Spatial data are key to fishery management; however, most often the spatial distribution of marine populations and fishing dynamics are poorly documented, especially for recreational fish species. The combination of fisheries-dependent data (FDD) obtained from logbooks, and local ecological knowledge (LEK) gathered from key stakeholders could be a powerful approach to inform data gaps in data-limited fisheries. In this study, we used both FDD from guides' catch reports and LEK using an online survey and key-informant interviews to reconstruct the spatial changes in bonefish (Albula vulpes) catch and fishing effort throughout South Florida over the past 35-40 years, and better understand the extent and spatial patterns of the bonefish decline described in previous studies. Although anglers perceived a decline of bonefish numbers across all fishing areas (26 to 53% drop across Biscayne Bay, the Florida Keys, and Florida Bay), the start of the bonefish decline in Florida Bay resulted in the highest drop in bonefish number (53%); thus, indicating both regional and localized decline events affecting bonefish abundance. Within Florida Bay, LEK and FDD concurred with an initial drop in bonefish at Inner Bay, followed by a greater magnitude of decline at Outer Bay. Metrics of effort derived from the survey and interviews depicted a shrinkage and aggregation in the spatial distribution of fishing and a shift of fishing activities toward the Lower Keys. In sum, the spatiotemporal patterns of catch and effort obtained from LEK and FDD allowed us to understand where, when and how this data-limited species declined in South Florida. Pharmaceuticals have been acknowledged as an important contaminant of emerging concern with the potential to cause adverse effects in exposed fauna. Most research has focused on temperate freshwater systems; therefore, there is a pressing need to quantify pharmaceutical exposure in subtropical coastal marine systems. This study investigated the prevalence of pharmaceutical exposure to bonefish (Albula vulpes) in subtropical South Florida, USA, and evaluated the relative risk of detected concentrations to elicit pharmacological effects. The influence of sampling region, season (within or outside spawning season), and bonefish length on pharmaceutical assemblage, detection frequency, and risk was assessed. Both spatial (multiple regions) and temporal (spawning season) components were considered in order to incorporate bonefish biology biological in our exploration of pharmaceutical exposure and potential risk of effect. To quantify risk of pharmacological effects, concentrations were compared to a 1/3 threshold of the human therapeutic plasma concentration (H T PC). In total, 53 different pharmaceuticals were detected with an average of 7.1 pharmaceuticals per bonefish and 52.3 % had at least one pharmaceutical exceeding the 1/3 H T PC threshold. The presence of pharmaceutical cocktails at concentrations capable of eliciting pharmacological effects is of particular concern considering the potential for unknown interactions. For exposure and risk of pharmacological effect, region and season were significant, while bonefish length was not. Pharmaceutical exposure and risk were highest in the most remote sampling region. To date, the presence of pharmaceuticals has been extensively documented across a wide range of aquatic systems and biota. Further, substantial progress has been made in transitioning from laboratory assessments of pharmaceutical fate and effects in fish to in situ assessments of exposure and effects; however, certain research areas remain understudied. Among these is investigation of differential accumulation across multiple internal tissues in wild marine fish beyond the species commonly sampled in laboratory and freshwater field settings. This study examined the presence of pharmaceuticals across four tissues (plasma, muscle, brain, and liver) in a wild marine fish, bonefish (Albula vulpes), throughout coastal South Florida, USA. Differential accumulation across tissues was assessed for the number and concentration, identity, and composition of accumulated pharmaceuticals by sampling 25 bonefish and analyzing them for 91 pharmaceuticals. The concentration of pharmaceuticals was highest in plasma > liver > brain > muscle, while the number of pharmaceuticals was highest in liver > brain > plasma > muscle. The identity of detected pharmaceuticals was tissue specific, and there was an inverse relationship between the number of detections for each pharmaceutical and its log K ow . The composition of pharmaceuticals was tissue specific for both pharmaceutical presence/absence and concentration. Across all tissues, the greatest similarity was between brain and liver, which were more similar to plasma than to muscle, and muscle was the most distinct tissue. For tissue compositional variability, muscle was the most diverse in accumulated pharmaceuticals, while plasma, brain, and liver were similarly variable. Pharmaceutical uptake involves processes that vary across aquatic systems and biota. However, single studies examining multiple environmental compartments, microhabitats, biota, and exposure pathways in mesoconsumer fish are sparse. We investigated the pharmaceutical burden in bonefish (Albula vulpes), pathways of exposure, and estimated exposure to a human daily dose. To evaluate exposure pathways, the number and composition of pharmaceuticals across compartments and the bioconcentration in prey and bonefish were assessed. To evaluate bioaccumulation, we proposed the use of a field-derived bioaccumulation factor (fBAF), due to variability inherent to natural systems. Exposure to a human daily dose was based on bonefish daily energetic requirements and consumption rates using pharmaceutical concentrations in prey. Pharmaceutical number and concentration were highest in prey, followed by bonefish, water and sediment. Fifteen pharmaceuticals were detected in common among bonefish, prey, and water; all of which bioconcentrated in prey and bonefish, and four bioaccumulated in bonefish. The composition of detected pharmaceuticals was compartment specific, and prey were most similar to bonefish. Bonefish were exposed to a maximum of 1.2 % of a human daily dose via prey consumption. Most research on pharmaceutical presence in the environment to date has focused on smaller scale assessments of freshwater and riverine systems, relying mainly on assays of water samples, while studies in marine ecosystems and of exposed biota are sparse. This study investigated the pharmaceutical burden in bonefish (Albula vulpes), an important recreational and artisanal fishery, to quantify pharmaceutical exposure throughout the Caribbean Basin. We sampled 74 bonefish from five regions, and analyzed them for 102 pharmaceuticals. We assessed the influence of sampling region on the number of pharmaceuticals, pharmaceutical assemblage, and risk of pharmacological effects. To evaluate the risk of pharmacological effects at the scale of the individual, we proposed a metric based on the human therapeutic plasma concentration (H T PC), comparing measured concentrations to a threshold of 1/3 the H T PC for each pharmaceutical. Every bonefish had at least one pharmaceutical, with an average of 4.9 and a maximum of 16 pharmaceuticals in one individual. At least one pharmaceutical was detected in exceedance of the 1/3 H T PC threshold in 39% of bonefish, with an average of 0.6 and a maximum of 11 pharmaceuticals exceeding in a Key West individual. The number of pharmaceuticals (49 detected in total) differed across regions, but the risk of pharmacological effects did not (23 pharmaceuticals exceeded the 1/3 H T PC threshold). The most common pharmaceuticals were venlafaxine (43 bonefish), atenolol (36), naloxone (27), codeine (27), and trimethoprim (24). Findings suggest that pharmaceutical detections and concentration may be independent, emphasizing the need to monitor risk to biota regardless of exposure diversity, and to focus on risk quantified at the individual level. Previous modelling of the haploid-to-diploid ratio (H:D) in biphasic life cycles relied on estimates of the stable population growth rate and structure. This is a projective analysis that estimates the population dynamics given current conditions. However, the environment is rarely constant and has both periodicity and random instabilities. The objective of this work was to unveil how the H:D responds to them. It was found that ploidy phase dissimilarities on the demographic matrix and/or in the initial population structure cause an inevitable H:D time variability as a consequence of the life-cycle structure and independent of the environmental seasonal cycle. This variability depends on the type of life strategy, demographic processes involved and ploidy dissimilar vital rates. Furthermore, ploidy dissimilar fertility or growth rates cause cyclic oscillations mismatching the seasonal cycle, whereas ploidy dissimilarities in the ramet looping rates (survival related) induce a monotonical variation.

Primary hyperparathyroidism is an endocrine disorder with variable clinical expression, frequently presenting as asymptomatic hypercalcemia in Western countries but still predominantly as a symptomatic disease in developing countries. The objective of this retrospective study was to describe the diagnostic presentation profile, parathyroidectomy indication and post-surgical bone mineral density follow-up of patients with primary hyperparathyroidism seen at a university hospital. We found 115 patients (92 women, median age 56 years) with primary hyperparathyroidism diagnosed during the last 20 years. We defined symptomatic patients based on the presence of any classical symptom affecting bone, kidney or the neuromuscular system. Surgical criteria followed the guidelines of the National Institutes of Health regarding asymptomatic primary hyperparathyroidism. Symptomatic patients and patients meeting surgical criteria for parathyroidectomy were 66 and 93% of the sample, respectively. Median calcium and parathyroid hormone values were 11.9 mg/dL and 189 pg/mL, respectively. After surgical treatment, 97% of patients were cured, with increases in bone mineral density of 19.4% in the lumbar spine and 15.7% in the femoral neck 3 years after surgery. Greater bone mass increases were detected in pre-menopausal women, men, and in symptomatic and younger patients, both in the lumbar spine and femoral neck. Our results support the previous findings of a predominantly symptomatic disease with a presentation profile that could be mainly related to a delayed diagnosis. Nevertheless, genetic and racial backgrounds, and nutritional factors such as calcium and vitamin D deficiency may play a role in the clinical presentation of primary hyperparathyroidism of Brazilian patients.

In contrast to most developed countries, most patients with primary hyperparathyroidism in Brazil are still symptomatic at diagnosis. However, we have been observing a change in this pattern, especially in the last few years. We evaluated 104 patients, 77 females and 27 males aged 11-79 years (mean: 54.4 years), diagnosed between 1985 and 2002 at a University Hospital. Diagnosis was made on the basis of clinical findings and of high total and/or ionized calcium levels, high or inappropriate levels of intact parathyroid hormone and of surgical findings in 80 patients. Patients were divided into three groups, i.e., patients diagnosed from 1985 to 1989, patients diagnosed from 1990 to 1994, and patients diagnosed from 1995 to 2002. The number of new cases diagnosed/year increased from 1.8/year in the first group to 6.0/year in the second group and 8.1/year in the third group. The first group comprised 9 patients (mean serum calcium +/- SD, 13.6 +/- 1.6 mg/dl), 8 of them (88.8%) defined as symptomatic. The second group comprised 30 patients (mean calcium +/- SD, 12.2 +/- 1.63 mg/dl), 22 of them defined as symptomatic (73.3%). The third group contained 65 patients (mean calcium 11.7 +/- 1.1 mg/dl), 34 of them symptomatic (52.3%). Patients from the first group tended to be younger (mean +/- SD, 43.0 +/- 15 vs 55.1 +/- 14.4 and 55.7 +/- 17.3 years, respectively) and their mean serum calcium was significantly higher (P < 0.05). All of symptomatic patients independent of group had higher serum calcium levels (12.4 +/- 1.53 mg/dl, N = 64) than asymptomatic patients (11.4 +/- 1.0 mg/dl, N = 40). Our data showed an increase in the percentage of asymptomatic patients over the years in the number of primary hyperparathyroidism cases diagnosed. This finding may be due to an increased availability of diagnostic methods and/or to an increased awareness about the disease.

Intraoperative parathyroid hormone (IO-PTH) measurements have been proposed to improve operative success rates in primary, secondary and tertiary hyperparathyroidism (PHP, SHP and THP). Thirty-one patients requiring parathyroidectomy were evaluated retrospectively from June 2000 to January 2002. Sixteen had PHP, 7 SHP and 8 THP. Serum samples were taken at times 0 (before resection), 10, 20 and 30 min after resection of each abnormal parathyroid gland. Samples from 28 patients were frozen at -70 C for subsequent tests, whereas samples from three patients were tested while surgery was being performed. IO-PTH was measured using the Elecsys immunochemiluminometric assay (Roche, Mannheim, Germany). The time necessary to perform the assay was 9 min. All samples had a second measurement taken by a conventional immunofluorimetric method. We considered as cured patients who presented normocalcemia in PHP and THP, and normal levels of PTH in SHP one month after surgery and who remained in this condition throughout the follow-up of 1 to 20 months. When rapid PTH assay was compared with a routine immunofluorimetric assay, excellent correlation was observed (r = 0.959, P < 0.0001). IO-PTH measurement showed a rapid average decline of 78.8% in PTH 10 min after adenoma resection in PHP and all patients were cured. SHP patients had an average IO-PTH decrease of 89% 30 min after total parathyroidectomy and cure was observed in 85.7%. THP showed an average IO-PTH decrease of 91.9%, and cure was obtained in 87.5% of patients. IO-PTH can be a useful tool that might improve the rate of successful treatment of PHP, SHP and THP.

Context: Paranasal sinus cancer is considered rare, with an incidence of less than 1 per 100,000 per year, with the frontal sinus being the primary site in only 0.3%. We report a case of adenocarcinoma arising in the frontal sinus., Design: Case report., Case Report: A 59-year-old woman, secretary, came in February 1998 with a 4-month history of low intensity frontal headache. She denied contact with wood dust. On examination a non-tender swelling was noted over her right forehead next to the medial aspect of the right orbit. CT scan showed a soft-tissue mass involving frontal sinus with intracranial invasion through the posterior wall. The anterior ethmoid sinus and the medial aspect of the right orbit were also involved. MRI demonstrated dural thickening in communication with the frontal mass. She underwent an en-bloc tumor resection by craniotomy including orbital clearance. Histology revealed an adenocarcinoma. After surgery she had tumor recurrence, and chemotherapy and radiotherapy were started resulting in partial improvement.

In the present investigation, we studied the influence of co-dergocrine mesylate (dihydroergotoxin mesylate) on platelet aggregation induced by adrenaline, adenosine diphosphate (ADP) and collagen in vitro. Platelet-rich plasma was incubated with increasing concentrations of co-dergocrine (1.5, 3.0, 6.0, 12.0, 24.0, and 48.0 micrograms/ml). The lowest concentration of co-dergocrine tested (1.5 micrograms/ml) inhibited adrenaline-induced aggregation by 97%. Higher concentrations of the compound caused similar degrees of inhibition. ADP-induced aggregation was only decreased significantly (20%, P less than 0.001) by the highest concentration of co-dergocrine, whereas collagen-induced aggregation was not affected. Our results demonstrate that co-dergocrine selectively inhibits platelet aggregation triggered by adrenaline in vitro.
