1. Cognitive Behavioural Therapy to Optimize Post-Operative Recovery (COPE): a randomized controlled feasibility trial in extremity fracture patients

Abstract

Abstract Background Outcomes following surgery to operatively manage extremity fractures are variable, and up to two-thirds of patients report chronic post-surgical pain. Preliminary evidence suggests that psychotherapy directed at improving coping skills and reducing somatic vigilance may improve outcomes among fracture patients. The objective of this pilot study was to test the feasibility and acceptability of a randomized controlled trial comparing an online cognitive behavioural therapy (CBT) program versus usual care in patients with an operatively managed open or closed extremity fracture. Methods We conducted a single-centre internal pilot study over a 10-month period in patients with at least one operatively managed open or closed fracture of the appendicular skeleton. Participants were randomized to an online CBT program or usual care and followed for 12 months. The goals of our pilot study were to determine an acceptable rate of recruitment, the degree to which participants randomized to CBT were compliant with treatment, the site investigator’s ability to adhere to study protocol and data collection procedures, and our ability to achieve high follow-up rates. Feasibility criteria were evaluated using a graded “traffic light” approach, in which “green light” indicates moving forward with the definitive trial, “yellow light” indicates proceeding with modifications to the protocol and trial procedures, and “red light” indicates a definitive trial is not feasible without significant protocol and trial procedure modifications. Results We enrolled 94 participants over 10 months, which resulted in a “yellow light” for recruitment. Participant compliance with completion of the online CBT program received a “yellow light”, with 60% of participants who were randomized to CBT completing all seven modules. However, 40% of participants in the CBT-arm withdrew from the program, resulting in a “red light”. Adherence with the study protocol activities at baseline was relatively high (88%) which resulted in a “yellow light”. Follow-up was 85% (80 of 94) at 12 months, resulting in a “yellow light”. Conclusions These results suggest feasibility of a definitive, multi-centre trial to compare CBT versus usual care in the management of persistent post-operative pain in fracture patients despite the pilot phase identifying some challenges with enrollment timelines, compliance with the CBT program, and participant follow-up. For the definitive trial, we will expand participant recruitment to additional centres and implement strategies to optimize participant engagement and compliance with the CBT program and follow-up. Abstract

Chlorhexidine skin antisepsis is frequently recommended for most surgical procedures; however, it is unclear if these recommendations should apply to surgery involving traumatic contaminated wounds where povidone-iodine has previously been preferred. We aimed to compare the effect of aqueous 10% povidone-iodine versus aqueous 4% chlorhexidine gluconate on the risk of surgical site infection in patients who required surgery for an open fracture.We conducted a multiple-period, cluster-randomised, crossover trial (Aqueous-PREP) at 14 hospitals in Canada, Spain, and the USA. Eligible patients were adults aged 18 years or older with an open extremity fracture treated with a surgical fixation implant. For inclusion, the open fracture required formal surgical debridement within 72 h of the injury. Participating sites were randomly assigned (1:1) to use either aqueous 10% povidone-iodine or aqueous 4% chlorhexidine gluconate immediately before surgical incision; sites then alternated between the study interventions every 2 months. Participants, health-care providers, and study personnel were aware of the treatment assignment due to the colour of the solutions. The outcome adjudicators and data analysts were masked to treatment allocation. The primary outcome was surgical site infection, guided by the 2017 US Centers for Disease Control and Prevention National Healthcare Safety Network reporting criteria, which included superficial incisional infection within 30 days or deep incisional or organ space infection within 90 days of surgery. The primary analyses followed the intention-to-treat principle and included all participants in the groups to which they were randomly assigned. This study is registered with ClinicalTrials.gov, NCT03385304.Between April 8, 2018, and June 8, 2021, 3619 patients were assessed for eligibility and 1683 were enrolled and randomly assigned to povidone-iodine (n=847) or chlorhexidine gluconate (n=836). The trial's adjudication committee determined that 45 participants were ineligible, leaving 1638 participants in the primary analysis, with 828 in the povidone-iodine group and 810 in the chlorhexidine gluconate group (mean age 44·9 years [SD 18·0]; 629 [38%] were female and 1009 [62%] were male). Among 1571 participants in whom the primary outcome was known, a surgical site infection occurred in 59 (7%) of 787 participants in the povidone-iodine group and 58 (7%) of 784 in the chlorhexidine gluconate group (odds ratio 1·11, 95% CI 0·74 to 1·65; p=0·61; risk difference 0·6%, 95% CI -1·4 to 3·4).For patients who require surgical fixation of an open fracture, either aqueous 10% povidone-iodine or aqueous 4% chlorhexidine gluconate can be selected for skin antisepsis on the basis of solution availability, patient contraindications, or product cost. Abstract

Following the 2017 killing of Maxwell Adam Mahama by a lynch mob in Ghana, this article engages with the subject of lynching in Ghana through a content analysis of newsmedia items relating to the practice. While reactions to Mahama’s killing invite optimism that lynching as a form of instant (in)justice is being problematized in Ghana, this study leads to a less optimistic position. State, media and public responses to Mahama’s killing were compared vis-à-vis similar killings of two police officers ‐ Jerry Wornoo and Richard Owusu-Sekyere ‐ in 1998. Drawing on the relevant literature, this study concluded that a number of factors combined to make Mahama, Wornoo and Owusu-Sekyere ‘newsworthy victims’. In contrast, victims in several lynching cases reported by Ghanaian media between 1999 and May 2017 were not deemed newsworthy, thus attracting less attention. Consequently, there have been missed opportunities following the Wornoo and Owusu-Sekyere killings to make critical systemic interventions in Ghana to prevent lynching. Hence, it is argued, the mere sensationalization of Mahama’s lynching may not trigger the requisite systemic social change. Recommendations are made to harness the momentum from Mahama’s killing to address instant (in)justice.

Abstract

A 63-year-old woman was referred to the lung cancer clinic after presenting to the emergency department with a 10-day history of fever, chest tightness and breathlessness. Chest X-ray showed a left-sided retrocardiac mass, and an urgent CT scan was requested. On review, she reported a 2-month history of intermittent cough and night sweats. She denied weight loss or haemoptysis. She was an ex-smoker. Her medical history included two episodes of pneumonia in the 1980s. There was no history of recurrent childhood infections or exposure to tuberculosis. Performance status was zero and she worked for the NHS as an operating department practitioner.
