Background: Orthodontic treatment involves moving teeth within the alveolar ridge. Bone remodeling is associated with the activity of osteoblasts and osteoclasts. Procedures such as corticotomy-assisted orthodontic therapy (CAOT) or periodontally accelerated osteogenic orthodontics (PAOO) are intended to reduce bone density and negative stress on the grip side and therefore limit bone resorption during orthodontic movement or add bone substitute material so that the tooth does not cross the vestibular plate. Methods: The study was conducted in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. The study design was defined in the PICO format—Population (P): patients with full permanent dentition, both adolescents and adults; Intervention (I): orthodontic treatment with fixed appliances using additional supportive treatments such as CAOT or PAOO; Comparison (C): assessment of the impact of additional treatments during orthodontic treatment on the remodeling of the alveolar bone and the condition of the periodontium; Result (O): statistically significant/non-significant differences in the condition of the alveolar bone before and after orthodontic treatment. Search filters include the time of publication of the article, systematic reviews from the last five years, and publications that appeared in English. The information provided in the abstracts of systematic reviews that describe the effects of additional procedures during orthodontic treatment such as CAOT or PAOO on the health of periodontium was analyzed. Articles unrelated to the subject of the planned study and those in which tooth movement acceleration was analyzed were excluded. Results: Eight articles were selected in which a total number of 835 subjects took part. The changes in bone density and effects on periodontium were different after CAOT and PAOO. Conclusions: The validity of CAOT and PAOO procedures remains controversial. Better results are obtained when combined with tissue augmentation or thickening of the gingival phenotype rather than as stand-alone procedures, as their uses to protect periodontal tissues are limited. Excessive orthodontic force can induce inflammatory tooth root resorption due to sustained high stresses within the periodontal ligament (PDL). This study aimed to analyze the PDL pressures during upper incisor retraction using the en masse method with TISAD. The finite element method (FEM) ensured consistent conditions across cases. The models included bone geometry, adjacent teeth, PDL, and orthodontic hardware, analyzed with LS-Dyna. The pressure ranged from 0.37 to 2.5 kPa across the dental arch, with the central incisors bearing 55% of the load. The pressure distribution remained consistent regardless of the force or hook height. The critical pressure (4.7 kPa) was exceeded at 600–650 g force, with notable pressure (3.88 kPa) on the palatal root wall of the right central incisor. Utilizing 0.017 × 0.025 SS archwires in MBT 0.018 brackets provided good torque control and reduced the root resorption risk when forces of 180–200 g per side were applied, maintaining light to moderate stress. Triple forces may initiate resorption, highlighting the importance of nonlinear finite element analysis (FEA) for accurate oral cavity simulations. [ABSTRACT FROM AUTHOR]

Background: Understanding the anatomy of the incisive canal is crucial for effective diagnosis and treatment planning in clinical orthodontics. This is because, during orthodontic tooth movement, there is a risk of contact between the roots of the upper central incisors and the incisive canal. Objective: The aim of this study was to assess the anatomical variability of the incisive canal using cone beam computed tomography (CBCT), as well as to evaluate its correlation with age, sex, and the position of the maxillary central incisors. There are only a few studies on this topic. Materials and methods: We analysed CBCT data from 67 patients aged from 13 to 49 years. This study was conducted at the Wroclaw Medical University. Measurements were performed twice by two independent researchers, and intra-observer error and correlation were calculated. The mean difference between the first and second observations and between observers was also assessed. We examined the dimensions of the incisive canal and its relationship to the roots of the upper central incisors in relation to age and gender. Results: Our study results revealed a significant correlation between the width and length of the incisive canal. Males exhibited a significantly greater canal length at the lowest point of the incisive canal on the palatal wall. Additionally, males had wider canals compared to females. The analysis of canal width and distance between the most mesial point of the root and the line passing through the most anterior point of the incisive canal showed a negative correlation in all age groups of men. The analysis of incisal inclination and incisal canal inclination showed a very strong relationship, especially in the age group of 13 to 20 years. Several potential risk groups of contact between the roots of central incisors and the incisive canal have been identified based on their structure and the planned incisors' orthodontic movement. Conclusions and implications: Knowledge of the anatomy of the incisive canal and the use of 3D imaging in high-risk patients can prevent resorption of the incisor root by considering the individual anatomical conditions of the patient when planning orthodontic tooth movement. We recommend performing a CBCT scan before starting orthodontic treatment in the case of moderate and significant retraction of the incisors, or a significant change in their inclination due to the wide anatomical diversity of the incisive canal, especially in adult patients. Purpose: Conventional orthodontic treatment with stainless steel orthodontic wires may be detrimental to oral health, as it contributes to demineralized lesions and increases adhesion and bacterial biofilm formation, which contributes to cavity development. An alternative that has been investigated to reduce the side effects of orthodontic treatment is the use of coating materials with antimicrobial nanoparticles. This study aims to evaluate the antiadherent and antibacterial properties of TiO2-coated and TiO2:Ag-coated stainless steel orthodontic wires against S. mutans bacteria. Methods: In the sol--gel method, TiO2:Ag thin films were deposited on stainless steel orthodontic wires. Coated archwires were analyzed for their antibacterial and antiadherent properties. The evaluation of Streptococcus mutans adhesion to the orthodontic wires' surface was conducted according to the type of coating used, biofilm formation assay, and measurement of the pH of the bacterial community. Results: In the microbiological test, the TiO2:Ag coatings revealed a statistically significant difference in terms of microbial adhesion and biofilm formation by Streptococcus mutans. The TiO2:Ag coating on stainless steel wire increased pH levels in the saliva environment. Conclusions: It can be concluded that antimicrobial orthodontic wires coated with silver- TiO2 nanoparticles using the sol--gel thin film are a promising choice for improving orthodontic treatment. Background. Predicting the cooperation of orthodontic patients seems to be of the utmost importance for successful results in treatment with removable appliances, especially if their cost is to be covered from public funding. Therefore, the issue of unbiased pre-treatment assessment of cooperation still calls for an investigation. Objectives. The objective of this study was to check whether smile attractiveness and its importance, subjectively evaluated by the patients/their caregivers, are reliable predictors of a patient's compliance during treatment with removable appliances. Material and methods. The study group comprised 97 patients aged 9-12 years, treated with active plates or twin-blocks, equipped with the TheraMon® system. Before treatment, the caregivers and the patients filled out the questionnaires ranking both of the investigated variables. After 9 months of treatment, we analyzed the correlations of the daily wear time (DWT) and other variables. Results. The analysis revealed the following: no relevance of the DWT to the type of removable appliances; negative correlation between the DWT and smile attractiveness assessed by the children and their caregivers; evidently elongated DWT (up to 9.68 h in children), who, together with the caregivers, evaluated their smile attractiveness as poor; coherence of children's and their caregivers' responses evaluating smile attractiveness and its importance, as well as the lack of consistency when comparing responses provided separately by the children and their caregivers. Conclusions. Application of the TheraMon® sensors objectively proved that the patient's smile attractiveness ranked subjectively as low predicts the orthodontic patient's irreproachable cooperation. Therefore, our easy-to-use questionnaire calls for changing the protocol regarding the wearing time of removable appliances during treatment and introducing an evidence-based policy of reimbursement for such therapy from public funds. Further investigation of the effectiveness of removable appliances worn shorter than previously presumed and of the motivation to continue treatment, once smile attractiveness has been improved, is necessary. Background. The effectiveness of orthodontic treatment with removable appliances arouses controversy concerning the level of patients' compliance. The compliance depends on factors controlled by the orthodontists, by the patients themselves and by their caregivers, and those beyond any control. Objectives. The purpose of this study was to assess the extent to which the personality traits of patients and their caregivers affect the cooperation in removable appliance therapy. Material and methods. A total of 38 patients (19 boys, 19 girls) aged 9-12 years, presenting malocclusions suitable for removable appliance therapy were randomly selected. The EAS-C Temperament Questionnaire, Generalized Self-Efficacy Scale, NEO-Five Factor Inventory and Parental Attitude Scale developed by Plopa were employed to evaluate the patients' and the caregivers' temperamental dispositions. Electronic sensors were employed to objectively assess patients' cooperation. Results. Statistical analysis revealed a high positive correlation between removable appliance wear time and the results of the GSES and the SPR-R, a strongly positive correlation between wear time and the NEOFFI- C results and a strongly negative correlation between wear time and the EAS-C-Emo measurement. The level of a generalized sense of efficacy, conscientiousness and the severity of requirements imposed on the child were the factors related to the caregivers, positively correlating with patients' cooperation. Conclusions. The cooperation of children treated with removable appliances may be foreseen with observation of the patients' and their caregivers' traits. Objective evaluation of the patients' cooperation and psychological tests may be valuable tools for the re-assessment of the paradigm of early orthodontic treatment. [ABSTRACT FROM AUTHOR]

Background. Loading of the compromised periodontium with orthodontic forces produces different results than those achieved in patients with healthy periodontal support. Determining the force value at a level preventing further deterioration of the patient's periodontal status, thus delivering the most precisely individualized "dose" of loading, seems to be crucial for the successful intrusion of teeth with reduced periodontal support. Objectives. The aim of the study was to determine the range of force values efficiently intruding maxillary incisors without further compromising the initially-impaired periodontal status. Finite element analysis (FEA), allowing estimation of the stress and strain distribution, was the method of choice. Material and methods. The CT scans of a periodontally-compromised patient were segmented using InVesalius software. A model - based on NURBS surfaces - was adjusted to the CT scans in order to obtain both smooth and natural curvatures of each model segment. All relevant tissues were modeled as separate volumes. The geometric model was discretized in order to create a numerical model for applying Ansys software (v. 15.07) and using APDL. The central incisors were loaded with external intrusive forces, ranging from 0.1 to 0.4 N. Results. The simulation, performed iteratively, showed that even the lowest force value - 0.1 N - causes stress changes in the alveolus and on the root surfaces, with a tendency of stress increasing towards the bottom of the alveolus and root apex. It is also notable that during the application of forces of equal magnitude, the stress/strain distribution was significantly higher around tooth 21, which displayed the highest range of PDL reduction. Application of the same force level created a higher stress-strain response around tooth 21, and the characteristics were less homogenous. Conclusions. A force value of 0.1 N applied in vivo might produce the most effective tooth intrusion and bone modeling which favors bone defect regeneration. [ABSTRACT FROM AUTHOR]

Background. Objective assessment of daily wear time of removable appliances is possible, so the next step is to ascertain whether the severity of malocclusion influences patients' compliance. This could help resolve the controversy over the question of whether removable appliance therapy truly works. Objectives. The aim of the study was to investigate whether the patient's orthodontic treatment needs affect the cooperation between the patient and the doctor, and to find a correlation that could affect recommendations for orthodontic treatment. Material and methods. The study involved 58 patients (29 boys, 29 girls) aged 9-12 years, who qualified for treatment with removable appliances equipped with a sensor system. The patients were divided into four groups according to their Index of Orthodontic Treatment Need dental health component scores. Over a 9-month period, data stored in the sensors were compared with the recommended daily wear time (DWT) of the appliances, and a statistical analysis was conducted. Results. DWT differed considerably in all the groups. Statistically significant differences in the mean DWT values occurred only when extreme values of the IOTN DHC were compared. Conclusions. The degree of patient compliance depends to a small extent on the severity of malocclusion. Patients with mild malocclusion will probably be less likely to cooperate. Among patients with severe malocclusion, compliance may be unpredictable. Orthodontic treatment carries the risk of major complications such as enamel demineralization, tooth decay, gingivitis, and periodontal damage. A large number of elements of fixed orthodontic appliance results in the creation of additional plaque retention sites which increase the risk of biofilm creation. Modification of the surface of orthodontic elements may prevent the formation of bacterial biofilm. In this paper, surface modification of stainless steel orthodontic wires with TiO2: Ag was carried out by the sol-gel thin film dip-coating method. To obtain the anatase crystal structure, substrates were calcined for 2 h at 500 °C. The properties of the obtained coatings were investigated using scanning electron microscopy, X-ray diffraction, and electrochemical tests. Corrosion studies were performed in a Ringer's solution, which simulated physiological solution. SEM and XRD analyses of the coated surface confirmed the presence of Ag nanoparticles which may have antimicrobial potential. [ABSTRACT FROM AUTHOR]

Background: Orthodontic classifications make it possible to give an accurate diagnosis but do not indicate an objective orthodontic treatment need. In order to evaluate the need for treatment, it is necessary to use such indicators as the IOTN., Objectives: The aim of the study was to find (i) relationships between individual diagnosis and objective recommendations for treatment and (ii) an answer to the question whether and which occlusal anomalies play an important role in the objectification of treatment needs., Material and Methods: Two hundred three 18-year-old adolescents (104 girls, 99 boys) were examined. In order to recognize occlusal anomalies, the classifications proposed by Orlik-Grzybowska and Ackerman-Proffit were used. The occlusal anomalies were divided into three categories: belonging to both classifications, typical of OrlikGrzybowska classification and typical of Ackerman-Proffit classification. In order to determine the objective need for orthodontic treatment, the Dental Health Component (DHC) of the IOTN was used., Results: The occurrence of the following malocclusions covered by both classifications, namely abnormal overjet, crossbite and Angle's class, had a statistically significant (p < 0.05) impact on an increase of treatment needs in the subjects (DHC > 3). As for the classification by Orlik-Grzybowska, dental malpositions and canine class significantly affected the need for orthodontic treatment, while in the case of the Ackerman-Proffit scheme, it was asymmetry and crowding. There was no statistically significant correlation between past orthodontic treatment and current orthodontic treatment need., Conclusions: IOTN may be affected by a greater number of occlusal anomalies than it was assumed. Orthodontic treatment received in the past slightly reduces the need for treatment in 18-year-olds.

Background: Cognizance of the mechanical properties of nickel-titanium archwires is necessary for the management of orthodontic therapy with fixed appliances. Acting on the periodontium with forces that are too heavy may lead to such complications as: pain, tooth root resorption and destruction of the alveolar bone and may also lead to retardation in tooth movement., Objectives: The aim of the study was to assess the activation and deactivation forces of nickel-titanium archwires: Titanol Superelastic, Copper NiTi 35oC and NeoSentalloy., Material and Methods: The examined material was 90 samples of Titanol Superelastic, Copper NiTi 35oC and NeoSentalloy with diameters of 0.016 and 0.016 x 0.022. All tests were carried out on the Zwick mechanical tests machine at a temperature of 30o C., Results: In the group of archwires with diameters 0.016, the levels of deactivation forces were, respectively, from highest to lowest: Titanol Supertelastic, NeoSentalloy, Copper NiTi 35oC. In the group of rectangular archwires 0.016 x 0.022, the highest deactivation forces were released in Titanol Superelastic. With the high levels of deflection, 0.016 x 0.022 NeoSentalloy archwires released statistically significantly higher levels of force than 0.016 x 0.022 Copper NiTi 35o C, but this force diminished rapidly with lower deflection and below 3 mm of deflection, the highest forces were released by Copper NiTi 35o C., Conclusions: Testing the mechanical properties of the nickel-titanium wires of various diameters, it was found that round section wires release forces which fall within the range of optimal forces.
