1. Propiedades farmacológicas de Annona muricata como terapia alternativa en el tratamiento del cáncer: revisión sistemática

Abstract

[Objetivo] El objetivo fue determinar las propiedades farmacológicas y terapéuticas de la Annona muricata, como terapia alternativa o preventiva para el tratamiento del cáncer. [Métodos] Se realizó una búsqueda sistemática en tres bases de datos (Scopus, Pubmed/MEDLINE y Web of Science) utilizando palabras claves y vocabularios controlados sobre Annona muricata, terapia alternativa o preventiva y tratamiento del cáncer, siguiendo la metodología de la declaración Prisma. Después de la búsqueda inicial, los estudios seleccionados se exportaron al gestor bibliográfico de referencias Mendeley. Para la extracción de los datos de los estudios incluidos, se utilizó el programa Excel, y se empleó un formulario referencial. [Resultados] Se seleccionaron diez estudios, los cuales evidenciaron el efecto farmacológico de los extractos de hojas de Annona muricata sobre las células cancerígenas diversas. Todos los estudios seleccionados correspondieron a ensayos in vitro/in vivo. Asimismo, los estudios demostraron la presencia de diversos compuestos bioactivos presentes en la Annona muricata, los cuales le confieren un poder citotóxico y anticancerígeno. [Conclusiones] Esta revisión sistemática demostró que la Annona muricata, es una planta de excelente potencial anticancerígeno y citotóxico, para ser utilizada como terapia alternativa o complementaria en diversos tipos de cáncer, debido a que presenta compuestos bioactivos, como acetogeninas, compuestos fenólicos, flavonoides, alcaloides, entre otros, cuyo efecto principal es la apoptosis de células cancerígenas. Sin embargo, se requieren de mayores análisis, principalmente de intervención, para probar su efectividad en beneficio de la salud humana.

Abstract

El presente artículo analiza el papel crucial de las estrategias disruptivas en la supervivencia y adaptación de la industria gastronómica durante la pandemia de la COVID-19. Mediante una investigación cualitativa basada en entrevistas en profundidad con empresarios, emprendedores y gerentes gastronómicos, se identificaron tendencias y prácticas comunes que reflejan un enfoque proactivo y adaptable para enfrentar la crisis. Los resultados revelaron una amplia gama de estrategias innovadoras, que van desde la personalización de servicios de entrega hasta la diversificación del producto y la colaboración estratégica con otros establecimientos. La rápida adaptación a las nuevas condiciones del mercado, la gestión eficaz de riesgos y la búsqueda activa de alianzas estratégicas emergieron como elementos clave para la resiliencia empresarial. Además, se destacó la importancia de la transformación digital y la comunicación efectiva con los clientes como factores determinantes para mantener la relevancia y competitividad en un entorno empresarial en constante cambio.

Abstract

(1) The prevention of cytomegalovirus (CMV) in lung transplant recipients (LTx) is based on the administration of VGC for a period of 6–12 months, but there is little information on the premature discontinuation of the drug. Our objective was to evaluate the reasons for early cessation of VGC and the dynamics of CMV replication after discontinuation. (2) We carried out a retrospective study of LTx on VGC prophylaxis according to guidelines, with an outpatient follow-up period of >90 days. The detection of any level of CMV-DNA in the plasma (Cobas, Roche Diagnostics®) during a period of 18 months after the discontinuation of VGC was considered positive. (3) We included 312 patients (64% male, mean age 53.50 ± 12.27; 71% D+R+, 15% D−R+, and 14% D+R−) in our study. The prescribed prophylaxis was completed by 179 patients (57.05%). The mean duration of prophylaxis was 7.17 ± 1.08 months. The recorded reasons for VGC discontinuation in 133 patients (43%) were myelotoxicity (n = 55), impaired renal function (n = 32), and gastrointestinal disturbances (n = 11). The reason for discontinuation was not recorded for 29 patients. CMV-DNA was detected in 79% (n = 246) of cases, and D+R+ and D+R− recipients showed a high risk of detection (p < 0.001). The median times to onset of CMV-DNA detection were 35 days in D+R−, 73 days in D+R+, and 96 days in D−R+ (p < 0.001). (4) Adverse effects of VGC are frequent in LTx. CMV-DNA detection is very common after the discontinuation of VGC and is related to the CMV donor and recipient serostatus.

10. Impact of Neutropenia on Clinical Outcomes after Lung Transplantation

Abstract

Background/Objectives: Neutropenia is a frequent complication among solid organ transplant (SOT) recipients receiving immunosuppressive therapy and antimicrobial prophylaxis. However, there are limited studies analysing the frequency and impact of neutropenia in lung transplant recipients (LTRs). Our aim was to analyse the frequency of neutropenia, the need for granulocyte colony-stimulating factor (GCSF) treatment within the first 18 months post-transplant and its association with acute rejection, chronic lung allograft dysfunction (CLAD), overall survival and the development of infections. Methods: This observational and retrospective study recruited 305 patients who underwent lung transplantation between 2009 and 2019, with outpatient quarterly follow-up during the first 18 months post-surgery.Results: During this period, 51.8% of patients experienced at least one episode of neutropenia. Neutropenia was classified as mild in 50.57% of cases, moderate in 36.88% and severe in 12.54%. GCSF treatment was indicated in 23.28% of patients, with a mean dose of 3.53 units. No statistically significant association was observed between neutropenia or its severity and the development of acute rejection, CLAD or overall survival. However, the patients who received GCSF treatment had a higher mortality rate compared to those who did not. Sixteen patients (5.25%) developed infections during neutropenia, with bacterial infections being the most common. Conclusions: Neutropenia is common in the first 18 months after lung transplantation and most episodes are mild. We did not find an association between neutropenia and acute rejection, CLAD, or mortality. However, the use of GCSF were associated with worse post-transplant survival.

14. The demands and boundaries of consent for medical treatment in the National Health Service

Abstract

It is widely accepted that consent is underpinned by the principle of respect for autonomy. Accordingly, doctors are instructed to obtain consent for medical treatment through decision-making in partnership with patients. Yet, decision-making involves preceding judgments about the treatment-options that should be offered, and sometimes conflicting considerations of the doctor's duty to care for the patient. This thesis engages Amartya Sen's capability approach as a theoretical tool to understand the idea of consent and propose a model that responds to the broader context. Central to the capability approach is the argument that a person's well-being is achieved through her freedom to 'be and do' what she has reason to value. In the context of health, this can be understood as securing a person's capability to achieve medical treatment in line with her agency; that is, her unique health-goals. This capability has two aspects: treatments that are available to a person, as the means to her goals; and her opportunities to achieve or reject these treatments. The capability approach supplies critical conceptual clarity and distinction of these two aspects, and it enables the articulation a clear model of the wider demands and boundaries of consent. Key to attaining this model is Sen's focus on public reason: a way of interpreting the underpinning principles that would be acceptable to a reasonable person. As per John Rawls, superior courts are fora of public reasoning. This thesis analyses significant, consent-focused case law within a capabilities framework to expose evolving ideas of justice that culminate in the Supreme Court's decision in Montgomery. Traditionally, judges relied on the private reasoning of doctors for both aspects of a patient's capability to achieve treatment; then, there is shift to public reason. Yet, reliance persists on medical professionalism, and Montgomery illuminates judicial strategy as well as reveals ongoing challenges.

Abstract

Abstract Multi‐omics analyses are used in microbiome studies to understand molecular changes in microbial communities exposed to different conditions. However, it is not always clear how much each omics data type contributes to our understanding and whether they are concordant with each other. Here, we map the molecular response of a synthetic community of 32 human gut bacteria to three non‐antibiotic drugs by using five omics layers (16S rRNA gene profiling, metagenomics, metatranscriptomics, metaproteomics and metabolomics). We find that all the omics methods with species resolution are highly consistent in estimating relative species abundances. Furthermore, different omics methods complement each other for capturing functional changes. For example, while nearly all the omics data types captured that the antipsychotic drug chlorpromazine selectively inhibits Bacteroidota representatives in the community, the metatranscriptome and metaproteome suggested that the drug induces stress responses related to protein quality control. Metabolomics revealed a decrease in oligosaccharide uptake, likely caused by Bacteroidota depletion. Our study highlights how multi‐omics datasets can be utilized to reveal complex molecular responses to external perturbations in microbial communities.

19. Impact of Neutropenia on Clinical Outcomes after Lung Transplantation.

Abstract

Background/Objectives: Neutropenia is a frequent complication among solid organ transplant (SOT) recipients receiving immunosuppressive therapy and antimicrobial prophylaxis. However, there are limited studies analysing the frequency and impact of neutropenia in lung transplant recipients (LTRs). Our aim was to analyse the frequency of neutropenia, the need for granulocyte colony-stimulating factor (GCSF) treatment within the first 18 months post-transplant and its association with acute rejection, chronic lung allograft dysfunction (CLAD), overall survival and the development of infections. Methods: This observational and retrospective study recruited 305 patients who underwent lung transplantation between 2009 and 2019, with outpatient quarterly follow-up during the first 18 months post-surgery.Results: During this period, 51.8% of patients experienced at least one episode of neutropenia. Neutropenia was classified as mild in 50.57% of cases, moderate in 36.88% and severe in 12.54%. GCSF treatment was indicated in 23.28% of patients, with a mean dose of 3.53 units. No statistically significant association was observed between neutropenia or its severity and the development of acute rejection, CLAD or overall survival. However, the patients who received GCSF treatment had a higher mortality rate compared to those who did not. Sixteen patients (5.25%) developed infections during neutropenia, with bacterial infections being the most common. Conclusions: Neutropenia is common in the first 18 months after lung transplantation and most episodes are mild. We did not find an association between neutropenia and acute rejection, CLAD, or mortality. However, the use of GCSF were associated with worse post-transplant survival. [ABSTRACT FROM AUTHOR]

Abstract

(1) The prevention of cytomegalovirus (CMV) in lung transplant recipients (LTx) is based on the administration of VGC for a period of 6–12 months, but there is little information on the premature discontinuation of the drug. Our objective was to evaluate the reasons for early cessation of VGC and the dynamics of CMV replication after discontinuation. (2) We carried out a retrospective study of LTx on VGC prophylaxis according to guidelines, with an outpatient follow-up period of >90 days. The detection of any level of CMV-DNA in the plasma (Cobas, Roche Diagnostics®) during a period of 18 months after the discontinuation of VGC was considered positive. (3) We included 312 patients (64% male, mean age 53.50 ± 12.27; 71% D+R+, 15% D−R+, and 14% D+R−) in our study. The prescribed prophylaxis was completed by 179 patients (57.05%). The mean duration of prophylaxis was 7.17 ± 1.08 months. The recorded reasons for VGC discontinuation in 133 patients (43%) were myelotoxicity (n = 55), impaired renal function (n = 32), and gastrointestinal disturbances (n = 11). The reason for discontinuation was not recorded for 29 patients. CMV-DNA was detected in 79% (n = 246) of cases, and D+R+ and D+R− recipients showed a high risk of detection (p < 0.001). The median times to onset of CMV-DNA detection were 35 days in D+R−, 73 days in D+R+, and 96 days in D−R+ (p < 0.001). (4) Adverse effects of VGC are frequent in LTx. Abstract

On 19 July 2021, Israel became one of the first countries in the world to address the crisis of its ageing population through a binding resolution. The plans included a formal system of measuring progress, a set of Indicators for Optimal Ageing, and a shared vision to promote urban planning policy that integrates innovation, inclusion, and new partnerships. As a result, Israel is currently experiencing robust growth in evidence-based, measurable, multisectoral, and sustainable initiatives to enhance older adults' well-being, adding health to years, and not only years, to the average Israeli life expectancy. This paper highlights national and regional applications of that initiative, describing how this approach mobilizes diverse stakeholders in government, local authorities, civil society, and the private sector, to collaborate in addressing the challenges that face older persons on a large scale. Showcasing strategies and solutions in Israel's multicultural society may inform other national efforts in Europe and beyond. [ABSTRACT FROM AUTHOR]

Abstract

Copper homeostasis mechanisms are essential for microbial adaption to changing copper levels within the host during infection. In the opportunistic fungal pathogen Cryptococcus neoformans (Cn), the Cn Cbi1/Bim1 protein is a newly identified copper binding and release protein that is highly induced during copper limitation. Recent studies demonstrated that Cbi1 functions in copper uptake through the Ctr1 copper transporter during copper limitation. However, the mechanism of Cbi1 action is unknown. The fungal cell wall is a dynamic structure primarily composed of carbohydrate polymers, such as chitin and chitosan, polymers known to strongly bind copper ions. We demonstrated that Cbi1 depletion affects cell wall integrity and architecture, connecting copper homeostasis with adaptive changes within the fungal cell wall. The cbi1Δ mutant strain possesses an aberrant cell wall gene transcriptional signature as well as defects in chitin / chitosan deposition and exposure. Furthermore, using Cn strains defective in chitosan biosynthesis, we demonstrated that cell wall chitosan modulates the ability of the fungal cell to withstand copper stress. Given the previously described role for Cbi1 in copper uptake, we propose that this copper-binding protein could be involved in shuttling copper from the cell wall to the copper transporter Ctr1 for regulated microbial copper uptake.

Abstract

Considering the relevance that foreign investments have for companies, governments, and society in general, their socioeconomic consequences, and the legal issues that often arise, the objective of this Doctoral Thesis is to investigate the problems derived from the expropriatory and nationalizing processes undertaken in Latin America in recent years. Specifically, it is about determining whether the States receiving foreign investment offer legal security to said investments and, in the event of expropriation or nationalization, whether the mechanisms for resolving possible disputes are efficient. This general objective is deployed in the following specific objectives. Firstly, analyze the characteristics of foreign investment in Latin America, which will allow us to better understand its specific behavioral dynamics. Secondly, study the expropriation and nationalization processes in Latin America in recent years. Third, address the effectiveness of the mechanisms that currently exist to protect foreign investment in Latin America and examine their effectiveness in nationalization and expropriation processes. The work confirms foreign investment as a driver of economic, political, and social development in the countries of Latin America. However, certain trends are suspicious of these processes, which is reflected in the increase in cases registered with the ICSID in certain countries. Secondly, the evidence collected during the analysis of expropriation processes in LA suggests that States receiving foreign investment in the region face significant challenges in terms of legal certainty for foreign investors. The lack of a comprehensive multilateral legal framework, together with unethical business practices and the proliferation of expropriation and nationalization processes, contribute to generating insecurity and risks for foreign investments in the region. Thirdly, the case studies analyzed reveal the importance of taking into account not only economic factors, Partiendo de la relevancia que tienen las inversiones extranjeras para empresas, gobiernos y la sociedad en general, sus consecuencias socioeconómicas y las cuestiones jurídicas que muchas veces se plantean, el objetivo de la presente Tesis Doctoral es investigar la problemática derivada de los procesos expropiadores y nacionalizadores emprendidos en AL en los últimos años. Concretamente, se trata de determinar si los Estados receptores de inversión extranjera ofrecen seguridad jurídica a dichas inversiones y, en caso de que se den la expropiación o nacionalización, si los mecanismos de resolución de posibles controversias resulten eficientes. Este objetivo general se despliega en los siguientes objetivos específicos. En primer lugar, analizar cuáles son las características propias de la inversión extranjera en AL, lo cual permitirá comprender mejor sus dinámicas específicas de comportamiento. En segundo lugar, estudiar los procesos de expropiación y nacionalización en AL de los últimos años. En tercer lugar, abordar la eficacia de los mecanismos que existen actualmente para proteger la inversión extranjera en AL y examinar la efectividad de estos en los procesos de nacionalización y expropiación. En el trabajo se constata la inversión extranjera como motor de desarrollo económico, político y social en los países de AL. Si bien, existen ciertas tendencias que recelan de estos procesos, lo que se refleja en el incremento de casos registrados ante el CIADI en determinados países. En segundo lugar, la evidencia recopilada durante el análisis de los procesos de expropiación en AL sugiere que los Estados receptores de inversión extranjera en la región enfrentan desafíos significativos en cuanto a la seguridad jurídica para los inversores extranjeros. Se plantea describir, caracterizar y resaltar la importancia del departamento de housekeeping en centros hospitalarios. Para ello, en el marco teórico se exponen las particularidades de este departamento cuando se desarrolla en el ámbito de la salud. Se toma como referencia los resultados primarios del estudio titulado: Servicio de housekeeping en las habitaciones de la clínica Ricardo Palma y su relación con el bienestar del paciente - 2018 (Vizcarra, 2019). Los hallazgos permiten señalar la importancia del servicio de este departamento en los centros hospitalarios, así como del profesional hotelero para la gestión del mismo.

In Montgomery v Lanarkshire Health Board, the Supreme Court declared that patients are entitled to choose among all available treatments. But, who decides what is ‘available’? Doctors rely upon evidence-based medicine (EBM) – a complex epistemology that generates normative recommendations – for determining treatment availability. EBM is required to be applied through exercise of the treating doctor’s clinical judgement, which inescapably involves some value judgements. EBM attempts to accommodate patient values through shared decision-making (SDM). However, SDM remains contingent upon EBM for its starting points, and this contingency provokes a charge of epistemic injustice because it privileges the treating doctor’s judgement over conflicting views. Such a charge was implicit in Montgomery. The Court responded by adopting a strategy that retains reliance on clinical judgement, yet attempts to restrict the treating doctor’s privilege by admitting other professional views. This strategy permits the induction of a three-step test for availability of treatment, which is theoretically appealing but challenges the practical organisation of the medical profession.

Abstract

Diversas publicaciones, reportan efectos dañinos por contaminación sonora siendo de importancia médica y social; que provocan en las personas efectos como estrés, ansiedad, problemas cardiovasculares, deterioro cognitivo y disminucion del estado fisico entre otros. El objetivo de la investigación fue monitorear el sonido in situ en la ciudad de Barranca, Lima – Perú e identificar los decibeles al cual estarían sometidos algunas zonas de protección especial y comercial. El registro de sonido se realizó con un sonometro, en los frontis de instituciones educativas, centros de salud, y zonas comerciales. Los resultados mostraron que el nivel sonoro superó el límite máximo tolerable de 50 dB establecido en el Decreto Supremo Nº 085-2003-PCM, en las zonas de protección especial como el Colegio Ventura Ccalamaqui y Hospital de Barranca Cajatambo y zonas mixtas como mercado y centro comercial. En conclusión, la contaminación sonora en las zonas evaluadas en la ciudad de Barranca, supera los límites máximos establecidos, obteniéndose máximos de 79,32±2,07 dB, sobre todo el primer día laborable en inicio de semana; los altos niveles sonoros provocarían consecuencias negativas en la salud.

Abstract

Introduction: Cytomegalovirus (CMV) infection is a well-known factor associated with invasive aspergillosis in immunocompromised hosts. However, its association with COVID-19-associated pulmonary aspergillosis (CAPA) has not been described. We aimed to examine the possible link between CMV replication and CAPA occurrence. Methods: A single-center, retrospective case–control study was conducted. A case was defined as a patient diagnosed with CAPA according to 2020 ECMM/ISHAM consensus criteria. Two controls were selected for each case among critically ill COVID-19 patients. Results: In total, 24 CAPA cases were included, comprising 14 possible CAPA and 10 probable CAPA. Additionally, 48 matched controls were selected. CMV replication was detected more frequently in CAPA than in controls (75.0% vs. 35.4%, p = 0.002). Probable CMV end-organ disease was more prevalent in CAPA (20.8% vs. 4.2%, p = 0.037). After adjusting for possible confounding factors, CMV replication persisted strongly associated with CAPA (OR 8.28 95% CI 1.90–36.13, p = 0.005). Among 11 CAPA cases with CMV PCR available prior to CAPA, in 9 (81.8%) cases, CMV replication was observed prior to CAPA diagnosis. Conclusions: Among critically ill COVID-19 patients, CMV replication was associated with CAPA and could potentially be considered a harbinger of CAPA. Further studies are needed to confirm this association.

Abstract

Background Homeless populations (HPs) have difficulties obtaining necessary medical care, and primary health care service outreach (PHSO) might be useful to bridge this gap. Objective Using the Centre for Evidence-Based Management Critically Appraised Topics framework, to provide systematic evidence of the usefulness of PHSO interventions for HPs. Methods A systematic search was conducted in 4 electronic databases: PubMed, Web of Science, CINAHL, and Cochrane (publication dates between January 1980 and November 2020). In total, 2,872 articles were identified. Primary research about PHSO for HPs in high-income countries were included. Data were extracted from eligible studies, summarized, and collated into a narrative account. Results Twenty-four studies that described and evaluated PHSO interventions for adults experiencing homelessness were selected in the final synthesis. Most studies had a nonrandomized design. PHSO was found to successfully address some barriers to health care access for HPs through flexible appointments in convenient locations, fostering an understanding relationship between doctor and patients, and provision of additional basic necessities and referrals. Outreach was provided for a range of health care concerns, and several solutions to engage more HPs in primary care, improve continuity of care and to decrease the running costs were identified. Outreach also helped to implement preventative measures and reduced emergency service admissions. Conclusion Our review adds to the evidence that PHSO likely improves health care access for HPs. Further studies over longer time periods, involving collaborations with experts with lived experience of homelessness, and utilizing randomized study designs are needed to test outreach efficacy.

Abstract

The current research work aims to determine the relationship between soft skills and work stress in education specialists from the UGEL in the coastal area of Ancash. From a methodological connotation, it was developed from a quantitative and descriptive approach with a non-experimental design. A Spearman's Rho coefficient was obtained, resulting in 0.207, which is located in a weak positive correlation; indicating the existence of a relationship between the variables, denoting that soft skills contribute to improving stress in teachers. From a curricular appreciation, soft skills and stress are elements to be taken into consideration as transversal axes in the educational curriculum, requiring that it be flexible, dynamic, and complex, having as its center the potentialization of the actors involved in the educational process. Soft skills should contribute to adaptive leadership of social and emotional realities., El actual trabajo de investigación tiene por objetivo determinar la relación entre las habilidades blandas y el estrés laboral en especialistas de educación de la UGEL de la zona costa de Ancash. Desde una connotación metodológica se desarrolló desde un enfoque cuantitativo y tipo correlacional con diseño no experimental. Se obtuvo un coeficiente de Rho de Spearman tiene como resultado 0,207 el cual se ubica en una correlación positiva débil; indicando la existencia de relación entre las variables, denotándose que las habilidades blandas contribuyen en mejorar el estrés en los docentes. Desde una apreciación curricular, las habilidades blandas y el estrés, son elementos para tener en consideración como ejes transversales en el currículo educativo, requiriéndose que este sea flexible, dinámico, complejo, teniendo como centro la potencialización de los actores involucrados en el proceso educativo. Las habilidades blandas deben contribuir a un liderazgo adaptativo de las realidades sociales y emocionales.

Abstract

The objective of this study was to determine the relationship between nonverbal teacher communication and the use of didactic material by pre-school and elementary school teachers of CEBE N° 01 of Chimbote, Peru. The research methodology was based on a correlational type with a non-experimental design. A correlation of 0.108 was obtained, being considered as low positive, indicating the existence of a relationship between both variables, therefore, it is indicated that nonverbal communication is positively related to the didactic material used in the classroom; however, it is necessary in a future research, to study the causes that cause the correlation to be low. The usefulness of socio-affective education is necessary to contribute to the integral formation of the personality and effective nonverbal communication in the classroom, from where the teacher can provide viability to the use of didactic material., Se tiene por objetivo determinar la relación entre la comunicación no verbal docente y uso de material didáctico por docentes de inicial y primaria del CEBE N° 01 de Chimbote, Perú. La metodología de la investigación se basó en un tipo correlacional con diseño no experimental. Se obtuvo una correlación de 0,108 siendo considerada como positiva baja, indicando la existencia de relación entre ambas variables, por lo tanto, se indica que la comunicación no verbal se relaciona positivamente en el material didáctico empleado en el aula de clases, sin embargo, es necesario en una próxima investigación, estudiar las causas que originan que la correlación sea baja. Es necesario la utilidad de la educación socioafectiva como vía para contribuir a la formación integral de la personalidad y de una efectiva comunicación no verbal en el aula, desde donde el docente puede brindar viabilidad al uso del material didáctico.
