Abstract

Rheumatoid arthritis (RA) is an autoimmune disease, in which the body inappropriately makes antibodies against its own tissues. The exact role of immune cell activation, however, has been unknown until recently. Knowledge in this area has been made possible by technological advances which enabled identification of cytokines, activating proteins and other molecules released by different types of immune cells such as T- and B-lymphocytes (T and B cells). Synovitis - inflammation of the synovial lining of joints - is a key feature of RA, and both T and B cells have been associated with this process. Interleukin-4 (IL-4) is a cytokine made by T cells, which stimulates both T and B cells, and may therefore contribute to the development of synovitis. To assess this possibility, IL-4 activity and related immune functions were evaluated in synovial fluid (SF) from 29 RA patients. IL-4 was absent or at very low levels in RA SF. RA SF contained substances which inhibited IL-4 to cause proliferation of lymphocytes, as well as inhibiting similar activity of another cytokine, IL-2. Antibody against transforming growth factor beta (TGF beta), another cytokine, blocked the inhibitory activity of RA SF, but IL-4 was not needed for enhanced proliferation; the SF inhibitor apparently acted on the cells rather than by interfering with IL-4 effects. Appreciable levels of TGF beta were found in RA SF. However, inhibitory activity was not completely removed by the antibody. The study suggests that T cell-produced lymphokines are not associated with the chronic stage of RA, but a possible function in early RA is not ruled out. Abstract

Multiple myeloma is a cancer of antibody-producing cells in the bone marrow. This cancer is frequently accompanied by excessive resorption of bone in the vicinity of the cancerous cells and a resulting hypercalcemia, or excess level of calcium in the blood. To appreciate more fully the processes that are going on within the bone in response to multiple myeloma, levels of Gla protein were measured. Gla, also called osteocalcin and serum BGP, is synthesized only in osteoblasts, the bone-forming cells. Serum BGP was measured in a number of patients, including a series of 117 patients with multiple myeloma. Gla protein levels were abnormal in 25 cases, or 21 percent. In 14 patients, the Gla levels were increased, and reflected more indolent disease in the patient, whereas in 11 patients decreased levels correlated with lytic bone lesions, hypercalcemia, and a poorer prognosis. Since Gla protein is thought to be an indicator of bone formation and to be independent of bone resorption, these results indicate that bone formation by the osteoblasts is an important factor in multiple myeloma. That is, a major difference among multiple myeloma patients may not be the rate at which bone is resorbed; rather, although bone is being resorbed in all multiple myeloma patients, some can continue to rebuild bone as it is torn away. Abstract

OBJECTIVE. To assess objective and subjective evidence of sleep disturbances in patients with rheumatoid arthritis (RA) and to examine correlations between parameters of inflammatory activity and sleep pathology. METHODS. Nineteen RA patients and 19 age-matched healthy control subjects underwent all-night polysomnography on 2 consecutive nights. RA patients were also evaluated for daytime sleepiness by mean sleep latency test and responded to a self-report questionnaire on their first night. RESULTS. Whereas normal sleep architecture is conserved in RA, we confirmed former findings of severe sleep fragmentation and an enhanced presence of primary sleep disorders. No correlation exists between RA activity and the sleep disorders. Subjective assessment was not consistent with the objective evidence of sleep disruption, unlike the findings in patients with fibrositis. CONCLUSION. Sleep is severely disturbed in patients with RA, regardless of the inflammatory disease activity. The specificity of the sleep disorders assessed needs confirmation, as does specific sleep therapy for these patients.
