BackgroundInvasive fungal diseases (IFD) caused by Cryptococcus and dimorphic fungi are associated with significant morbidity and mortality. Isavuconazole (ISAV) is a novel, broad-spectrum, triazole antifungal agent (IV and by mouth [PO]) developed for the treatment of IFD. It displays potent activity in vitro against these pathogens and in this report we examine outcomes of patients with cryptococcosis or dimorphic fungal infections treated with ISAV.MethodsThe VITAL study was an open-label nonrandomized phase 3 trial conducted to evaluate the efficacy and safety of ISAV treatment in management of rare IFD. Patients received ISAV 200 mg 3 times daily for 2 days followed by 200 mg once-daily (IV or PO). Proven IFD and overall response at end of treatment (EOT) were determined by an independent, data-review committee. Mortality and safety were also assessed.ResultsThirty-eight patients received ISAV for IFD caused by Cryptococcus spp. (n = 9), Paracoccidioides spp. (n = 10), Coccidioides spp. (n = 9), Histoplasma spp. (n = 7) and Blastomyces spp. (n = 3). The median length of therapy was 180 days (range 2-331 days). At EOT 24/38 (63%) patients exhibited a successful overall response. Furthermore, 8 of 38 (21%) had stable IFD at the end of therapy without progression of disease, and 6 (16%) patients had progressive IFD despite this antifungal therapy. Thirty-three (87%) patients experienced adverse events.ConclusionsISAV was well tolerated and demonstrated clinical activity against these endemic fungi with a safety profile similar to that observed in larger studies, validating its broad-spectrum in vitro activity and suggesting it may be a valuable alternative to currently available agents.Clinical trials registrationNCT00634049.

In terms of model predictive control (MPC) performance degradation caused by operational faults, in this article, a robust MPC strategy with active fault tolerance properties is proposed. The proposed strategy incorporates a fault supervision layer into the structure of conventional cost‐contracting formulation‐based robust MPC for the online update of the nominal controller model in the event of faults. The robust MPC is based on multiplant uncertainty, while the supervisory layer consists of a bank of unknown input observers and a decision‐making algorithm. Simulation results in a nonlinear polymerization reactor subject to process faults demonstrate that the proposed approach offers superior performance compared to the conventional strategy. Synthetic dyes are not so desired today, some studies point them as causing health problems such as allergies and environmental problems because they are substances of difficult natural degradation, generating a quest for the return to natural. Fungi have a far relation in the production of dyes, this is the case of angkak, a type of rice fermented by fungi of the genus Monascus, a food adopted by some Asian countries. In the search for new colorants of fungal origin, our research group have been working identifying some fungi for this purpose, being Penicillium sclerotiorum LM 5679 one of the most promising, became the focus of this work. A small curiosity observed during the purification of this fungi was that it had a more intense coloration when it was not pure. This work aims to study the influence of abiotic and biotic stresses on dye production. To study the abiotic stresses, a submerged culture was performed with univariate analysis of the factors: pH, temperature, inoculum size, water activity, osmolarity and light exposure. For the biotic stresses, the same bioprocess was carried out by simultaneous inoculation, co-culture, of C. albicans, S. aureus and E. coli with P. sclerotiorum LM 5679. The concentration of pigments was estimated by UV-VIS Spectrophotometry, the biomass of some experiments was also estimated. Among the analyzed factors none exceeded the amount of pigment produced by the standard bioprocess, the highest values were obtained in the tests conducted at pH 3 and inoculum size of 106 cells/mL. The results indicate that there is no relationship between the amount of pigment and the concentration of biomass. This work presents how the stress factors have influence on the bioprocess, it is necessary more to conduct more investigation focusing on the optimization of the bioprocess. The importance of this work was to explain how the stress factors influence the production of pigment by P. sclerotiorum LM 5679 and should be investigated with a focus on the optimization of the bioprocess. Colorantes de origem natural acompanharam o desenvolvimento humano, mas, com o avan??o tecnol??gico, surgiram os colorantes sint??ticos, mais f??ceis de serem obtidos por n??o dependerem da sazonalidade comum ??s plantas, que ainda s??o as principais fontes de colorantes naturais. Atualmente, os colorantes sint??ticos atualmente n??o s??o t??o desejados, haja vista alguns estudos apontarem tais corantes como causadores de problemas de sa??de como alergias, e de problemas ambientais, por se tratarem de subst??ncias de dif??cil degrada????o natural, gerando uma busca para o retorno ao natural. Fungos possuem uma rela????o milenar na produ????o de colorantes, ?? o caso do angkak, um tipo de arroz fermentado por fungos do g??nero Monascus, um alimento adotado por alguns povos asi??ticos. Na busca por novos colorantes de origem f??ngica, em trabalhos deste grupo foram isolados e identificados alguns fungos para esta finalidade, sendo Penicillium sclerotiorum LM 5679 um dos mais promissores, motivo pelo qual se tornou o foco deste trabalho. Este trabalho tem o objetivo de estudar a influ??ncia de estresses abi??ticos e bi??ticos na produ????o de colorante. Para estudar os estresses abi??ticos foi realizado cultivo em meio submerso com an??lise univariada dos fatores: pH, temperatura, tamanho do in??culo, atividade de ??gua, osmolaridade, velocidade de agita????o e exposi????o a luz. Para os estresses bi??ticos, foi realizado o mesmo bioprocesso realizando inocula????o simult??nea, co-cultura, de C. albicans, S. aureus e E. coli com P. sclerotiorum LM 5679. Os n??veis de colorantes foram estimados por espectroscopia no ultravioleta vis??vel (UV/VIS), a biomassa de alguns experimentos tamb??m foi estimada. Dentre os fatores analisados nenhum superou a quantidade de pigmento produzido no bioprocesso adotado como padr??o, os valores mais altos foram obtidos nos testes conduzidos em pH 3 e tamanho de in??culo de 106 c??lulas/mL. Os resultados apontam que n??o h?? rela????o entre a quantidade de pigmento e a concentra????o de biomassa. A import??ncia deste trabalho foi de expor como os fatores de estresse influenciam na produ????o de pigmento por P. sclerotiorum LM 5679 e, por isso, esses fatores devem ser investigados com foco na otimiza????o do bioprocesso.

Furthermore, 8 of 38 (21%) had stable IFD at the end of therapy without progression of disease, and 6 (16%) patients had progressive IFD despite this antifungal therapy. Thirty-three (87%) patients experienced adverse events. Conclusions. ISAV was well tolerated and demonstrated clinical activity against these endemic fungi with a safety profile similar to that observed in larger studies, validating its broad-spectrum in vitro activity and suggesting it may be a valuable alternative to currently available agents.

In advanced process control, the Model Predictive Control (MPC) may be considered the most important innovation in recent years and the standard tool for industrial applications due to the fact that it keeps the plant operating in the constraints more profitable. However, like every control algorithm, the MPC after some time in operation rarely works as originally designed. Thus, to preserve the benefits of MPC systems for a long period of time, their performance needs to be monitored and evaluated during the operation. This task require the presence of reliable and effective tools to detect when the controller performance is below of the desirable, to define the need, or not, of recommissioning the system. Thus, the objective of this work is development of techniques for monitoring and evaluating the performance of multivariable predictive controllers, being developed two new tools: LQG benchmark Modified and IHMC benchmark. The results obtained from numerical simulations were satisfactory and consistent with the technical literature applied in the developments of the evaluators, which were used in the monitoring of the control system MPC of the oil-water-gas three-phase separation process, offering an appropriate solution and providing subsidies for implementations in real industrial systems. Em controle avançado de processos, o controlador preditivo ou MPC (Model Predictive Control) pode ser considerado como a mais importante inovação dos últimos anos e a ferramenta padrão para aplicações industriais, devido ao fato do MPC manter a planta operando dentro das suas restrições de forma mais lucrativa. Entretanto, como todo algoritmo de controle, o MPC depois de algum tempo em operação dificilmente funciona como quando fora inicialmente projetado. Desta forma, com o objetivo de manter os benefícios dos sistemas MPC por um longo período de tempo, seu desempenho precisa ser monitorado e avaliado durante a operação. Esta tarefa requer a presença de ferramentas efetivas e confiáveis para detectar quando o desempenho do controlador estiver abaixo do desejável, para definir a necessidade, ou não, de um recomissionamento do sistema. Destarte, aborda-se neste trabalho o desenvolvimento de técnicas para monitoramento e avaliação de desempenho de controladores preditivos multivariáveis, sendo desenvolvidas duas novas ferramentas: LQG benchmark Modificado e IHMC benchmark. Os resultados obtidos a partir de simulações numéricas foram satisfatórios e coerentes com a literatura técnica aplicada no desenvolvimento dos avaliadores, os quais foram utilizados no monitoramento do sistema de controle MPC do processo de separação trifásica água-óleo-gás, oferecendo assim uma solução apropriada e fornecendo subsídios para implementações em sistemas industrias reais.

OBJECTIVE: To present the ongoing researches for biomarkers in Alzheimer disease METHODS: Bibliographic review, starting from 2000, utilizing PubMed data bank. RESULTS: This revision presents the actual evidences of the main markers involved in Alzheimer disease including the molecular (presented in blood and cerebrospinal fluid), the associated with image (pósitron emission tomography, single photon emission computed tomography and magnetic ressonance) and neural networks. CONCLUSIONS: Finding an efficient biomarker will enable significative progress in the clinic evaluation and in the disease development approaches, also, will contribute to the increment of new therapeutic estrategies.

PURPOSE: To describe and analyze the features of a cases series of patients with primary epithelial neoplasms of the lacrimal gland, its surgical treatment, and histopathological findings. METHODS: Retrospective evaluation of files from patients with primary epithelial neoplasms of the lacrimal gland in the period from 1997 to 2007. All patients with primary epithelial tumors of the lacrimal gland were included in this study. Data on gender, age, clinical features, surgical treatment, histopathological findings and follow-up were collected. The slides with histological sections of the tumors were reviewed by the same pathologist. RESULTS: During the study period, there were 12 patients, 5 (41.7%) with benign tumors, all pleomorphic adenomas (benign mixed tumor) and 7 (58.3%) with malignant neoplasms, thus distributed: four cases of adenoid cystic carcinoma, two of mucoepidermoid carcinoma and one carcinoma expleomorphic adenoma. Globally, patients mean age was 54.1 years-old (ranging from 14 to 70 years-old), with mean age of 52.4 years-old (ranging from 14 to 65 years-old) for benign neoplasms, and 55.3years-old for malignant neoplasms (ranging from 26 to 70 years-old). Clinical follow-up information, ranging from 2 to 10 years-old, was available for all patients. Three patients developed distant metastasis and died of disease. CONCLUSIONS: The most frequent primary epithelial neoplasms of the lacrimal gland were pleomorphic adenoma and adenoid cystic carcinoma during the study period. Malignant tumors were more frequent than benign tumors. The histopathological diagnosis and the disease initial stage can play a significant role in patient's survival. OBJETIVO: Descrever e analisar as características de uma série de casos de portadores de neoplasias epiteliais primárias da glândula lacrimal, o tratamento cirúrgico, assim como os achados histopatológicos. MÉTODOS: Avaliação retrospectiva dos arquivos de pacientes com neoplasias epiteliais primárias da glândula lacrimal, no período de 1997 até 2007. Todos os pacientes com tumores epiteliais primários da glândula lacrimal foram incluídos neste estudo. Foram analisados os dados sobre sexo, idade, características clínicas, tratamento cirúrgico, achados histopatológicos e seguimento dos pacientes. As lâminas com secções histológicas dos tumores foram revisadas pelo mesmo patologista. RESULTADOS: No período do estudo, foram encontrados 12 pacientes, sendo 5 (41,7%) portadores de tumores benignos, todos adenomas pleomórficos (tumor benigno misto), e 7 (58,3%) com neoplasias malignas, assim distribuídos: quatro casos de carcinoma adenóide cístico, dois de carcinoma mucoepidermóide e um de carcinoma ex-adenoma pleomórfico. Analisando-se de modo global, a idade média dos portadores foi de 54,1 anos (variando de 14 a 70 anos); com média de idade de 52,4 anos (variando de 14 a 65 anos) para neoplasias benignas, e 55,3 para neoplasias malignas (variando de 26 a 70 anos). Informações do seguimento, variando de 2 a 10 anos, estavam disponíveis para todos os pacientes. Três pacientes desenvolveram metástases distantes e morreram devido à doença. CONCLUSÕES: A maioria das neoplasias epiteliais primárias da glândula lacrimal foi o adenoma pleomórfico e o carcinoma adenóide cístico no período de estudo. Os tumores malignos foram mais frequentes que os benignos. O diagnóstico histopatológico e o estadiamento inicial da doença podem desempenhar uma papel significante na sobrevida do paciente.

OBJETIVOS: Definir a taxa de detecção de tracoma em escolares do Estado de Alagoas - Brasil. MÉTODOS: A amostra foi constituída de 6.424 crianças e adolescentes. A avaliação clínica dos escolares foi realizada por graduandos da Faculdade de Medicina sob supervisão de professores do Departamento de Oftalmologia, seguindo os critérios da Organização Mundial de Saúde. A avaliação clínica foi caracterizada por exame dos cílios, das pálpebras, da conjuntiva e da córnea de ambos os olhos. Os estudantes foram catalogados, anotando-se nome, sexo, idade, diagnóstico da forma clínica e município. Quanto à idade, os escolares foram divididos em três grupos: grupo A (14 anos). RESULTADOS: A taxa de detecção de tracoma foi de 4,5%. Dos 3.280 estudantes do sexo masculino, 161 (4,9%) casos foram considerados confirmados, e dos 3.144 estudantes do sexo feminino, 131 (4,2%) casos foram considerados confirmados. Do total de escolares do grupo A, B e C, respectivamente 175 (5,3%), 113 (8,0%) e 4 (1,6%), foram considerados casos confirmados. CONCLUSÃO: Nesta região, o tracoma parece não ter sido erradicado e, portanto, deve permanecer como diagnóstico diferencial de conjuntivite folicular crônica em crianças e adolescentes advindos dela.

The authors report a case of a 19-year-old woman admitted for the investigation of fever and hemolytic anemia for the previous 2 months. As an inpatient, she had convulsions and sudden loss of consciousness, developing hemoptysis, hypoxia, and respiratory insufficiency. Examination showed pericardial effusions on the echocardiogram and bilateral alveolar condensations on the thoracic radiograph. A hypothetical diagnosis of systemic lupus erythematosus was made, and measurement of the antinuclear factor was requested along with daily pulse therapy methylprednisolone, in spite of which the outcome was fatal. Afterwards, the result of the antinuclear factor test was positive, with a titer of 1:5120, showing a fine punctiform pattern, fulfilling the criteria for systemic lupus erythematosus according to the American College of Rheumatology. Secondary pulmonary hemorrhage in this connective tissue disease is an uncommon but serious complication that involves a high level of mortality in spite of intensive treatment, as is also reported in the literature. Os autores relatam o caso de uma paciente de 19 anos admitida para investigação de febre e anemia hemolítica, com dois meses de evolução. Durante a internação apresentou crises convulsivas tônico-clônicas e queda súbita no nível de consciência, evoluindo com hemoptise, hipoxemia e insuficiência respiratória. Os exames evidenciaram derrame pericárdico ao ecocardiograma e condensações alveolares bilaterais à radiografia de tórax. Foi levantada a hipótese diagnóstica de lúpus eritematoso sistêmico sendo solicitada a pesquisa do fator antinuclear e iniciada terapia com pulsos diários de metilprednisolona, a despeito da qual a paciente evoluiu para o óbito. Posteriormente o resultado do fator antinuclear mostrou-se positivo, em título de 1:5120, padrão puntiforme fino, preenchendo critérios para lúpus eritematoso sistêmico segundo o Colégio Americano de Reumatologia. A hemorragia pulmonar secundária a esta doença do tecido conjuntivo é uma forma incomum e grave de manifestação da doença que evolui com alta mortalidade, apesar do tratamento intensivo, conforme descrito na literatura.

Studies have been evaluating instructional methods to teach it. Learning based on the construction of disease presentation schemes has been suggested as an effective strategy, but its use has not been tested to support the development of CR. Although medical semiology is important to develop CR, in general, its teaching occurs separately from the practice of CR. Objectives: To evaluate the effect of teaching chest pain semiology through the construction of disease presentation schemes in the diagnostic accuracy of medical students. Materials and methods: Experimental study, carried out on medical students from 4th and 6th terms, in 3 phases. 26 students were recruited and randomly divided into 2 groups - SR and MM. Group SR versus Group MM, 4th term versus 6th term and study phases were compared. In the initial evaluation, students resolved 8 clinical cases which presented chest pain (CP). In the training phase, after a CP semiology mini-class, students were randomly divided into 2 groups, one of which was submitted to the teaching of CP semiology based on mind maps (MM), and the other used the structured reflection strategy (SR). After a week, students were submitted to an evaluation of 8 new cases with the same diagnoses. In all phases, the following were evaluated: the identification of the responsible organic system and of the structure responsible for the clinical manifestation, the diagnostic performance, the resolution time, and the participants' confidence in their answers. Results: In both groups there was an improvement in diagnostic performance, a reduction in the resolution time and an improvement in confidence in the diagnosis. There was not any difference in performance between Group SR and Group MM. The study revealed a strong correlation between identifying the anatomical structure involved in the manifestation of chest pain and a correct diagnosis. Conclusions: Both strategies were effective in teaching semiology and CR. The identification of the system and anatomical structure involved in the clinical presentation showed a strong correlation with the accuracy of the diagnosis Introdução: o raciocínio clínico (RC) se refere ao processo cognitivo pelo qual o médico é capaz de estabelecer o diagnóstico correto e propor conduta adequada ao problema clínico encontrado. Estudos têm avaliado métodos instrucionais para seu ensino. O aprendizado, baseado na construção de esquemas de apresentação das doenças, tem sido sugerido como estratégia eficaz, mas seu uso não foi testado para o desenvolvimento do RC. Apesar do conhecimento da semiologia médica ser importante para o RC, em geral, seu ensino ocorre separadamente da prática do RC. Objetivos: avaliar o efeito do ensino da semiologia de dor torácica, por meio da construção de esquemas de apresentação das doenças na acurácia diagnóstica em estudantes de medicina. Materiais e métodos: estudo experimental, realizado em estudantes de medicina do 4° e 6° períodos, em 3 fases. Foram recrutados 26 alunos, divididos aleatoriamente em 2 grupos (RE e MM) e comparados RE versus MM, 4° período versus 6° período, e fases do estudo. Na avaliação inicial, os alunos resolveram 8 casos clínicos com apresentação de dor torácica (DT). Na fase de treinamento, após a miniaula de semiologia da DT, os alunos foram aleatoriamente divididos em 2 grupos, sendo que um foi submetido ao ensino da semiologia da DT baseado em mapas mentais (MM), e o outro utilizou a estratégia de reflexão estruturada (RE). Após uma semana os alunos foram submetidos a avaliação, com 8 novos casos, com os mesmos diagnósticos. Em todas as fases foram avaliados: a identificação do sistema orgânico e a estrutura anatômica responsável pela manifestação clínica, a performance diagnóstica, o tempo de resolução e a confiança dos participantes em suas respostas. Resultados: Em ambos os grupos (RE e MM) ocorreu melhora de performance diagnóstica, redução do tempo de resolução e melhora da confiança no diagnóstico. Não houve diferença de performance entre os grupos RE e MM. O trabalho comprovou forte correlação da identificação da estrutura anatômica, envolvida na manifestação da dor torácica, ao acerto do diagnóstico. Conclusões: ambas as estratégias foram eficazes no ensino da semiologia e do RC. A identificação do sistema e estrutura anatômica, envolvida na apresentação clínica, apresentou forte correlação com a acurácia do diagnóstico

In despite of its importance, it has been observed that this ability is prone to decrease during the medical training. The pedagogical methods for teaching and practicing empathy over the period that physicians are trained have been widely discussed. The Health Empathy Map (HEM) is a structured instrument that enables the practice of this skill in a health care environment. Objectives: Assess the effectiveness of the use of HEM as empathy skill practice method on the scores of empathy of the orthopedics and traumatology resident physicians and also the patients perception of empathy. Materials and Methods: Experiential study, carried out by 22 orthopaedics and traumatology resident physicians at São Francisco Hospital Complex. In the first phase, the empathy of resident physicians was assessed using the Jefferson Scale of Medical Empathy (JSE), and in the patients seen in the first-aid room, the Consultation and Relational Empathy (CARE) instrument was applied in order to assess the empathy based on the patient perspective. Sociodemographic questionnaires were applied together to characterize residents and patients. In the second phase, the implementation of the use of HEM was carried out by the resident in the outpatient service for four weeks. One week after the interruption of the use of MES (3rd phase), JSE and CARE were again applied. Results: The statistics analysis by the student´s t-test showed that the empathy scores of JSE did not show significant differences between the first and the third phases. Furthermore, the chi-squared test demonstrated that the intervention was not enough to increase the self-perception of residents level of empathy. When the patients perception of empathy is assessed, the comparison of the scores from the CARE scale by the student´s t-test, showed an increase of these scores between the first and third phases, showing an improvement of empathy perceived by the patient. The Pearson analysis of correlation revealed that there is no dependence between the results of these two scales. Conclusion: The HEM, for four weeks, did not produce an increase on the scores of self-report of the resident physicians empathy. Notwithstanding, after the use of HEM there have been an increase in the perception of the resident physician empathy during the medical care from the patients point of view. There was no correlation between the in-training physicians empathy self-assessment and their patients assessment. From the patient’s viewpoint, there was an improvement of the resident’s empathy after the intervention, demonstrating that an empathic behaviour can be taught integrating the training and the HEM use in the Medical Residency Programmes Introdução: A empatia representa um dos domínios da inteligência emocional e, sua prática é associada a melhores resultados diagnósticos e terapêuticos. Apesar da sua importância, foi observado que, durante o período da formação médica, esta habilidade tende a decrescer. Muito se discute sobre os métodos pedagógicos para o ensino e prática da empatia durante o período de formação médica. O Mapa da Empatia em Saúde (MES) é um instrumento estruturado que permite a prática dessa habilidade no ambiente assistencial. Objetivos: Avaliar o efeito do uso do MES como método de prática da habilidade da empatia nos escores de empatia dos médicos residentes de Ortopedia e Traumatologia e na empatia percebida por parte dos pacientes. Materiais e métodos: Estudo experimental, com 3 fases, realizado com 22 médicos residentes de Ortopedia e Traumatologia do Complexo Hospitalar São Francisco. Na 1ª fase, avaliou-se a empatia dos médicos residente através da Escala Jefferson de Empatia Médica (JSE) e nos pacientes atendidos no ambulatório, foi aplicado o instrumento Consultation and Relational Empathy (CARE), que avalia a empatia do médico percebida pelo paciente. Juntamente foram aplicados questionários sociodemográficos, para a caracterização dos residentes e dos pacientes. Na 2ª fase, implantado o uso do MES pelo residente no atendimento ambulatorial por quatro 4 semanas. Uma semana após a interrupção do uso do MES (3ª fase), foram novamente aplicadas a JSE e a CARE. Resultados: A análise estatística pelo teste t de Student mostrou que os escores de empatia da JSE não apresentaram diferenças significativas entre a 1ª e a 3ª fase. Além disso, o teste do qui-quadrado demonstrou que a intervenção não foi suficiente para aumentar a autopercepção da empatia por parte dos residentes. Ao se avaliar a percepção da empatia na perspectiva dos pacientes, a comparação dos escores da escala CARE pelo teste t de Student, evidenciou aumento dos escores entre a 1ª e a 3ª fase, indicando uma melhora da empatia percebida por parte do paciente. A análise de Correlação de Pearson revelou que não existe dependência entre os resultados dessas duas escalas. Conclusão: O MES, por 4 semanas, não promoveu aumento dos escores de autorrelato da empatia nos médicos residentes. Apesar disso, após o uso do MES, houve aumento da percepção da empatia do médico residente na perspectiva do paciente. Não houve correlação entre a autoavaliação da empatia dos médicos e as avaliações de seus pacientes. Do ponto de vista dos pacientes, encontramos melhorias na empatia dos residentes após a intervenção, demonstrando que o comportamento empático pode ser ensinado integrando treinamento e o uso do MES nos programas da residência médica

Thus, one way to encourage engagement, group learning, collaboration and interaction is the use of educational games integrated into classes and seminars. Being part of a game encourages greater approximation to the learning aim, enabling the student to establish and apply the concepts that are being dealt with. As well as benefiting memory and performance, games and other interactive learning methods have important social benefits for the students. Aim: To assess the affects of adding gamification to traditional lessons that are contextualized through clinical case discussions for large groups of medical students. Methodology: This is an experimental study, given to 39 students from the third period of the medical school at the Presidente Tancredo de Almeida Neves University Center in São João Del Rei (MG), during the first semester of 2019. The same class was given to all the participants, dealing with the diagnostic differentials of Thyrotoxicosis and, following this, the sample was randomized into two groups: one group for clinical cases discussion and another for gamification of the clinical cases. At the end of the activities, assessment was made of student motivation, knowledge retention (short and long term) and knowledge transfer for the resolution of the Thyrotoxicosis clinical cases. Results: Analysis of the groups showed greater attention, confidence and satisfaction, among the motivation constructs, in the students that used the game compared with the students that were submitted to traditional clinical case discussion. Knowledge retention, in the short and long term, as well as knowledge transfer had similar levels between the two groups, showing that the educational game, in this context, is not inferior to the traditional method used. Conclusion: Therefore, the introduction of gamification, for learning reinforcement in traditional classes contextualized by clinical cases, may generate its own effects in increasing motivation in instructional activities Introdução: Gamificação é a utilização de mecanismos baseados em games, sua estética e lógica para engajar pessoas, motivar atitudes, promover aprendizado e solucionar problemas. Assim, uma forma de incentivar a elaboração, o aprendizado em grupo, bem como a colaboração e a interatividade, é a utilização de jogos educacionais integrados às aulas e seminários. A participação em um jogo incentiva o aprofundamento no objetivo de aprendizagem, possibilitando que o aluno elabore os conceitos que estão sendo abordados e os aplique. Além dos benefícios de memória e desempenho, os jogos e os outros métodos de aprendizagem interativa têm importantes benefícios sociais para os alunos. Objetivo: Avaliar os efeitos da adição de gamificação, nas aulas tradicionais contextualizadas por discussão de casos clínicos, para grupos grandes de estudantes de medicina. Metodologia: Trata-se de pesquisa experimental, aplicada a 39 alunos do terceiro período do curso de medicina do Centro Universitário Presidente Tancredo de Almeida Neves, em São João Del Rei (MG), no primeiro semestre de 2019. Foi ministrada aula comum a todos os participantes versando sobre o diagnóstico diferencial das tireotoxicoses e, posteriormente, a amostra foi randomizada em dois grupos: um grupo para discussão de casos clínicos e o outro grupo para gamificação de casos clínicos. Ao final das intervenções, foram avaliadas a motivação dos estudantes, a retenção do conhecimento (curto e longo prazo) e a transferência do conhecimento para resolução de casos clínicos de tireotoxicose. Resultados: A análise dos grupos revelou maior atenção, confiança e satisfação, dentre os domínios da motivação, dos estudantes que utilizaram o jogo proposto quando comparados aos estudantes do grupo submetido à discussão tradicional de casos clínicos. A retenção do conhecimento, no curto e longo prazo, bem como a transferência do conhecimento foi semelhante entre os dois grupos, mostrando que o jogo educacional, nesse contexto, não é inferior ao método tradicionalmente utilizado. Due to its importance for the training of doctors, there has been considerable effort for methodological changes in the last decades, with a view to develop a new teaching-learning process, student-centred and supported by the teacher as a facilitator and mediator in the process. In this context, educators have been seeking after alternatives for new practices. Among them, gamification stands out, which consists in the implementation of fundamental characteristics of games, within an educational setting, which are capable of positively influencing students' motivation, attention, understanding, memorisation, and metacognition, rendering meaningful learning outcomes. Objectives : To evaluate the effects of the inclusion of gamification in laboratory lessons of Human Anatomy in relation to the motivation of medical students in addition to short and long-term memorisation. Methodology: a transversal experimental research with 41 subjects in their first term as medical school students at UNIFENAS-BH, in the first semester of 2019, who were taking the Human Anatomy course. The participants attended a mini class, being subsequently and randomly divided into two groups, one of which proceeded with a traditional laboratory class whereas the other participated in a gamified laboratory class. A card-based game model was used; it was a specific educational game developed to be used for the purpose of this work (ANATOCARD). During the intervention, students were evaluated by doing pre-tests (pre-test, post-test 1, post-test 2 and post-test 3) and by completing an IMMS questionnaire to assess their motivation. Seven days after doing post-test 2, students did post-test 3, without previous notice. Conclusion: An educational game was developed - a gamified educational object - which was named ANATOCARD. The game proved to be useful, making it possible to reach the goals of this research, in addition to being reproducible and inexpensive, which easing its diffusion. In the group where gamification was added, compared to the control group, we observed a higher, statistically significant, motivation level, which was noticeable in all of its dimensions (Attention, Relevance and Satisfaction), not only Trust. Regarding proficiency, results show an adequate correspondence among the effects of gamification, fully comparable to those of the traditional methods of teaching Anatomy. Furthermore, students in the gamification-group showed greater knowledge retention in post-test 3, with statistical significance. Therefore, the results of this work make it possible to conclude using this gamified educational object as a valid alternative for teaching Human Anatomy to medical school students Introdução: O ensino da anatomia humana nos primeiros semestres dos cursos de medicina é desafiador e, eventualmente, desmotivador, dada sua alta carga teórico-prática e seu distanciamento da prática clínica. Devido à sua importância para a formação do médico, nas últimas décadas houve esforço considerável para mudanças metodológicas, objetivando desenvolver um novo processo de ensino-aprendizagem centrado no aluno e apoiado no professor como facilitador e mediador deste processo. Neste contexto, educadores têm buscado alternativas de novas práticas. Dentre elas destaca-se a gamificação, que consiste na aplicação de características elementares de jogos, em cenário educacional, capazes de influenciar positivamente a motivação, a atenção, a compreensão, a memorização e a metacognição dos estudantes, traduzindo-se em aprendizagem significativa. Objetivos: avaliar os efeitos da adição da gamificação em aulas laboratoriais de anatomia humana em relação à motivação de estudantes de medicina e memorização em curto e longo prazo. Metodologia: pesquisa experimental transversal aplicada a 41 alunos do primeiro período do curso de medicina da UNIFENAS-BH, no primeiro semestre de 2019, cursando a disciplina de anatomia humana. Os participantes assistiram uma miniaula, sendo posteriormente e aleatoriamente divididos em dois grupos, um dos quais seguiu com a aula laboratorial tradicional e o outro participou da adição de gamificação (aula laboratorial gamificada). O modelo de jogo utilizado foi de cartas, tendo sido desenvolvido um jogo educacional específico para ser utilizado neste trabalho (ANATOCARD). Durante a aplicação da intervenção os alunos foram avaliados por pré-testes (pré-teste, pós-teste1, pós-teste 2 e pós-teste 3) e pela aplicação de questionário IMMS para avaliação de motivação. Após sete dias da aplicação do pós-teste 2, os alunos foram submetidos à aplicação do pós-teste 3, sem aviso prévio. Conclusão: desenvolveu-se um jogo educacional, um objeto educacional gamificado, que foi denominado ANATOCARD. O jogo mostrou-se útil, permitindo atingir os objetivos desta pesquisa, além de ser reprodutível e de baixo custo, o que facilita sua disseminação. No grupo onde foi feita a adição de gamificação, comparativamente com o grupo controle, observamos motivação mais elevada, estatisticamente significativa, que se estendeu a todas as suas dimensões (atenção, relevância e satisfação), não alcançando apenas a confiança. Em relação à proficiência, encontramos resultados mostrando adequada correspondência entre os efeitos da gamificação aplicada, plenamente comparáveis aos do método tradicional de ensino de anatomia. Adicionalmente os alunos do grupo jogo mostraram maior retenção de conhecimentos no pós-teste 3, com relevância estatística. Isto nos permite concluir pela possibilidade de usar este objeto educacional gamificado como alternativa válida para o ensino da anatomia humana para estudantes de medicina

The popularization of access to health information, as well as the strengthening of person-centered medicine, has the potential to influence the doctor - patient relationship, as the doctor is no longer the sole holder of knowledge and needs to share “power” with the patient in their guidelines and conduct. Objective: To evaluate the perception of patients’ use of the internet to obtain information about their illness and the association with the Power-sharing attitude in the doctor-patient relationship between medical students and their teachers. Methods: Quantitative, observational, cross-sectional study, which included medical students from 3 ͬ ͩ and 6 ͭ ͪ year of UNIFENAS – BH and their respective teachers of outpatient practice. The outcomes of interest were assessed by means of a questionnaire adapted to assess people`s perception of internet use to obtain information about their illness and the patientpractitioner orientations scale (PPOS) power subscale. Results: 215 students participated in the study, 60% female, with a mean age of 24±3,1 yars, and 30 teachers, 50% female, with a mean age of 42±7,8 years. Abstract: Alzheimer''s dementia (AD) is a degenerative brain disorder characterized mainly by cholinergic failure, but other neuro-transmitters are also deficient especially at late stages of the disease. Misfolded β-amyloid peptide has been identified as a causative agent, however inflammatory changes also play a pivotal role. Even though the most prominent pathology is seen in the cognitive functions, specific abnormalities of the central nervous system (CNS) are also reflected in the periphery, particularly in the immune responses of the body. The aim of this study was to characterize the dopaminergic and serotonergic systems in AD, which are also markedly disrupted along with the hallmark acetyl-choline dysfunction. Peripheral blood mono-nuclear cells (PBMCs) from demented patients were judged against comparison groups including individuals with late-onset depression (LOD), as well as non-demented and non-depressed subjects. Cellular sub-populations were evaluated by mono-clonal antibodies against various cell surface receptors: CD4/CD8 (T-lymphocytes), CD19 (B-lymphocytes), CD14 (monocytes), and CD56 (natural-killer (NK)-cells). The expressions of dopamine D3 and D4, as well as serotonin 5-HT1A, 5-HT2A, 5-HT2B and 5-HT2C were also assessed. There were no significant differences among the study groups with respect to the frequency of the cellular sub-types, however a unique profound increase in 5-HT2C receptor exclusively in NK-cells was observed in AD. The disease-specific expression of 5-HT2C, as well as the NK-cell cyto-toxicity, has been linked with cognitive derangement in dementia. These changes not only corroborate the existence of bi-directional communication between the immune system and the CNS, but also elucidate the role of inflammatory activity in AD pathology, and may serve as potential biomarkers for less invasive and early diagnostic purposes as well. Not only is it one of the most mentioned humanistic dimensions in Medicine, but it is also associated with better diagnosis and therapeutic results in a clinical context. Due to its importance in the context of the physician-patient relationship, the behavior of the students’ empathy levels during the undergraduate years has become a research object in Medical Education. This study is an effort to evaluate the empathy of Medicine students at UNIFENAS BH (a private medical college) over the undergraduate years by using the Jefferson scale, already validated for the Portuguese language. Methods: Cross-sectional study held at UNIFENAS BH, with the application of the social-demographic questionnaire and the Jefferson empathy scale, already validated in Brazil and designed to measure the Medicine students’ empathy level with respect to patients. The sample consisted of 247 regular students of the even-numbered undergraduate semesters. Results: The analysis of the results shows, comprehensively, that there was not a statistically significant difference in empathy levels among the academic terms studied over the undergraduate years, with respect to gender and future specialty. Exceptions were observed only in the fourth academic semester, in connection with the variable “gender” with respect to the scores: compassion, ability to put oneself in somebody else’s place and general score, in which the scores for the female gender showed themselves higher when compared to those of the male gender. The data obtained may reflect the result of various educational strategies that have been implemented at the college, aimed at the development of a person-centered care, in addition to teaching and research activities developed over the undergraduate and graduate years. Conclusions: No statistical differences were observed, in the analysis of this sample of students, in empathy levels among students of different academic terms, with respect to the variables “gender” and “future specialty” over the undergraduate years at UNIFENAS BH. Medical colleges should include the topic of empathy in their curricula, to establish the teaching of that important socioemotional skill A empatia é uma competência que permite perceber e entender o sentimento e a perspectiva do outro. Além de ser uma das dimensões humanísticas mais mencionadas na medicina, a empatia também está associada a melhores resultados diagnósticos e terapêuticos no contexto clínico. Em função de sua importância no contexto da relação médico-paciente, o nível de empatia do estudante de medicina durante o curso de graduação passou a ser alvo de pesquisa em educação médica. O estudo avaliou a empatia dos alunos do curso de medicina da UNIFENAS-BH ao longo de toda a graduação através da aplicação da escala de empatia de Jefferson já validada para a língua portuguesa. A amostra foi composta de 247 alunos regulares dos períodos pares da graduação. Resultados: A análise dos resultados mostrou de forma abrangente que não houve diferença estatística significativa nos níveis de empatia entre os períodos estudados, ao longo da graduação, em relação ao gênero e área de atuação futura. As exceções observadas foram verificadas apenas no 4º período, relacionadas à variável gênero em relação aos escores: compaixão, capacidade de colocar-se no lugar do outro e escore geral, onde os escores do sexo feminino se mostraram mais elevados quando comparados ao masculino. Os dados obtidos podem refletir o resultado de diversas estratégias educacionais que vem sendo implementadas na escola, com o objetivo do desenvolvimento da prática assistencial centrada na pessoa, além de atividades de ensino e pesquisa desenvolvidas na graduação e pós-graduação. Conclusões: Nesta amostra de estudantes avaliados não houve diferença estatística nos níveis de empatia entre os estudantes dos diferentes períodos em relação ao sexo e área de atuação futura ao longo da graduação no curso de Medicina – UNIFENAS BH. As escolas de medicina devem incluir o tema da empatia em seus currículos de modo a formalizar o ensino desta importante habilidade socioemocional

On the other hand, structured oral examinations (SOE) have much simpler application and lower cost. Nonetheless, SOE may have lower validity and reliability. Objective: To compare Medical students’ academic performance and perception in regard of assessing skills through OSCE and SOE. Method: Two tests were designed (OSCE and SOE) to evaluate five obstetrical skills in a mannequin (the first three Leopold maneuvers, fundal height measurement, and fetal heart rate auscultation). The SOE assessed skills separately and the OSCE evaluated the skills after the analysis of a contextualized case. Students of the 4th term of Medical School were distributed into two groups. In the first phase of the study, Group 1 performed SOE, and Group 2, OSCE. Three weeks later, in the second phase, the same tests were applied inversely. Tests were applied by the same teacher, who employed the same checklist in both tests. In the second phase, a survey about students’ perception towards both test types was also applied. The mean score in each question was compared, as well as the final score, concerning the following crossings: test type in each of the two phases; test type regardless of phase; intragroup OSCE and SOE, and scores of the first and second stages, regardless of test type, as well as between groups, regardless of phase. Students’ perception was analyzed according to frequency distribution and grouping of open-ended answers by similarity. Results: 21 students participated in the study: 13 in Group 1, and 8 in Group 2. No difference was found in all the other crossings, except for the scores between phases. As for the intragroup comparison, the final score and the question 2 (fundal height measurement) score were superior in the second phase in both groups. The comparison between mean scores of the first and second phases, regardless of test type, demonstrated that the scores of the second phase were superior in the final score and in all questions, except for the second and third Leopold maneuvers. The scores’ improvement on the second day may be related to the testing effect. Conclusion: the test type did not influence students’ performance. Most candidates preferred the OSCE type. Apesar do exame clínico objetivo estruturado (OSCE) ser considerado padrão ouro para avaliação de habilidades clínicas, ele é uma prova de organização complexa e de alto custo. Por outro lado, as provas do tipo prática-oral estruturada (POE) são de aplicação mais simples e de menor custo, ainda que possam apresentar menor validade e confiabilidade. Objetivo: comparar o desempenho acadêmico e a percepção de alunos de Medicina na avaliação de habilidades por OSCE e POE. Método: foram elaboradas duas provas (OSCE e POE) para avaliação de cinco habilidades obstétricas em manequim (três primeiras manobras de Leopold, medida de útero-fita e ausculta de batimentos cardíacos fetais). A POE avaliou as habilidades isoladamente e o OSCE avaliou as habilidades após a análise de um caso clínico contextualizado. Estudantes do 4º período do curso de Medicina foram distribuídos em dois grupos. Na primeira fase, o Grupo 1 realizou a POE e o Grupo 2 o OSCE. Na segunda fase, 3 semanas após, aplicou-se novamente as mesmas provas, de modo invertido. As provas foram aplicadas por um único avaliador, que utilizou o mesmo checklist nas duas provas. Na segunda fase, aplicou-se também um questionário sobre a percepção dos alunos em relação aos dois tipos de prova. Comparou-se a nota média em cada questão e a nota total nos seguintes cruzamentos: tipo de prova em cada uma das fases; tipo de prova independentemente da fase; OSCE e POE intragrupo e notas da primeira e da segunda fases, independentemente do tipo de prova, bem como entre os grupos, independentemente da fase. A percepção do aluno foi analisada por distribuição de frequência e agrupamento das respostas abertas por semelhança. Resultados: 21 alunos participaram do estudo, sendo 13 do Grupo 1 e oito do Grupo 2. Não houve diferença entre as notas das questões e a nota total, entre os dois tipos de prova, nas duas fases do estudo. Também não se observou diferenças em todos os outros cruzamentos, exceto quando se comparou as notas entre as fases do estudo. Na comparação intragrupo, a nota total e a nota da questão 2 (útero-fita) foi superior na segunda fase nos dois grupos. A comparação entre as médias das notas na primeira fase e na segunda fase, independentemente do tipo de prova, demonstrou que as notas da segunda fase foram superiores na nota total e em todas as questões, exceto na 2ª e 3ª manobras de Leopold. A melhora das notas, no segundo dia, pode estar relacionada ao efeito teste. Conclusão: o tipo de prova não influenciou o desempenho do aluno. A maioria dos alunos preferiu a prova tipo OSCE.

43. [Trachoma: epidemiologic study in students in Alagoas State--Brazil].

- Abstract
Purpose: To define the detection rate of trachoma in scholars from Alagoas--Brazil., Methods: The sample included 6,424 children and adolescents. Scholar's clinical evaluation was performed by medical students under the supervision of professors of the Department of Ophthalmology, according to the World Health Organization guidelines. The clinical evaluation was characterized by eyelashes, eyelids, conjunctiva and cornea examination in both eyes. The scholars were cataloged and data of name, sex, age, clinical forms diagnosis and city were recorded. Regarding the age, the students were divided into three groups: group A (<10 years old), group B (10-14 years old) and group C (>14 years old)., Results: The detection rate of trachoma was 4.5%. From 3,280 male scholars, 161 (4.9%) cases were confirmed; from 3,144 female scholars, 131 (4.2%) cases were considered confirmed. From the total scholars in group A, B and C; respectively 175 (5.3%), 113 (8.0%) and 4 (1.6%) were considered confirmed cases., Conclusion: In this region, it seems that trachoma has not been eradicated and, therefore, must remain as a differential diagnosis of chronic follicular conjunctivitis in children and adolescents from this region.
