BACKGROUND // The PaRIS Survey is an initiative led by the Organization for Economic Cooperation and Development in twenty-one countries, including Spain, to promote people-centred health care. The objective of the study aimed to describe PROMS and PREMS (Patient Reported Outcomes and Experience Measures, respectively) from patients who were in contact with Primary Care Centres, in order to establish a set of reliable, valid and internationally comparable indicators. METHODS // A cross-sectional study with two questionnaires will be carried out: one applied online for professionals from Primary Care Centres and another by telephone or online for patients aged forty-five and older of the same Health Care Centres. The domains covered are: characteristics of the Health Centre and of the professionals; sociodemographic characteristics of the patients; lifestyles; health care capacities; PROMs and PREMs. CONCLUSIONS // The study offers a unique opportunity to evaluate the health outcomes and experiences of the care received in Primary Care from patient’s perspective. This information is essential to help policymakers better understand the performance of their health system and how it could be improved, particularly in relation to chronic care in Primary Care., FUNDAMENTOS // La Encuesta de Indicadores Referidos por los Pacientes (PaRIS, por sus siglas en inglés) es una iniciativa liderada por la Organización para la Cooperación y el Desarrollo Económicos en veintiún países, incluyendo España, para promover una atención sanitaria centrada en las personas. El objetivo del estudio fue describir los resultados en salud y las experiencias referidas por los pacientes (PROMs y PREMs, por sus siglas en inglés Patient Reported Outcomes and Experience Measures, respectivamente) que tuvieron contacto con los Centros de Atención Primaria (CAP), con la finalidad de establecer un conjunto de indicadores confiables, válidos e internacionalmente comparables. MÉTODOS // Se realizará un estudio transversal, con dos cuestionarios: online para profesionales de los Centros de Atención Primaria y telefónico u online para los pacientes de cuarenta y cinco años y más, que acudieron a los CAP. Los dominios abarcados son: características del Centro de Atención Primaria y de los profesionales; características sociodemográficas de los pacientes; estilos de vida; capacidades; PROMs y PREMs. CONCLUSIONES // PaRIS ofrece una oportunidad para medir resultados de salud y las experiencias en Atención Primaria desde la perspectiva del paciente. Esta información es fundamental para ayudar a los gestores a comprender mejor el desempeño del sistema de salud y detectar posibilidades de mejora, particularmente en relación con la atención crónica.

CXC chemokine receptors 1 (CXCR1) and 2 (CXCR2) have high sequence similarity and overlapping chemokine ligand profiles. Residue positions 3.32 and 7.39 are critical for signal transduction in the related CXCR4, and in these positions CXCR1 and CXCR2 contain oppositely charged residues (Lys3.32 and Glu7.39). Experimental and computed receptor structures reveal the possible formation of a salt bridge between transmembrane (TM) helices 3 and 7 via these two residues. To investigate the functional importance of Lys1173.32 and Glu2917.39 in CXCR1, along with the flanking Glu1183.33, we performed a signaling study on 16 CXCR1 mutants using two different CXCL8 isoforms. While single Ala-mutation (K1173.32A, E2917.39A) and charge reversal (K1173.32E, E2917.39K) resulted in nonfunctional receptors, double (K1173.32E-E2917.39K) and triple (K1173.32E-E1183.33A-E2917.39K) mutants rescued CXCR1 function. In contrast, the corresponding mutations did not affect the CXCR2 function to the same extent. Our findings show that the Lys3.32-Glu7.39 salt bridge between TM3 and −7 is functionally important for CXCR1 but not for CXCR2, meaning that signal transduction for these highly homologous receptors is not conserved.

Decreased antigen presentation contributes to the ability of cancer cells to evade the immune system. We used the minimal gene regulatory network of type 1 conventional dendritic cells (cDC1) to reprogram cancer cells into professional antigen-presenting cells (tumor-APCs). Enforced expression of the transcription factors PU.1, IRF8, and BATF3 (PIB) was sufficient to induce the cDC1 phenotype in 36 cell lines derived from human and mouse hematological and solid tumors.Within 9 days of reprogramming, tumor-APCs acquired transcriptional and epigenetic programs associated with cDC1 cells. Reprogramming restored the expression of antigen presentation complexes and costimulatory molecules on the surfaces of tumor cells, allowing the presentation of endogenous tumor antigens on MHC-I and facilitating targeted killing by CD8+ T cells. Functionally, tumor-APCs engulfed and processed proteins and dead cells, secreted inflammatory cytokines, and cross-presented antigens to naïve CD8+ T cells. Human primary tumor cells could also be reprogrammed to increase their capability to present antigen and to activate patient-specific tumor-infiltrating lymphocytes. In addition to acquiring improved antigen presentation, tumor-APCs had impaired tumorigenicity in vitro and in vivo. Injection of in vitro generated melanoma-derived tumor-APCs into subcutaneous melanoma tumors delayed tumor growth and increased survival in mice. Antitumor immunity elicited by tumor-APCs was synergistic with immune checkpoint inhibitors. Our approach serves as a platform for the development of immunotherapies that endow cancer cells with the capability to process and present endogenous tumor antigens.

[Resumo] A fractura bimaleolar é unha lesión que ve aumentada a súa frecuencia ano a ano. O caso clínico presenta inestabilidade no nocello e un rango restrinxido de mobilidade articular da tibioperoneoastragalina como secuela dunha fractura bimaleolar dun ano de evolución. O obxetivo principal é proporcionar un plan de actuación estructurado que permita a recuperación da funcionalidade do nocello e mellorar a inestabilidade propia deste tipo de patoloxías. Metodoloxía: Realizouse unha exploración clínica en camilla, en estática e en dinámica, que permitiron valorar rangos articulares, grupos musculares e funcionalidade do pé tanto en descarga como en carga, así como cuestionarios autocompletados sobre calidade de vida e funcionalidade do pé subxetivos, e test de equilibrio estático e dinámico. Cos resultados obtivos propónse un plan de tratamento adecuado ás comorbilidades que presenta o doente. Resultados: Despois e 15 días de tratamento o doente presenta unha clara melloría en aspectos como equilibrio estático e dinámico, así como na súa percepción da funcionalidade do seu nocello, aínda que a rango de mobilidade articular da tibioperoneoastragalina dereita segue restrinxido Conclusións: As fractura bimaleolares aumentan en incidencia, e por conseguinte as súas secuelas; a inestabilidade crónica de nocello, é unha das máis frecuentes pero non hai estudios que traten o tema nin protocolos de actuación fronte a mesma desde un punto de vista conservador., [Resumen] La fractura bimaleolar es una lesión que ve aumentada su frecuencia año a año. El paciente del presente caso clínico presenta inestabilidad en el tobillo y un rango restringido de movilidad articular de la tibioperoneoastragalina como secuela de una fractura bimaleolar de un año de evolución. El objetivo principal es proporcionar un plan de actuación estructurado que permita la recuperación de la funcionalidad del tobillo y mejorar la inestabilidad propia de este tipo de patologías. Metodología: Se realizó una exploración clínica en camilla, en estática y en dinámica, que permitió valorar rangos articulares, grupos musculares y funcionalidad del pie tanto en descarga como en carga, así como cuestionarios autocompletados sobre calidad de vida y funcionalidad del pie subjetivos, junto con test de equilibrio estático y dinámico. Con los resultados obtenidos se propone un plan de tratamiento adecuado a las comorbilidades que presenta el paciente. Resultados: Después de 15 días de tratamiento el paciente presenta una clara mejoría en aspectos como equilibrio estático y dinámico, así como en su percepción sobre la funcionalidad de su tobillo, aunque el rango de movilidad articular de la tibioperoneoastragalina derecha sigue restringido Conclusiones: Las fractura bimaleolares aumentan en incidencia, y por conseguinte sus secuelas; la inestabilidad crónica de tobillo es una de las más frecuentes pero no hay estudios que traten el tema ni protocolos de actuación frente a la misma desde un punto de vista conservador., [Abstract] Bimalleolar fracture is an injury that is increasing in frequency year by year. The patient in the present clinical case presents with ankle instability and a restricted range of tibiofibularastragalus joint mobility as a sequel to a bimalleolar fracture of one year's evolution. The main objective is to provide a structured action plan that allows the recovery of ankle functionality and to improve the instability inherent to this type of pathology. Methodology: A clinical examination was carried out on a stretcher, in static and dynamic, which allowed us to assess joint ranges, muscle groups and functionality of the foot in both unloading and loading, as well as self-completed questionnaires on quality of life and functionality of the subjective foot, together with static and dynamic balance tests. The results obtained are used to propose a treatment plan appropriate to the patient's comorbidities. Results: After 15 days of treatment the patient shows a clear improvement in aspects such as static and dynamic balance, as well as in his perception of the functionality of his ankle, although the range of joint mobility of the right tibiofibularastragalus joint is still restricted. Conclusions: Bimalleolar fractures are increasing in incidence, and consequently their sequelae; chronic ankle instability is one of the most frequent, but there are no studies that deal with the subject or protocols for dealing with it from a conservative point of view.
