However, in some cases we see misplacement of pacemaker leads which is most often related to the presence of underlying cardiac anomalies. We report the case of misplacement of a pacemaker lead into the left ventricle of a 56-year-old patient paced in VVI/R mode and with a tined type pacemaker lead because of a symptomatic complete atrioventricular block. Electrocardiogram showed a pacemaker-generated rhythm with a right bundle branch block pattern. Chest X-ray showed the pacemaker lead located relatively high in relation to the diaphragm. Echocardiography visualized the pacemaker lead in the left heart chambers (atrium and ventricle), hence confirming its aberrant course. Further, the defect causing its passage to the left heart chambers was a sinus venosus atrial septal defect. The patient reported no complication related to the misplacement of the lead. After a brief period of oral anticoagulation, the lead was inserted into the right ventricle by percutaneous technique. Keywords: pacemaker, lead misplacement, sinus venosus atrial septal defect

6. Essays on resources and institutions

Abstract

This thesis consists of four essays covering two sets of issues linking resources to institutions. Chapter 1 provides a summary of the thesis. Chapter 2 provides a general overview of the resource curse literature, emphasising the role of institutions, the nature of the political regimes in resource-rich countries and the link with civil conflicts. Chapter 3 examines the implications of liberal lending practices of international credit markets to dictators during resource booms. We show that the combination of institutional weaknesses such as unaccountable leadership and unsound lending may give autocrats perverse incentives to loot and destabilise their countries, which impedes economic growth. Chapter 4 investigates what motivates some dictators in resource-rich countries to invest in productivity enhancing public goods while others deliberately choose predatory or repressive policies. We find that the ruler is more likely invest in public goods when the productivity of the non-resource sector is high, and when he is relatively ineffective in controlling the country's resources. Chapter 5 presents an overview of the literature on intellectual property rights focusing on the problems raised by sequential innovations for the design of patents and the role of legal institutions in resolving disputes. Chapter 6 examines the nature of the North-South divide in the bioscience industries as a hold-up problem caused by the lack of coordination between North and South property rights systems. We develop a model of bargaining in a sequential R&D framework that demonstrates the mechanism by which underinvestment in maintaining biodiversity and inefficient flow of information occurs. Chapter 7 assumes that the coordination problem is resolved and investigates the number and placement of the property rights to provide incentives for efficient investment in information generation. We show that the existence of a property right in the genetic resources is necessary for the South to share in the rents from the R&D sector. When traditional knowledge is the South's private information, it is not necessary to establish a separate property right in it to appropriate its return.

Abstract

Using the culturomics method, two strains were isolated, identified, and characterised following the taxonogenomics concept. Bacillus marasmi sp. nov. strain Marseille-P3556 (= CSURP3556) is isolated from a 13-month-old girl living in Niger. The phylogenetic tree, phenotypic criteria, and genomic analysis described here clearly show that this bacterium is different from previously known bacterial species withstanding in nomenclature and new members of Bacillus genus.

Abstract

Background: Major surgery is a term frequently used but poorly defined. The aim of the present study was to reach a consensus in the definition of major surgery within a panel of expert surgeons from the European Surgical Association (ESA). Methods: A 3-round Delphi process was performed. All ESA members were invited to participate in the expert panel. In round 1, experts were inquired by open- and closed-ended questions on potential criteria to define major surgery. Results were analyzed and presented back anonymously to the panel within next rounds. Closed-ended questions in round 2 and 3 were either binary or statements to be rated on a Likert scale ranging from 1 (strong disagreement) to 5 (strong agreement). Participants were sent 3 reminders at 2-week intervals for each round. 70% of agreement was considered to indicate consensus. Results: Out of 305 ESA members, 67 (22%) answered all the 3 rounds. Significant comorbidities were the only preoperative factor retained to define major surgery (78%). Vascular clampage or organ ischemia (92%), high intraoperative blood loss (90%), high noradrenalin requirements (77%), long operative time (73%) and perioperative blood transfusion (70%) were procedure-related factors that reached consensus. Regarding postoperative factors, systemic inflammatory response (76%) and the need for intensive or intermediate care (88%) reached consensus. Consequences of major surgery were high morbidity (>30% overall) and mortality (>2%). Conclusion: ESA experts defined major surgery according to extent and complexity of the procedure, its pathophysiological consequences and consecutive clinical outcomes.
