Objective: Human Papilloma Virus (HPV) infections have been suggested to be associated with the squamous epithelial lesions. Certain HPV types have been detected in squamous cell carcinoma (SCC). The goal of this study is to assess pan HPV expression in laryngeal squamous cell carcinomas and adjacent mucosal epithelium and to correlate its expression with tumor grade, nodal status, and tumor stage.Methods: The present study includes 39 cases of laryngeal SCC treated by radical laryngectomy. The tumor tissues were originally formaline-fixed and paraffin-embedded. The sections representing both tumor and the adjacent mucosa were analyzed for pan HPV expression by immunohistochemical method.Results: Pan-HPV staining was positive in 14 cases (35%) either at the tumor cells or at the adjacent mucosa. Among these, positive staining only at the tumor cells was observed in 1 case (7%), and 2 cases (15%) were positive both at the tumor cells and at the adjacent mucosa. The remaining 11 cases (78%) showed positivity only at the adjacent mucosa, Regarding the tumor stage, the adjacent mucosa showed positivity in 8 cases (53 %) among 15 T3 and T4 tumors while only 2 cases (13%) were positive at tumor cells. A proportional distribution was seen among the groups regarding tumor grade and nodal status.Conclusion: The present study demonstrates that although not providing direct evidence, adjacent mucosa should also be investigated for achieving higher detection rate in laryngeal SCC. The detectionof HPV at tumor cells by this method is of limited value. The advanced stage was not correlated statistically with HPV expression, although a higher rate of HPV positivity in T3-T4 tumors was found. This study also did not indicate any correlation with tumor grade and nodal status. However for a definite conclusion, wider series should be studied.

Retrorektal bölgede izlenen gelişimsel kistler tür, köken ve histopatolojik özelliklerine göre epidermoid kist, rektal duplikasyon (enterik) kisti ve retrorektal kistik hamartoma (“tailgut kisti”) olarak sınıflandırılabilir. Bu kistler benign gelişimsel lezyonlar olmakla birlikte, nadir de olsa malign dönüşüm bildirilmektedir. Bu nedenle klinik olarak erken tanı çok önemlidir. Nadir görülen bu lezyonlar patolog için de ayırıcı tanı açısından sıkıntı yaratabilir. Embriyolojik hindgut kalıntılarından gelişen tailgut kistleri genellikle multikistik olup, skuamöz epitel, silli kolumnar epitel ya da transisyonel epitel ile döşelidir. Literatürde kist duvarında organize bez yapısı tanımlanan olgu bulunmamaktadır. Burada, altmış sekiz yaşında retrorektal kitlesi bulunan ve histopatolojik tanısı tailgut kisti ile uyumlu, kist duvarında serömüköz bezler bulunan bir olgu sunulmuş ve retrorektal gelişimsel kistlerin ayırıcı tanısı tartışılmıştır

The aim of the study is to evaluate the pattern and level of expression of glucose transporter-1 (GLUT-1) in rectal carcinoma in relation to outcome as a potential surrogate marker of tumour hypoxia. Formalin-fixed tumour sections from 43 patients with rectal carcinoma, who had undergone radical resection with curative intent, were immunohistochemically stained for GLUT-1. A mean of three sections per tumour (range 1-12) were examined. Each section was semiquantitatively scored; 0, no staining; 1, <10%; 2, 10-50%; 3, >50% and a score given for the whole section, the superficial (luminal) and deep (mural) part of the tumour. Staining was seen in 70% of tumours. Increased staining was noted adjacent to necrosis and ulceration. A diffuse and patchy pattern of staining, with and without colocalisation to necrosis was seen. Patients with high GLUT-1-expressing tumours (score 3 vs 0-2) had a significantly poorer overall survival (P=0.041), which was associated with poorer metastasis-free survival with no difference in local control. No significant correlation was seen with other prognostic factors. There was a strong correlation between the score for the superficial and deep parts of the tumour (r=0.81), but a significant relationship with outcome was only found in the deep part (P=0.003 vs P=0.46). In conclusion, increased GLUT-1 expression in rectal tumours was an adverse prognostic factor and is worth further evaluation as a predictive marker of response to therapy. [ABSTRACT FROM AUTHOR]

19. 184 Radiotherapy results in early stage (T1-T2) vocal cord carcinoma

- Abstract
In early stage larynx carcinoma, the function preserving treatment can be performed only if patients are evaluated properly. In this study we present the preliminary results of 44 patients of early stage larynx carcinoma which are treated according to the prospective larynx carcinoma protocol of Dokuz Eylül Head And Neck Cancer Group. Forty-three patients were male and 1 patient was female. The average age is 61 (27–87). Thirty-seven patients were staged as TIN0 and 7 patients as T2N0. In 40 cases, the tumor localization was in the glottic and in 4 cases in the supraglottic area. The tumor histology was squamous cell carcinoma in 36 patients, in-situ carcinoma 7 and small cell carcinoma in 1 patient. Lung cancer was detected as a second primary in 1 patient. Diagnosis was made by biopsy in 28 cases, by stripping in 12 cases, by cordectomy in 1 case, by polypectomy in 1 case, and by mass excision in 2 cases. All cases were treated by radiotherapy alone. Treatments were completed in all cases. The delivered total dose is 66–70 Gy in 33–35 fractions (in 200 cGy fractions). There was total tumor regression in all cases, 2 months after radiotherapy. No serious side effects were observed. The self evaluation of voice quality was revealed as very satisfactory in 43 patients. Only in the cordectomy case the voice quality was unsatisfactory. The case with lung cancer is lost to follow up. Two cases died because of other reasons and the rest are disease free.

21. Evaluation of colon mucosa using screening colonoscopy and flexible spectral imaging color enhancement in patients with long lasting ulcerative colitis.

Aim: To determine which flexible spectral imaging color enhancement (FICE) channel best visualizes colon mucosa in ulcerative colitis (UC) and to compare FICE imaging with standard imaging., Methods: The study enrolled patients with ulcerative colitis in remission who had inflammatory bowel disease for at least 8 years. All patients underwent screening colonoscopy. The entire colon, especially the suspicious areas in terms of dysplasia, were imaged with standard endoscopy and FICE. Random and target biopsies were obtained. Histopathological diagnosis was made and image patterns were evaluated. Seven endoscopists evaluated normal, colitis, and polyp images obtained with FICE., Results: One hundred and twenty-three colon segments were evaluated and 1831 images were obtained from 18 patients. A total of 1652 images were FICE and 179 were standard images. Separate FICE images were obtained for normal colon mucosa, polypoid lesions, and colitis areas. Normal colon mucosa was best visualized using the second, sixth, and ninth FICE channel; polyps using the third, seventh, and ninth channel; and colitis using the second, third, and ninth channel. When all images were analyzed, the second and ninth channel were significantly better than the other channels. A total of 584 biopsies were obtained, including 492 (84.2%) random biopsies and 92 (15.7%) target biopsies. Random biopsies detected no dysplasia, but target biopsies detected low-grade dysplasia in three diminutive polyps., Conclusion: FICE was not significantly better at dysplasia screening than the standard procedure, but it effectively detected diminutive polyps and evaluated surface patterns without using magnification. FICE might contribute to the assessment of inflammation severity in patients with UC in clinical remission. However, more extensive studies are necessary to confirm these findings.

- Abstract
Molecular pathological analysis has an expanding role in patient diagnosis and management. The performance of these techniques relies on excellent laboratory procedures. However, the crucial step is obtaining the best samples for molecular analysis. Archiving and selection of these are the responsibilities of all pathologists even if they are not working at a center with molecular pathological facilities. This review focuses on the features of different types of materials for molecular pathological analysis. Many steps that might affect the results, including communication between the pathologist and the oncology team, features of different types of materials (cytological, tissue blocks, biopsy, circulating tumor cells (CTCs) and cell-free circulating nucleic acids), effects of tissue processing, methods for selecting the best material, and tissue saving and tumor enrichment methods are discussed. The procedures for referral to a center for molecular pathological analysis are also mentioned. Awareness of the importance of the cytopathological and histopathological material of the patients for future molecular pathological analysis by pathologists is of the utmost importance.

23. Minor salivary gland evaluation: Sjögren's syndrome.

Background/aim: We aimed to analyze the value of 3 serial sections, spaced 200 µm apart, for quantification of lymphocyte and plasma cell foci in minor salivary gland biopsy (MSGB)., Materials and Methods: Labial MSGBs from 69 patients with Sjögren's syndrome (SS) and scleroderma were used for this study. Each sample was prepared as 3 serial sections spaced 200 µm apart. Lymphocytic and plasma cell focus score (LFS, PFS) were determined for each section, and the diagnostic results were compared to those obtained from a single section., Results: For 22 of the 69 patients, all 3 sections were scored at <1 and interpreted as inconclusive for the presence of SS. For 20 cases, all 3 sections were scored at ≥1 and interpreted as diagnostic for SS. In the remaining 27 cases, the score was found to vary between sections. Plasma cell foci were observed in 11 cases, with 5 cases exhibiting a PFS of ≥1. Of those 5 cases, 4 also had a LFS of ≥1., Conclusion: Assessment of 3 serial sections in MSGB has the potential to improve accuracy of SS diagnosis by detecting specific features that may not have been detected in a single section. We concluded that data about the PFS require further evaluation.

- Abstract
Aim: We aimed to investigate the value of PET-CT in therapy response and the correlation of quantitative PET parameters with histopathologic results in patients with locally advanced rectal cancer (LARC) before and after neoadjuvant chemoradiotherapy. We also analyzed the correlation of PET-CT parameters between Ki-67 and glucose transporter 1 (GLUT1)., Patients and Methods: A total of 29 patients diagnosed with LARC who had undergone a biopsy between 2009 and 2012 were included in our study. Quantitative PET parameters [standardized uptake value (SUV)max-mean, lean body mass SUV(max-mean), tumor/liver SUV, retention index , and [INCREMENT]SUV(max)] were measured before and after therapy using PET-CT. Tumor regression grade (TRG) was evaluated according to Wheeler's classification. Patients in grade 1 were considered responders, whereas patients at grades 2 and 3 were considered nonresponders. Immunohistochemical staining with Ki-67 and GLUT1 was performed on biopsy and surgical specimens. The correlation between staining ratios and SUV was also investigated., Results: SUV parameters were significantly decreased after therapy (P < 0.001). Twelve (41%) patients were at TRG1, 10 (35%) were at TRG2, and seven (24%) were at TRG3. A cutoff SUV(max) of 5.05 to discriminate between responders and nonresponders after treatment revealed a sensitivity of 57%, specificity of 73%, negative predictive value of 65%, positive predictive value of 67%, and accuracy of 66%. Using a cutoff of 3.55 for the SUV(mean) (standardized measurement of SUV with 1.2-cm-diameter region of interest) revealed a sensitivity, specificity, negative predictive value, positive predictive value, and accuracy of 67, 76, 67, 76, and 72%, respectively. For a cutoff of 1.95 for the tumor SUV(mean)/liver SUV(mean), these diagnostic values after therapy were 73, 78, 82, 67, and 76%, respectively. We found a moderate correlation between liver-based SUV(max) (r = -0.35, P = 0.019) and SUV(mean )(r = -0.31, P = 0.036) with GLUT1 after therapy. Quantitative PET parameters and retention index were moderately correlated with Ki-67., Conclusion: PET-CT is a useful method for assessing the response to neoadjuvant chemoradiotherapy in patients with LARC. The most significant parameter for assessing treatment response using SUV parameters is the tumor/liver ratio.

- Abstract
Laryngeal leiomyosarcoma is an extremely rare malignancy originating from the smooth muscles of blood vessels. Herein, we present the first case of leiomyosarcoma arising from the glottic area of the larynx with coexistent tuberculous mediastinal lymphadenopathy. The patient was treated with vertical laryngectomy and anti-tuberculous medication. He has been disease-free for 24 months since initial treatment.

- Abstract
Osteochondroma is the most common benign tumor of the bone, seen mostly during adolescence. In the current study, we report a 19-year-old male patient with a two-week history of pain and swelling of the medial side of his right thigh just above the knee without any trauma. CT angiography revealed a popliteal artery pseudoaneurysm and its close relationship with a femoral osteochondroma. Surgical repair consisted of repair of pseudoaneurysm and removal of osteochondroma. In young patients, a non-traumatic pseudoaneurysm of distal femoral artery may be a complication of an osteochondroma and this treatable pathology should be looked for to prevent recurrence., (© The Author(s) 2013 Reprints and permissions: sagepub.co.uk/journalsPermissions.nav.)

27. Assessment of the effect of mesangial hypercellularity in childhood nephropathies to the clinical and laboratory findings.

Aim: To assess the relationship between mesangial hypercellularity in various childhood nephropathies and clinical and laboratory parameters., Methods and Patients: The reports of the renal biopsies were evaluated retrospectively. The patients with diagnosis of IgA nephropathy (isolated and Henoch-Schönlein nephritis), IgM nephropathy, or isolated mesangial proliferative glomerulonephritis were included. Each nephropathy group was divided into two subgroups according to the severity of mesangial hypercellularity as mild and severe. The biochemical data and histopathological findings of the patients were recorded., Results: When the groups were compared, it was found that the patients with IgA nephropathy had hematuria (p = 0.043) and the patients with IgM nephropathy had nephrotic syndrome more frequently than the other patients (p = 0.01). No difference was detected between the groups regarding the severity of mesangial hypercellularity. On the other hand, when the groups were evaluated within themselves, no significant association was detected between the severity of mesangial hypercellularity and clinical and laboratory parameters. It was determined that the renal biopsy was performed earlier in patients with Henoch-Schönlein nephritis compared to the other cases (p = 0.004). Compared to the isolated IgA nephropathy group, it was found that the number of cases with severe mesangial hypercellularity was higher and the level of proteinuria was more prominent in patients with Henoch-Schönlein nephritis. Additionally, when the patients with Henoch-Schönlein nephritis were evaluated, the degree of proteinuria was found to be higher in patients with severe mesangial hypercellularity compared to those of showing mild mesangial hypercellularity (p = 0.002)., Conclusion: It was observed that there is no direct relation between the severity of mesangial hypercellularity and clinical and laboratory findings in various childhood nephropathies. However, when Henoch-Schönlein nephritis is compared with IgA nephropathy, it was found that the severity of mesangial hypercellularity was higher in cases with Henoch-Schönlein nephritis and the level of proteinuria was more prominent in those cases. However, no difference was detected in glomerular filtration rates and biochemical data with regard to the level of mesangial hypercellularity.

- Abstract
Objective: Tumor-stroma proportion of tumor has been presented as a prognostic factor in some types of adenocarcinomas, but there is no information about squamous cell carcinomas and laryngeal carcinomas., Material and Method: Five digital images of the tumor sections were obtained from 85 laryngeal carcinomas. Proportion of epithelial tumor component and stroma were measured by a software tool, allowing the pathologists to mark 205.6 μm2 blocks on areas as carcinomatous/stromal, by clicking at the image. Totally, 3.451 mm2 tumor areas have been marked to 16.785 small square blocks for each case., Results: Median follow up was 48 months (range 3-194). The mean tumor-stroma proportion was 48.63+18.18. There was no difference for tumor-stroma proportion when tumor location, grade, stage and perinodal invasion were considered. Although the following results were statistically insignificant, the mean tumor-stroma proportion was the lowest (37.46±12.49) for subglottic carcinomas, and it was 52.41±37.47, 50.86+19.84 and 44.56±16.91 for supraglottic, transglottic and glottic cases. The tumor-stroma proportion was lowest in cases with perinodal invasion and the highest in cases without lymph node metastasis (44.72±20.23, 47.77±17.37, 50.05±17.34). Tumor-stroma proportion was higher in the basaloid subtype compared with the classical squamous cell carcinoma (53.76±14.70 and 48.63±18.38 respectively). The overall and disease-free survival analysis did not reveal significance for tumor-stroma proportion (p=0.08, p=0.38). Only pathological stage was an independent factor for overall survival (p=0.008)., Conclusion: This is the first series investigating tumor-stroma proportion as a prognostic marker in laryngeal carcinomas proposing a new method, but the findings do not support tumor-stroma proportion as a prognostic marker.

- Abstract
Background/aims: The effect of kefir on peptic ulcer disease was evaluated in an experimental model, with non-steroid anti-inflammatory drugs, together with the determination of gastric mucus secretion by quantitative digital histochemistry., Materials and Methods: The experimental group included 28 male albino Wistar rats. After a diet with standard rat bait for 7 days, 14 rats were fed with kefir for 7 days while the others were kept on the same diet. At the 14th day, indomethacin was injected to 7 of the rats fed on kefir and to 7 of the rats on standard rat bait. All the rats were sacrificed after 4 hours. Gastric erosion and ulceration were scored histopathologically. Mucosal mucus was quantified by image analysis, and periodic acid-Schiff stained area percentage was determined., Results: Erosion and ulceration were identified only in cases that received indomethacin. In the cases on kefir, erosion was identified in 6 cases (86%) and ulceration in 1 case. Rats fed on standard diet had erosion in 4 cases (57%) and ulceration in 3 (43%), but the difference was statistically insignificant (Mann-Whitney test, p=0.25). The stained area percentage for gastric mucus was not different between the four groups (Kruskal-Wallis test, p=0.313)., Conclusions: These findings suggest that kefir does not change gastric mucus secretion. Although statistically insignificant, as there were more cases with ulceration in cases on the rat diet, kefir might have a beneficial effect on peptic ulcer disease induced by non-steroid anti-inflammatory drug. This requires further evaluation in larger series.

30. Juvenile linear scleroderma with unique forms of renal involvement.

- Abstract
Linear scleroderma is the most common type of juvenile localized scleroderma, which usually involves the limbs. The linear lesions may extend through the dermis, subcutaneous tissue, muscles, and the underlying bone, leading to significant functional and cosmetic deformities. Renal involvement in juvenile localized scleroderma is a rare manifestation. Here we report two cases with juvenile linear scleroderma and unique renal manifestations, including unilateral renal arterial stenosis and immunoglobulin M nephropathy.

- Abstract
Immunoglobulin A nephropathy (IgAN) is associated with mucosal IgA defect. Probiotics regulate specific and innate immunity. We evaluated the effect of Saccharomyces boulardii on experimental IgAN in mice. Four groups of BALB/c mice (eight for each) were formed. Group 1 was immunized by oral poliovirus vaccine (OPV) at 0, 14, and 28 days. Group 2 was also given S. boulardii in addition to OPV. Group 3 was given only S. boulardii, whereas group 4 received no treatment. At week 6, after urine and serum samples were obtained for urinalysis and serum creatinine and IgA measurements, all animals were sacrificed to get their kidneys for histopathological evaluation. Urinalysis and serum creatinine levels were normal in all groups. Serum IgA level was increased only in group 1. Whereas group 1 had mesangial proliferation, histology was normal in the other groups. Predominant IgA deposition was universal in group 1, whereas it was either not present or minimal in other groups. Three mice in group 1 also had C3 deposition, which was absent in other groups. Electron microscopy revealed mesangial proliferation, matrix expansion, focal glomerular basement membrane thickening and electron-dense deposits in group 1 only, whereas the other groups were normal. In conclusion, enteral S. boulardii prevented OPV-induced IgAN in mice.

- Abstract
Background: Vascular endothelial growth factor (VEGF) expression influences tubular repair and promotes angiogenesis. The aim of the present study was to determine the relation of VEGF expression and cortical vascularity with renal pathological changes and clinical parameters in allograft biopsies., Materials and Methods: Sections from 50 renal allograft biopsies were evaluated by streptavidine-biotin immunohistochemistry by primary antibodies against VEGF and CD34. Cortical tubulointersititial (TI) VEGF expression was scored by light microscopic examination considering intensity and density. Glomerular expression was scored as 0: no staining; 1: faint staining in less than 50% of glomeruli; 2: moderate to strong staining in more than 50% of glomeruli. We determined the number of vessels per cortical high power field (Nves) highlighted by CD34 staining. The clinical and pathological features were retrieved from patient files., Results: Nves was decreased with interstitial fibrosis (IF): 56.3 +/- 3.7; 53.3 +/- 9.8, 46.6 +/- 10.5, 36.75 +/- 1.89 for cases with no IF to mild, moderate, and severe forms, respectively (P << .000). There was increased TI VEGF expression: 1.86 +/- 2.12, 5.8 +/- 3.1, 5.85 +/- 4.4, 10.25 +/- 2.06, respectively (P = .004). The NVes values were not different for cases with high and low to negative VEGF expression scores. There was a negative correlation between Nves values and creatinine at the time of biopsy and time from transplantation to biopsy (r = -.325, P = .024 and r = -.294, P = .038, respectively). Nves and VEGF scores were not different when acute rejection scores or cyclosporine toxicity were considered (P > .05), while Nves were significantly different for chronic allograft nephropathy scores (P = .05)., Conclusions: Chronic renal changes seemed to be associated with decreased cortical vascularity in renal allografts, while the TI VEGF expression was increased. In contrast Nves was not increased with VEGF expression in this series. It seems that along with VEGF, other factors are required for protection against vascular reduction. The aging of the allograft is also a negative influence on cortical vascularity.

- Abstract
We investigated the question of whether IgM deposition causes any difference in the histomorphometry of children with mesangial proliferative glomerulonephritis (MePGN). We retrospectively compared the urinalysis, blood pressure measurements, and serum creatinine levels of children with IgM (+) and IgM (-) MePGN. For histopathological evaluation, the outlines of Bowman's capsules and glomerular tufts of 10 glomeruli were rimmed manually, and the surrounded areas were calculated as pixels. The cells at each tuft of the largest four glomeruli were counted. Bowman's capsular areas, glomerular tuft areas, glomerular cellularities, and the number of cells per tuft area were determined for each patient. There were 24 (M/F: 14/10) patients with MePGN. Twelve of them were IgM (+). Mean age at biopsy was 6.8+/-4.1 years. IgM (+) patients mostly presented with nephrotic syndrome (NS) (n=9) and IgM (-) patients mostly with macroscopic hematuria (n=7). None of the clinical or laboratory findings was statistically different between the groups (p>0.05) except increased NS frequency in IgM (+) patients (p: 0.038). The histomorphometric parameters were similar in both groups (p>0.05). IgM (+) patients with MePGN mostly presented with NS in our study, but their histomorphometric parameters demonstrated no significant difference.

Purpose: To determine the interaction of heavy silicone oil with various intraocular lens (IOL) materials and whether heavy silicone oil covers the silicone IOL optic as silicone oil does., Setting: Department of Ophthalmology, Dokuz Eylul University, Izmir, Turkey., Methods: The study group comprised 5 poly(methyl methacrylate) (PMMA) IOLs, 4 foldable silicone IOLs, 5 foldable hydrophilic acrylic IOLs, and 5 foldable hydrophobic acrylic IOLs. Each IOL was bathed in balanced salt solution (BSS) for 10 minutes and then placed in heavy silicone oil dyed with Sudan Black for another 10 minutes. Afterward, each IOL was reimmersed in BSS for 5 minutes and examined under the light microscope. Digital images were analyzed to determine the optic area covered with heavy silicone oil., Results: The mean heavy silicone oil coverage was 7.05% +/- 7.88% (SD) (range 1.13% to 20.54%) on PMMA IOLs, 100% on silicone IOLs, 12.17% +/- 11.43% (range 1.25% to 31.52%) on hydrophobic acrylic IOLs, and 34.64% +/- 13.28% (range 12.57% to 44.42%) on hydrophilic acrylic IOLs. Heavy silicone oil coverage of silicone IOLs was statistically significantly greater than the coverage of other IOL materials., Conclusion: Heavy silicone oil acted the same as silicone oil and covered the entire surface of silicone IOLs.

- Abstract
The effect of altitude on renal failure and bone mineralization is not well known. This topic is studied in a 5/6 nephrectomy rat model. After hemoglobin, creatinine clearance and proteinuria were determined in 28 Wistar rats. Two 5/6 nephrectomy (Nx1-Nx2, n=7 each) and two sham (Sh1-Sh2, n=7 each) groups were formed. The Nx1-Sh1 and Nx2-Sh2 groups were kept at sea level and at 1200 m altitude, respectively. The same analyses were performed after 3 months just before sacrifices in order to harvest kidneys and femurs for histopathologic examination. Hemoglobin, creatinine clearance, and proteinuria were similar in all groups at the onset. Final hemoglobin was higher in Nx2-Sh2, but only Sh2 vs. Sh1 was significant (p=0.001). Creatinine clearance decreased (p=0.001 for Nx1) and proteinuria increased (p=0.002 for Nx1 and p=0.005 for Nx2) after 5/6 nephrectomy, but Nx1 vs. Nx2 was similar. Histopathological changes in the remnant kidneys were prominent, but Nx1 vs. Nx2 was not different. Although the relative osteoid volume increased in Nx groups, only Nx1 vs. Sh1 was different (p=0.006). In conclusion, exposure to 1200 m altitude, compared to the sea level, preserved the creatinine clearance better in 5/6 nephrectomized rats. No change was observed in proteinuria, renal histopathology, and bone mineralization.

Objective: To evaluate the expression of HSP70 and pS2 and to determine whether it may be an additional prognostic variable in the prediction of recurrence and survival in rectal adenocarcinomas., Methods: The paraffin sections of 45 patients with rectal carcinoma who were treated with surgical resection were stained with HSP70 and pS2 antibodies by using the standard biotin immunoperoxidase method. Cytoplasmic staining for both antibodies was scored semiquantitatively., Results: Only 16 (35.6%) tumors showed a positive cytoplasmic reaction with HSP70 antibody, while pS2 expression was observed in 26 (57.8%) tumors. There was an association between HSP70 and pS2 expression (P=0.002). No correlations were found between HSP70 and pS2 expression and tumor recurrence or overall survival and other prognostic factors. However, the type of surgical resection was significantly associated with pS2 expression status (P=0.013). Significant correlations were detected between tumor recurrence and other clinicopathologic parameters, such as clinical stage, lymph node involvement, and resection type (P=0.015, P=0.015, and P=0.03, respectively). Resection type was significantly associated with clinical outcome, recurrence, and metastasis (P=0.009, P=0.03, P<0.01, respectively). In addition, there was a statistically significant relationship between clinical stage and final outcome (P=0.005)., Conclusions: The strong correlation between pS2 expression and incomplete surgical resection suggests that pS2 may be related to invasive tumor behavior and may also play a role in tumor recurrence, although this latter association did not reach statistical significance in this study. HSP70 expression does not appear to be related to tumor invasiveness or tumor recurrence.

Many checkpoint proteins that are involved in the control of the cell cycle and apoptosis have been investigated, but only a few studies have evaluated the prognostic significance of multiple factors only in rectal carcinomas. The aim of this study was to determine the role of p53, p21, and p27 protein expression as a prognostic factor in rectal carcinomas. Formalin-fixed, paraffin-embedded tissue blocks from 45 rectal adenocarcinomas with appropriate clinical and prognostic data were examined. The standard streptavidin-biotin immunoperoxidase method was used for immunostaining with p53 protein, p21 WAF1/Cip1 protein, and p27 Kip1 protein. The extent of positive p53, p21, and p27 staining was graded semiquantitatively. The clinicopathologic and prognostic features were statistically analyzed. No significant association was found between p53 status and p21 or p27 protein expression (chi2 test, P=0.42 and P=0.18 respectively). There was no correlation between the expressions of p53, p21, and p27, and conventional clinicopathologic features. The mean time interval to recurrence was 25.7+/-24.7 months (range, 0-54 months). p53, p21, and p27 expression was not associated significantly with recurrence and distant metastasis. However, a significant relationship was found between the expression of p27 protein and hepatic metastasis (independent samples t-test, P=0.007). The authors concluded that p53, p27, and p21 protein expression was not related to the clinicopathologic parameters, tumor aggressiveness, metastatic potential, and survival in rectal carcinomas. Further studies are needed to evaluate the predictors of outcome in rectal cancer, considering a variety of prognosticators.

38. Sarcoidosis with an uncommon presentation: apropos of a case.

- Abstract
A 17-year-old male presenting with chronic renal failure whose supporting clinical manifestations of the disease had appeared independently over a four-year period is reported. The renal biopsy specimen of the patient revealed tubulointerstitial nephritis and membranous glomerulonephritis. He never had hilar adenopathy, but maculopapular rashes, erythema nodosum, arthritis, chronic lymphocytopenia, hepatomegaly, splenomegaly, and lymphadenomegaly had been observed at different periods over four years. The presence of non-caseating granulomatous lesions in the liver biopsy accompanying uveitis verified the diagnosis of sarcoidosis. Low dose steroid was applied to this hepatitis-C carrier, and uveitis was suppressed. No recurrence has been observed in two-year follow-up.

Background: We examined the effect of L-carnitine on experimental renal ischemia-reperfusion (I-R) injury., Methods: Seventy-two Wistar albino rats were divided into five groups, four of which underwent right nephrectomy and left renal I-R for 60 min (groups 2 and 4) and 90 min (groups 3 and 5). The sham operated group (n=8) was left as controls. Intraperitoneal carnitine (200 mg/kg) was given to groups 4 and 5 three hours before operation. For histopathologic studies, half of the rats in the study groups (n=8) and the remaining rats were sacrificed 15 min and seven days after reperfusion, respectively. Blood urea nitrogen (BUN) and serum creatinine levels were also measured at 15 min and on Day 3 and Day 7., Results: Concerning the 90 min I-R groups, survival was improved in carnitine-treated rats (7/8 vs 5/8). Of untreated groups, the 90 min I-R group exhibited significantly higher BUN (p=0.0023) and serum creatinine (p=0.0086) levels on Day 3. Although BUN and serum creatinine levels did not show significant differences between carnitine-treated and untreated rats in the two 60 min I-R groups throughout the study period, carnitine-treated rats in the 90 min I-R group exhibited significantly improved BUN (p=0.0063) and serum creatinine (p=0.0013) levels on Day 3. The severity of histopathologic changes was lower in all carnitine-treated rats 15 min after reperfusion. On Day 7, the mean histopathologic score was significantly lower in carnitine-treated rats in the 90 min I-R group than that of untreated rats exposed to I-R injury of same duration (p=0.0078)., Conclusion: Our results suggest that L-carnitine has beneficial effects on renal function, histopathologic changes, and survival in renal ischemia-reperfusion injury.

There is still controversy about the classification of laryngeal preneoplastic lesions. In this study, we compared the World Health Organization (WHO) and Ljubljana histological classifications with regard to laryngeal epithelial hyperplastic-dysplastic lesions in a single series to determine their relation to statistically significant criteria when the mechanisms of neoplastic progression are considered. Emphasis was put on p53 expression, proliferative activity, and angiogenesis. Fifty-four laryngeal biopsies with preneoplastic changes were re-evaluated and classified according to both classifications. The streptavidin-biotin method was used for immunohistochemical staining for Ki-67, p53, and CD34 antibodies. A positive correlation was obtained between the histological categories and Ki-67, p53, and CD34 expressions using both classifications (Spearman's Correlation test). There was a significant difference between the histological categories of both the WHO and the Ljubljana classifications, when the expression of the three markers was compared (Kruskal Wallis test, p = 0.000 for each). Further evaluation revealed a statistically significant difference between all categories of both classifications, excluding the p53 overexpression scores and vessel counts in mild and moderate dysplasia categories (Mann-Whitney U-test, p = 0.209, and 0.091 respectively), and the p53 overexpression scores in severe dysplasia and carcinoma in situ categories (Mann-Whitney U-test, p = 0.249) of the WHO classification. Similarly, no significant differences were found between severe dysplasia and carcinoma in situ, as well as between atypical hyperplasia and carcinoma in situ categories using both classifications for the Ki-67 expression (Mann-Whitney U-test, p = 0.806, and 0.111, respectively). Our results suggest that regarding the mechanisms of neoplastic progression such as p53 expression and angiogenesis, the histological categories of the Ljubljana classification seem to depend on additional evidence. Therefore, we support the use of the Ljubljana classification.

- Abstract
CD44 is a type I transmembrane glycoprotein serving as a cell adhesion receptor, whose main ligand is hyaluronic acid, but also may interact with collagen, laminin, fibronectin, and osteopontin. This marker is involved in cell migration, homing, activation, metastasis, and inflammation. Tubular CD44 expression has been shown to correlate with scarring in renal diseases, but there is little data on allograft biopsies. This deficiency is important since experimental studies have shown that blockade of the CD44-hyaluronic acid interaction may prolong allograft survival. In an attempt to clarify the role of tubular CD44 expression in renal allografts, CD44 expression was determined immunohistochemically in 37 allograft and 10 implantation biopsies, as the percentage of tubules expressing this marker. For implantation biopsies the mean tubular CD44 expression was 6% +/- 14%; for allograft biopsies, 13% +/- 20% (P =.17, Mann-Whitney U). By the Spearman correlation test, CD44 expression did not correlate with Banff scores, but was moderately correlated with serum creatinine values at the time of biopsy (P =.017, r =.4). These findings suggest an important role of tubular CD44 expression in renal allografts. It appears to be induced by more than one pathway, resulting in a pattern of expression that correlated with renal function. However larger series are required before recommending the routine use of this marker.

- Abstract
Objectives: We investigated the incidence of level I metastasis in patients with laryngeal and hypopharyngeal squamous cell carcinoma (SCC)., Patients and Methods: The records of 126 patients who underwent primary tumor excision with radical neck dissection (RND) or its modifications for laryngeal or hypopharyngeal SCC were retrospectively reviewed. Preoperative tumor and neck stages, the sites and the number of metastatic lymph nodes were recorded. Patients treated with selective neck dissection (SND) or preoperative chemotherapy and/or radiation therapy were excluded., Results: Of 155 RND or modified RND performed for 113 laryngeal and 13 hypopharyngeal SCC, lymph node metastases were detected in 51 specimens, all of which spared level I. The most frequently involved levels were II and III., Conclusion: Selective neck dissection sparing level I may be appropriate for clinically and radiologically N0 patients with laryngopharyngeal carcinoma.

- Abstract
Maintenance of serum albumin levels within normal limits is difficult to achieve in nephrotic children with normal renal functions who are unresponsive to specific treatment. One approach in such children is unilateral nephrectomy with rapid progression to renal failure. Peritoneal membrane is permeable to fluids, electrolytes and proteins, and peritoneal space has been used for total parenteral alimentation. Experimental ureteroperitoneostomy has been reported not to cause any significant side effect. The aim of this study was to evaluate the effects of unilateral ureteroperitoneostomy on serum and urine protein levels in rats with adriamycin-induced nephrosis. Adriamycin nephrosis was induced in 45 male Wistar rats. After two weeks, unilateral nephrectomy (Nx), unilateral ureteroperitoneostomy (Up) and sham operated (Sh) groups, each including 15 rats were formed. Serum creatine (S(Cr)) and albumin (S(alb)), and daily urinary protein excretion (U(pro)) were determined before adriamycin injection (week 0), before operations (week 2) and at the end of 6th week in all rats. In addition, percent change in serum albumin (deltaS(alb)) and urine protein levels (deltaU(pro)) between weeks 0-2, 0-6 and 2-6 were calculated for each group (e.g.; deltaS(alb) 0-2 = [S(alb) week 2-S(alb) week 0]/S(alb) week 0 x 100). Then, these parameters were compared within and between the groups. Furthermore, peritoneal tissue samples were obtained from the rats in Sh and Up groups to be examined for pathological changes. S(Cr) did not change within and in between the groups during the study period. S(alb) decreased significantly at weeks 2 and 6 with respect to week 0 in all three groups. In addition, although S(alb) tended to decrease at week 6 with respect to week 2 in all groups, this was significant only in Sh group. U(pro) increased significantly at weeks 2 and 6 with respect to week 0, and at week 6 with respect to week 2 in all groups. However, S(alb) and U(pro) were not different between the three groups at weeks 0.2 and 6. On the other hand, deltaS(alb) and deltaU(pro) were not different between Sh vs. Nx and Nx vs. Up rats, but deltaS(alb) 0-6, deltaS(alb) 2-6 and deltaU(pro) 0-6 were significantly lower in Up group compared to Sh group. Histopathological examination of peritoneal samples revealed significantly higher fibrosis score in Up group compared to Sh group. In conclusion, unilateral ureteroperitoneostomy may one important therapeutic selection in the treatment of intractable nephrotic syndrome. However, peritoneal fibrosis could be a concern for further use of peritoneum in case of end stage renal failure.

- Abstract
Hypothesis: The goal of this study was to evaluate the effects of hyaluronic acid, epidermal growth factor, and mitomycin C on the healing of acute experimental traumatic perforations of the tympanic membrane., Background: Most acute perforations of the tympanic membrane heal spontaneously. However, some form of surgical treatment (i.e., myringoplasty) is needed for nonhealing perforations. Because the closure occurs by squamous epithelial migration, drugs that stimulate this regenerative process may aid in the closure of the perforation, obviating the need for more extensive treatments., Methods: Bilateral perforations of the tympanic membrane were created in 30 rats, divided into three groups (A, B, C). The perforations in the right ears were treated with hyaluronic acid, epidermal growth factor, or mitomycin C. Those in the left ears were left untreated for comparison. To examine the healing process in different periods, 5 animals were killed in each group at days 3, 5, 7, 9 and 14. The other 5 animals in each group were observed daily to determine the duration of perforation closures. Thirty surgical specimens (5 right sides from each group and all 15 left sides in all groups) were histopathologically examined for tympanic membrane thickness, fibroblastic reaction, neovascularization, and crust morphology., Results: Hyaluronic acid and epidermal growth factor applications significantly shortened the healing in acute experimental traumatic perforations of the tympanic membrane (p = 0.0432); however, the difference between them was not significant (p = 0.3160). On the other side, tympanic membrane perforations treated with topical mitomycin C showed no evidence of closure. There were no significant differences in the histologic parameters between the treated groups and their contralateral control ears., Conclusion: Hyaluronic acid and epidermal growth factor accelerated the closure of acute tympanic membrane perforations in rats. This may make them clinically useful in augmenting the efficiency of conservative treatments of acute perforations of the tympanic membrane.

- Abstract
The aim of this study was to evaluate the interaction between various widely used intraocular lenses (IOLs) and silicone oils of different viscosities. Four groups of IOLs, including monoblock foldable hydrophilic acrylic IOLs (Morcher, type 92s); monoblock hydrophobic acrylic IOLs (Acrysof-SA60AT, Alcon); single-piece rigid polymethylmethacrylate (PMMA) IOLs (Intraocular Optical International-IOI-65130) and a three-piece foldable silicone optic IOL (CeeOn Edge 911A, Pharmacia UpJohn) were analyzed in vitro to determine the percentage adherence 1,000-centistoke, 1,300-centistoke or 5,000-centistokes silicone oil on the IOL optic. For each IOL type, there was no statistically significant difference in the mean silicone oil coverage (MSC) of the IOL optics for the different viscosities of silicone oil. Silicone IOLs had the highest MSC percentage (79.9%) whereas hydrophilic acrylic IOLs were the least silicone-covered IOLs (7.8%) compared to the other IOL types tested in this study. It is not the concentration of silicone oil that affects silicone oil coverage. When performing small-incision cataract surgery in patients who may require silicone oil injection, foldable hydrophilic acrylic or hydrophobic acrylic lenses should be preferred over standard foldable silicone lenses., (Copyright 2003 S. - Abstract
Objective: Although 25-45% of all schwannomas are reported to occur in the head and neck region, nasal cavity and paranasal sinus involvement is rare, with 32 such cases described till 1999. Of these cases, only three were cystic. Herein we present a cystic schwannoma of the maxillary sinus of a 66-year-old, otherwise healthy male patient., Methods: The tumor tissue was routinely processed, embedded in paraffin, and stained with H&E. Immunostaining was performed for S-100 protein, epithelial membrane antigen (EMA) and cytokeratin., Results: A mass with sudden enlargement, inhomogeneous enhancement with cystic areas, S-100 positive membrane-like structures lining the cystic cavity of an otherwise classical schwannoma were the main features encountered in our case., Conclusion: We recommend that cystic schwannoma should be kept in mind in the differential diagnosis of cystic masses of the maxilla.

- Abstract
We report a nine-year-old girl who presented with the clinical and laboratory findings of rapidly progressive glomerulonephritis. She was found to be positive for both pANCA and cANCA. However, renal histopathology revealed immune complex type of crescentic glomerulonephritis. Thus, although testing for ANCA is an important tool in the prediction of the subtype of crescentic glomerulonephritis, a renal biopsy is still required to establish the diagnosis.

- Abstract
Objective: Phenotypical changes in the tubular epithelial cells (TEC) seem to be important in the progression of renal diseases. The present study was designed to identify the relation between the expression of major histocompatibility complex (MHC) class II antigens and CD44 by TEC, with parameters of renal scarring in primary and systemic renal diseases., Material and Methods: Expression of MHC class II and CD44 antigens was determined immunohistochemically in 71 renal biopsies and eight nephrectomy specimens with chronic pyelonephritis (CP)., Results: CD44 expression was increased in renal diseases compared with autopsy cases and was strongly correlated with parameters of renal scarring and MHC class II antigen expression in primary and systemic renal diseases. CD44 expression was demonstrated in chronic pyelonephritis, postinfectious glomerulonephritis, diabetic nephropathy and hypertensive nephropathy, as well as other diseases described previously. Similar results were obtained for MHC class II antigen expression by TEC and these results were correlated with serum creatinine values., Conclusions: CD44 expression by TEC is a common pathway in renal scarring, like MHC class II antigen expression, and both of these may be important in renal scarring in CP cases as well as other primary and systemic renal diseases.

- Abstract
Renal scarring due to pyelonephritis was shown to improve in rats given vitamin A. We evaluated the effect of vitamin A in a renal ablation nephropathy model. Four groups, each including 7 rats with 5/6 nephrectomy, were formed: group I (no vitamin A), group II (60 kIU vitamin A), group III (120 kIU vitamin A), and group IV (180 kIU vitamin A). Four sham-operated rats comprised the control group. After 6 weeks of 5/6 nephrectomy, the rats were sacrificed and serum creatinine, vitamin A, and beta-carotene levels were determined in addition to histopathological evaluation of the remnant kidneys. The tubulointerstitial and glomerular changes were graded as "0-3" and "0-5" respectively, in accordance with the severity of the lesions. Tubulointerstitial score (TIS), mean glomerulosclerosis score (MGS, arithmetical mean of the sclerosis scores of 100 glomeruli), and severity of glomerulosclerosis index (SGI, ratio of the number of glomeruli with grade > or = 3 sclerosis to the total number of glomeruli examined) were calculated for each rat. Serum creatinine levels were higher in the study groups than the control rats (P < 0.05), but there was no significant difference between the study groups (although the levels increased as the dose of vitamin A increased). Serum vitamin A levels were significantly higher in the groups given vitamin A than the control rats and group I (P < 0.05). In addition, serum vitamin A levels increased significantly in parallel to increasing doses of vitamin A (P < 0.05). Serum beta-carotene levels did not differ between the groups, except for group II, which had lower levels than controls (P = 0.01). MGS and SGI were significantly higher in the study groups than control rats (P < 0.05), but did not differ between the study groups. Study and control rats were not different with respect to TIS, but there was a difference between the control group and group III (P = 0.04). Group II had the lowest MGS, SGI, and TIS scores among the study groups. When all the rats were considered together, vitamin A levels did not correlate with the MGS and SGI, but correlated positively with the TIS (r = 0.391, P = 0.027). beta-Carotene levels also did not correlate with the MGS, SGI, and TIS. In conclusion, vitamin A administration did not significantly affect the clinical and pathological course of renal ablation nephropathy in rats. Furthermore, higher doses of vitamin A might even damage renal tissue.
