Abstract

Background & Aims: Patients with alcoholic hepatitis and a modified Maddrey's discriminant function (mDF) <32 have a low risk of short-term mortality. However, few data exist concerning long-term outcomes. The aims of this study were to evaluate 5-year survival rates and to identify predictive factors for long-term prognosis in this patient population. Methods: We studied patients from 2 centers who were admitted for hepatic decompensation (ascites, hepatic encephalopathy, or jaundice) and who had histological findings of steatohepatitis and an mDF <32. Clinical and biological parameters were recorded at the time of liver biopsy and alcohol consumption was recorded during follow-up. We performed Cox proportional hazard survival analysis to identify factors associated with 5-year survival. Results: One hundred and twenty-one patients were included (male: 64%, mean age: 51.5 ± 10.3 years, presence of cirrhosis: 84%). The median model for end-stage liver disease and mDF scores were 14 (IQR 11.7–16.1) and 19 (IQR 11.1–24), respectively. During follow-up, 30% of the patients remained abstinent. Survival rates at 1, 6, 12, 24, and 60 months were 96.7 ± 1.6%, 90.1 ± 2.7%, 80.8 ± 3.6%, 69.9 ± 4.3%, and 50.7 ± 4.9%, respectively. The majority of deaths (80%) were liver related. In multivariable analysis, encephalopathy at baseline and alcohol abstinence were predictive of 5-year survival. The 5-year survival rates of patients without and with encephalopathy at baseline were 60.5 ± 5.8% and 29.7 ± 8.0%, respectively, and the 5-year survival rates of abstinent and non-abstinent patients were 74.0 ± 8.0% and 40.9 ± 5.8%, respectively. Conclusions: The mortality rate of patients with alcoholic hepatitis and an mDF <32 is around 50% at 5 years. Hepatic encephalopathy at baseline and lack of alcohol abstinence impair long-term prognosis. New treatment strategies, including measures to ensure abstinence, are required. Abstract

Secondary tumours of the ampulla of Vater are rare. Underlying primary tumours, clinical presentation, macroscopic appearance, treatment strategies and outcome of secondary ampullary lesions have not been systematically analysed. The present case study reported a 57-year old patient with an ampullary metastasis from renal cancer and a literature review was performed in which a further 32 patients were included. The most common responsible primary tumours were malignant melanoma and renal clear cell carcinoma, followed by breast cancer. The time interval between the diagnosis of the primary tumour and the ampullary metastasis was highly variable, and may be as long as 10 years, particularly for renal cancer. Patients may present with unspecific abdominal discomfort, jaundice or upper gastrointestinal bleeding. The gross appearance was largely indistinguishable from that of a primary tumour. Lesions may present as polypoid or irregular, soft and friable tumour mass, in certain cases with superficial ulceration. In ~50% of cases, the ampullary metastasis was the only metastatic lesion, while in the remaining cases, the cancer had spread to one or more organs. The prognosis was generally poor. Abstract

Bilateral testicular tumors are rare, accounting for only 0.5% to 5% of all testicular cancers. Most are metachronous, discovered in the contralateral testis during follow-up; less frequently (≈35%), they are synchronous lesions.1 Although the same histologic type is usually supposed for bilateral tumors, lesions with different histologic types are not unusual. Large studies on the incidence and clinical and histologic characteristics of bilateral germ cell tumors report that the number of patients with lesions of different histologic types varies widely, from 25% up to 70%.1–5 Different histologic types in patients with bilateral, synchronous lesions have been reported to occur in 31.6% of cases.1 The widespread use of imaging for any scrotal problem has shown that not all focal lesions in the testis are malignancies, especially when small.6 An effort to characterize them preoperatively is then worthwhile. When the lesion does not have malignant characteristics, a conservative surgical approach is suggested, and decisions are made after confirmation of its nature by a frozen section pathologic examination.6 There are only a few case reports of the imaging appearances of synchronous bilateral lesions of different histologic types.7–9 The purpose of this article is 2-fold: (1) to describe the imaging features encountered in 4 patients with bilateral synchronous testicular tumors that proved to be of different histologic types at pathologic examination; and (2) to discuss the role of imaging in guiding the surgical approach to patients with those lesions

Abstract

The wide range of drug related changes seen in gastrointestinal biopsies Abstract. Interpretation of biopsies from the gastrointestinal tract has becoming more challenging as the number of new medications, especially in cancer patients, is constantly increasing. Distinctive drug-associated mechanisms can cause different types of mucosal injury. These features are frequently overlapping since number of histological patterns is limited. This review focuses on range of drug-induced changes seen in GI biopsies and their key diagnostic features that can help the pathologist to differentiate between different drugs and other possible differential diagnosis. Furthermore, a good clinical correlation in these cases is of an outermost importance for the correct diagnosis and treatment.

Abstract

Background: Leiomyosarcoma represents a rather uncommon malignancy, and reports of cases characterized by a renal genesis are particularly rare., Cases: We describe 2 cases of leiomyosarcoma of the kidney, in a 63-year-old woman and in a 53-year-old man, respectively. Both tumors share a common immunohistochemical profile with a strong positivity for smooth muscle actin, a focal positivity for desmin, and negativity for cytokeratins and other markers., Conclusion: We provide a comparison between our findings and the data available in the literature, and we note an interesting relatively long survival in our patients (10 months for the first case and 20 months for the second case).
