Abstract

This observational retrospective real-world study examined changes in healthcare resource utilization (HCRU) pre- and post-initiation of aripiprazole once-monthly (AOM 400) in patients with schizophrenia or bipolar I disorder. Electronic health record-derived, de-identified data from the NeuroBlu Database (2013–2020) were used to identify patients ≥18 years with schizophrenia (n = 222) or bipolar I disorder (n = 129) who were prescribed AOM 400, and had visit data within 3, 6, 9, or 12 months pre- and post-initial AOM 400 prescription. Rates of inpatient hospitalization, emergency department visits, inpatient readmissions, and average length of stay were examined and compared over 3, 6, 9, and 12 months pre-/post-AOM 400 using a McNemar test. Statistically significant differences were seen in both schizophrenia and bipolar I disorder patient cohorts pre- and post-AOM 400 in inpatient hospitalization rates (p <.001 all time points, both cohorts) and 30-day readmission per patient rates (p <.001 all time points, both cohorts). Statistically significant improvement in mean length of stay was observed in both cohorts at all time points, except for at six months in patients with schizophrenia. Emergency department visit rates were significantly lower after AOM 400 initiation for both cohorts at all time points (p <.001). A reduction in the rate of hospitalizations, emergency department visits, 30-day readmissions, and average length-of-stay was observed for patients diagnosed with either schizophrenia or bipolar I disorder, which suggests a positive effect of AOM 400 treatment on HCRU outcomes and is supportive of earlier analyses from different data sources. 32. Prostitution and male perspectives: A study on sex work and violent sexual exploitation in Bangladesh

Abstract

The primary purpose of this thesis is to examine the factors influencing the violent attitudes of Bangladeshi males towards female sex workers (FSWs). It has been found that male attitudes toward women generally are responsible for increased sexual violence, even at home. Underpinned by patriarchal attitudes and religious doctrine, Bangladeshi culture rarely acknowledges the larger problem of males’ social and associated health responsibilities when they purchase sex. With the escalating demand for female prostitutes in recent times, the spread of sex related diseases is also rising in Bangladesh. Rather than the conventional research approach directed solely at FSWs, by focusing on male customers’ perspectives this thesis raises questions regarding violent sexual exploitation rarely researched in Bangladesh. In this connection, this study will also examine how violent sexual behavior of male clients towards FSWs contributes to the spread of sexually transmitted diseases. By extensively reviewing the available literature on these issues, this study will thus expand the scope to look at the problems caused by male clients and will recommend the necessity of sex education and gender awareness to bring about equitable outcomes for FSWs as well as women more generally in Bangladesh.

40. Enhancing pre-trained contextual embeddings with triplet loss as an effective fine-tuning method for extracting clinical features from electronic health record derived mental health clinical notes

Abstract

The development and application of real-world evidence in the field of mental health trails other therapeutic areas like oncology and cardiovascular diseases, largely because of the lack of frequent, structured outcomes measures in routine clinical care. A wealth of valuable patient-level clinical data resides in an unstructured format in clinical notes documented at each clinical encounter. Manual extraction of this information is not scalable, and heterogeneity in recording patterns and the heavily context-dependent nature of the content renders keyword-based automated searches of little practical value. While state-of-the-art natural language processing (NLP) models based on the transformer architecture have been developed for information extraction tasks in the mental health space, they are not trained on unstructured clinical data that capture the nuances of different dimensions of mental health (e.g., symptomology, social history, etc.). We have developed a novel transformer architecture-based NLP model to capture core clinical features of patients with major depressive disorder (MDD). Initialized on MentalBERT model weights, we pre-trained our model further on clinical notes from routine mental health care and fine-tuned using triplet loss, an effective feature embedding regularizer which boosts classification and extraction of 3 specific features in patients with MDD: anhedonia, suicidal ideation with plan or intent (SP), and suicidal ideation without plan or intent (SI) or where plan or intent are unknown. Training and testing data were annotated by mental health clinicians. Using triplet loss for fine tuning led to improvement in model performance benchmarked against other standard models (MentalBERT and BioClinicalBERT) on the same tasks, achieving F1 scores of 0.99 for anhedonia, 0.94 for SP, and 0.88 for SI. Model robustness was tested by testing sensitivity of model predictions on modifications to test sentences. The application of such an NLP model can be further scaled to capture clinical features of other disorders as well as other domains like social history or history of illness.

Abstract

Objective During lung transplantation, cells in the pulmonary parenchyma are subjected to ischemia, hypothermic storage, and reperfusion injury. Platelets, whose granular contents include adhesion receptors, chemokines, and coactivating substances that activate inflammatory and coagulant cascades, likely play a critical role in the lung allograft response to ischemia and reperfusion. The platelet response to the pulmonary allograft, however, has never been studied. Here we report significant platelet activation immediately after lung transplantation. Methods We performed a prospective cohort study comparing markers of platelet activation in patients undergoing lung transplantation and patients undergoing nontransplant thoracotomy. Plasma levels of soluble P-selectin, soluble CD40 ligand, and platelet–leukocyte conjugates were measured before surgery, after skin closure, and at 6 postoperative hours. Results Both soluble P-selectin and soluble CD40 ligand levels increased significantly after lung transplantation but not after thoracotomy. Additionally, platelet–monocyte conjugate fluorescence was significantly higher after lung transplantation than after thoracotomy alone. Conclusion These findings suggest that platelet activation is significantly increased after lung transplantation beyond that expected from the postoperative state. The increase in circulating platelet–monocyte conjugates suggests an important interaction between platelets and inflammatory cells. Further research should examine whether platelet activation affects early graft function after lung transplantation.

Abstract

Background: Real world data on clinical outcomes and quality of care for patients with coronary artery disease (CAD) are fragmented. We describe the rationale and design of the Singapore Cardiovascular Longitudinal Outcomes Database (SingCLOUD). Methods and Results: We designed a health data grid to integrate clinical, administrative, laboratory, procedural, prescription and financial data from all public-funded hospitals and primary care clinics, which provide 80% of health care in Singapore. Here, we explain our approach to harmonize real-world data from diverse electronic medical and non-medical platforms to develop a robust and longitudinal dataset. We present pilot data on patients with myocardial infarction (MI) treated with percutaneous coronary intervention (PCI) between 2012 and 2014. The initial data set had 53,395 patients. Of these, 35,203 had CAD confirmed on coronary angiography, of whom 21,521 had PCI. Eventually, limiting to 2012-2014, 3,819 patients had MI with PCI, while 5,989 had MI. Compared with the quality improvement registry, Singapore Cardiac Data Bank, which had 189 fields for analysis, the SingCLOUD platform generated an additional 313 additional data fields, and was able to identify an additional 250 heart failure events, 664 major adverse cardiovascular events at 2 years, and low-density lipoprotein levels to 1 year for 3,747 patients.
