Background: The consumption of some food groups is associated with the risk of diabetes. However, there is no evidence from meta-analysis which evaluates the consumption of ultra-processed products in the risk of diabetes. This study aimed to review the literature assessing longitudinally the association between consumption of ultra-processed food and the risk of type 2 diabetes and to quantify this risk through a meta-analysis.Methods: We conducted a systematic review and meta-analysis with records from PubMed, Latin American and Caribbean Literature in Health Sciences (LILACS), Scielo, Scopus, Embase, and Web of Science. We included longitudinal studies assessing ultra-processed foods and the risk of type 2 diabetes. The review process was conducted independently by two reviewers. The Newcastle Ottawa scale assessed the quality of the studies. A meta-analysis was conducted to assess the effect of moderate and high consumption of ultra-processed food on the risk of diabetes.Results: In total 2272 records were screened, of which 18 studies, including almost 1.1 million individuals, were included in this review and 72% showed a positive association between ultra-processed foods and the risk of diabetes. According to the studies included in the meta-analysis, compared with non-consumption, moderate intake of ultra-processed food increased the risk of diabetes by 12% [relative risk (RR): 1.12; 95% confidence interval (CI): 1.06-1.17, I2 = 24%], whereas high intake increased risk by 31% (RR: 1.31; 95% CI: 1.21-1.42, I2 = 60%).Conclusions: The consumption of ultra-processed foods increased the risk for type 2 diabetes as dose-response effect, with moderate to high credibility of evidence. Objetivo: Avaliar a prevalência de consumo de bebidas alcoólicas no período pré- e gestacional e os fatores que se associam ao consumo em uma coorte de gestantes. Métodos: Coorte prospectiva de gestantes que realizavam pré-natal em um centro municipal de saúde da cidade do Rio de Janeiro, RJ. Os dados foram coletados de novembro de 2009 a outubro de 2011, nos três trimestres de gestação. Para ingressar no estudo as gestantes deveriam estar entre a 5° e 13° semana gestacional; não apresentar gestação gemelar; ter de 20 a 40 anos de idade; não possuir doenças infecciosas/crônicas; realizar o pré-natal na unidade de coleta do estudo. Para coleta de dados foram utilizados instrumentos padronizados para informações de dados demográficos, socioeconômicos, antropométricos, estilo de vida, saúde mental e apoio social. Foram realizadas análises descritivas para estimar as prevalências de consumo de bebidas alcoólicas. Para a associação das variáveis independentes foi conduzida análise longitudinal de regressão logística. Resultados: A prevalência de consumo de bebidas alcoólicas foi 74% entre as entrevistadas antes da gestação, 20,1% no primeiro, 22,6% no segundo e 15,5% no terceiro trimestres. O tipo de bebida mais consumida pelas gestantes foi a cerveja/chopp com uma mediana de volume semanal de 89ml (IC 95% 83,2-93,0). O tabagismo, renda, paridade e apoio social apresentaram associação significativa com o consumo de bebidas alcoólicas. Conclusão: O estudo ressalta a necessidade de abordar o tema no planejamento familiar e nas consultas pré-natal. Salienta-se ainda a importância do profissional de saúde realizar uma abordagem ampla e preventiva do consumo de álcool incluindo os fatores associados que levam ao consumo. Introduction: Alcohol is considered a risk factor for chronic noncommunicable diseases, and its consumption is increasing every year worldwide among men, women and even pregnant women. Objective: To evaluate the prevalence of alcohol consumption in the pre and gestational period and the factors associated with drinking in a cohort of pregnant women followed at a public health center. Methods: The study was conducted using data from a prospective cohort of pregnant women undergoing prenatal care at a public health center in the city of Rio de Janeiro, Brazil. Data were collected from November 2009 to October 2011, in the three trimesters of pregnancy with each pregnant woman. To participate in the study the pregnant woman should be between the 5th and 13th gestational week; not have twin pregnancy; being between 20 and 40 years old; absent of infectious/chronic diseases; perform prenatal care at the study unit. Standardized instruments were applied to collect data on demographic, socioeconomic, anthropometric data, lifestyle, mental health and social support. Descriptive analyzes were performed to estimate the prevalence of alcohol consumption. The association of independent variables was tested using longitudinal logistic regression analysis. Results: Alcohol consumption was 74.1% in the pregestational period, 20.1% in the first, 22.6% in the second and 15.5% in the third trimesters. The type of alcohol drinking most frequently consumed was beer/chopp with a median weekly volume (ml) of 89 (95% CI 83.2-93.0). Smoking habit, income, parity and social support were associated with alcohol consumption. Conclusion: The study highlights the need to address the alcohol consumption in familiar planning program and prenatal assistance. Health professionals need to provide a broader and preventive intervention, including the associated factors associated to alcohol consumption.

Este profissional representa o elo entre a atenção básica de saúde e a comunidade. Objetivo: avaliar o perfil nutricional e comportamental de agentes comunitários de saúde, bem como autopercepção de saúde, consumo de medicamentos de uso contínuo e presença de doença diagnosticada por médico no município de Pelotas, RS. Metodologia: estudo transversal, tipo censo com Agentes Comunitários de Saúde da zona rural e urbana. Para avaliação do estado nutricional foram aferidos peso e altura. O estado nutricional foi classificado de acordo com o índice de massa corporal. Consumo alimentar, prática de atividade física no lazer, consumo de álcool, tabagismo, autopercepção de saúde, presença de doença diagnosticada por médico e consumo de medicamentos de uso contínuo foram identificados por meio de questionário aplicado por entrevistadoras treinadas. Após a coleta de dados, estes foram duplamente digitados no programa Epidata 3.1 e as análises descritivas e teste qui-quadrado de Pearson foram realizadas no programa estatístico STATA 12.0, com nível de significância de 5%. Resultados: observou-se elevada prevalência de excesso de peso (80,3%) e inatividade física no lazer (70,7%). Em relação aos marcadores de consumo alimentar, 49,6% consumiram dois ou mais alimentos não saudáveis e 75,7% consumiram dois ou mais alimentos saudáveis no dia anterior à entrevista. Tabagismo foi reportado por 14,9% dos profissionais e 54,8% consumiram bebida alcoólica nos últimos 30 dias. Mais da metade dos participantes reportou autopercepção boa de saúde, 50,8% tiveram diagnóstico médico de doença e 57,0% usavam medicamentos de uso contínuo. Conclusão: o estudo identificou elevadas prevalências de excesso de peso e inatividade física dentre os fatores comportamentais avaliados, sugerindo a necessidade de medidas voltadas para o controle destes dois fatores, tendo em vista que são passíveis de modificação. Além disso, mais da metade dos profissionais apresentava diagnóstico médico de doença e usava medicamentos de forma contínua. Destaca-se a importância de ações que visem a promoção da saúde desses profissionais, independente da zona de atuação, pois pouca diferença foi identificada entre as duas áreas. Introduction: Among the primary responsibilities of the Community Health Worker is the implementation of actions at health promotion. This professional represents the link between primary health care and the community. Objective: To evaluate the nutritional and behavioral profile of Community Health Workers, as well as self-perception of health, consumption of medicines of continuous use and presence of disease diagnosed by a physician, in the city of Pelotas, RS. Methodology: A cross-sectional, census-type study with Community Health Workers from rural and urban areas. To assess nutritional status, weight and height were measured. Nutritional status was classified according to body mass index. Food consumption, leisure-time physical activity, alcohol consumption, smoking, self-perception of health, presence of a physiciandiagnosed disease, and continuous use of medication were identified through a questionnaire administered by trained interviewers. After data collection, these were double entered in the Epidata 3.1 program and the descriptive analyses and Pearson's chi-square test were performed in the STATA 12.0 statistical program, with a significance level of 5%. Results: This study observed a high prevalence of overweight (80.3%) and leisure-time physical inactivity (70.7%). Regarding food intake markers, 49.6% consumed two or more unhealthy foods and 75.7% consumed two or more healthy foods in the day before the interview. Smoking was reported by 14.9% of professionals and 54.8% consumed alcohol in the last 30 days. More than half of the participants reported good self-rated of health, 50.8% had a medical diagnosis of the disease, and 57.0% used continuous medication. Conclusion: the study identified a high prevalence of overweight and physical inactivity among the behavioral factors evaluated, suggesting the need for measures to control these two factors. Also, more than half of the professionals had a medical diagnosis of the disease and used medications continuously. Highlighting the importance of actions that objective promoting of the health of these professionals, regardless of their area of operation, due little difference identified between the two areas.

- Abstract
This paper evaluates the knowledge of community health workers in complementary feeding and their association with sociodemographic characteristics, work routines and describes the resources available in primary care facilities to master this topic. We applied questionnaires to the responsible for health services and to community health workers, the latter consisting of a knowledge test that allowed the calculation of scores according to the number of correct answers in multiple-choice questions. There was a positive association with age, length of service, home visits to children under 24 months, providing guidance or seeking information about complementary feeding with the knowledge in feeding in the first 24 months of life. There were discrepant responses by health services and the community health workers regarding training, government materials and child growth monitoring. Greater mastery was noted in breastfeeding in relation to complementary feeding. Therefore, primary care facilities should provide support and resources to increase knowledge in complementary feeding through training and access to government materials.
