Sarah Butler looks at these myths and clears up some of the misunderstandings that are often repeated In the UK there is a need to promote and increase the number of general practice nurses working in primary care. In comparison to their counterparts in secondary care, practice nurse numbers are significantly reduced. In recent years emphasis was placed on keeping individuals well and out of hospital and their conditions managed in primary care where possible. The role of the general practice nurse is still lesser known compared to the role of a staff nurse. Myths still remain around the general practice nurse position including those eligible to apply and the development that is offered.

Despite many experienced nurses opting to work a practice nurses, the number of newly qualified nurses choosing practice nursing as their first post is low. Newly qualified nurses, instead, prefer to gain experience in secondary care first, with many not realising practice nurses do not need to have worked in secondary care. Pressures such as increasing workload and an ageing workforce will only deplete the number of practice nurses in post already, therefore increasing practice nurse numbers is essential.

Cervical screening for those individuals with a cervix routinely occurs every 3 years for those aged 25–49 (24½ in England) and every 5 years for those aged 50–64. Over 99.7% of cervical cancers are caused by human papillomavirus. Cervical cancer is preventable and curable; primary HPV screening can detect early changes in cervical cells allowing for effective monitoring and treatment.

The emerald ash borer, Agrilus planipennis Fairmaire (Coleoptera: Buprestidae), is a serious pest of ash (Fraxinus spp.) (Oleaceae) in North America. Control of emerald ash borer is difficult in natural forest settings; therefore, a classical biological control programme is the most feasible management option for this invasive, nonnative insect. Here, we report the first Canadian release and establishment of parasitoids Tetrastichus planipennisi Yang (Hymeoptera: Eulophinae), Oobius agrili Zhang and Huang (Hymenoptera: Encyrtidae), and Spathius galinae Belokobylskij and Strazanac (Hymenoptera: Braconidae) in natural forests in Ontario, Quebec, and New Brunswick, Canada for the control of emerald ash borer. Releases of T. planipennisi were made from 2013 to 2019, O. agrili from 2015 to 2019, and S. galinae from 2017 to 2019. Trees from release sites were destructively sampled to rear out adult emerald ash borers and parasitoids 1–3 years after parasitoid release. Recoveries of T. planipennisi were made at 81% of release sites (13 of 16) 1–2 years after release, and O. agili were recovered from 29% of release sites (4 of 14) 1–3 years after release. Spathius galinae was not recovered. These data provide important information for the development and deployment of a successful biological control programme for the management of emerald ash borer in Canada.

This article uses edvertising as a vehicle through which to examine autonomy and control for key agents in education when market-like reforms are combined with privately led management of schools. We begin by outlining the philosophical foundations of school choice from the perspective of autonomy and control, and then lay out the case of edvertising. Guided by Cribb and Gewirtz’s theoretical discussion of autonomy and control, we explore degrees and types of autonomy and control for various agents in schools. We then examine the differences between levels of autonomy for key agents—schools, principals, teachers, parents, and the State—as idealized in the philosophical underpinning of market-based policies with the actual autonomy exhibited with the introduction of edvertising.

Objective: To evaluate whether elective induction of labour (eIOL) influences the rate of caesarean birth in uncomplicated pregnant women at term, compared to expectant management. Design: Retrospective cohort study. Setting: Births in Victoria between 2010 and 2018. Population: Term, singleton, vertex births from low-moderate risk pregnancies (n=396,164). Methods: Preliminary analyses compared eIOL at 37 weeks with expectant management both beyond that gestational age (preliminary analysis I) and at that gestational age and beyond (preliminary analysis II). Similar comparisons were made for eIOL at 38, 39, 40 and 41 weeks’ gestation and expectant management. The primary analysis repeated these comparisons, limiting the population to nulliparous women whose recorded indication for induction did not include one of a specified list of conditions. We estimate the value of non-personally identifying information to consumers of online services through a discrete choice experiment based on hypothetical streaming video services. Non-personally identifying information for online services is typically information on the ways in and times at which customers use the service, and is distinct from personally identifying information such as email addresses or telephone numbers. For most of our survey respondents, we find no evidence that they were willing to pay to avoid sharing their non-personally identifying information with third parties. A smaller group of respondents never selected a service that shared information with third parties.

The National Association of School Nurses (NASN) developed the Framework for 21st Century School Nursing Practice to reflect current school nurse practice. The Framework of practice was introduced in June 2015, and feedback was requested and obtained from practicing school nurses in a variety of ways. The final version of the Framework is introduced in this article. This article updates (and replaces) the articles in the July 2015 NASN School Nurse related to the Framework. Central to the Framework is student-centered nursing care that occurs within the context of the students’ family and school community. Surrounding the student, family, and school community are the nonhierarchical, overlapping key principles of Care Coordination, Leadership, Quality Improvement, and Community/Public Health.These principles are surrounded by the fifth principle, Standards of Practice, which is foundational for evidence-based and clinically competent quality care. Each of these principles is further defined by practice components. Suggestions are provided regarding how the Framework can be used in a variety of settings to articulate and prioritize school nursing practice. The ultimate goal is to provide a resource to guide school nurses in their practice to help students be healthy, safe, and ready to learn.

A 25% risk of acute esophagitis is experienced by patients as a result of the volume of esophagus encompassed within a conformal radiotherapy technique. We reviewed our institutional experience administering the radioprotectant amifostine prior to daily radiotherapy to determine its effects on the onset of esophagitis. Materials and methods: From 2005 to 2016, 49 patients diagnosed with stage 3 small cell lung carcinoma received concurrent chemoradiotherapy. Chemotherapy (CT) consisted of cisplatin and etoposide with radiotherapy (RT) encompassing CT-identified gross tumor volume. In 32 patients (group 1), amifostine was delivered (500 mg subcutaneously divided in two injections) prior to the second daily RT fraction. The remaining 17 patients (group 2) did not receive amifostine due to choice or drug intolerance. Results: Metrics of esophagitis included weight loss and opiate requirement during treatment. About 31% of group 1 required opiates at a median RT dose of 3300 cGy, and 41% of group 2 required opiates at a median dose of 2250 cGy. The dose of radiotherapy delivered to 50% of the esophageal volume for group 1 was significantly greater than that in group 2 (3000 cGy vs 576 cGy). Conclusion: In this modern retrospective series of thoracic chemoradiotherapy in the treatment of stage 3 small cell lung cancer, amifostine that was delivered subcutaneously postponed the onset of esophagitis. Incontinence is associated with mental illness and neuroleptic medications but diagnosis and treatment is often poor or non-existent. Problems of incontinence are compounded in secure psychiatric services for women by poor health, obesity, and a sedentary lifestyle. Addressing the physical health of this group necessitates a more accurate picture of the nature, incidence, and management of incontinence. A point-in-time survey of 108 women who agreed to be interviewed (93%) covered presence, frequency, and nature of incontinence, and information on management case note data was used to gather demographic and previous medical history, comparisons were made between patients with and without problems of incontinence. Findings indicate a problem of incontinence in 48% of women with a dominance of problems of stress and urge enuresis. Of modifiable factors that contribute to enuresis, the current study highlighted the contribution of obesity, smoking and clozapine medication. A further finding was the preference for managing rather than treating problems of incontinence. Actions to improve the detection and treatment of this problem are described.

The purpose of this project was to determine the effectiveness of the Helping Administer to the Needs of the Student with Diabetes in Schools (H.A.N.D.S.sm) continuing education program in improving the level of experience and competence in performing services associated with diabetes care. This program is a live course for school nurses providing clinical information about diabetes management and their professional role in the care of students with diabetes. Pre- and post-surveys were administered via e-mail to assess their level of experience and competence in diabetes care. A total of 105 nurses completed both surveys and were included in the analysis. The changes between pre- and post-survey questions were assessed. The H.A.N.D.S. participants’ levels of experience and competence for each of the four categories of diabetes care improved significantly, and a greater number of nurses reported being able to perform the services independently and having the ability to teach others.

Over the past 20 years, market-based choice initiatives have become a popular approach to education reform. Since 2002, the New York City Department of Education has opened over 250 high schools, creating a marketplace so widespread that many students no longer have a zoned or neighborhood school. This article uses two New York City–based case studies to examine branding or marketing practices at new small schools. It explores how and to whom schools market themselves and the nature and type of information provided to students and parents. The article raises questions about the implications of marketing and branding practices for public education.

Accessible summary Much has been written about how best to help people with learning disabilities if they feel sad or worried. Often this is about thinking. But it might be better to look at doing instead. This is the story of how we tried to help a lady feel happier, and worry less. Summary A substantial body of literature exists concerning the adaptation of Cognitive Behavioural Therapy for people with learning disabilities. However, it is possible that cognitive approaches have been prioritised at the expense of behavioural techniques that are simpler and more effective. This case conceptualisation considers a behaviourally focused approach to helping a woman with a mild learning disability who was experiencing low mood and anxiety, delivered by a trainee clinical psychologist working within a multidisciplinary team.

This article begins to unpack the complex effects of the policies of both the small schools and choice on students with special needs. Drawing on qualitative data collected throughout the 2008-2009 academic year and a range of quantitative data from New York City’s public high schools, the author shows that while small schools and choice are intended to expand schooling options for all, students with special needs often find that when entering the public high school choice process, their selection set is narrowed. For families of students with special needs, the lack of adequate special needs resources can preclude schools from being considered as viable options. In addition, schools of choice engage in practices to deter higher need students from applying. The combined influences of parent decision and school choice processes result in between-school sorting, with larger institutions receiving the bulk of the higher need population of students, which is reflected in the city’s enrollment data. The findings raise questions about not only the effects of the small school reforms, but also about the role of choice policies in the public educational system.
