Objective: The study is performed to compare the analgesic effect of tenoxicam supposotories and paracetamol pill for management of right mediolateral episiotomy repair. Materials and Methods: A total of 66 patients who gave birth vaginally with right mediolateral episitomy were randomly assigned to receive 40mg tenoxicam supposotories plus 3x1 placebo pill/po. (Group 1, n=33) or 3X500 mg paracetamol pill/po. plus placebo supposotories ( Group 2, n=33) after episiotomy repair and postpartum three days. Pain ratings were recorded before, first and 24 hours after medications. Verbal rating scale (VRS) and visual analog scale (VAS) used for pain record. Student T test, X 2 test, Mann Whitney U and Wilcoxon Rank Test used for statistical analysis and Spearman correlation analysis used for comparison between VRS and VAS (rs, n, p). Results: Tenoxicam supposotories is more effective analgesic than paracetamol pill for right mediolateral episiotomies pain. Objective: To evaluate whether oocyte quality, implantation and pregnancy rates in intra-cytoplasmic sperm injection (ICSI) cycles are related to the duration of gonadotrophin-releasing hormone antagonist (GnRH-ant) use. Material and Method: A total of 138 patients who were treated with GnRH-ant protocol at the IVF Clinic from March 2010 to January 2012 were enrolled in this study. The patients were classified into three groups according to duration of GnRH-ant use. Group 1: 4 days (n=51); group 2:5 days (n=57); group 3:6 days (n=30) antagonist application. Main outcome measures were implantation rate, pregnancy rate, fertilization rate, number of oocytes retrieved (NOR), number of mature oocytes (NMO). Results: The NOR and NMO were the lowest in group 1, intermediate in group 2, and the highest in group 3 respectively. There is no statistically significant difference between the groups in regard to total gonadotrophins used, fertilization, implantation or pregnancy rates. The pregnancy rates per ET and cycle were 37.9% and 31.8%, respectively. Conclusion: We noticed that longer GnRH-ant use was associated with more oocytes retrieved and more mature oocytes, but no difference in fertilization, implantation or pregnancy rates concluding that longer GnRH-ant use does not have a detrimental effect on in vitro fertilization outcome., Amaç: Gonadotropin salgılatıcı hormon antagonisti (GnRH-ant) kullanım süresinin, in vitro fertilizasyon (IVF)/ intra-sitoplazmik sperm enjeksiyonu (ICSI) sikluslarında oosit kalitesi, implantasyon ve gebelik oranlarına etkisi olup olmadığını araştırmak. Gereç ve Yöntem: IVF kliniğimizde Mart 2010- Ocak 2012 tarihleri arasında GnRH-ant protokolü ile tedavi edilen 138 hasta çalışmaya dahil edildi. Hastalar GnRH-ant kullanım sürelerine göre 3 gruba ayrıldı. Grup 1: 4 gün (n=51); grup 2: 5 gün (n=57); grup 3: 6 gün (n=30). İmplantasyon, fertilizasyon ve gebelik oranları, toplanan oosit sayısı (TOS) ve matür oosit sayısı (MOS) bakılan ana parametrelerdir. Gruplar arasında kullanılan toplam gonadotropin dozu, fertilizasyon oranı, implantasyon ve gebelik oranları açısından istatistiki anlamlı bir fark görülmedi. Siklus ve embriyo transferi başına görülen gebelik oranlarımız sırasıyla %31.8 ve %37.9 idi. Sonuç: Çalışmamızda GnRH-ant kullanım süresi uzadıkça toplanan oosit ve matür oosit sayısının arttığını, fakat fertilizasyon, implantasyon ve gebelik oranlarının fark etmemesi üzerinden, uzamış antagonist uygulamasının IVF sonucuna negatif etki oluşturmadığını gözlemledik.

Objective: Investigation of the effects of liposoluble / watersoluble contrast media used in hysterosalpyngography on fertility in unexplained infertility cases. Material and Method: Sixty primary or secondary infertile cases diagnosed as unexplained infertility were retrieved from the entire infertility cases admitted between June 1998 –March 2001 and they were analyzed by randomized, prospective, single blinded method and classified as the study group (G1=n:30, Lipiodol) and the control group (G2=n:30, Urovision). Hysterosalpyngography was provided two days after the end of menses. The patients who were diagnosed as unexplained infertility and with normal basal clinical and laboratory investigations for infertility have been just monitered for over three months after hysterosalpyngography was done. They were suggested to return back when menstruation delay occur. If pregnancy not achieved over 3 months patients directed for ovulation induction with clomiphene citrate + intrauterine insemination. Statistically Student T test, Mann Whitney U test and X2 were used for continous, ordinal and nominal data, respectively. Objective: To evaluate the intrauterine pathologies using office-based hysteroscopy (OH) in patients scheduled for assisted reproductive technologies (ART). Materials and Methods: This study was conducted at the In Vitro Fertilization (IVF) Unit of Firat University Hospital, between March 2010- January 2012. The 219 patients with no OH within the previous 6 months were enrolled the study with rigid hysteroscope (continous flow; 30-degree forward-oblique view) assembled in a 4-mm diameter diagnostic sheath with an atraumatic tip. After OH investigation, endometrial sampling was performed with biopsy catheter. The patients in whom the findings were normal proceeded to an IVF cycle within 1 month. Results: The procedure was succesful in 219 patients, with mean (±SD) age of 31.9 (±5.2) years, duration of infertility of 6.2 (±4.5) years and number of previous ART trials 1.6 (±1.1). The type of infertility were as follows: 150 primary (68.5%) and 69 secondary infertility (31.5%). Endometrial polyp and uterine subseptum were the major intracavitary abnormalities. Chronic endometritis and endometrial polyp were the major histopathologic abnormalities. Objective: To examine the effect of X ray, lipiodol and melatonin used during HSG on hystopathologic changes in the rat uterus. Material and Method: A total of 40 Spraque-Dawley type female rats were randomly allocated to 4 groups. Group 1 (n=10): Abdomen was opened and closed without X-ray exposure, group 3 (n=10): X-ray exposure, group 3 (n=10): Abdomen was opened and 0.1 ml Lipiodol was applied to each uterine horn, group 4 (n=10): 10 mg/kg/ip melatonin was administered and 15 minutes later, 0.1 ml Lipiodol was applied to each uterine horn. The rats in group 2, group 3 and group 4 were exposed to 3 doses of X-ray with 2-minute intervals after the closing of the abdomen (total dose = 15-20 milli Rad). The abdomen was opened 3 hours later in all rats. Uterine horn was removed and fixated in 10% formaldehyde. The preparations were examined under a light microscope to evaluate hystopathologic changes. Kruskal Wallis variance analysis was used of data. Objective: To examine the effect of applying 3%, 50%, 75% and 95% concentrations of trichloroacetic acid by attachment and instillation methods for endometrial ablation on ovarian reserve and dysplasic changes in ovarian surface epithelium in rats. Material and Method: Ninety rats were included in a randomized, bloc-designed, concentration-matched study program. Group1: (n=10): Abdomen was opened and closed. The groups in which 3% (Group 2, n=10), 50% (Group 3, n=10), 75% (Group 4, n=10), and 95% (Group 5, n=10) TCA attachment was applied, after abdomen was opened and a 2 cm. lengthwise incision was made in the left uterine horn. The groups in which 0,1 ml of 3% (Group 6, n=10), 50% (Group 7, n=10), 75% (Group 8, n=10) and 95% (Group 9, n=10) TCA instillation was applied to the left uterine horn, after the abdomen was opened. Abdomens of all rats were opened 21 days later. Left oophorectomy was performed. Preparations were evaluated in terms of ovarian follicle reserve and changes in ovarian surface epithelium. Group 2 and Group 6, Group 3 and Group 7, Group 4 and Group 8, Group 5 and Group 9 were compared (MWU test). Kruskall Wallis variance analysis was employed in the comparison including all groups (p0.05). Cornual pregnancy is a rare type of ectopic pregnancy, and diverse therapeutic options exist for the management. Medical treatment despite high initial beta HCG values is not thought to be safe. We reported a 39-year-old woman with an initial beta HCG value of 22000 mIU/mL and diagnosed of a cornual pregnancy. Patient was managed successfully with the administration of combined systemic and ultrasonographically guided local injection of methotrexate into the gestational sac. Amaç: Bipolar koter kullanılarak yapılan unilateral proksimal tubal oklüzyon işleminin birinci ve altıncı aylarda rat over histopatolojisi üzerine etkilerinin incelenmesi. Yöntem: Erişkin 28 Wistar albino rat östrus fazında rastgele 4 gruba ayrıldı. G1 (n=7): Batın açılıp kapatılan ve 1 ay sonra sol ooferektomi yapılan grup. G2 (n=7): Sol proksimal tubal oklüzyon yapılıp 1 ay sonra sol ooferektomi yapılan grup. G3 (n=7): Batın açılıp kapatılan ve 6 ay sonra sol ooferektomi yapılan grup. Material and Methods: Adult 28 Winstar albino rats at eustrous phase randomly divided into 4 groups. G1 (n=7): Left oopherectomy, one month after laparatomy. G2 (n=7): Left oopherectomy, one month after proximal tubal oclusion. G3 (n=7): Left oopherectomy, six months after laparatomy. G4 (n=7): Left oopherectomy, six months after proximal tubal oclusion. Left ovary specimens fixed with formaldehyde, dyed with Hematoxylen-Eosin and ovarian reserve examined. The number of atretic follicles, corpus luteum and corpus albicans determined. Angiogenesis in corpus luteum and presence of ovarian stromal fibrosis examined. G1and G2 were compared with G3 and G4. Statistical analysis of ordinal results were examined by Mann Whitney U test and nominal results by x2 test f. P0.05, Mann Whitney U test ). Angiogenesis regression in corpous luteum were lower in both G2 and G4. No macroscopic or microscopic cyst formation obtained in all groups. Conclusion: Proximal tubal occlusion procedure by bipolar coagulation has no harmful effect on rat oavarian histopathology at both early and late period.

Amaç: Gestasyonel koryokarsinomun tubal ektopik gebeliklerle birlikteliği son derece nadir bir durumdur. Ektopik gebelik bulguları ile birlikte olması ve nadir görülmesi nedeniyle olgu sunumu yapılmıştır.Olgu Sunumu: S.A. 31 yaşında G3, P2, A0, C1 olan olgu, yaklaşık bir aylık menstrüel gecikme sonrası şiddetli pelvik ağrı ile başvurdu. Yapılan pelvik muayenesinde serviks adet kanamalı, servikal hareketlerde hassasiyet, batın ve pelvik bölgesinde yaygın defans ve rebound tespit edildi. Gebelik testi pozitifti. Ultrasonografide yaygın serbest mayi görünümü vardı. Mevcut bulgularla rüptüre ektopik gebelik ön tanısı ile acil laparatomi yapıldı. Çıkarılan spesmenin histopatoloji sonucunun koryokarsinom gelmesi üzerine EMA-CO protokol ü uygulandı. Kemoterapi sonrasında tam remisyon sağlandı.Sonuç: Ektopik gebeliklerle birliktelik gösteren koryokarsinom olguları, tanı konulduğunda çoğu zaman metastaz yapmış olması nedeniyle sıklıkla agresif seyreder. Küçük ve insitu değişiklikleri tanımlayabilmek için tubal gebelikler mutlaka histopatolojik olarak incelenmeli ve olgular Beta hCG düzeyi ile takip edilmelidir., Objective: The gestational choricarcinoma with tubal ectopic pregnancy is very rare. This case is represented due to this rarity.Case Report: Thirty-one years old, gravida 3, parity 2, curettage 1 woman with severe pelvic pain after 1 month menstruation delay accepted to the clinic. Passive cervical bleeding and tenderness with cervical motion was obtained during pelvic examination. Marked defence and rebound was the sign of the abdominal physical examination. Pregnancy test were positive. Ultrasonography showed marked free fluid in the abdomen. Emergency laparotomy is applied to the patient because of the ruptured ectopic pregnancy diagnosis. Final histopathology of the specimen was choriocarcinoma, so EMA-CO protocol administered and after the chemotherapy complete remission is achieved.Conclusion: Choriocarcinomas with ectopic pregnancies have agressive progression since usually they have had metastases when diagnosed. Tubal pregnancies must be examined histopathologically for awareness of minor and in situ changes and cases must be followed by β- HCG levels.

Amaç: Sistemik tek doz Methotrexate (MTX) verilmesinin over folikül rezervi üzerine etkisinin araştırılması.Materyal ve Metot: Çalışmada toplam 20 adet Wistar Albino cinsi kullanıldı. Düzenli sikluslara sahip olduğu tespit edilen ratlar rastgele iki gruba ayrıldı. Grup I (n=10)de : İntra musküler (IM) olarak tek doz halinde 50mg/m2 MTX, Grup II (n=10)de IM olarak tek doz halinde aynı hacimde serum fizyolojik (SF) uygulandı. Son MTX dozundan sonra bir hafta süreyle tüm ratlara günlük smear takipleri yapılarak siklus düzensizlikleri olup olmadıkları araştırıldı. Tedavi sonrası üçüncü haftanın sonunda ratlar servikal dislokasyon ile sakrifiye edildi ve bilateral overleri çıkarıldı. Çıkarılan overlerden hazırlanan hemotoksilen-eozin ile boyanmış preparatlarda primordial, primer, sekonder, tersiyer folikül, korpus luteum sayıldı ve folikül rezervi hesaplandı. Fibrozis varlığı ve histiyosit infiltrasyonu araştırıldı.Bulgular: Gruplar arası karşılaştırmalarda tek doz MTX verilen grupta primer, sekonder folikül sayılarında azalma, atretik folikül, fibrozis ve histiyosit skorlarında artışlar olduğu tespit edildi. Fark anlamlı bulunmadı (p>0.05). Tedavi sonrası smear takiplerinde MTX verilen ratların bir tanesinde estrus fazı görülmedi (p>0.05). Korpus luteum ve folikül kisti MTX grubunda azalmış olarak bulundu (p>0.05).Sonuç: MTXın over folikül rezervlerinde azalmalara neden olması menapoz yaşında etkili olabilir. Group I (n=10) was administered single-dose intramuscular (im) 50 mg/m2 MTX, while Group II (n=10) received single-dose serum physiologic at the same volume. After administration of the last MTX dose, daily smear follow-ups of all rats were made for one week and these were examined for cycle irregularities. At the end of the third week after treatment, rats were sacrified and bilateral ovaries were taken out. Primordial, primary, secondary, tertiary follicle and corpus luteum were counted in the preparations of ovarian samples stained with hematoxylin eosine. Presence of fibrosis and histiocyte infiltration was examined. Results: Inter-group comparison showed that there was a decrease in primary, secondary follicle counts and a increase atretic follicle counts, fibrosis and histiocyte infiltration scores in the group that was administered single-dose MTX. The differences were insignificant (p>0.05). It was seen in the post-treatment smear follow-ups that one rat that was given MTX did not have estrus phase (p>0.05). No difference was found between groups in terms of corpus luteum and follicle cyst (p>0.05). Conclusion: The reductions by MTX in ovarian follicle reserves may affect menopause age. Further studies are needed to determine its effects on humans.
