Women's pregnancy increases the odds of developing anxiety, depression, and low self-esteem. Physical activity can reduce the severity of maternal mental disorders and postpartum symptoms. The aim was to evaluate self-esteem, anxiety, and physical activity patterns among healthy pregnant women during three gestational trimesters (GTs). A longitudinal prospective study involving 248 Portuguese pregnant women, monitored for a year during all GTs, was conducted. Self-reported questionnaires were used to collect personal and obstetric data, and Rosenberg's Self-Esteem Scale, Zung Anxiety Scale, and a Pregnancy Physical Activity Questionnaire were applied. A total of 152 women presented valid follow-up data considered for analysis. All test scores do not present normality (p < 0.001). Over the gestational trimesters, the Zung anxiety scores did not show a significant change between GTs. However, the self-esteem values reduced significantly from 1st to 2nd GT: (1st GT = 43.94 (±9.94) [95%CI: 42.37 to 45.53]; 2nd GT = 27.27 (±9.64) [95%CI: 25.74 to 28.81]; 3rd GT = 25.71 (±10.52) [95%CI: 24.04 to 27.39]; p < 0.001), remaining stable in the last GT. Moderate to vigorous (2nd GT = 73.43 (±88.69) [95%CI: 59.34 to 87.54]; 3rd GT = 65.05 (±82.65) [95%CI: 51.92 to 78.19]; p = 0.008), vigorous (1st GT = 0.45 (±3.95) [95%CI: 0.83 to 2.08]; 2nd GT = 1.63 (±3.52) [95%CI: 1.07 to 2.19]; 3rd GT = 0.81 (±2.16) [95%CI: 0.46 to 1.15]; p = 0.0475), and light (2nd GT = 101.68 (±48.77) [95%CI: 93.93 to 109.44]; 3rd GT = 98.42 (±49.05) [95%CI: 90.62 to 106.22]; p < 0.001) physical activity levels were significantly reduced throughout the GTs, but the time spent in sedentary activities did not change. In this follow-up study, it was possible to identify a critical point in the 2nd GT, when the drastic reduction in physical activity levels coincided with periods of lower self-esteem scores. - Abstract
[Abstract] Background: Obesity is common in women of reproductive age and increases the risk during pregnancy. Exercising during this period reduces health complications. Home e-health programs are effective in overcoming exercise barriers as pregnant women use technology and the internet for health information. Methods: A single-blind randomized controlled feasibility study with pregnant women with obesity (body mass index [BMI] ≥30 kg/m2) was conducted in the University Hospital Center of São João between January and April 2023. Pregnant women were randomized to a control group with standard care and to an experimental group with 8-week remote exercise program using a Phoenix® biofeedback device. Feasibility outcome measures were recruitment rate (≥35%), loss to follow-up (≤15%), and program fidelity (≥1 session/week). Secondary outcomes were evaluated through Pregnancy Physical Activity Questionnaire, Oswestry Index on Disability, and weight assessments at baseline and at the end of the program. Results: Of the 63 eligible participants, 24 (38.1%) were successfully randomized and completed the baseline assessment. Of these, 3 (4.8%) from experimental group did not perform the initial onboarding. The control group had 8.3% of follow-up losses and for the experimental group there were no follow-up losses. Program fidelity (mean ≥1 session/week) was fulfilled by 66.7% of successfully randomized participants. Regarding secondary outcomes assessed between baseline and the 8th week, experimental group compared to control group had higher levels of physical activity for sports activities, a lower level of inactivity, and lower disability rates caused by low back pain. Conclusions: Based on the recruitment rate, losses to follow-up, and fidelity rate, the GROB (obesity in pregnancy) study was deemed feasible and worthy of consideration for a larger study. There is evidence that the quality of life and mental health of pregnant women change during pregnancy. To evaluate the impact of physical activity on the quality of life and mental health of pregnant women with obesity or overweight, a systematic review was performed using six electronic databases (PubMed, Cochrane (CENTRAL), ScienceDirect, Scielo, BVS and PEDro). In total, 205 articles were collected, and after screening in accordance with the PRISMA declaration, six randomized clinical trials were selected. Methodological quality was assessed using the Cochrane RoB2 tool and a narrative synthesis of the results was performed. Physical activity interventions did not demonstrate statistically significant results on the quality of life and mental health of pregnant women with obesity or overweight. The effects of physical activity during pregnancy for women with obesity or overweight are varied due to the diversity of interventions implemented. Nonetheless, a discernible positive association emerges between stringent adherence to the prescribed physical activity regimen and enhanced physical well-being, weight management, and heightened aerobic capacity. In order to ascertain more definitive conclusions, rigorous clinical trials are needed that take into account the heterogeneity of interventions and ensure adequate adherence to the protocol. To assess the effectiveness of exercise interventions during pregnancy in managing gestational weight gain (GWG), excessive GWG, gestational diabetes (GD), hypertensive disorders, 2-h post–oral glucose tolerance test (OGTT), and birth weight in pregnant women with overweight/obesity (OW/OB). The search strategy was conducted in five electronic databases, restricting to articles published within the past 10 years. Randomized controlled trials comparing exercise intervention with usual prenatal care in pregnant women with OW/OB were considered. Two reviewers extracted data and assessed the risk of bias using version 2 of the Cochrane risk-of-bias tool for randomized trials and the quality of studies using Grading of Recommendations Assessment, Development and Evaluation classification. Continuous data were calculated as mean differences (MDs) and dichotomous data as risk ratios (RRs). Seven trials comprising 1648 pregnant women were included. Exercise interventions were associated with lower GWG (MD, −1.19 kg [95% confidence interval, CI, −1.79 to −0.60]) and lower incidence of GD (RR, 0.56 [95% CI, 0.40–0.78]). When analyzing excessive GWG, 2-h post-OGTT, birth weight, and hypertensive disorders, there was no statistically significant difference between the exercise and usual care groups. The strength of evidence was considered moderate. Exercise interventions during pregnancy in women with OW/OB were shown to influence GWG and the incidence of GD.

Ankylosing spondylitis (AS) is a chronic inflammatory rheumatic disease characterised by inflammation of the spine and the sacroiliac joints and stiffness, which leads to a progressive decline in the quality of life of individuals. Bath indices are considered the principal functional outcome measure and Erythrocyte Sedimentation Rate (ESR) and C-Reactive Protein (CRP) are the most widely used biomarkers in AS to assess disease activity. To characterise individuals with AS and to examine the association between functional parameters and biomarkers. Thirty-one patients were recruited from the National Association of Ankylosing Spondylitis (ANEA) – Centre of northern region of Portugal, between February 2014 and June 2015, to be included in the present study. The Bath indices [Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Bath Ankylosing Spondylitis Functional Index (BASFI) and Bath Ankylosing Spondylitis Metrology Index (BASMI)], the Neck Disability Index (NDI), the Oswestry Disability Index (ODI) and laboratory parameters such as ESR and CRP were analysed. The participants’ mean age was 42.77 (±14.37) years, with 18 (58.1%) women and 13 (41.9%) men. The mean scores were 3.55 (BASDAI), 2.78 (BASFI) and 2.58 (BASMI). The final mean scores were 25.81± 18.36 (NDI) and 23.29±15.32 (ODI). We found a negative association between ESR and BASDAI (β=-0.038, p=0.025), and between ESR and BASFI (β=-0.044, p=0.013). As to CRP, there was no statistically significant association between Bath indices. This cross-sectional study in AS patients demonstrated that, on the one hand, this sample had a high education level, having been diagnosed with AS for more than 15 years, with a moderate disability and, on the other hand, with the adjustment of education level and years since diagnosis, there was an association between ESR and BASDAI and BASFI scores.

Background: Low back pain (LBP) is an increasingly reported condition, and physical activity (PA) may play an important role. The aim of the present study was to evaluate the proportion of pregnancy-related LBP and its association with type and intensity level of PA during pregnancy. Methods: A longitudinal prospective study was carried out with a cohort of 118 pregnant women. Participants were evaluated in all trimesters. LBP was assessed with a self-reported questionnaire and participants were categorized according to its occurrence. The type and intensity of PA were evaluated using the Pregnancy Physical Activity Questionnaire and categorized into tertiles. Binary logistic regression models were constructed to verify the relationship between LBP and type, the intensity of PA in all trimesters, and LPB pre-pregnancy. Results: LBP was reported by 40.7%, 52.2% and 66.7% of the subjects in the first, second, and third trimesters, respectively. No significant associations were found between LBP and type and intensity of PA. However,women who had LBP before pregnancy, compared to those who did not, had higher odds of expressing LBP during pregnancy (OR= 3.85, 95% CI: 1.344-11.025). Conclusions: LBP is a common condition and increased during pregnancy. Results of this study suggest that type and intensity of PA are not associated with emerging of LBP during pregnancy. A lack of physical activity in computer workers (CW) can contribute to the development of musculoskeletal symptoms (MSS). Aim: (i) Evaluate MSS over a 12 month and 7 days period, (ii) determine physical activity (PA) levels and compliance with World Health Organization (WHO) PA recommendations, and (iii) assess the relationship between compliance with WHO PA recommendations and MSS. A cross-sectional observational study comprising 119 computer workers. The Nordic Musculoskeletal Questionnaire (NMQ) was used to evaluate the MSS and the International Physical Activity Questionnaire (IPAQ) was used to analyse the perception of the level of PA. MSS occurred in the lumbar region (56.3%), neck (43.7%), and shoulders (39.5%). 44.7% of the participants reported a low level of PA. The percentage of compliance was similar among women and men (62.2% vs. 58.5%, respectively). Negative correlations were found between pain intensity and metabolic equivalent values. The participants who followed the WHO PA recommendations reported a lower frequency of MSS compared to those who did not, but the differences were not statistically significant. It was possible to conclude that computer workers presented a higher frequency of MSS in the lower back, neck, and shoulders. Regarding the level of PA, participants were mostly classified as low. Participants who followed the WHO PA recommendations reported lower MSS than those who did not. This finding could be important in obtaining successful programs that promote health-oriented physical activity in this group of workers.

Nas últimas duas décadas, houve um aumento exponencial da atenção pelo tema “saúde”, já que as pessoas perceberam que quanto maior a quantidade e qualidade de informações se obtêm sobre isto, melhores são os resultados para a saúde pessoal e coletiva (Liu et al., 2020). O termo “literacia em saúde” (LS) é definido como o processo que ocorre quando uma sociedade fornece informações precisas de saúde e serviços que as pessoas podem facilmente encontrar, entender e usar para tomar suas decisões e ações trazendo benefícios para a saúde individual, pública e também para a sustentabilidade dos sistemas de saúde (Department of health and human services, 2019). O Comitê de Literacia em Saúde refere que os resultados de uma pesquisa nacional realizada nos Estados Unidos mostram que há uma enorme falta de habilidades de conhecimento em saúde necessárias para responder às solicitações da sociedade do século XXI e que essa escassez tem causado um impacto negativo significativo no setor da saúde, tendência também vista em outros países como Portugal, mostrando a importância da melhora da LS da população ser uma meta global, daí a importância de materiais como este (Institute of Medicine Committee on Health Literacy, 2004). A atitude perante a atividade física (AF) durante a gravidez mudou radicalmente nos últimos trinta anos (Charlesworth, 2011). Os nossos antepassados tratavam a mulher grávida como se estivesse doente, sendo aconselhada a descansar e a evitar exercícios (Duncombe et al., 2009). A evidência científica suporta cada vez mais a importância da AF para a saúde individual (Duncombe et al., 2009) e comunitária (Beaglehole et al., 2011). Da pesquisa efetuada, não foram encontrados estudos que mostrassem efeitos negativos da AF de moderada intensidade durante o período gravídico (Da Costa, 2003). Atualmente, existem recomendações para a prática da AF durante a gravidez, pelos benefícios demonstrados, tanto para a mulher como para o bebé (Physical activity guidelines for Americans, 2008). O American College of Obstetricians and Gynecologists (ACOG) em 2002, emanou orientações para que as mulheres grávidas, sem complicações obstétricas, adotem as mesmas recomendações de exercício físico que as das mulheres não grávidas. Da mesma forma, o Centers for Disease Control and Prevention (CDC) e o United States Department of Health and Human Services (USDHHS), 2009 também recomendam a AF durante a gravidez. [...]

A unidade curricular (UC) de Fisioterapia na Comunidade (ESS|P.Porto) tem como objetivo o desenvolvimento de competências necessárias à intervenção em promoção da saúde. Esta unidade curricular envolve uma forte articulação com a comunidade e a aprendizagem em contexto real, potenciando a transferência das competências adquiridas. O modelo pedagógico Problem-Based Learning favorece uma forte articulação entre as diferentes áreas técnico-científicas e confere ao estudante um papel central na aprendizagem. 30. Developing community partnerships for primary healthcare: An integrative review on management challenges.

Objectives: The aim of this review was to identify healthcare management barriers, challenges and obstacles in the development of health partnerships in community intervention projects. Methods: We conducted an integrative review using the following data sources: PubMed, B-on, Medline and EBSCOhost. The keywords in the search were: Health Partnerships AND Community Health AND Primary Health Care. We grouped results based on six categories: Environment; Membership; Process and structure; Purpose; Communication; Resources. Findings: The search conducted identified 844 articles. After being submitted to the first filter, and applying the exclusion and inclusion criteria, a total of 56 were reviewed. Of these, 44 dealt with aspects relating to Environment. In 53 articles it was referred that the relationship partners were key to the development of partnerships. Factors relating to the process of collaboration were found in 45, while factors related to structural elements were mentioned in 40 articles. 31 identified factors relating to purpose. Communication was highly discussed, appearing in 34 articles. Factors relating to resources were given great importance in the literature, appearing in 48. In 23, the subject of leadership. Conclusions: The literature reviewed highlighted that dimensions such as relationships, commitment, communication, funding, and structure are key in the long-term sustainability of the partnership. Objetivo – Determinar e analisar o perfil comportamental dos estudantes do primeiro ciclo do ensino básico sobre o uso da mochila escolar. Método – Estudo observacional, analítico e transversal. A amostra foi constituída por 88 estudantes do 1º ciclo do ensino básico no concelho de Vila Nova de Famalicão (Portugal). Foi aplicado um questionário para avaliar o comportamento dos estudantes relativamente ao tipo, modo de transporte, organização do material escolar e peso da mochila. Resultados – Todas as mochilas apresentavam duas alças e eram reguláveis. A maioria dos estudantes de todos os anos letivos transportava a mochila de alças nos dois ombros. No geral, os estudantes do 1º ciclo distribuíam o material de forma desorganizada pelos compartimentos da mochila e colocavam o material mais pesado afastado da coluna vertebral, não se verificando diferenças significativas. Background: Engaging in physical activity (PA) and reducing sedentary behaviour (SB) are important for health during pregnancy, yet relatively few studies have examined these variables using objective measures and across trimesters during pregnancy. Objective: To determine the amount of objectively assessed PA and SB engaged in whether there was any weekday to weekend day variation in PA and SB during the first and second trimester of pregnancy. Method: PA and SB were determined using accelerometry worn over 7 consecutive days during each trimester in 137 Portuguese females (mean age ± SD = 29.6 ± 5.7). Results: In regard to the proportion of participants meeting the ACSM guidelines for PA, 37.5% of the participants in the first trimester and 29.6% of participants in the second trimester met the cut off of 30min or more of any type of moderate intensity activity on most (5) days of the week. Moderate intensity PA was significantly lower in trimester 2 compared to trimester 1 (P = 0.003). Moderate intensity PA was also significantly lower during weekends compared to weekdays irrespective of trimester (P = 0.003). SB, light and vigorous intensity PA were relatively stable from trimester 1 to trimester 2 and between weekdays and weekends (P < 0.05). Conclusion: The present study suggests that the majority of women do not meet PA guidelines for health during pregnancy and that moderate intensity PA declines form trimester 1 to 2 and is lower at weekends.

Introduction - During pregnancy, women experience physiological and biomechanical changes, that may result in low back pain and restrictions in physical and functional activity. Aim - To identify the parameters of physical exercise in low back pain and functional incapacity of the pregnant woman without obstetrical complications. Method - Computerized research of studies published between January 2013 and 26 June 2018 in databases and search engines, such as PubMed, Science Direct, Google Scholar, CINAHL Complete, MEDLINE Complete, Web of Science, SciELO, Cochrane Library, and PEDro to identify randomized controlled articles that included the following terms: 'physical exercise' AND 'low back pain' AND 'functional disability' AND 'healthy pregnant' AND 'randomized controlled trial'. The analysis and selection of articles were carried out by two independent researchers, obtaining a consensus through a third blind investigator. Results - A total of 26,012 articles were identified of which were included five articles that presented the had as principal outcome back pain, from this five only three presented the secondary outcome analysis functional incapacity. The mean of the totality of analyzed articles was 6.2 in 10 points on the PEDro scale. In these studies, participated 817 pregnant women, between the ages of 20 and 40 and with gestational age between the 12th and the 35th week. Aerobic training, muscle strengthening, and relaxation were the most common types of exercise. Exercise parameters varied between studies in frequency (1 to 4 times/week), duration of the session (30 to 60 minutes/session), and time of intervention (4 to 12 weeks), with/without supervision and in-group/individual. After intervention with physical exercise, low back pain intensity was reduced in 4/5 studies and functional disability was reduced in 2/3 studies. Conclusions - The lack of studies did not allow the identification of the parameters of the exercise to reduce low back pain either functional incapacity in pregnant women. The aims of this paper are to examine the accomplishment of the recommendations of physical activity (PA) practice in accordance with the recommendations of the American College of Sports Medicine (ACSM); verify if the sociodemographic variables (SDV) influence women’s self-esteem; investigate if there are differences in pregnant women’s self-esteem who practiced or not structured physical activity (SPA) before pregnancy, and of pregnant women who met the recommendations or not during pregnancy. A sample of 44 pregnant women was evaluated in two stages: between the 10th and 13th week of pregnancy (1st trimester- 1T) and between the 20th and 23rd week of gestation (2nd trimester - 2T). The PA was assessed by accelerometry and self-esteem was assessed using the Rosenberg Self-Esteem Scale. Sociodemographic variables and SPA before pregnancy were assessed through a self-administered questionnaire. Over 60% of the sample did not meet recommendations for PA practice in any of the trimester. There were no significant differences between self-esteem for each of the SDV, with the exception of the higher monthly income (1T) and marital status (2T), associated with better self-esteem. There were significant differences in self-esteem among pregnant women or unstructured PA performed before pregnancy (1T and 2T). The completion, or not, of ACSM recommendations did not seem to impact on self-esteem during pregnancy. Most pregnant women did not met the ACSM recommendations for PA practice. It was found that marital status and higher incomes have influenced positively self-esteem, as well as the practice of SPA, before pregnancy.

39. Predictors of adherence to the Mediterranean diet from the first to the second trimester of pregnancy

Background: Although changes in eating patterns may occur during gestation, predictors of these changes have not been explored. This study aimed to identify predictors of adherence to the Mediterranean diet (MD) from the first to second trimester of pregnancy. Methods: A prospective study was conducted with 102 pregnant women aged 18-40, from the city of Porto, Portugal. Socio-demographic and lifestyle characteristics were assessed through a questionnaire. Food consumption was assessed with a three-day food diary completed during the first and second trimesters. Participants were categorized according to their change in adherence to the MD into the negative change group (i.e., women who had low adherence in each trimester or had high adherence in the first trimester and then low adherence in the second) and the positive change group (i.e., women who had high adherence in both trimesters or had low adherence in the first trimester and then high adherence in the second). Conditional stepwise logistic regression models were performed to assess the potential predictors of negative MD change. Results: Among the 102 women, 39.2% had negative change from the first to the second trimester. The logistic model's results show that being married (OR=0.26, 95%CI: 0.10, 0.76) and having a higher intake of vegetables in the first trimester (OR=0.17, 95%CI: 0.10, 0.43) were associated with lower odds of having a negative change in adherence to the MD from the first to second trimester. Conclusion: Marital status and vegetable consumption seem to be associated with a lower occurrence of negative change in adherence to the MD from early to middle pregnancy. Introducción: Aunque los cambios en los patrones pueden ocurrir durante la gestación, predictores de estos cambios no han sido exploradas. Este estudio pretende identificar predictores de la adhesión a la dieta mediterránea (DM) desde el primer al segundo trimestre del embarazo. Metodología: Se realizó un estudio prospectivo con 102 embarazadas de edad 18-40, de la ciudad de Oporto, Portugal. Se evaluaron las características socio-demográficas y de estilo de vida a través de un cuestionario. Consumo de alimentos se evaluó con un diario de alimentos de tres días concluido durante los primeras y segundo trimestres. Los participantes fueron categorizados según su cambio en la adhesión a la DM en el cambio negativo (es decir, las mujeres que tenían poca adhesión en cada trimestre o tenido alta adhesión en el primer trimestre y luego baja adhesión en el segundo) y el grupo de cambio positivo (es decir, las mujeres que tenían alta adhesión en ambos trimestres o tenía baja adhesión en el primer trimestre y luego elevada adhesión en el segundo). Modelos de regresión logística stepwise condicional se realizaron para evaluar los posibles predictores de la variación negativa de DM. Resultados: Entre las 102 mujeres, 39,2% tenían cambio negativo del primer al segundo trimestre. Los resultados model's logística muestran que estar casada (OR = 0,26, IC95%: 0,10, 0,76) y tener una mayor ingesta de verduras en el primer trimestre (OR = 0,17, IC95%: 0,10, 0,43) se asociaron con menor probabilidad de tener un cambio negativo en la adhesión a la DM del primer al segundo trimestre. Conclusiones: Estado civil y el consumo de vegetal parecen estar asociado con una menor ocurrencia de cambio negativo en la adhesión a la MD desde temprano a medio embarazo.

International guidelines recommend a first line therapy in the treatment of female stress urinary incontinence (SUI), the pelvic floor muscle (PFM) training. This case report assesses the effects of the PFM training program in treating women with severe SUI. The urodynamic parameters allow diagnosed intrinsic sphincter deficiency and urethral hypermobility. The subjective and objective parameters were assessed at the beginning and after six-month of PFM training program. This case report confirms the efficiency of the intensive training program in severe SUI. The medical implications of the PFM training as first treatment option reflect favourable individual results and additionally contribute to the selection of the non-invasive treatment, the reduction of the incidence collateral effects, low costs and that does not prevent future treatment options.

Purpose: The objective of this study was to analyze the influence of a retraining urogynecology program in women with the diagnosis of stress urinary incontinence, compared to the amount of urine leakage, strength of the pelvic floor muscles and subjective perception of improvement. Methodology: This case study with multiple subjects, constituted a sample group of 14 women, who underwent a retraining urogynecology program (Manual Reeducation, Electrical Stimulation and Biofeedback), twice a week for a total of twenty sessions, lasting three months. Before and after the program, perineometry was performed to assess the strength of maximum voluntary contraction of the pelvic muscles, and pad-test to quantify urinary leakage. The final questionnaire allowed to assess the subjective perception of improvement. Results: After implementation of the program in the sample group, a total of 14 women with a mean age of 51,6 years (sd = 5,43), was obtained a significantly increased by 26,95% of maximum voluntary contraction of the pelvic floor muscles (3,1cmH2O, p=0,004), significantly reduced the amount of urinary leakage of 60,81% (Pad Test/1 Hour – 4,5 grams, p=0,008) and 92,9% of women feel improved, almost healed or healed. The correlation between the increase of maximum voluntary contraction and decrease the quantity of urine leakage was not statistically significant (r = -0,378, p= 0,183). Conclusion: After the application of the retraining urogynecology program, women with stress urinary incontinence of the sample group presented statistically significant improvements on the maximal voluntary contraction force of the pelvic floor muscles, in the amount of urinary leakage and the subjective perception of improvement.

43. Relationship of milk intake and physical activity to abdominal obesity among adolescents

Diet and physical activity (PA) are recognized as important factors to prevent abdominal obesity (AO), which is strongly associated with chronic diseases. Some studies have reported an inverse association between milk consumption and AO. This study examined the association between milk intake, PA and AO in adolescents. A cross-sectional study was conducted with 1209 adolescents, aged 15–18 from the Azorean Archipelago, Portugal in 2008. AO was defined by a waist circumference at or above the 90th percentile. Adolescent food intake was measured using a semi-quantitative food frequency questionnaire, and milk intake was categorized as ‘low milk intake’ (10 points) and low-active groups ( 10 points) on the basis of their reported PA. They were then divided into four smaller groups, according to milk intake and PA: (i) low milk intake/low active; (ii) low milk intake/active; (iii) high milk intake/low active and (iv) high milk intake/active. The association between milk intake, PA and AO was evaluated using logistic regression analysis, and the results were adjusted for demographic, body mass index, pubertal stage and dietary confounders. In this study, the majority of adolescents consumed semi-skimmed or skimmed milk (92.3%). The group of adolescents with high level of milk intake and active had a lower proportion of AO than did other groups (low milk intake/low active: 34.2%; low milk intake/active: 26.9%; high milk intake/low active: 25.7%; high milk intake/active: 21.9%, P = 0.008). After adjusting for confounders, low-active and active adolescents with high levels of milk intake were less likely to have AO, compared with low-active adolescents with low milk intake (high milk intake/low active, odds ratio [OR] = 0.412, 95% confidence intervals [CI]: 0.201– 0.845; high milk intake/active adolescents, OR = 0.445, 95% CI: 0.235–0.845).Conclusion: High milk intake seems to have a protective effect on AO, regardless of PA level

A incontinência urinária de esforço (IUE) é a perda involuntária de urina desencadeada com o esforço, riso ou tosse e está habitualmente relacionada com a hiper-mobilidade uretral e/ou uma deficiência do esfíncter interno. Os exercicios dos músculos do pavimento pélvico (EMPP) têm sido recomendados no tratamento da IUE. A primeira intervenção no tratamento da incontinência urinária de stress consiste no treino dos músculos do pavimento pélvico (TMPP). A Agency for Healthcare Research and Quality Cochrane Incontinence Group a concluírem que o TMPP é invariavelmente melhor do que ausência de tratamento ou tratamento placebo. A base científica para o TMPP é fundamentada nos princípios da fisiologia do exer cício. A eficácia do treino depende do treino específico dos músculos do pavimento pélvico, da intensidade da carga, duração, frequência e adesão aos protocolos de treino. O objectivo do presente artigo é expor os princípios da fisiologia do exercício e perceber o papel desses princípios nos protocolos de TMPP.

Introdução: A Incontinência Urinária (IU) feminina é um importante problema de saúde pública, quer pela sua elevada prevalência, quer pelo elevado impacte físico, psíquico e social na vida da mulher. O objectivo deste estudo foi determinar a prevalência, o impacte da Incontinência Urinária de Stresse (IUS) antes e durante a gravidez em parturientes do distrito de Viana do Castelo e determinar a percentagem de mulheres que procura apoio de um profissional de saúde para o seu problema. Método: Realizou-se um estudo transversal considerando uma amostra representativa do distrito, constituída por 336 mulheres, cujo parto ocorreu no Hospital de Santa Luzia, no período compreendido entre 15 de Janeiro a 29 de Março de 2002. Todas as mulheres foram submetidas a um questionário no pós-parto hospitalar. Resultados: A prevalência da IUS, definida como Alguma vez teve perda de urina durante a realização de um esforço? foi de 5,4% (IC 95%: 3,0-7,8) antes da gravidez e 51,5% (IC 95%: 46,1-56,9) durante a gravidez actual. Os factores associados à ocorrência da IUS antes da gravidez foram a multiparidade (OR=9,96), a presença de diabetes (OR=4,61) e obesidade (OR=4,76), e à IUS durante a gravidez foram a multiparidade (OR=1,66), a diabetes (OR=2,62) e a obstipação (OR=1,73). A grande maioria (88,9%) das mulheres com IUS sente-se incomodada por se sentir húmida, 48,5% sente-se nervosa ou ansiosa e 57,3% tem medo que os outros se apercebam do odor. Durante a gravidez, apenas menos de metade das mulheres que tiveram perdas de urina procurou apoio de um profissional de saúde, apesar de a maioria ter interesse em tratar o problema. Conclusões: A IUS afecta um grande número de mulheres deste distrito antes e durante a gravidez. A IUS tem reflexos em várias dimensões da saúde, sendo o bem-estar físico e emocional os mais afectados, mas apenas uma pequena percentagem de mulheres revela o seu problema de IUS a um profissional de saúde. Perante esta evidência tornase importante que os profissionais de saúde conheçam esta realidade e se preocupem em dar resposta a estes problemas de saúde., Introduction: Female Urinary Incontinence (UI) is a major public health issue given its high prevalence and its physical, psychological and social impact on women’s lives. The aim of this study was to determine the prevalence and impact of Stress Urinary Incontinence (SUI) before and during pregnancy in parturients from the Viana do Castelo district. Methods: A cross-sectional study was carried out considering a representative sample of the district comprising 336 women who gave birth in the Santa Luzia Hospital between 15th January and 29th March 2002. A questionnaire was submitted to all the women during the postpartum period at hospital. Results: The prevalence of SUI, defined as Have you ever experienced urine loss while performing an effort? was of 5.4% (IC 95%:3.0-7.8) before pregnancy and 51.5% (IC 95%:46.1-56.9) during the current pregnancy. The factors associated with the occurrence of SUI before pregnancy were multiparity (OR=9.96), diabetes (OR=4.61) and obesity (OR=4.76), and with SUI during pregnancy were multiparity (OR=1.66), diabetes (OR=2.62) and constipation (OR=1.73). The vast majority (88.9%) of women suffering from SUI feel uncomfortable as they feel wet, 48.5% feel nervous or troubled and 57.3% fear that others may notice the smell. During pregnancy, only less than half of the women who experienced urine loss sought help from a healthcare professional, although the majority was interested in having this problem treated. Conclusions: SUI affects a great number of women from the district before and during pregnancy. SUI is reflected in many aspects of health and mostly affects physical and emotional well-being, but only a small percentage of women reveal the burden of SUI to a healthcare professional. In the face of such evidence, it turns out very important for healthcare professionals to be aware of this reality and concerned to give response to this healthcare problem.

Summary What is already known about this subject Diet and physical activity (PA) are recognized as important factors to prevent abdominal obesity (AO). Studies have found an inverse relationship between milk intake or milk products and body weight and/or body fat in children and adolescents. Evidence suggests that low levels of PA are associated with AO in youth. What this study adds Our study explored the combined association of milk intake and PA on AO in adolescents, which are most often studied in isolation. Our findings suggested that adolescents with high milk intakes, regardless of whether they were active or low active, were less likely to have AO. Our findings could have a great epidemiological interest and bring important evidence in the field of AO management among adolescents. Background Diet and physical activity (PA) are recognized as important factors to prevent abdominal obesity (AO), which is strongly associated with chronic diseases. Some studies have reported an inverse association between milk consumption and AO. Objective This study examined the association between milk intake, PA and AO in adolescents. Methods A cross-sectional study was conducted with 1209 adolescents, aged 15–18 from the Azorean Archipelago, Portugal in 2008. AO was defined by a waist circumference at or above the 90th percentile. Adolescent food intake was measured using a semi-quantitative food frequency questionnaire, and milk intake was categorized as ‘low milk intake’ (<2 servings per day) or ‘high milk intake’ (≥2 servings per day). PA was assessed via a self-report questionnaire, and participants were divided into active (>10 points) and low-active groups (≤10 points) on the basis of their reported PA. They were then divided into four smaller groups, according to milk intake and PA: (i) low milk intake/low active; (ii) low milk intake/active; (iii) high milk intake/low active and (iv) high milk intake/active. The aims of the this prospective study were to analyse physical activity (PA) engagement during the first and second trimesters, considering the different guidelines published on PA, to document the individual characteristics associated with the accomplishment of these guidelines and to examine pregnant women's perceived barriers to leisure PA, using a socioecological framework. A sample of 133 pregnant women in two stages - at 10-12 weeks' gestation (T1) and 20-22 weeks' gestation (T2) - were evaluated. PA was assessed by accelerometry during the T1 and T2 evaluation stages. Socio-demographic characteristics, lifestyle factors and barriers to leisure PA were assessed via questionnaire. A large proportion of women (ranging from 32% to 96%) did not reach the levels of PA recommended by the guidelines. There were no significant differences between T1 and T2 with regard to compliance with PA recommendations. A decrease in PA levels from T1 to T2 was noted for all recommendations. No associations were found between participants' characteristics and adherence to the recommendations in T1 and T2. No significant differences were found in barriers to leisure PA between T1 and T2. The most commonly reported barriers to leisure PA were intrapersonal, not health related. Our results indicate that there were no differences between trimesters regarding compliance of PA recommendations, and perceived barriers were similar in both trimesters.
