Aim: It is critical to categorize the risk factors that could disclose the severity of COVID-19. This study aimed to determine the effects of arterial blood gases on hospital mortality by examining the results retrospectively measured at the first admission to the emergency department of cases diagnosed with COVID-19. Material and Method: In this retrospective study, arterial blood gases of patients with COVID-19 were analyzed using univariate analysis to identify factors linked to 28-day all-cause in-hospital mortality. The patients were divided into two groups survivors and nonsurvivors. Results: The study included 159 survivors and 33 nonsurvivors with COVID-19. Serum levels of lactate, D-dimer, troponin, and CRP were higher and serum levels of base excess, bicarbonate, and albumin to creatinine ratio were lower in nonsurvivor patients than in survivors. The highest AUC was found for lactate and base excess. Conclusion: The arterial blood gases performed during the first admission to the emergency department are linked with disease severity and can be used to predict disease progression and mortality. Furthermore, patients with higher levels of lactate and lower levels of base excess and bicarbonate should be monitored closely and treated early.

The aim of this study was to determine the prevalence of Iron Deficiency Anemia (IDA) among adults living in Mugla and the relationship between demographic factors and IDA, retrospectively. The patients aged 15-99 who were admitted to the Outpatient Clinics of Mugla Training and Research Hospital between 01 January and 31 December 2021 were included to this study. Hb, HCT, MCV, MCH, serum iron (Fe), serum ferritin levels of patients were recorded. The anemia and IDA prevalence, relationship between anemia and biochemical and demographic factors were evaluated statistically. In our study, the laboratory tests of 82116 patients were evaluated. Anemia was found in 19326 (23.5%) patients in our study, while IDA was found in 2007 (11.5%) patients with anemia. Of the patients with IDA, 1734 (86.3%) were female and 273 (13.7%) were male. When the patients were evaluated according to the age groups, 44% of patients with IDA were in 25-44 aging group. Our study which investigates the anemia and IDA prevalence in Mugla region is the first study in literature. The IDA prevalence in Mugla is lower than previous studies which shows the better socio-economic status of our city., Bu çalışmada Muğla ilinde yaşayan erişkinlerde Demir Eksikliği Anemisi (DEA) prevalansının belirlenmesi ve demografik veriler ile DEA arasındaki ilişkinin retrospektif olarak değerlendirilmesi amaçlandı. Çalışma, 01 Ocak–31 Aralık 2021 tarihleri arasında Muğla Eğitim ve Araştırma Hastanesi’ne ayaktan başvuran 15-99 yaş arası hasta grubunda gerçekleştirildi. Tüm hastaların Hb, HCT, MCV, MCH, serum demir (Fe), serum ferritin düzeyleri çalışma kapsamında değerlendirildi. Anemi prevalansı, DEA prevalansı, anemi varlığının biyokimyasal ve demografik veriler ile ilişkisi istatistiksel olarak karşılaştırıldı. Çalışma kapsamında 82116 hastanın laboratuvar sonuçları değerlendirildi. Çalışmaya dahil edilen hastaların 19326 (%23.5)’inde anemi, anemi saptanan hastaların ise 2007 (%11.5)’sinde DEA saptandı. DEA tanısı alan hastaların 1734 (%86.3)’ü kadın, 273 (%13.7)’si erkektir. DEA tanılı hastalar yaş gruplarına göre değerlendirildiğinde hastaların %44’ünün 25-44 yaş aralığında olduğu görüldü. Çalışmamız, Muğla ilinde DEA ve anemi prevalansının saptanması amacıyla bu konuda yapılmış ilk çalışmadır. DEA prevalansının literatürde bildirilen değerlerden düşük olması ilimizin sosyoekonomik açıdan gelişiminin bir göstergesi olduğu düşünülmektedir.

Bu çalışmada, hastanemize başvuran gebe kadınların tiroid fonksiyon testi (fT4 veTSH) sonuçlarını ve bu sonuçlara göre hipertiroidi, ötiroidi, subklinik hipotiroidi ve aşikar hipotiroidi sıklığını yaş gruplarına göre değerlendirdik. Mevcut gebeliğinde ilk defa hastaneye başvuran birinci trimestrde olan 6843 gebe çalışmaya dahil edildi. Hastalar 19 yaş altı, 20-34 yaş arası ve 35 yaş üzeri şeklinde 3 gruba ayrıldı. fT4 ve TSH değerleri gruplar arasında karşılaştırıldı. Gebelerin ortalama TSH değerleri 1.61 (0.987, 2.41) mIU/mL’idi. Çalışma grupları arasında hipertiroidi, ötiroidi, subklinik hipotiroidi ve aşikar hipotiroidi açısından istatistiksel olarak anlamlı fark izlenmedi. Fakat, 35 yaş üzerindeki gebelerde hipertiroidi sıklığı 35 yaş altındaki gebelere göre daha fazla izlendi. Ülkemizdeki diğer çalışmalara benzer şekilde sırası ile; hipertiroidi, subklinik hipotiroidi ve aşikar hipotiroidi sıklığı %2.9, %22.1 ve %0.4 olarak saptanmıştır. Bu nedenle, özellikle ülkemiz gibi iyot eksikliği sık olan bölgelerde yaş ayrımı gözetmeksizin gebelerde tiroid fonksiyonlarının çalışılmasını öneriyoruz., We aimed to investigate the thyroid function test (fT4 and TSH) results and the prevalence of hyperthyroidism, euthyroidism, subclinical hypothyroidism, and overt hypothyroidism according to age groups in pregnant women who had applied to our hospital. Six thousand eight hundred and fourty-three pregnant women who were in the first trimester and who applied to the hospital for the first time in their current pregnancy were included in the study. Patients were divided into 3 age groups, namely, ≤19 years, 20-34 years, and ≥35 years. The fT4 and TSH levels were compared between the age groups. The mean serum TSH value of patients was 1.61(0.987, 2.41) mIU/mL. There was no statistically significant difference in the rates of hyperthyroidism, euthyroidism, subclinical hypothyroidism and overt hypothyroidism between the three age groups. However, the incidence of hyperthyroidism in pregnant women aged 35 years and older was significantly higher when compared with those aged 35 years and younger. Consistently with the previous studies in our country, the prevalence of hyperthyroidism, subclinical hypothyroidism and overt hypothyroidism was detected as 2.9%, 22.1%, and 0.4%, respectively. Therefore, we recommend regular thyroid testing in pregnancy regardless of age, especially in countries where iodine deficiency is common.

testisi bulunan genç erişkin dönemindeki kişilerin endokrin testiküler fonksiyonlarının ve spermatogenez durumlarının araştırılması. Gereç ve Yöntemler: Polikliniğe başvuran diğer testis normal olmak üzere tek taraflı inguinal yerleşimli testisi bulunan 18 yaş üzeri genç erişkin erkekler sayı=14 olgu çalışmaya alındı. Bu olguların hasta kayıtları kullanılarak spermiyogram değerleri sayı=8 olgu ile toplam testosteron, luteinizan hormon LH ve folikül stimülan hormon FSH düzeyleri sayı=11 olgu retrospektif olarak değerlendirildi. Bulgular: Çalışmaya yaş ortalaması 21.92 ± 2.23 yıl olan 14 hasta alındı. Hastaların büyük kısmı primer hasta iken bir hasta 9 yaşında testisinin inguinale yerleştiğini belirtmiş ve bir hastada 5 yaşında başarısız orşiopeksi uygulandığını belirtmiştir. Hastaların 9 tanesinde sağ, 5 tanesinde sol inguinal testis saptandı. Olguların ortalama testosteron, LH, FSH değerleri sırasıyla 4.42±1.24 ng/ml; 7.56±4.1mIU/ml; 8.59±6.68mIU/ml olarak bulundu. Spermiogram sonuçlarında ise sadece bir olguda azoospermi saptanırken ortalama ejekulat hacimleri 1.93±1.2ml, mililitrede ortalama sperm sayısı 53.8±41.3 milyon, ortalama normal morfolojideki sperm yüzdesi 78.1±17.1 ve ortalama hareketli sperm yüzdesi a+b 41.4±24.2 olarak bulunmuştur. Sonuç: Diğer testis normal olmak üzere tek taraflı inguinal testis varlığı belirgin sperm kalitesi bozukluğu oluşturmaktadır., Objective: To analyze status of endocrine functions and spermatogenesis in adult patients with non-treated unilateral inguinal testis. Material and Methods: A total of 14 adult men with unilateral testis were studied. We evaluated sperm quality n=8 and total testosterone, luteinizing hormone, follicular stimulating hormone levels n=11 retrospectivly. Results: Mean age was 21.92 ± 2.23 years. Ascending testis at 9 years old was reported in one patient and non successful orchiopexy at 5 years old was reported in one patient. Other patients that studied were primary cases. Nine of the cases were right sided and five were left. Mean total testosterone, luteinizing hormone, follicular stimulating hormone levels were 4.42±1.24 ng/ml;7.56±4.1mIU/ml; 8.59±6.68mIU/ml . Azoospermia was observed in only one patient. Mean sperm volume was 1.93±1.2ml, mean of sperm concentration was 53.8±41.3 sperm/mL x106 , mean percent of normal sperm morphology was 78.1±17.1% and mean percent of rapid and slow progressive motility a + b was 41.4±24.2%. Conclusions: There were abnormality in sperm quality in patients with unilateral inguinal testis and normal contralateral testis.

Aims: Clinical detection of inflammatory markers is useful to assess systemic inflammation in patients with obstructive sleep apnea syndrome (OSAS). The aim of the study was to evaluate whether the lymphocyte-to-C-reactive protein (CRP) ratio (LCR) was a predictive marker in diagnosing and determining the severity of OSAS. Settings and Design: This was a retrospective clinical study. Subjects and Methods: One hundred and forty-one patients who had undergone polysomnography were included in the study. The sex, age, Apnea-Hypopnea Index (AHI), body mass index, and complete blood count parameters of the patients were recorded. AHI scores were used to classify the severity of OSAS. Statistical Analysis Used: Differences among the groups for each parameter were analyzed using Student's t-test and one-way analysis of variance with Tukey correction for normally distributed parameters and the Mann-Whitney U test and Kruskal-Wallis test for nonnormally distributed parameters. The correlation between LCR, neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, CRP, and AHI scores was assessed using Pearson's correlation coefficient. Results: First, the patients were divided into four groups according to their AHI results, as control group, mild, moderate, and severe OSAS groups. The median LCR levels were 2.57 (1.59, 3.51) in the control group, 1.28 (0.74, 3.27) in Group 2, 1.63 (0.86, 2.6) in Group 3, and 1.05 (0.62, 2.31) in Group 4. In the comparison of all patients with OSAS and the control group, the median LCR level was 1.27 (0.7, 2.74) in patients with OSAS and 2.57 (1.59, 3.51) in the control group (P = 0.002). Conclusions: This study demonstrated that LCR is an important marker for systemic inflammation in patients with OSAS. LCR may be a new predictive marker in the diagnosis and prognosis of patients with OSAS.
