Abstract

Objectives To assess the sensitivity to change of ultrasound halo features and their association with disease activity and glucocorticoid (GC) treatment in patients with newly diagnosed giant cell arteritis (GCA). Methods Prospective study of patients with ultrasound-confirmed GCA who underwent serial ultrasound assessments of the temporal artery (TA) and axillary artery (AX) at fixed time points. The number of segments with halo and maximum halo intima-media thickness (IMT) was recorded. Time points in which >80% of patients were assessed were considered for analysis. Halo features at disease presentation and first relapse were compared. Results 49 patients were assessed at 354 visits. Halo sensitivity to change was assessed at weeks 1, 3, 6, 12 and 24 and showed a significant standardised mean difference between all time points and baseline for the TA halo features but only after week 6 for the AX halo features. The number of TA segments with halo and sum and maximum TA halo IMT showed a significant correlation with erythrocyte sedimentation rate (0.41, 0.44 and 0.48), C reactive protein (0.34, 0.39 and 0.41), Birmingham Vasculitis Activity Score (0.29, 0.36 and 0.35) and GC cumulative dose (-0.34,-0.37 and-0.32); no significant correlation was found for the AX halo features. Halo sign was present in 94% of first disease relapses but with a lower mean number of segments with halo and sum of halo IMT compared with disease onset (2.93±1.59 mm vs 4.85±1.51 mm, p=0.0012; 2.01±1.13 mm vs 4.49±1.95 mm, p=0.0012). Conclusions Ultrasound is a useful imaging tool to assess disease activity and response to treatment in patients with GCA.

Abstract

Stress fractures are common in young and active individuals, associated with aggressive or repetitive physical activity and their early detection is fundamental to optimise patient care, decrease complications and avoid unnecessary exams. Currently, magnetic resonance imaging is the standard of care for detecting these lesions. Recently, ultrasound has been getting an increasing interest for the detection of stress fractures. In this article, we describe a clinical case that involved a second metatarsal stress fracture diagnosed by ultrasound and review the literature regarding the use of ultrasound in the diagnosis of stress fractures, particularly of the metatarsals. Abstract

Background: Frailty represents a complex clinical syndrome of decreased physiological reserve and increased vulnerability to stressors. Little is known regarding interaction between frailty status and outcomes in patients with acute coronary syndrome. Our study aims to assess the prevalence of frailty and its impact on in-hospital adverse outcomes of patients aged = 75 years admitted for acute coronary syndromes. Methods: In this retrospective, single centre, observational study were included patients aged = 75 years admitted for acute coronary syndromes between January 2011 and December 2015. Frailty was assessed using the Fried criteria. The primary endpoint was all-cause in-hospital mortality. Secondary endpoints included the occurrence of re-infarction, stroke and major bleeding. Results: Of the 502 patients included, 126 were classified as frail. These patients were older (mean age 78±5.5 vs. 76.2±5.5 years; p=0.020), more often male (68.3%) and had a higher risk profile according to GRACE (151.4±18.8 vs. 132.1±16.8; p<0.001), TIMI (4.3±1 vs. 3.1±1; p<0.001) and CRUSADE (34.6±9.4 vs. 25.8±9.5; p<0.001) scores at admission. All-cause in-hospital mortality was significantly more frequent in frail patients (11.9% vs. 5.6%; p<0.001), as well as re-infarction (7.4% vs. 4.8%; p<0.001), stroke (8.7% vs. 0.5%; p=0.002) and major bleeding (7.9% vs. 1.6%; p=0.002). On multivariate analysis, frailty remained independently associated with the primary endpoint (OR 5.63, [2.05-10.35]; p<0.001). Conclusion: Frailty, identified by Fried criteria, is frequent in elderly patients with acute coronary syndromes, and it is an independent prognostic predictor for in-hospital mortality. 46. Development of press forming techniques for thermoplastic composites: Investigation of a multiple step forming approach

Abstract

Thermoplastic composites are very attractive for aerospace structures due to their high specific strength and stiffness, enhanced toughness and high temperature resistance. One of the manufacturing methods used to produce thermoplastic composite parts is press forming. Press formed parts typically have constant thickness and fibre orientation. However, composite parts may be optimized by tailoring the stiffness and strength, thereby reducing structural weight. In this thesis, the processing techniques for this manufacturing method were developed by investigating the feasibility of a multiple step forming approach for the production of parts with variable thickness and fibre orientation. This manufacturing approach was proven from both a formability and structural integrity viewpoints. By using an efficient blankholder system with clamps and springs, thermoplastic composite laminates can be multiple step formed into three-dimensional shapes with no occurrence of visual forming defects such as wrinkling, fibre buckling or ply folding. Furthermore, good quality bonds can be achieved between laminates formed and co-consolidated in different steps. To achieve a high degree of bonding between GF/PEI composite laminates, the co-consolidation process window was estimated to be between 270ºC and 300ºC. The use of a post-consolidation stage in a hot press was needed to improve the degree of bonding between co-consolidated laminates formed in different steps, which led to very long production cycle times. For future research, the maximum temperature limit of the metal mould should be increased and tooling active cooling mechanisms must be incorporated to allow the mould to be rapidly cooled down, thereby decreasing production cycle times. Abstract

The Consultative Committee for Length (CCL) identified several key comparisons in the field of dimensional metrology at its meeting in September 2008. It decided that the formally individual key comparisons on short gauge blocks and on length bars should be combined under the designation CCL-K1. Key Comparison EURAMET.L-K1 was instigated following a decision at the 2010 meeting of the EURAMET Length contact persons held at SP Technical Research Institute of Sweden. A goal of the CCL key comparisons for topics in dimensional metrology was to demonstrate the equivalence of routine calibration services offered by NMIs to clients. The comparison was coordinated by the BEV (AT) as the pilot laboratory with substantial help by PTB (DE) for the long gauge blocks stability measurements.
