3. Perioperative Assessment and Clinical Outcomes of Percutaneous Atrial Septal Defect Closure with Three-Dimensional Transesophageal Echocardiography.

Background/Objectives: To close the atrial septal defect (ASD) with the transcatheter method, correctly defining the defect and selecting the appropriate closure device size are the most critical steps for the procedure's success. Although ASD can be successfully closed under the guidance of three-dimensional (3D) transesophageal echocardiography (TEE) and two-dimensional (2D) TEE, measurement comparisons between different types of defects are still needed. Methods: Our study was designed retrospectively. Sixty-one patients who underwent transcatheter ASD closure with 2D TEE and 3D TEE between 2020 and 2024 were included. The patients were divided into three groups according to the defect shape: circular, oval, and complex; and the measurement results, perioperative process, and clinical outcomes were compared in each group. Results: The average age of the patients was 35.05 ± 13.87 years, and 41 (67.2%) were women. The average follow-up period of the patients was 15.3 ± 9.18 months. No statistical significance was observed in the comparison of measurements obtained with 3D TEE and 2D TEE in the circular and oval defect groups. The differences between the minimum defect diameters of complex defects measured by 2D TEE and 3D TEE (p: 0.037), IVC rims (p < 0.001), aortic rims (p: 0.012), and the differences between implanted device dimensions and the maximum defect diameters measured by both methods were compared; statistical significance was observed (p: 0.025). Conclusions: In circular and non-complex oval defects, it has been observed that the size of the closure device selected with 2D TEE or 3D TEE is optimal, and the procedure is practical and feasible. While the closure of complex ASDs with 3D TEE provides reliable and optimal results, using only 2D TEE in complex ASDs may lead to selecting a smaller-sized device. [ABSTRACT FROM AUTHOR]

17. Predictive value of cardiovascular risk scoring systems for the detection of myocardial injury following carbon monoxide intoxication.

OBJECTIVES: This single-center, retrospective study investigates the predictive value of cardiovascular (CV) risk-calculation systems in patients admitted to the emergency department with carbon monoxide (CO) intoxication for the identification of potential myocardial injury. METHODS: The total CV risk of 558 patients presenting to the emergency department with CO intoxication were calculated on admission using different CV risk scoring systems, including SCORE Turkey, European Heart SCORE, and FRAMINGHAM to predict potential myocardial injury secondary to poisoning, and the risk levels were categorized based on the calculated scores. The presence of myocardial injury was identified based on the level of elevation of a cardiac biomarker (Serum cardiac troponin-I >99th percentile upper reference limit). RESULTS: Myocardial injury due to CO intoxication was detected in 132 (23.7%) of the patients. A comparison of the risk scoring systems' ability to detect the presence of myocardial injury revealed that all had significant, similar, but low predictive values (the "area under the curve" values of SCORE Turkey, European Heart SCORE and FRAMINGHAM were 0.653, 0.632, and 0.629, respectively; P < 0.001). Among the three risk scoring systems, SCORE Turkey was the most successful test in diagnosing myocardial injury with 87% specificity, while FRAMINGHAM scoring was the most successful test in excluding the presence of myocardial injury with 72.1% sensitivity. CONCLUSION: Among the tested CV risk-calculation systems SCORE Turkey, was found to be the most effective in the prediction of myocardial injury secondary to CO poisoning, but all produced similar and significant results. Amaç: Bu çalışmada Transkatater Aort Kapak İmplantasyonu(TAKİ) ve Cerrahi Aort Kapak Replasmanı(CAKR) geçiren İleri Aort Stenozu tanısı almış hastaların erken ve geç dönem klinik sonuçlarının literaturdeki prensipler ile birlikte merkezimizin klinik deneyiminin değerlendirilmesi ve karşılaştırılması amaçlanmıştır.Gereç ve Yöntem: Çalışmamıza 2011-2018 yılları arasında Atatürk Üniversitesi Tıp Fakültesi Hastanesi Kardiyoloji ve Kalp Damar Cerrahisi polikliniğine başvuran İleri Aort Stenozu tanılı hastalar arasından kalp ekibi tarafından değerlendirilip, TAKİ veya CAKR' ye yönlendirilen 60 yaş üstü triküspit yapıda aort kapağı olan, dejeneratif etiyolojiye sahip, güncel kılavuzlarda önerilen cerrahi risk skorlarına göre hesaplanmış orta riskli cerrahi gruba giren 128 hasta alındı.Çalışmaya dahil edilen hastaların demografik verileri, laboratuvar sonuçları ve diğer klinik bilgilerine arşiv dosyalarından ulaşıldı. Log.EuroSCORE, EuroSCORE 2 ve Göğüs Cerrahisi Derneği (STS) skorları web tabanlı sistemden hesaplandı. Bulgular: Orta riskli cerrahi hastası TAKİ grubunda 67, CAKR grubunda ise 61 hastaydı. TAKİ grubunun %38 (26 hasta)' i, CAKR grubunun ise %36 (22 hasta)' sı kadındı. Çalışmamızda TAKİ gurubunun yaş ortalaması 76,4, CAKR grubunda ise 70,7 olarak görüldü (p0,05).Kalp yetersizliği(KY), koroner arter hastalığı(KAH) ve Miyokart İnfarktüsü(Mİ) nedeniyle erken ve geç dönemde hastaneye yeniden yatışlar her iki grupta da benzer olarak görüldü (p>0,05). Sonuç: Çalışmamızda, merkezimizde TAKİ'nin CAKR'ye noninferior olduğu gösterilmiştir. Önceleri bu hastalarda operasyonel ve hastane içi erken dönemde mortalite oranlarının belirlenmesinde mevcut klasik cerrahi risk hesaplamalarının uygun olabileceği düşünülse de yakın dönemde yapılan çalışmalar ve bizim çalışmamızda da görüldüğü gibi bu klasik risk skorlamaları TAKİ yapılacak hastalarda hastane içi erken dönemde hesaplanan mortalite riskini çalışmalarda elde edilen oranlardan daha fazla göstermektedir. Bu nedenle yeni skorlama sistemlerine ihtiyaç duyulmaktadır.Yaş mortalite skorlarında önemli bir yer tutsa da hastaların frajilite durumu ve ek komorbiditeleriyle birlikte anlamlılık kazanmaktadır. Çalışmamız ve diğer literatür çalışmaları doğal olarak yaşlı popülasyonda yapılmış olup, düşük cerrahi riskli hasta gruplarında TAKİ 'nin tartışıldığı bu günlerde TAKİ protezlerinin(biyoprotez) uzun dönem klinik sonuçları sınırlı olup, mevcut tedavi yönteminin genç veya düşük riskli hasta grubunda da uygulanması için klinik çalışmalara daha fazla ihtiyaç vardır. Anahtar Kelimeler: İleri Aort Stenozu, Transkateter Aort Kapak İmplantasyonu, Cerrahi Aort Kapak Replasmanı, Mortalite, Cerrahi Orta Riskli Grup Aim: The aim of this study was to evaluate the clinical experience of our center on Transcatheter Aortic Valve Implantation (TAVI) and to compare the early and late clinical outcomes of patients with severe Aortic Stenosis who underwent TAVI or Surgical Aortic Valve Replacement (SAVR) Materials and Methods: The study included a total of 128 patients over 60 years of age with Severe Aortic Stenosis who were admitted to Cardiology and Cardiovascular Surgery outpatient clinic of Atatürk University Faculty of Medicine between 2011 and 2018 and underwent TAVI or SAVR. The study population was in the middle-risk surgical group according to the surgical risk scores which are proposed in the current guidelines with degenerative etiology of an aortic valve with tricuspid structure. The demographic data, laboratory results and other clinical information of the patients were obtained from the hospital database. Log.EuroSCORE, EuroSCORE II and Society of Thoracic Surgery (STS) scores were calculated from the web-based system.Results: The number of medium-risk surgical patient was 67 in the TAVI group and 61 in the SAVR group. 38 % (26 patients) of the TAVI group and 36 % (22 patients) of the SAVR group were female. The mean age was 76,4 years in the TAVI group and 70,7 years in the the SAVR group (p 0,05).Early and late hospitalizations due to Heart Failure(HF), Coronary Artery Disease(CAD), Myocardial İnfarction(MI) were similar in the both groups(p> 0,05). Conclusions: In this study, we found that TAVI was noninferior to SAVR .Although classical surgical risk calculations are thought to be appropriate for determining the mortality rates in these patients in the early postoperative and in-hospital early stages, as shown in the recent studies and in our study, these classical surgical risk scores show that the calculated risk of early in-hospital mortality in patients undergoing TAVI is more than rates obtained in the studies. Therefore, new scoring systems are needed. Although age plays an important role in mortality scores, it gains significance with the patients' fraility status and additional comorbidities. Our study and other literature studies were conducted in the elderly population, and long-term durability data of the TAVI prostheses (bioprosthesis) are currently limited When TAVI is considered in low-surgical patient groups, long - term clinical studies are also needed in this patient population for the indication of modalty.Keywords: Tavı, Savr, Advanced Aortic Stenosis, Surgical İntermediate Risk Group,Mortalıty 62

Türkiye Sualtı SporlarıFederasyonu (TSSF) bünyesinde yer alan sualtı hokeyi ve paletli yüzmebranşlarında son dönemlerde elde edilen uluslararası başarılar, bu branşlarayönelik yapılacak olan çalışmaların giderek önem kazanacağını göstermektedir.Bu çalışma; TSSF tarafından desteklenen bir dizi araştırmanın, testniteliğindeki başlangıç ölçümlerine ait değerlendirmeleri içermektedir. Havuzsporlarında, sporcuların vücut yağ yüzdesi (VYY), performansı etkileyenfaktörlerden biridir. Sporcuların VYY ise; kalıtsal özellikler, beslenme,antrenman kapsamı, antrenman süresi, yaşam tarzı, genel sağlık durumu gibifarklı parametrelerden etkilenmektedir. Bu çalışmada; vücut yağ yüzdeleribelirlenen sporcuların, haftalık antrenman süreleri dikkate alınarak, branşlararasında karşılaştırma yapmak istenmiştir. Bu amaçla, farklı spor kulüplerinden9 elit erkek sualtı hokeyi; 8 elit erkek çift palet ve 8 elit erkek mono paletsporcusu, kulüplerinden ,izinli ve gönüllü olarak çalışmaya katılmışlardır.Ölçüm öncesi her sporcu için bilgi formu oluşturulmuş, daha sonra ölçümalınacak olan vücut bölgesi belirlenerek, işaretlenmiştir. Deri kıvrımıkalınlıkları Holtain T/WSkinfoldCaliper(productcode 98.610)kaliper ile kol(triceps ve biceps),bel(suprailiac) ve sırt(subscapular) bölgelerden ikişer ölçüm yapılarakgerçekleştirilmiştir. İki ölçüm arasında %5'den fazla fark bulunduğunda ölçümtekrar edilmiştir. Sporcuların vücut yoğunlukları(g/ml), yaş ortalamaları dadikkate alınarak, erkekler için geliştirilmiş olan DurninWomersley formülüyle hesaplanmıştır. Vücut yoğunluklarınınelde edilmesi neticesinde, Siri formülükullanılarak VYY hesaplanmıştır. Çalışmanın istatistiksel değerlendirmesi SPSS20 programında yapılmıştır. Tanımlayıcı istatistikler, varyans analiziyle;branşlar arası VYY karşılaştırması OneWayANOVA; haftalık antrenman süresi ile VYY arasındaki ilişki non-parametrikkorelasyon yöntemiyle değerlendirilmiştir. Çalışmanın sonucunda; branşlararasında VYY açısından anlamlı bir fark bulunmamasına karşın; sporcularınhaftalık antrenman süreleri ile VYY arasında ters orantı olduğu gözlenmiştir.Sporcuların haftalık antrenman süreleri uzadıkça, VYY'nin azaldığı bulunmuştur.Sualtı hokeyi ve paletli yüzme sporcularında optimal VYY aralığını belirten birindekse literatürde rastlanmamıştır. Çalışmadan elde edilen veriler, bubranşların sporcularına beslenme eğitimi verilmesine, sporcuların monitörizeedilmesine ve antrenmanlarının planlanmasına ışık tutacak nitelikte ilkbulgular olarak önem taşımaktadır., Body fat percentage (BFP) is one of the factors affecting athletic performance. Hence, it's affected bydifferent parameters such as genetics, diet, volume and duration of training, life style and generalhealth status. In this study; BFP of underwater hockey and finswimming athletes were evaluated whiletheir weekly training duration was taken into consideration. For this aim, 9 elite underwater hockey,8 elite double fin and 8 elite mono finswimming athletes from different clubs participated in this studyvoluntarily and by having permission from their clubs. Skinfold measurements, body density (g/ml) andBFP of athletes were measured and SPSS 20 program was used for statistical analysis. As a result;statistical difference was not found the comparisons of BFP among the groups. However, inverse ratiowas determined between weekly training duration and BFP of the athletes. In literature, there wasn'tany index about BFP for underwater hockey and finswimming athletes. Therefore data of this study isimportant for educating athletes about dietary behaviors, monitoring athletes' fitness levels especiallybody compositions and planning training sessions.

Türkiye'de 1980'li yıllardan itibaren dünyada yaşanan gelişmelere paralel olarak sivil toplumun geliştiği görülmektedir. Bu yıllardan itibaren hem akademik hem de kamusal söylem içerisinde sivil toplum kavramı canlanmış ve sivil alanın en önemli öğesi olan sivil toplum kuruluşlarının faaliyetleri ülke genelinde artmış ve yaygınlaşmıştır. Son yıllarda ise ülke içerindeki demokratikleşme gayreti ve Avrupa Birliği kriterlerine uyum sağlama noktasında yapılan çalışmalarla sivil toplum kuruluşları Türkiye'nin önemli aktörlerinden biri haline gelmiştir. Türkiye'de sivil toplumun canlanış süreci genel itibariyle iki farklı eksen üzerinden gerçekleşmiştir. Bu eksenlerden biri kendisini laik cumhuriyetin koruyucusu olarak tanımlayan ve bu bağlamda söylemler üreten STK'lar iken, diğeri ise muhafazakâr çevrelerce kurulan ve İslami buyrukları kendisine referans gösteren muhafazakâr sivil toplum kuruluşlarıdır. Bu çalışma özellikle 1990'lı yılların başından itibaren kurulan ve Türkiye'de sivil toplum alanında her geçen dönem daha fazla etkin hale gelen muhafazakâr sivil toplum kuruluşlarını, Kimse Yok Mu? Dayanışma ve Yardımlaşma Derneği örnekleminde ele almaktadır. It is seen that civil society in Turkey has been developing in parallel with the development in the world since 1990s. Since then, the statement of civil society in both academical and and public speech has quickened, and the activities of civil associations, the most important subject of civil area, have increased and become widespread countrywide. With the democratization effort and studies done to comply with European Community Criterions in the country, civil associations have been one of the most important actor of Turkey. The process of the revival of civil society in Turkey generally took place over two different axes. One of these axes is NGOs (non-govermental organizations) that define themselves as the guardians of the secular republic and produce discourses in this context, and the other one is conservative non-governmental organizations that were founded by the conservative circles and refer to Islamic commandments.This study dealt with conservative civil associations such as KİMSE YOK MU? The Solidarity and Helping Association, which have been founded and had much influence day by day since the begining of 1990s. 142

Institution of kirvelik which has a long history in Anatolian geography and in the regions related with this geography corresponds to the relation that builds a new kinship regardless of blood relationship between the person circumcised, the person who bears the expenses of circumcision and their families. Kirvelik tradition that is widely practiced in some cities in Eastern and South-eastern regions of Turkey along with some cities in Central Anatolia is frequently practiced among different social groups. Kirve, in general sense, is not only a part of an acknowledgement that is economically limited, but also is the person who additionally undertakes legal and social sanctions. Kirve, who becomes much closer to the person circumcised and his family than their first degree relatives, can establish a new social lifelong relationship. However, nowadays, the institution of kirvelik which undergoes significant transformations with the wide-spread globalisation process seems to exist in symbolic terms. In this study, on the basis of Şanlıurfa example, the transformation of kirvelik tradition in time and space boundaries is scrutinised within indepth interviews with Sunni, Alevi and Yezidi groups and by discourse analysis method. Sixty five million tons of residues from field crops (wheat, barley, tobacco, cotton, rice etc.) having fundamental importance in Turkish agricultural sector and 160 million tons of animal manure from animal production are existed. Wastes, which are not used for feeding, are burned in the field or abandoned to degrade naturally. Animal manure is usually either applied to the field directly without implementing any process or accumulated in a pile outside. As a result, this situation causes to environmental and health problems. Although our country has wide range of energy sources, the reserves are limited and 60% of total energy consumption is met by imports. Even though the ratio of utilization of domestic resources is currently low, that of hydraulic, solar, and geothermal and wind energy is getting wide spread. Direct combustion of biomass for energy conversion is commonly applied. The utilization of animal and other agricultural wastes for energy conversion under anaerobic conditions using biogas systems almost does not exist. Considering above mentioned environmental, economical and social benefits, the project "The Production of Biogas From Agricultural and Animal Wastes and its Utilization in the Integrated Energy System" was prepared. The tasks in the project includes: review of biogas and energy production technologies, system simulations and decision support system (DSS) modeling, design and development of two lab-scale (one for animal waste and one for agricultural waste) and one pilot scale (250 kWe capacity) integrated biogas plant, integration of the pilot plant with energy conversion systems, experimental studies, techno-economical analysis of the biogas production, and life- cycle analysis. Within this study, the summary of each completed task of the project has been presented with the study methods and results. In addition, the studies which are/will be done at the ongoing work packages and study methods which will be followed are provided as a summary. [ABSTRACT FROM AUTHOR]

Bu inceleme yapılırken özellikle gemi işletmeciliği temel alınmış, kullanılan güç kaynaklan bu çerçevede; performans, işletme, maliyet ve egzoz emisyonları açısından değerlendirilmiştir. Halihazırda gemilerde en yaygın olarak kullanılan güç kaynağı olan dizel makineler özellikle işletme açısından en yüksek verimin alınabileceği ve en ekonomik işletme maliyetine sahip güç kaynaklandır. Stim sistemlerinin gemilerde güç kaynağı olarak kullanımı gün geçtikçe azalırken, gaz türbinleri yüksek performanslanyla stim sistemlerinin yerini almaktadır. Ancak kısmi yüklerdeki düşük verimi ve yüksek yakıt tüketimleri gaz türbinlerinin kullanım alanım daraltmaktadır. The operational effectiveness of the ships is accepted as the primary touchstone of this analysis and in this manner, power plants are evaluated with regard to their performance, operational profile, cost and their emissions. Diesel engines which are the most common power plants at present, have an economic and effective operational profile. The number of the steam power plants are decreasing gradually and they are being supplented by gas turbines. The utilisation of gas turbines are limited by their low efficiency at partial load and their high fuel consumption. Fuel cells dont have any utilisation in ships yet. Enerji kaynaklarının sınırlı olması ve giderek tükenmekte oluşu, zaman zaman çeşitli nedenlerle temininde karşılaşılan zorluklar, firmaları enerji tassarrufuna iten bir etken olsada enerji tassarrufu ihtiyacını doğuran ana neden enerji maliyetlerinin ulaştığı boyutlardır. Firmaları asıl etkileyen faktör enerji maliyetinin, toplam üretim maliyeti içindeki payıdır. Bu pay giderek büyümekte ve aynı mamulün üretiminde enerji tassarufuna ve geri kazanılmasına önem vermeyen ve bu konuda yatırım yapmayan firmalar rekabet edemez hale gelmişlerdir. Yapılan bu çalışmada ülkemiz endüstri tesislerinden biri olan Pakmaya A.Ş.' deki Atık Isı Kazanlı Gaz Türbini Kojenerasyon sistemi incelenmiş ve sistemin ekonomik açıdan karlılığı irdelenmiştir. Sistemin kurulması ile beraber yıllık bazda yapılan hesaplarla geri ödeme süresinin 2,3 yıl, sekiz yıl sonunda kapital değerinin ise 9.791.825 DM olacağı görülmüştür. Ayrıca genel bazda sanayi tesislerinde kullanılmakta olan mevcut kojenerasyon sistemlerinin üzerinde de durulmuş ve bu sistemler hakkında bilgi verilmiştir. This ratio is increasing everyday and companies, which do not give importance to energy regain and energy economy became incompetitive in the market. In this study cogeneration system with waste heat boilered gas turbine of PAKMAYA A.Ş. `one of an industrial companies in our country` has been examined and profitableness of the system was discussed. With annual calculations it has been understood that the system amortizes itself in 2,3 years and has been seen that after 8 years the profit made will be 9.791.825 DM. Deneylerde kullanılan optik yöntem sayesinde, akış rahatsız edilmeden sınır tabakadaki sıcaklık dağılımı kesin olarak belirlenmiştir. Oluşan hologramları değerlendirmek için dijital görüntü prosesi kullanılmıştır. Deneylerde basınç düşümünü belirli sınırlar içerisinde tutarak, ısı transferini arttırmak amaçlanmıştır. Silindirik ve üçgen prizma şeklinde iki farklı kanat formu seçilerek deneyler yapılmıştır. Deneyde kullanılan test kanalının uzunluğu 300 mm, genişliği 150 mm ve yüksekliği 14 mm'dir. Deney düzeneğinde ısı transferi ve akış prosesini, hız ve hidrolik çapın fonksiyonu olarak incelemek amacıyla, Reynolds sayısının (250 < Re < 7250) değerleri için 250'şer artarımla ölçümler yapılmıştır Laser Doppler Anemometresi(LDA) kullanarak test bölgesinin giriş ve çıkış bölümünde lokal ısı transfer prosesi, akım profilleri, hız profilleri ve hız bileşenleri Reynolds sayışırım (250 < Re < 7250) değerleri için irdelenmiş olup, grafikler halinde sunulmuştur. Deneyler hem Holografik înterferometri Metodu ile hemde konvansiyonel yöntem olan Global Ölçüm Metodu ile yapılmıştır. Bu iki ölçüm sonuçları birbirleriyle mukayese edilmiş olup, Global ölçüm Metodundaki hata oranının % 7 olduğu bulunmuştur. Deneylerin sonuçlan, pürüzsüz(düz) yüzeyli ısı değiştiricilerle karşılaştırılmıştır. Bu mukayese ile basınç düşümü ve ısı transferinin optimum olduğu kanat formunun silindirik yüzey olduğu deneysel olarak bulunmuştur. INVESTIGATION OF FLOW AND HEAT TRANSFER IN A COMPACT HEAT EXCHANGER WITH CYLINDRICAL AND TRIANGULAR RIPS WITH TECHNIQUES OF HOLOGRAPHIC INTERFEROMETRY İbrahim KÎLIÇASLAN Key Words: Holographic Interferometry, Heat Exchanger, Forced Convection Abstract: In this study, heat transfer and flow process were investigated in a compact heat exchanger with techniques of Holographic Interferometry. This optical measuring method offers the advantage to deliver information about the temperature distribution without disturbing the flow pattern. A digital image processing system was used for the evaluation of the interferograms. Here, the purpose was enhancement of the heat transfer rate while keeping pressure drop in certain level. Two different type of geometries for compact heat exchangers were investigated using air as a test fluid. The test duct consist of 300 mm length, 150 mm width and 14 mm height. In order to investigate heat transfer and flow process in the function of velocity and hydraulic diameter, experiments were done for the Reynolds number range extended from 250 to 7250 with adding 250 for each Re values. Velocity profile, streamline profile, local heat transfer and velocity components were investigated for the Reynolds number range from 250 to 7250 by using Laser Doppier Anemometry (LDA). Experiments were performed both techniques of Holographic Interferometry and Global Measurement. 7 % of measurement error was determined in Global Measurement Techniques at the end of the comparison of these two techniques. Experiments results were compared with smooth surface of heat exchanger. As a result of all experiments, It was found clylindirical rip as a optimum geometry of compact heat exchanger. n 137

Ülkeler ellerinde bulunan kısıtlı enerji kaynaklarını mümkün mertebe ekonomik kullanmaya başlamışlar, yeni enerji kaynaklarının arayışına yönelmişlerdir. Çağımız ekonomi çağıdır. 21. yüzyıla girerken her konuda olduğu gibi ülkemizde de petrol ve ürünlerinin ekonomik kullanılması gelecek günlerin en büyük amacıdır. Bu çalışmada turbo doldurmalı, su soğutmalı dizel bir motorda soğutma suyu sıcaklığının yakıt tüketimine etkileri incelenmiştir. Motor, farklı çalışma sıcaklıklarında test edilmiştir. Elde edilen deneysel veriler kullanılarak çeşitli yük ve devir kademelerindeki karakteristik değerler, tablo ve grafik halinde sunulmuştur. Bulunan sonuçlara göre soğutma suyu sıcaklığındaki değişimin yakıt tüketimindeki değişim oranı hesaplanmış, bu konuda ekonomik çalışma bölgesinin ve diğer motor karakteristiklerindeki değişikliklerin tespitine çalışılmıştır. In this study, the effect of cooling water's temperature on the consumption of the diesel motors having turbo charge and cooling by water were investigated. The engine was tested under various temperatures. By using the experimental datum, the characteristic values on the various load and the rotation are presented cut at table. According to result, of the ratio changes of the cooling water temperature and the ratio of the consumption of the fuel was calculated, related to this, it was tried to determine the economical working area. - Abstract
Background: Dual antiplatelet therapy (DAPT) is the treatment of choice for patients with acute and chronic coronary syndromes as it reduces mortality and prevents recurrent thrombotic complications. The assessment of both ischaemic burden and bleeding risk is crucial in deciding which DAPT to choose and how long it should be continued., Objectives: The aim of our study was to perform prospective clinical follow-up of patients receiving fixed-dose combination therapy (ASA 75 mg + clopidogrel 75 mg). Our study is a multicentric, cross-sectional, observational, cohort study., Methods: A total of 1500 patients who were started on fixed-dose combination DAPT for acute or chronic coronary syndrome were included in the study. Primary endpoints were hospitalization for any reason, hospitalization for cardiovascular cause, acute myocardial infarction, stent thrombosis, target vessel revascularization and bleeding; the secondary endpoints were death for any reason or cardiovascular cause and stroke. The significance level adopted in the statistical analysis was 5%., Results: Median age was 63 years; 78.5% of the patients were receiving DAPT treatment for acute coronary syndrome. The rates of hospitalization for cardiovascular reasons, acute myocardial infartion, stent thrombosis and target-vessel revascularization were 7.9%, 2.3%, 1.3% and 4.2%, respectively. While the rate of BARC type 1 bleeding was 3.3%, the rate of BARC type 5, 3, or 2 bleeding was 0.6%. The secondary endpoints which were death from any cause, cardiovascular death and stroke were 0.5%, 0.3% and 0.3%, respectively. Conclusion: Our study shows that fixed-dose combination therapy is effective and safe in appropriately selected patients with acute or chronic coronary syndromes.

- Abstract
Objective: Heart failure is a leading cause of death and the most common diagnosis leading to hospitalization. Its awareness is lower than that of other cardiovascular diseases, both in the general population and among patients with heart failure (HF). This study aimed to establish the current level of knowledge about HF in patients with HF with reduced ejection fraction (HFrEF) and mildly reduced ejection fraction (HFmrEF) in Türkiye., Methods: This questionnaire-based survey study is multicenter, conducted across 34 centers from December 2021 to July 2022. We performed a survey consisting of two sets of questions focusing on individual characteristics of the patients and HF-related knowledge., Results: The study included a total of 2,307 outpatient HF patients, comprising 70.5% males and 29.5% females with a mean age of 64.58 ± 13 (56-74) years and a mean body mass index value of 32.5 ± 10 kg/m2. HFrEF and HFmrEF were determined in 74.7% and 25.3% of patients, respectively. Thirty percent of the patients were unaware that they had HF. While 28.7% of the patients thought that they had sufficient information about HF, 71.3% believed they lacked adequate knowledge. In the study, 25.2% of the participants identified dyspnea, 22% identified tiredness, and 25.4% identified leg edema as the most common symptoms of HF. Only 27.4% of patients recognized all three typical symptoms of HF., Conclusion: We found that the study population's knowledge about HF symptoms and the nature of the disease was poor. Educational and awareness activities are necessary to optimize outcomes and benefits.

- Abstract
Background: Heart failure (HF) remains a significant health problem despite advances in diagnosis and treatment options. Malnutrition and increased inflammation predict poor disease prognosis. The parameters of the Naples prognostic score (NPS) include albumin, total cholesterol, neutrophil-lymphocyte ratio (NLR), and lymphocyte-monocyte ratio (LMR). We aimed to assess the potential of NPS as a predictor of long-term mortality in patients with HF., Methods: A total of 1728 patients with HF who applied to our center between 2018 and 2022 were included in this study. The NPS was computed and the patients were divided into three groups according to their NPS values as follows: NPS = 0 (Group 1), NPS = 1-2 (Group 2), and NPS = 3-4 (Group 3). We also evaluated the association between NPS value and HF mortality., Results: The patients were followed for a mean follow-up duration of 30 months. The mortality rate was 8.3% (145 patients). We carried out Model-1 and -2 Cox regression analyses to identify long-term mortality determinants. Model-2 was constructed by adding NPS to Model-1. NPS was significantly associated with HF mortality (Hazard Ratio: 2.194, 95% Confidence Interval: 1.176-4.091, p = 0.014). According to the Kaplan-Meier plot and log-rank analyses, there was a statistically significant difference in the long-term mortality of patients with HF and their NPS values for the entire cohort., Conclusion: Based on our findings, NPS showed promise as an independent predictor of long-term mortality in individuals with HF.
