNAs and the arrestin mRNA level is also dependent on age. Consistent with these results, both mRNA levels are higher in foragers than in in-hive bees under natural conditions. This study thus shows that the expression of genes encoding visual components is regulated by multiple factors and is adjusted to the honeybees' need for vision during the day, and throughout their lives. Comparison of data obtained with honeybees and other organisms indicates that there is a link between the regulation of phototransduction components and vision-related animal behaviour.

The role of the worker honey bee Apis mellifera L. changes depending on age after eclosion (age polyethism): young workers (nurse bees) take care of their brood by synthesizing and secreting brood food (royal jelly), while older workers (foragers) forage for nectar and process it into honey. Previously, we showed that the major proteins synthesized in the hypopharyngeal gland of the worker change from brood food proteins to alpha-glucosidase at the single secretory cell level in parallel with this age polyethism [Kubo et al., J. Biochem. 119, 291-295 (1996); Ohashi et al., Eur. J. Biochem. 249, 797-802 (1997)]. Here, we examined whether the function of the hypopharyngeal gland has flexibility depending on the colony conditions, by creating a dequeened colony in which the older workers were compelled to feed the drone larvae. It was found that most of the older workers in the dequeened colony synthesized brood food proteins as did nurse bees. Furthermore, the percentage of workers that synthesized brood food proteins was maintained at 80-90% of the total workers for at least two months, as in a normal colony. These results indicate that the function of the hypopharyngeal gland cells of the worker has flexibility and can, if necessary, be maintained as that of the nurse bee, depending on the condition of the colony.

The roles of adult worker honeybees change with age; young workers nurse brood by secreting bee milk (royal jelly), and older workers forage for nectar and pollen and process nectar into honey. The electrophoretic profile of worker hypopharyngeal-gland proteins changes with age and/or role. Immunoblotting analysis using affinity-purified antibodies against three major proteins (50, 56, and 64 kDa) of the nurse-bee gland showed that they are synthesized selectively and secreted as bee-milk proteins. Immunofluorescence study showed that the proteins condense in the duct after secretion from acini. However, a major 70-kDa protein synthesized specifically in the forager-bee hypopharyngeal gland was identified as an alpha-glucosidase. Therefore, the hypopharyngeal gland seems to have two distinct states differentiated by synthesizing of different major proteins depending on the age-dependent role change.

A method for the determination of synthetic tar dyes used as food additives using capillary electrophoresis with photodiode-array detection was investigated. The dyes Erythrosine (R-3), Phloxine (R-104), Rose Bengal (R-105), Acid Red (R-106), Amaranth (R-2), New Coccine (R-102) and Allura Red AC (R-40) were separated on a capillary column (50 cm x 75 microns I.D.) and identified from the absorbance spectra of each peak. The electrophoresis buffer used was a mixture of 25 mM sodium phosphate buffer and 25 mM sodium borate buffer (1:1) (pH 8.0) containing 10 mM sodium dodecyl sulfate (SDS). Substitution of beta-cyclodextrin for SDS in the electrolyte buffer was effective for the separation of R-2 and R-102. This modified method could be employed as an additional assay method for these two dyes.

The authors have developed a new actuator to drive an electrohydraulic totally implantable artificial heart. The basic concept of this artificial heart is that the blood pumps are implanted in the thorax and an actuator is placed separately in the abdominal region. The actuator is a regenerative pump that pumps fluids against high pressures and is thin enough for easy implantation. The rotor-magnet of the brushless DC motor is mounted on the impeller of the pump to miniaturize the actuator and reduce the number of moving parts. The height, diameter, and weight of the actuator are 32.5 mm, 73 mm, and 360 g, respectively. A pair of oil ports is connected to the left and right blood pumps with mesh reinforced tubes filled with silicone oil. The blood pumps are alternately driven by bidirectional rotation of the motor. Performance of the system was evaluated in in vitro and in vivo experiments. Maximum output of the right heart was 6.7 L/min in both experiments. Systemic circulation was well maintained in acute animal experiments using 49 and 50 kg goats. The feasibility of the actuator was confirmed.

Even in the severe tetralogy of Fallot the performance of right ventricle is maintained normally without loaded pattern in ECG compensated by the blood flow derived through the ductus arteriosus at early neonatal period. Thereafter, stenotic murmur and reversed shunt appear, because the degree of right ventricular stenosis presumably increases gradually and right ventricular resistance11) become higher than systemic one.
