We also thank Efrain Perini (PhD) and Jair Mengatti (PhD) from the Radiopharmacy Center of the Nuclear and Energy Research Institute (IPEN/CNEN), São Paulo, Brazil Abstract: Introduction Radiosynovectomy (RS) with Y-90-hydroxyapatite (Y-90-HyA) aims to control knee hemarthrosis in hemophiliac patients to prevent secondary arthropathy. However, knee RS using Sm-153-hydroxyapatite (Sm-153-HyA) is considered less suitable due to the lower average soft tissue range and energy of Sm-153 for large joints, such as the knees. Purpose The objective of this investigation was to assess the efficacy and safety of knee RS with Sm-153-HyA, compared to Y-90-HyA. Methods Forty patients were prospectively assigned to undergo knee RS with Sm-153-HyA (n = 19) or with Y-90-HyA (n = 21). The frequency of hemarthrosis episodes before and after treatment were compared. Results After six months of knee RS, Sm-153-HyA and Y-90-HyA promoted a similar reduction of hemarthrosis episodes (50% and 66.7%, respectively). However, after 12 months of knee RS, the reduction of hemarthrosis episodes was significantly (p = 0.037) higher using Sm-153-HyA (87.5%) compared to Y-90-HyA (50.0%). This discrepancy was more pronounced (p = 0.002) for Sm-153-HyA compared to Y-90-HyA in adults/adolescents. Conclusion Knee radiosynovectomy with Sm-153-HyA is safe, reduces hemarthrosis episodes after 12 months of treatments, especially in adults/adolescents and even with grades III/IV arthropathy, similar to Y-90-HyA. Agradecimentos: This study was financed in part by the Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - Brasil (CAPES) - Finance Code 001 Abstract: The purpose of this study was to compare the anthropometric methods by body segment with the values predicted by dual energy radiological absorptiometry (DXA) in athletes with spinal cord injury. Eight quadriplegic and six paraplegic athletes participated in this study. Body circumferences were measured at seven sites, skinfold thickness was measured at nine sites (on the right side of the body), and body composition was measured using DXA. Linear regression analysis was used to verify the associations between body composition and anthropometric measurements. Segmental measures that best explain fat mass as predicted by DXA were: a sum of the skinfold thickness in the arm (R2 = .66; p < .01); in the trunk, subscapular (R2 = .75, p < .01), midaxillary axillary (R2 = .67, p < .01) and abdominal skinfolds (R2 = .67, p < .01), and sum of trunk skinfold thickness (R2 = .67, p < .01); and in the leg, the calf skinfold and the sum of the skinfolds of the leg (R2 = .70, p < .01; R2 = .68, p < .01). The circumferences of the relaxed and tensed arms showed relevant relationships (R2 = .52, p < .01, and R2 = .57, p < .01, respectively) with the fat mass predicted by DXA. This suggests that segmental analysis of body composition through circumferences and skinfold thickness may be a good option for the accurate determination of body composition in athletes with spinal cord injury. Materials and methods Sixty-two patients with recently diagnosed multiple myeloma were submitted to whole-body(18)F-FDG-PET/CT and whole-body MIBI scans plus SPECT/CT of the chest and abdomen/pelvis. Number of focal lesions, contiguous soft tissue involvement (CSTI), extramedullary lesions (EMLs) and diffuse bone marrow (BM) involvement were recorded. Results PET/CT was positive in 59 patients (95%) and MIBI SPECT/CT in 58 (93%) (P = 0.69). MIBI detected more diffuse bone marrow involvement than PET/CT (respectively 78 vs. 58% of the patients), while PET/CT demonstrated more focal lesions than MIBI SPECT/CT (81 vs. 54% of the patients) (P = 0.002). PET/CT detected EMLs in four subjects and MIBI in one subject. CSTI was found in 28 (45%) and 23 (37%) patients on PET/CT and MIBI images, respectively (P = 0.36). Three patients with lytic lesions and no FDG uptake were MIBI positive, and two subjects with lytic lesions without MIBI uptake were FDG positive. Conclusion MIBI SPECT/CT performs similarly to(18)F-FDG-PET/CT in identifying sites of active disease in multiple myeloma staging. MIBI is more efficient than FDG for detecting the diffuse involvement of bone marrow but less efficient for detecting focal lesions. CDR has a research grant from CNPq (National Council of Research) proc 311841/2018-0 Many efforts have been made to standardize the interpretation of F-18-FDG PET/CT in multiple myeloma (MM) with qualitative visual analysis or with quantitative metabolic parameters using various methods for lesion segmentation of PET images. The aim of this study was to propose a quantitative method for bone and bone marrow evaluation of F-18-FDG PET/CT considering the extent and intensity of bone F-18-FDG uptake: Intensity of Bone Involvement (IBI). Whole body F-18-FDG PET/CT of 59 consecutive MM patients were evaluated. Compact bone tissue was segmented in PET images using a global threshold for HU of the registered CT image. A whole skeleton mask was created and the percentage of its volume with F-18-FDG uptake above hepatic uptake was calculated (Percentage of Bone Involvement - PBI). IBI was defined by multiplying PBI by mean SUV above hepatic uptake. IBI was compared with visual analysis performed by two experienced nuclear medicine physicians. IBI calculation was feasible in all images (range:0.00-1.35). Visual analysis categorized PET exams into three groups (negative/mild, moderate and marked bone involvement), that had different ranges of IBI (multi comparison analysis, p < 0.0001). There was an inverse correlation between the patients' hemoglobin values and IBI (r = -0.248;p = 0.02). IBI score is an objective measure of bone and bone marrow involvement in MM, allowing the categorization of patients in different degrees of aggressiveness of the bone disease. The next step is to validate IBI in a larger group of patients, before and after treatment and in a multicentre setting. Fibrodysplasia ossificans progressiva is a rare genetic disease characterized by widespread soft tissue ossification and congenital stigmata of the extremities. We report on a male child followed for ten years since the age of 3 years and 9 months, when the diagnosis was made. He was born with bilateral hypoplasic hallux valgus and ventricular septal defect, corrected by transsternal approach when 32 months old. Restriction of neck mobility followed and foci of ectopic ossification appeared. Four crises of disease exacerbation were treated with oral prednisone and/or other antiinflammatory drugs. Sodium etidronate 5 to 10 mg/kg/day was prescribed intermittently during about six years but was discontinued due to osteopenia. The disease course has been relentless, with severe movement restriction including the chest wall. A review showed few similar case reports in the Brazilian literature. We revisit the criteria for diagnosis and the essentials of management and treatment.

A 26 year-old female was admitted with abdominal pain, fever and weight loss. The clinical and laboratory investigations led to the diagnosis of paracoccidioidomycosis. Gallium-67 whole body images correlated well with the clinical course of the disease and with the patient's prognosis. Paciente do sexo feminino de 26 anos foi internada com dor abdominal, febre e emagrecimento. A investigação clínico-laboratorial estabeleceu o diagnóstico de paracoccidioidomicose. Os achados cintilográficos com citrato de gálio-67 correlacionaram-se com o curso clínico da paciente.

Fibrodysplasia ossificans progressiva is a rare genetic disease characterized by widespread soft tissue ossification and congenital stigmata of the extremities. We report on a male child followed for ten years since the age of 3 years and 9 months, when the diagnosis was made. He was born with bilateral hypoplasic hallux valgus and ventricular septal defect, corrected by transsternal approach when 32 months old. Restriction of neck mobility followed and foci of ectopic ossification appeared. Four crises of disease exacerbation were treated with oral prednisone and/or other antiinflammatory drugs. Sodium etidronate 5 to 10 mg/kg/day was prescribed intermittently during about six years but was discontinued due to osteopenia. The disease course has been relentless, with severe movement restriction including the chest wall. A review showed few similar case reports in the Brazilian literature. We revisit the criteria for diagnosis and the essentials of management and treatment. Fibrodisplasia ossificante progressiva é uma doença genética rara, caracterizada por ossificação disseminada em tecidos moles e estigmas congênitos nas extremidades. Relatamos sobre uma criança do sexo masculino, branca, que vem sendo acompanhada desde seus 3 anos e 9 meses quando diagnosticamos a doença. Nascida com halux valgo hipoplásico bilateral e defeito do septo interventricular, submeteu-se a correção cirúrgica através de esternotomia, aos 32 meses de vida. Seguiram-se progressiva restrição de movimentos cervicais e aparecimento de nódulos duros e dolorosos em regiões paravertebrais cérvico-torácicas. Em dez anos, assistimos a quatro episódios bem definidos de exacerbação da doença, com acréscimos de ossificação ectópica. Nessas ocasiões, prescrevíamos prednisona oral e/ou antiinflamatórios não hormonais. Etidronato de sódio, 5 a 10 mg/kg/dia, foi utilizado em períodos de dois ou três meses, com intervalos de dois meses, durante seis anos. Osteopenia obrigou-nos a interromper o uso do quelante. A evolução tem sido inexorável, com limitação intensa de movimentos torácicos e corpóreos. A revisão da literatura brasileira revelou poucos casos semelhantes. Enfatizamos os critérios para diagnóstico e princípios básicos de tratamento.

Objectives: In sickle cell/β-thalassemia, mutations in the corresponding β-globin genes are responsible for complex pathological events resulting in diverse clinical complications. The objective of this study was to provide an overview of the clinical and laboratory characteristics of patients with the syndrome, and of the degree of severity of clinical manifestations resulting from the β-thalassemia mutation. Methods: A retrospective chart review was performed on 46 patients with sickle cell/β-thalassemia (31 Sβ
o and 15 Sβ+ ), evaluating hematological parameters and end organ damage. Statistical analyzes were carried out in order to highlight differences between the two groups according to the nature of the thalassemia mutation. Results: As expected, patients with the Sβo phenotype had a higher degree of hematological involvement in comparison to Sβ+ patients; with lower hemoglobin levels, and signs of more intense chronic hemolysis. However, Sβ+ patients were more prone to the occurrence of acute chest syndrome. The impact of the thalassemia mutation upon total body and bone composition was also evident, as Sβo patients presented lower body mass index (BMI) and bone mineral density. The degree of bone damage correlated to lower BMI and hemoglobin levels, as well as plaquetosis, monocytosis and elevated lactate dehydrogenase, possibly reflecting the effects of hemolysis and inflammation upon bone metabolism and body constitution. Conclusions: This study identified significant differences among sickle cell/β-thalassemia patients according to the beta mutation involvement, pointing to an important predictor of disease severity. A osteoporose é uma doença óssea multifatorial que afeta alguns pacientes neurológicos. Ataxia não é conhecida como fator de risco para osteoporose/osteopenia. A perda progressiva de movimentos pode levar a perda de massa óssea que associada à alta frequência de quedas podem favorecer a ocorrência a fraturas. OBJETIVO: O objetivo deste estudo é investigar a densidade mineral óssea (DMO) de pacientes com DMJ, a frequência de fraturas vertebrais nesses pacientes e fatores clínicos que podem influenciar as alterações da DMO. PACIENTES E MÉTODOS: Trinta pacientes consecutivos com confirmação molecular da DMJ foram incluídos prospectivamente. Foram obtidos dados clínicos e DMO da coluna lombar e do fêmur por densitometria óssea (DXA). Os Z-scores foram utilizados para análise de DMO. A radiografia lombar foi obtida na posição lateral para pesquisa de fraturas. Os resultados obtidos de DMO foram correlacionados com os dados clínicos dos pacientes. RESULTADOS: Dez pacientes (33,3%) apresentaram DMO baixa em pelo menos um dos locais estudados com base nos Z-scores. O Z-score da DMO da população total do estudo foi comparado aos valores normativos e foram encontradas diferenças significativas no quadril total e na coluna lombar. Os Z-scores correlacionaram-se diretamente com o índice de massa corporal na coluna lombar, fêmur total e colo do fêmur. A expansão do CAG foi inversamente correlacionada com o Z-score no colo do fêmur. Não houve correlação entre os anos de sintomas, a gravidade da doença, o tipo de ataxia, a idade do início dos sintomas, a duração da doença, a dependência de cadeira de rodas, o histórico de fraturas, a frequência de quedas ou a presença de fraturas nos raios-X lombares e Z-scores. CONCLUSÃO: Baixa DMO é muito frequente entre os pacientes com DMJ e esse achado pode justificar a avaliação precoce da DMO desses pacientes Abstract: INTRODUCTION: Machado-Joseph Disease (MJD) is the most common spinocerebellar ataxia worldwide. Osteoporosis is a multifactorial bone disease that affects some neurologic patients. Ataxia is not known as a risk factor to osteoporosis/osteopenia. Progressive loss of movements may lead to bone loss and the high frequency of falls in these patients may contribute to fractures. PURPOSE: The aim of this pilot study is to investigate the bone mineral density (BMD) of patients with MJD, the frequency of vertebral fractures in these patients and clinical factors that may influence BMD alterations. PATIENTS AND METHODS: Thirty consecutive patients with molecular confirmation of MJD were prospectively enrolled. Clinical data and BMD of lumbar spine and femur were obtained. BMD was measured by bone densitometry (DXA). Z-scores were used for BMD analysis. Lumbar X-ray was obtained in the lateral position for fracture assessment. BMD results were correlated to clinical data of the patients. RESULTS: Ten patients (33.3%) showed low BMD in at least one of the sites studied based on Z-scores. BMD Z-score of total study population was compared to normative values, and significant differences were found on the total hip and lumbar spine. Z-scores correlated directly with body mass index in lumbar spine, total hip and femoral neck. CAG expansion was inversely correlated to Z-score at femoral neck. There was no correlation between years of symptoms, severity of disease, type of ataxia, age of onset of symptoms, disease duration, wheelchair dependence, history of fractures, frequency of falls or presence of fractures on lumbar X-ray and Z-scores. Orientador: Celso Dario Ramos Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Objetivos: A avaliação da função renal, em particular da taxa de filtração glomerular (TFG), é extremamente importante em pacientes submetidos à quimioterapia com cisplatina, um fármaco nefrotóxico. Os objetivos deste trabalho foram comparar a estimativa da TFG pela fórmula Crockcroft-Gault (TFGe), que se baseia na creatinina sérica, com a TFG medida pelo método do EDTA-51Cr (TFG-EDTA), considerado "padrão-ouro", em pacientes com diagnóstico de carcinoma de cabeça e pescoço (CCP) antes e 30 dias após quimioterapia com cisplatina e acompanhar a evolução da função renal desses pacientes após 6 meses. Pacientes e Métodos: Foram avaliados 69 pacientes com diagnóstico de CCP submetidos à quimioterapia com cisplatina, sendo 6 mulheres e 63 homens. Esses pacientes foram submetidos ao exame com EDTA-51Cr e coleta de sangue para cálculo da TFGe antes e 30 dias após a quimioterapia. A função renal de 40 destes pacientes também foi analisada através da TFG-EDTA 6 meses após a quimioterapia. Resultados: A média da TFGe e TFG-EDTA pré-quimioterapia foi de 91,21 ml/min/1.73m2 e 84,33 ml/min/1.73m2, respectivamente. Trinta dias após a quimioterapia com cisplatina esta média foi de 78,79 ml/min/1.73m2 para a TFGe e 64,97 ml/min/1.73 m2 para a TFG-EDTA. Orientadores: Heraldo Mendes Garmes, Allan de Oliveira Santos Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Acromegalia é uma doença crônica causada pela hipersecreção do hormônio do crescimento (GH), que estimula a produção de IGF-1 pelas células hepáticas e tecidos sistêmicos, sendo a principal etiologia o somatotropinoma hipofisário. O excesso de GH e IGF-1 está associado a uma série de comorbidades e ao aumento da mortalidade. O tratamento de primeira linha da doença é a ressecção transesfenoidal do adenoma hipofisário secretor de GH, porém, pela prevalência aumentada de macroadenomas ao diagnóstico, muitos pacientes, cerca de 50%, necessitam de tratamento medicamentoso adjuvante e a primeira escolha é o análogo de somatostatina (SSA), que suprime a secreção de GH pelos somatotrofos por meio de sua ligação ao receptor SSTR2. No entanto, cerca de 50% dos pacientes são parcialmente ou totalmente resistentes aos análogos de somatostatina e, portanto, a busca por fatores preditivos de resposta ao medicamento é de relevante valia para individualizar o tratamento medicamentoso. Diante do cenário de emergência do "68Ga-DOTATATO" PET/CT como método padrão ouro no diagnóstico e seguimento de tumores neuroendócrinos gastroenteropancreáticos, que também expressam SSTR, nosso objetivo específico foi avaliar se a captação (SUV max) do radiofármaco na região hipofisária de pacientes com o diagnóstico de acromegalia em uso de SSA seria útil como preditor de reposta ao medicamento. Para tanto, foram recrutados 15 pacientes com acromegalia em uso de SSA há pelo menos 6 meses, acompanhados no serviço de endocrinologia do Hospital de Clínicas da Universidade Estadual de Campinas - UNICAMP, que foram submetidos ao "68Ga-DOTATATO" PET/CT no serviço de medicina nuclear. Os pacientes foram divididos em um grupo com resposta completa ao SSA (n = 5), definida como GH < 1 µg/L e IFG-1 na faixa de normalidade para sexo e idade, e um grupo que não atingiu tal critério (n = 10). Como resultado, não encontramos maior SUV max no grupo com resposta completa (p = 0.0576), porém evidenciamos uma relação inversa significativa entre valores de GH pós uso de SSA e o SUV max na região da sela túrcica, possivelmente refletindo a maior expressão tumoral de SSTR2 nos pacientes que respondem ao tratamento. Em relação a outras variáveis, como sexo, idade, níveis de GH e IGF-1, intensidade de sinal em T2 na RNM não encontramos associação com a resposta aos SSA. Dessa forma, após essa avaliação inicial, concluímos que o "68Ga-DOTATATO" PET/CT deve ser, ainda, mais estudado para avaliar sua utilidade no seguimento de pacientes acromegálicos Abstract: Acromegaly is a chronic disease caused by growth hormone (GH) hypersecretion, which stimulates the production of IGF-1 by liver cells and systemic tissues, and the main cause is pituitary somatotropinoma. GH and IGF-1 excess is associated with many comorbidities and increased mortality. The first line treatment of the disease is the transsphenoidal resection of the GH-secreting pituitary adenoma. However, due to the increased prevalence of macroadenomas at diagnosis, many patients, about 50%, need adjuvant drug treatment and the first choice is the analogue of somatostatin (SSA), which suppresses GH secretion by somatotrophs by binding to the SSTR2 receptor. However, about 50% of patients are partially or totally resistant to somatostatin analogues, so the search for predictive factors of adequate response is of great value in individualizing drug treatment. Because of the emergence of "68Ga-DOTATATE" PET/CT as the gold standard method in the diagnosis and follow-up of gastroenteropancreatic neuroendocrine tumors, which also express SSTR2, our specific objective was to evaluate whether radiopharmaceutical uptake (SUV max) in the pituitary region of patients with acromegaly diagnosis on SSA therapy would be useful as a predictor of drug response. Surpreendentemente, não há dados sistemáticos na literatura relatando o uso da cintilografia renal com o ácido dimercaptosuccinico-99mTc (DMSA-99mTc) em AF. Este estudo teve como objetivo avaliar o uso de imagens planas e SPECT/CT com DMSA-99mTc para a quantificação da função renal relativa e absoluta na AF. Nestes pacientes foi realizada a comparação da função tubular e glomerular, através da medição simultânea de captação renal absoluta de DMSA-99mTc (DMSA%) e da taxa de filtração glomerular (TFG) usando EDTA-51Cr. Neste contexto, duas questões técnicas também foram levantadas e avaliadas: o melhor método para quantificar a DMSA% e se é possível medir com precisão o TFG com EDTA-51Cr utilizando uma gama-câmara convencional. Materiais e Métodos: trinta e três pacientes (19 do sexo feminino) com idades entre 23-69 anos, com AF e sem evidência clínica de insuficiência renal foram recrutados para este estudo prospectivo. Todos os indivíduos deram consentimento informado. Foram injetados simultaneamente nos pacientes doses convencionais de DMSA-99mTc e EDTA-51Cr. Imagens planas e de SPECT/CT foram obtidas após 3 horas. As imagens foram analisadas em busca de lesões renais. A DMSA% foi calculada como uma percentagem da dose injetada. Amostras de sangue foram colhidas para determinar a TFG com EDTA-51Cr. Dois métodos para quantificar a DMSA% foram comparados: com o uso de imagens planas e images SPECT/CT. Um dispositivo de baixo custo que permite a medição da TFG com EDTA-51Cr utilizando uma câmara de cintilação também foi desenvolvido e testado com amostras radioativas in vitro e com amostras de sangue de 8 pacientes injetados com doses convencionais de EDTA-51Cr. Resultados: Foi identificado um padrão característico de imagens de DMSA-99mTc em 29/33 (88%) pacientes, que consiste em afilamento cortical difuso e colunas renais proeminentes, tanto nas imagens planas como nas imagens SPECT. Lesões corticais também estavam presentes em 26/33 pacientes (79%), mais claramente identificadas por SPECT do que por imagens planas. A DMSA% estava reduzida na maioria dos pacientes e a função tubular relativa média foi semelhante em ambos os rins. A correlação entre a DMSA% (24,6% ± 5,5%) e taxa de filtração glomerular (89,4 ± 24,5 mL/min/1,73m2) foi baixa (r = 0,448). A correlação entre a DMSA% obtida por imagens planas com aquela obtida por SPECT/CT foi: r = 0,722, p =0,99943 . Os valores de TFG obtidos com o método convencional por contador de poço variaram de 27 a 73 mL/min/1,73m2 (média de 56,7 ± 14,0 mL/min/1,73m2) e utilizando o novo dispositivo adaptado para a gama-câmara, de 31 a 82 ml/min/1,73m2 (média 60,9 ± 14.5 mL/min/1,73m2), com sensibilidade de 300Bq, expressando uma correlação de r = 0,9010 e p = 0,5628 (95% de confiança). Conclusão: A cintilografia com DMSA-99mTc identifica afilamento cortical, colunas renais proeminentes e lesões focais do córtex renal na maioria dos pacientes com anemia falciforme, apresentando um padrão de imagem característico. A correlação entre a DMSA% e a TFG é baixa na AF, sugerindo que a disfunção glomerular não é necessariamente acompanhada por uma disfunção tubular proporcional nestes pacientes. Os dois métodos para medir a DMSA% (usando imagens planas ou imagens SPECT/CT) mostram resultados semelhantes. É possível medir a TFG com EDTA-51Cr em uma gama-câmara utilizando o dispositivo colimador descrito no estudo, do qual foi solicitado o registro de patente Abstract: Introduction: Different functional and structural abnormalities of the kidneys are common in patients with sickle cell anemia (SCA). Surprisingly, there is no systematic data in the literature reporting the use of 99mTc-dimercaptosuccinic acid (DMSA) renal scintigraphy in SCA. This study aimed to evaluate the utility of DMSA planar and SPECT/CT images and relative renal function quantification in SCA. We also compared tubular and glomerular function in these patients by performing simultaneous measurement of absolute renal uptake of 99mTc-DMSA (DMSA%) and glomerular filtration rate (GFR) using 51Cr-EDTA. In this context, two technical issues have also been raised and evaluated: what is the best method to quantify DMSA% and if it is possible to accurately measure the GRF with 51Cr-EDTA using a conventional gamma camera. Materials and Methods: Thirty-three patients (19 female) aged 24-58 years with SCA and no clinical evidence of renal insufficiency were recruited for this prospective study. All subjects gave informed consent. Conventional doses of 99mTc-DMSA and 51Cr-EDTA were simultaneously injected in the patients. Planar and SPECT/CT images were obtained after 3 hours. The images were analyzed in search of renal lesions. DMSA% was calculated as a percentage of the injected dose. Blood samples were taken to determine 51Cr-EDTA based GFR. Two methods for quantifying DMSA% were compared: using planar images and SPECT/CT images. An inexpensive device that allows the measurement of 51Cr-EDTA based GFR using a scintillation camera was also developed and tested with radioactive in vitro samples and with blood samples of 8 patients injected with conventional doses of 51Cr-EDTA. Results: A characteristic image pattern of prominent renal columns and cortical thinning was identified in 29/33 patients (88%) on planar and SPECT images. Focal cortical lesions were present in 26/33 patients (79%), more clearly identified on SPECT than on planar images. DMSA% was reduced in most patients and the mean relative tubular function was similar for the right and left kidneys. The correlation between DMSA% (24.6% ± 5.5%) and GFR (89.4 ± 24.5 mL/min/1.73m2) was low (r= 0.447). DMSA% correlations obtained with planar and SPECT/CT images were: r = 0,722, p = 0.99943. The GFR values obtained with the conventional method ranged from 27 to 73 ml/min/1.73m2 (mean 56.7 ± 14.0) and using the new device adapted to the gamma camera, from 31 to 82 ml/min/1.73m2 (mean 60.9 ± 14.5), with sensitivity of 300Bq, expressing a correlation of r = 0.901 and p = 0.9486 (95% confidence). Conclusion: 99mTc-DMSA scintigraphy identifies prominent renal columns, cortical thinning and focal cortical lesions in most patients with SCD, with a distinctive image pattern. The correlation between DMSA% and GFR is low in SCD, suggesting that glomerular dysfunction is not necessarily followed by a proportional tubular dysfunction in these patients. The two methods for measuring DMSA% (using planar images or SPECT/CT) show similar results. Orientador: Celso Dario Ramos Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Contexto: Apesar do importante potencial dos cálculos dosimétricos para uso clínico em radiosinoviortese (RSV), há poucos dados na literatura sobre este assunto. Neste trabalho, é proposta uma metodologia para dosimetria beta paciente-específica utilizando imagens SPECT/CT para avaliar a dose absorvida por RSV em articulações tratadas com hidroxiapatita marcada com samário-153 (HA-153Sm) e hidroxiapatita marcada com ítrio-90 (HA-90Y). Metodologia: Uma análise voxel-por-voxel foi realizada para construir um mapa de dose absorvida tridimensional para três pacientes tratados com HA-90Y (185MBq) ou HA-153Sm (740MBq). Uma equação genérica de dose absorvida foi utilizada para calcular a dose absorvida no voxel, a partir da atividade intra-voxel obtida da quantificação da imagem SPECT e a sua massa correspondente obtida a partir da imagem CT. Considerando o tamanho do voxel (7.79mmX7.79mmX7.79mm), a energia média e o alcance terapêutico das partículas beta emitidas do 90Y e 153Sm, é assumido que a dose em cada voxel não é afetada pela atividade dos voxels vizinhos. Dados sobre a razão de dose entre HA-90Y e HA-153Sm foram comparados. Resultados: Foram obtidas distribuição espacial da dose absorvida, dose absorvida total e dose absorvida por atividade injetada para cada paciente. A dose absorvida relativa entre HA-90Y e HA- 153Sm foi cerca de 5.5:1. Conclusão: Dosimetria paciente-específica em RSV pode ser uma poderosa ferramenta, especialmente em distribuições heterogêneas de radiotraçadores intra-articulares, quando doses terapêuticas complementares podem ser necessárias. Usando esta metodologia, foi possível demonstrar que podem ser obtidos efeitos terapêuticos comparáveis entre as RSVs realizadas com HA-90Y e HA-153Sm se for utilizada uma atividade cerca 5.5 vezes maior de HA-153Sm Abstract: Background: Despite the potential clinical importance of dosimetric calculations in radiosynovectomy (RSV), there are very few data in the literature about this topic. In this study, we propose a patient-specific methodology for beta dosimetry using SPECT/CT images to evaluate the absorbed dose of RSV in human joints treated with HA-90Y and HA-153Sm. Methods: A voxel-by-voxel analysis was performed to build a 3D absorbed dose map for three patients treated with HA-90Y (185MBq) or HA-153Sm (740MBq). A generic absorbed dose equation was used to calculate the voxel dose, supported by the voxel activity obtained from quantified SPECT image and the corresponding voxel mass obtained from CT image. Considering the voxel size (7.79mmX7.79mmX7.79mm), the average energy and the therapeutic range of emitted beta particles from 90Y and 153Sm, we assumed that the dose in each voxel is not affected by the activity of neighboring voxels. Data about ratio of absorbed dose between HA-90Y and HA-153Sm were compared. Results: Spatial distribution of absorbed dose, total absorbed dose and absorbed dose per injected activity were obtained for each patient. The calculated relative absorbed dose between HA-90Y to HA-153Sm was about 5.5:1. Conclusions: Patient-specific dosimetry in RSV can be a powerful tool, especially in heterogeneous intra-articular distributions of radiotracer when additional therapeutic doses can be administered. Using this methodology we estimated that it is possible to achieve comparable therapeutic effects for RSV using HA-90Y if one uses activities about 5.5 times greater of HA-153Sm Mestrado Clínica Médica Mestra em Clínica Médica

Orientadores: Celso Darío Ramos, Elizabeth João Pavin Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Introdução: A doença arterial coronariana (DAC) é frequentemente assintomática em pacientes diabéticos, o que faz os testes não-invasivos serem uma ferramenta essencial para a detecção precoce da doença. O objetivo deste estudo foi avaliar a perfusão miocárdica de pacientes com diabetes mellitus tipo 1 (DM1) e tipo 2 (DM2) assintomáticos, sem diagnóstico prévio de DAC ou acidente vascular cerebral. Materiais e métodos: Cinqüenta e nove pacientes consecutivos assintomáticos (16 DM1, 43 DM2) foram submetidos a cintilografia de perfusão miocárdica com sestamibi- 99mTc (CPM). Eles foram avaliados quanto ao índice de massa corpórea (IMC), controle metabólico do diabetes, dislipidemia, tipo de terapia para o diabetes, hipertensão arterial sistêmica, nefropatia, retinopatia, neuropatia periférica, tabagismo e história familiar de DAC. Resultados: CPM foi anormal em 25,4%: 12 com alterações de perfusão (2 DM1, 10 DM2) e 3 com disfunção ventricular esquerda isolada (3 DM2). Os mais fortes preditores de perfusão miocárdica anormal foram: idade igual ou maior a 60 anos (p = 0,017, OR = 6,0), neuropatia periférica (p = 0,028, OR = 6,1), nefropatia (p = 0,031, OR = 5,6) e ECG de esforço positivo para isquemia (p = 0,049, OR = 4,08). Conclusão: A isquemia miocárdica silenciosa ocorre em mais de um em cada cinco diabéticos assintomáticos. Os mais fortes preditores de isquemia foram: idade avançada, neuropatia periférica, nefropatia, retinopatia e ECG de esforço positivo para isquemia Abstract: Introduction: Coronary artery disease (CAD) is often asymptomatic in diabetic patients, which makes non-invasive tests an essential tool for early detection of the disease. This study was aimed to evaluate myocardial perfusion in asymptomatic patients with type 1 (DM1) and type 2 diabetes mellitus (DM2) without previous diagnoses of CAD or stroke. Methods: Fifty nine consecutive asymptomatic patients (16 DM1, 43 DM2) underwent myocardial perfusion scintigraphy with 99mTc-sestamibi (MPS). They were evaluated for body mass index (BMI), metabolic control of DM, type of therapy, systemic arterial hypertension, dyslipidemia, nephropathy, retinopathy, peripheral neuropathy, smoking, and familial history of CAD. Results: MPS was abnormal in 15 patients (25.4%): 12 (20,3%) with perfusion abnormalities (2 DM1, 10 DM2), and 3 with isolated left ventricular dysfunction (3 DM2). The strongest predictors for abnormal myocardial perfusion were: age 60 years and above (p=0.017; OR=6.0), peripheral neuropathy (p=0.028; OR=6.1), nephropathy (p=0.031; OR=5.6), and stress ECG positive for ischemia (p=0.049; OR=4.08). Conclusions: Silent myocardial ischemia occurs in more than one in five asymptomatic diabetic patients. Objective: To evaluate the performance of
18 F-fluorodeoxyglucose positron-emission tomography/computed tomography (18 F-FDG PET/CT) in localizing epileptogenic zones, comparing18 F-FDG injection performed in the traditional interictal period with that performed near the time of a seizure., Materials and Methods: We evaluated patients with refractory epilepsy who underwent18 F-FDG PET/CT. The reference standards for localization of the epileptogenic zone were histopathology and follow-up examinations (in patients who underwent surgery) or serial electroencephalography (EEG) recordings, long-term video EEG, and magnetic resonance imaging (in patients who did not). The18 F-FDG injection was performed whether the patient had an epileptic seizure during the EEG monitoring period or not. The18 F-FDG PET/CT results were categorized as concordant or discordant with the reference standards., Results: Of the 110 patients evaluated, 10 were in a postictal group (FDG injection after a seizure) and 100 were in the interictal group. The18 F-FDG PET/CT was concordant with the reference standards in nine (90%) of the postictal group patients and in 60 (60%) of the interictal group patients. Among the nine postictal group patients in whom the results were concordant, the18 F-FDG PET/CT showed hypermetabolism and hypometabolism in the epileptogenic zone in four (44.4%) and five (55.6%), respectively., Conclusion: Our data indicate that18 F-FDG PET/CT is a helpful tool for localization of the epileptogenic zone and that EEG monitoring is an important means of correlating the findings. In addition, postictal18 F-FDG PET/CT is able to identify the epileptogenic zone by showing either hypometabolism or hypermetabolism.- Published

Objective: To determine whether the whole-body tumor burden, as quantified by
18 F-fluorodeoxyglucose positron-emission tomography/computed tomography (18 F-FDG PET/CT), is a prognostic indicator in advanced (stage III or IV) non-small cell lung cancer (NSCLC)., Materials and Methods: This was a prospective study in which we evaluated18 F-FDG PET/CT staging parameters to quantify tumor burdens in patients with stage III or IV NSCLC. The following parameters were evaluated for the whole body (including the primary tumor) and for the primary tumor alone, respectively: maximum standardized uptake volume (wbSUVmax and tuSUVmax ); metabolic tumor volume (wbMTV and tuMTV); and total lesion glycolysis (wbTLG and tuTLG). To determine whether the18 F-FDG PET/CT parameters were associated with overall survival (OS) and progression-free survival (PFS), we evaluated the wbSUVmax /tuSUVmax , wbMTV/tuMTV, and wbTLG/tuTLG ratios., Results:18 F-FDG PET/CT was performed for staging in 52 patients who were followed for a median of 11.0 months (mean, 11.7 months). The estimated median PFS and OS were 9.6 months and 11.6 months, respectively. In the univariate analysis, OS was found to correlate significantly with wbTLG (hazard ratio [HR] = 1.001; 95% confidence interval [95 CI]: 1.000-1.001; p = 0.0361) and with the wbTLG/tuTLG ratio (HR = 1.705; 95% CI: 1.232-2.362; p = 0.0013). In the multivariate analysis, only the wbTLG/tuTLG ratio was independently associated with OS (HR = 1.660; 95% CI: 1.193-2.310; p = 0.0027)., Conclusion: The wbTLG/tuTLG ratio is an independent prognostic indicator of OS in advanced-stage NSCLC.- Published

Objective: The purpose of this study was to evaluate the effects of electrical muscle stimulation (EMS) on muscles, using
99m Tc-sestamibi SPECT/CT., Materials and Methods: We prospectively enrolled 20 consecutive male professional water polo players. The mean age was 25 years (range, 18-36 years). All athletes underwent99m Tc-sestamibi SPECT/CT of the thigh (rectus femoris and vastus medialis muscle groups) before and after EMS. Images were quantified to identify increases in perfusion after EMS., Results: Before EMS, there were no significant differences between the right and left thigh (rectus femoris and vastus medialis muscles) in terms of perfusion ( p = 0.4). However, the comparison between the pre- and post-EMS analyses of the same muscle groups showed significant differences in radiotracer uptake ( p < 0.001), with a mean increase in perfusion of 128% for the rectus femoris muscle group (95% CI: 0.86-1.61) and 118% for the vastus medialis muscle group (95% CI: 0.96-1.79)., Conclusion:99m Tc-sestamibi SPECT/CT is an objective means of evaluating blood flow in muscles submitted to EMS, which appears to promote significant increases in such blood flow.- Published

Objective: To determine whether
18 F-fluorodeoxyglucose positron emission tomography/computed tomography performed immediately after percutaneous ablation (iPA 18 F-FDG PET/CT) is useful in evaluating the outcomes of the procedure., Materials and Methods: This was a retrospective study of 20 patients (13 males, 7 females; mean age, 65.8 ± 12.1 years) submitted to percutaneous ablation of metastases. All of the lesions treated had shown focal uptake on a18 F-FDG PET/CT scan obtained at baseline. The primary tumors were mainly colorectal cancer (in 45%) or lung cancer (in 40%).iPA 18 F-FDG PET/CT was performed to identify any residual viable tumor cells. The treatment was considered a success (no viable tumor cells present) if no uptake of18 F-FDG was noted on theiPA 18 F-FDG PET/CT scan., Results: Twenty-six lesions were submitted to percutaneous ablation with either cryoablation (n = 7) or radiofrequency ablation (n = 19). The mean lesion diameter was 2.52 ± 1.49 cm. For the detection of viable tumor cells,iPA 18 F-FDG PET/CT had a sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of 66.7%, 95%, 88.5%, 80%, and 90.5%, respectively. There was a significant correlation between theiPA 18 F-FDG PET/CT findings and the results of the follow-up studies (kappa = 0.66; p < 0.01)., Conclusion:iPA 18 F-FDG PET/CT studies appear to constitute a useful means of evaluating the outcomes of percutaneous ablation. By detecting residual viable tumor cells, this strategy might allow early re-intervention, thus reducing morbidity. Studies involving larger numbers of patients are needed in order to confirm our findings.- Published

The objective of this study was to provide an overview of the clinical and laboratory characteristics of patients with the syndrome, and of the degree of severity of clinical manifestations resulting from the β-thalassemia mutation., Methods: A retrospective chart review was performed on 46 patients with sickle cell/β-thalassemia (31 Sβ° and 15 Sβ
+ ), evaluating hematological parameters and end organ damage. Statistical analyzes were carried out in order to highlight differences between the two groups according to the nature of the thalassemia mutation., Results: As expected, patients with the Sβ0 phenotype had a higher degree of hematological involvement in comparison to Sβ+ patients; with lower hemoglobin levels, and signs of more intense chronic hemolysis. However, Sβ+ patients were more prone to the occurrence of acute chest syndrome. The impact of the thalassemia mutation upon total body and bone composition was also evident, as Sβ0 patients presented lower body mass index (BMI) and bone mineral density. The degree of bone damage correlated to lower BMI and hemoglobin levels, as well as plaquetosis, monocytosis and elevated lactate dehydrogenase, possibly reflecting the effects of hemolysis and inflammation upon bone metabolism and body constitution., Conclusions: This study identified significant differences among sickle cell/β-thalassemia patients according to the beta mutation involvement, pointing to an important predictor of disease severity.- Published

- Abstract
Context and Objective: Studies using radionuclides are the most appropriate method for estimating renal function. Dimercaptosuccinic acid chelate labeled with technetium-99m (99mTc-DMSA) is the radiopharmaceutical of choice for high-resolution imaging of the renal cortex and estimation of the functional renal mass. The aim of this study was to evaluate a simplified method for determining the absolute renal uptake (ARU) of 99mTc-DMSA prior to nephrectomy, using the radioactivity counts of nephrectomy specimens as the gold standard., Design and Setting: Prospective study at the Division of Nuclear Medicine, Department of Radiology, Universidade Estadual de Campinas., Methods: Seventeen patients (12 females; range 22-82 years old; mean age 50.8 years old) underwent nephrectomy for various reasons. Renal scintigraphy was performed three to four hours after intravenous administration of a mean dose of 188.7 MBq (5.1 mCi) of 99mTc-DMSA, which was done six to 24 hours before surgery. The in vivo renal uptake of 99mTc-DMSA was determined using the radioactivity of the syringe before the injection (measured using a dose calibrator) and the images of the syringe and kidneys, obtained from a scintillation camera. After surgery, the reference value for renal uptake of 99mTc-DMSA was determined by measuring the radioactivity of the nephrectomy specimen using the same dose calibrator., Results: The ARU measurements were very similar to those obtained using the reference method, as determined by linear regression (r-squared = 0.96)., Conclusion: ARU estimation using the proposed method before nephrectomy seems to be accurate and feasible for routine use.
