Donepezil, immune response, abeta - Abstract
Donepezil (DNPZ), an acetyl-cholinesterase inhibitor (AChEI), is widely used as a symptomatic drug in AD, given the involvement of cholinergic damage in the onset of memory loss. DNPZ, besides being able to improve acetylcholine (ACh) transmission, might have a neuroprotective effect reducing toxic Abeta fibrils and regulating the immune response. Considering that we previously showed a specific increase of plasma anti-Abeta1–42 antibodies in AD patients treated with AChEI, we evaluated in this study the role of DNPZ in favouring a Th2 phenotype, involved in modulating the immune humoral response. Moreover we hypothesized that this mechanism might be mediated by the a7-nicotinic ACh receptor expressed in lymphocytes. 60 patients with mild or moderate AD, either treated (n=22) or not (n=38) with DNPZ, and 30 controls were enrolled. AD DNPZ+ showed significantly higher plasma levels of anti-Abeta antibodies than DNPZ− (+40%) and lower levels of Abeta 1–42 than controls (−50%). In a subgroup of subjects, GATA-3, a transcription factor involved in Th2 differentiation, and a7nAChR expression was evaluated. No differences were found in GATA-3 mRNA in AD DNPZ+ when compared to DNPZ− and controls. On the opposite, we found by chromatine immunoprecipitation a significant three-fold increase of the association of GATA-3 with the IL-5-promoter in DNPZ+ patients, which also showed a significant twofold increase in a7-nAChR mRNA, with respect to DNPZ-ones. In vitro analyses demonstrated that the capacity of DNPZ to modulate GATA-3 expression is mediated by a7nAChR, since MLA, a specific antagonist, prevents it. Further studies are needed to better understand the role of DNPZ in modulating the immune response against Abeta, possibly ameliorating therapeutic strategies for AD

Background— Moderate, regular alcohol consumption is generally associated with a lower risk of cardiovascular events but data in patients with chronic heart failure are scarce. We evaluated the relations between wine consumption, health status, circulating biomarkers, and clinical outcomes in a large Italian population of patients with chronic heart failure enrolled in a multicenter clinical trial. Methods and Results— A brief questionnaire on dietary habits was administered at baseline to 6973 patients enrolled in the Gruppo Italiano per lo Studio della Sopravvivenza nell’Insufficienza Cardiaca-Heart Failure (GISSI-HF) trial. The relations between wine consumption, fatal and nonfatal clinical end points, quality of life, symptoms of depression, and circulating biomarkers of cardiac function and inflammation (in subsets of patients) were evaluated with simple and multivariable-adjusted statistical models. Almost 56% of the patients reported drinking at least 1 glass of wine per day. After adjustment, clinical outcomes were not significantly different in the predefined 4 groups of wine consumption. However, patients with more frequent wine consumption had a significantly better perception of health status (Kansas City Cardiomyopathy Questionnaire score, adjusted P P =0.01), and lower plasma levels of biomarkers of vascular inflammation (osteoprotegerin and C-terminal proendothelin-1, adjusted P P =0.01) after adjusting for possible confounders. Conclusions— We show for the first time in a large cohort of patients with chronic heart failure that moderate wine consumption is associated with a better perceived and objective health status, lower prevalence of depression, and less vascular inflammation, but does not translate into more favorable clinical 4-year outcomes. - Abstract
OBJECTIVES: The one-carbon metabolism, also known as methionine-homocysteine cycle, governs the dynamics of DNA methylation, epigenetically regulating gene expression, and has been reported altered in anorexia nervosa (AN) adult patients. The aim of this study consisted in assessing whole-blood DNA methylation in adolescent AN patients, assessing its significance in relationship to clinical and hormonal variables. METHODS: Whole-blood global DNA methylation was measured as incorporation of [(3)H]dCTP following HpaII cut in 32 adolescent females affected by restrictive type AN and compared to 13 healthy controls. Homocysteine, vitamin B12 and folate plasma levels were assessed as well as fasting plasma levels of leptin and steroid hormones. Clinical variables, including severity and associate states and traits, were assessed by means of the EDI-3, CDI and STAI-Y scales. RESULTS: We confirm that whole-blood global DNA methylation is modestly albeit significantly reduced in AN adolescents with respect to controls, correlating with plasma leptin and steroid hormone levels. Conversely, clinical traits did not correlate with the outcome variable. CONCLUSIONS: A better definition of the epigenetic dysregulation underlying AN pathology or vulnerability might lead to develop useful markers for diagnosis, prognostic classification and tailored therapeutic interventions in these vulnerable patients since the earliest phases of their disease.

- Abstract
Donepezil (DNPZ), an acetyl-cholinesterase inhibitor (AChEI), is widely used as a symptomatic drug in AD, given the involvement of cholinergic damage in the onset of memory loss. DNPZ, besides being able to improve acetylcholine (ACh) transmission, might have a neuroprotective effect reducing toxic Abeta fibrils and regulating the immune response. Considering that we previously showed a specific increase of plasma anti-Abeta1–42 antibodies in AD patients treated with AChEI, we evaluated in this study the role of DNPZ in favouring a Th2 phenotype, involved in modulating the immune humoral response. Moreover we hypothesized that this mechanism might be mediated by the a7-nicotinic ACh receptor expressed in lymphocytes. 60 patients with mild or moderate AD, either treated (n=22) or not (n=38) with DNPZ, and 30 controls were enrolled. AD DNPZ+ showed significantly higher plasma levels of anti-Abeta antibodies than DNPZ− (+40%) and lower levels of Abeta 1–42 than controls (−50%). In a subgroup of subjects, GATA-3, a transcription factor involved in Th2 differentiation, and a7nAChR expression was evaluated. No differences were found in GATA-3 mRNA in AD DNPZ+ when compared to DNPZ− and controls. On the opposite, we found by chromatine immunoprecipitation a significant three-fold increase of the association of GATA-3 with the IL-5-promoter in DNPZ+ patients, which also showed a significant twofold increase in a7-nAChR mRNA, with respect to DNPZ-ones. In vitro analyses demonstrated that the capacity of DNPZ to modulate GATA-3 expression is mediated by a7nAChR, since MLA, a specific antagonist, prevents it. Further studies are needed to better understand the role of DNPZ in modulating the immune response against Abeta, possibly ameliorating therapeutic strategies for AD

- Abstract
Background: The diagnosis of amyotrophic lateral sclerosis (ALS) requires the careful exclusion of mimic syndromes in presence of involvement of both upper and lower motor neurons progressing over time. In the differential diagnosis myopathic processes must be considered. In particular, inclusion body myositis patients are often misdiagnosed as having ALS, due to the fact that these two conditions share some clinical and serological similarities. Other myopathies, such as polymyositis are even rarer and, as a general rule, a diagnosis of inflammatory myopathy tends to exclude ALS. Aim: Describe the case of an ALS patient that was subsequently co-diagnosed with polymyositis. Case Report: A 51-year-old woman with a 4-month history of distal muscle weakness and progressive hypotrophy at the upper limbs came to our attention. Her past medical history was significant for hypothyroidism. Neurological examination showed intact cranial nerves, diffuse mild bilateral weakness with wasting of the interossei muscles at the hands and evident fasciculations; deep tendon reflexes were increased at the upper limbs and decreased at the lower ones. Both brain and spinal cord MRI were normal, as it was the exam of the CSF. An EMG study revealed signs of acute denervation and fasciculations in both distal and proximal muscle of the arms; myopathic changes were not reported. Due to the unexpected bilateral worsening of proximal weakness at the lower limbs, the patient underwent a muscle biopsy (vastus lateralis), which revealed the coexistence of neurogenic alterations and myopathic features, compatible with an inflammatory myopathy. An oral steroid course was started but the patient failed to respond, deteriorating further until tracheostomy. Discussion: Polymyositis can rarely mimic ALS but this is, to our knowledge, the first report of cooccurrence of these two disorders. - Abstract
Aims To analyse the safety and impact on maintenance of sinus rhythm of transoesophageal echocardiographically guided early cardioversion associated with short-term anticoagulation in a large series of patients with atrial fibrillation and atrial flutter. Methods and Results Patients who were candidates for cardioversion were eligible for inclusion if they had atrial fibrillation or atrial flutter lasting longer than 2 days or of unknown duration. Patients received short-term anticoagulation with warfarin or heparin and underwent transthoracic echocardiography followed by transoesophageal echocardiography. Early cardioversion was performed if no thrombus was seen on the transoesophageal study. Warfarin was maintained for 1 month after cardioversion. In patients with atrial thrombi, cardioversion was deferred and prolonged anticoagulation was prescribed. The study population included 183 patients. One hundred and sixty nine patients without atrial thrombi underwent early cardioversion. Fourteen patients with atrial thrombi (7·6%) underwent a second transoesophageal echocardiogram after a median of 4 weeks of oral warfarin, and cardioversion was performed if clot regression was documented. No patient in our study population had a clinical thromboembolic event at 1 month follow-up (95% C.I. 0–0·016). The immediate success rate of cardioversion was better among patients with atrial fibrillation <4 weeks duration compared with patients with atrial fibrillation of longer or of unknown duration: 96·6% vs 85%, respectively (P=0·014). At 1 month follow-up, the percentage of arrhythmia relapses in patients with initially successful cardioversion was similar in the two groups (29% vs 26%, P=ns); thus the initial better outcome in patients with recent-onset arrhythmia was not lost. Conclusion Transoesophageal echocardiography-guided early cardioversion in concert with short-term anticoagulation is safe. This approach permits abbreviation of the overall duration of atrial fibrillation and has a better impact on the maintenance of sinus rhythm for patients in whom the duration of atrial fibrillation is <4 weeks. - Abstract
Tilt induced prolonged asystole has been considered to identify a distinct subgroup of patients with neurally mediated syncope and management including permanent pacemaker implantation has been suggested. To evaluate the reproducibility of asystolic response during head-up tilt testing (HUT), 33 patients with neurally mediated syncope and asystolic response (≥3 seconds) during HUT prospectively underwent two consecutive tests 13±15 days apart. On repeat tilt testing asystole was reproduced in 12 patients (36%), while 8 patients still had a positive HUT, but without asystole. Remarkably, 13 patients (40%) had a negative repeat HUT. Among 12 patients with asystole on both HUTs there was no significant difference in duration of asystole (14 371±11 430 ms vs 13 707±10 470 ms,P =ns) and time to syncope (36±20 min vs 37±20 min, P=ns) during initial and repeat HUTs. - Abstract
Head-up tilt testing (HUTT) potentiated with sublingual nitroglycerin has gained acceptance as means of diagnosing neurally mediated syncope. To evaluate the reproducibility of HUTT potentiated with sublingual nitroglycerin, 48 patients with unexplained syncope prospectively underwent 2 consecutive tests 1 to 28 days apart. The initial test ended in syncope in 34 patients (71%). In 9 patients (19%) the test was positive during the drug-free phase, whereas 25 patients (52%) had syncope after nitroglycerin administration. Of these 34 patients with an initial positive test result, 27 (79%) had a reproducible outcome on repeat testing. Of 12 patients (25%) with an initial negative test result, 10 (83%) had a reproducible outcome on repeat testing. Of 2 patients (4%) with a first test ending in exaggerated response, both had a negative repeat test response. The overall reproducibility of sublingual nitroglycerin tilt-table testing was 77%. In a group of 23 patients with both positive tests, 19 (83%) had the same response modality (2 vasodepressor, 4 cardioinhibitory, 13 mixed response). In the same group of patients, individual trough heart rates correlated well with each other between tests. Finally, in the 27 patients with both positive tests, intrapatient time of onset of symptoms did not significantly correlate between tests. Thus, in patients with syncope of unknown origin, HUTT potentiated with sublingual nitroglycerin provides an adequate reproducibility when repeated on different days. [ABSTRACT FROM AUTHOR]

Background The independent prognostic impact of diabetes mellitus ( DM ) and prediabetes mellitus (pre‐ DM ) on survival outcomes in patients with chronic heart failure has been investigated in observational registries and randomized, clinical trials, but the results have been often inconclusive or conflicting. We examined the independent prognostic impact of DM and pre‐ DM on survival outcomes in the GISSI ‐HF (Gruppo Italiano per lo Studio della Sopravvivenza nella Insufficienza Cardiaca‐Heart Failure) trial. Methods and Results We assessed the risk of all‐cause death and the composite of all‐cause death or cardiovascular hospitalization over a median follow‐up period of 3.9 years among the 6935 chronic heart failure participants of the GISSI ‐ HF trial, who were stratified by presence of DM (n=2852), pre‐ DM (n=2013), and non‐ DM (n=2070) at baseline. Compared with non‐ DM patients, those with DM had remarkably higher incidence rates of all‐cause death (34.5% versus 24.6%) and the composite end point (63.6% versus 54.7%). Conversely, both event rates were similar between non‐ DM patients and those with pre‐ DM . Cox regression analysis showed that DM , but not pre‐ DM , was associated with an increased risk of all‐cause death (adjusted hazard ratio, 1.43; 95% CI , 1.28–1.60) and of the composite end point (adjusted hazard ratio, 1.23; 95% CI , 1.13–1.32), independently of established risk factors. In the DM subgroup, higher hemoglobin A1c was also independently associated with increased risk of both study outcomes (all‐cause death: adjusted hazard ratio, 1.21; 95% CI , 1.02–1.43; and composite end point: adjusted hazard ratio, 1.14; 95% CI , 1.01–1.29, respectively). Objective: Donepezil (DNPZ) is a drug commonly used for Alzheimer's disease (AD) that may favour a T helper 2 phenotype leading to increased naturally occurring auto-antibodies (NAb) against beta-amyloid (Aβ). We hypothesized the involvement of the cholinergic receptors [α7-nicotnic acetylcholine receptor (α7nAChR)] expressed on peripheral blood mononuclear cells (PBMC). Methods: Fifty patients with mild-to-moderate AD, DNPZ treated (DNPZ+, n = 25) or not (DNPZ−, n = 25), and 25 matched controls were enrolled and PBMC extracted for both in vitro cultures, and real-time polymerase chain reaction and chromatin immunoprecipitation assay. Plasma samples were also obtained for Aβ and NAb determination. Results: Donepezil increased in vitro the expression of the transcription factor GATA binding protein 3 (GATA3) through α7nAChR, because prevented by the specific antagonist methyllycaconitine. Ex vivo PBMC α7nAChR mRNA expression was increased in both AD groups, while GATA3 expression was not. A significant increase in the GATA3/interleukin 5 promoter association was found in DNPZ+ patients. Finally, DNPZ+ patients showed both significantly higher plasma levels of anti-Aβ NAb with respect to DNPZ− patients and Aβ 1-42 with respect to normal controls. Conclusions: Donepezil might modulate a T helper 2 bias via α7nAChR leading to increased expression of NAb. Further studies on the role of the modulation of the immune response against Aβ may pave the way to innovative therapeutic strategies for AD..

Objectives. The one-carbon metabolism, also known as methionine-homocysteine cycle, governs the dynamics of DNA methylation, epigenetically regulating gene expression, and has been reported altered in anorexia nervosa (AN) adult patients. The aim of this study consisted in assessing whole-blood DNA methylation in adolescent AN patients, assessing its signifi cance in relationship to clinical and hormonal variables. Methods. Whole-blood global DNA methylation was measured as incorporation of [ 3 H]dCTP following Hpa II cut in 32 adolescent females affected by restrictive type AN and compared to 13 healthy controls. Homocysteine, vitamin B 12 and folate plasma levels were assessed as well as fasting plasma levels of leptin and steroid hormones. Clinical variables, including severity and associate states and traits, were assessed by means of the EDI-3, CDI and STAI-Y scales. Results. We confi rm that whole-blood global DNA methylation is modestly albeit signifi cantly reduced in AN adolescents with respect to controls, correlating with plasma leptin and steroid hormone levels. Conversely, clinical traits did not correlate with the outcome variable. Conclusions. A better defi nition of the epigenetic dysregulation underlying AN pathology or vulnerability might lead to develop useful markers for diagnosis, prognostic classifi cation and tailored therapeutic interventions in these vulnerable patients since the earliest phases of their disease.

Background: The diagnosis of amyotrophic lateral sclerosis (ALS) requires the careful exclusion of mimic syndromes in presence of involvement of both upper and lower motor neurons progressing over time. In the differential diagnosis myopathic processes must be considered. In particular, inclusion body myositis patients are often misdiagnosed as having ALS, due to the fact that these two conditions share some clinical and serological similarities. Other myopathies, such as polymyositis are even rarer and, as a general rule, a diagnosis of inflammatory myopathy tends to exclude ALS. Aim: Describe the case of an ALS patient that was subsequently co-diagnosed with polymyositis. Case Report: A 51-year-old woman with a 4-month history of distal muscle weakness and progressive hypotrophy at the upper limbs came to our attention. Her past medical history was significant for hypothyroidism. Neurological examination showed intact cranial nerves, diffuse mild bilateral weakness with wasting of the interossei muscles at the hands and evident fasciculations; deep tendon reflexes were increased at the upper limbs and decreased at the lower ones. Both brain and spinal cord MRI were normal, as it was the exam of the CSF. An EMG study revealed signs of acute denervation and fasciculations in both distal and proximal muscle of the arms; myopathic changes were not reported. Due to the unexpected bilateral worsening of proximal weakness at the lower limbs, the patient underwent a muscle biopsy (vastus lateralis), which revealed the coexistence of neurogenic alterations and myopathic features, compatible with an inflammatory myopathy. An oral steroid course was started but the patient failed to respond, deteriorating further until tracheostomy. Discussion: Polymyositis can rarely mimic ALS but this is, to our knowledge, the first report of cooccurrence of these two disorders. Muscle biopsy is required in order to confirm or discard this hypothesis. Moreover, albeit our case did not respond to treatment, a correct diagnosis could be extremely important due to the possible favourable response of polymyositis to steroids.

- Abstract
The seizure type most frequently described in GLUT1 deficiency is generalized (mainly absence). We report the case of a young boy who, as the main clinical manifestation presented with focal non-motor, and then focal motor seizures. At the age of 3 months episodes of face pallor/cyanosis and hypotonus lasting about 1 min, occurred. They were initially misdiagnosed as gastroesophageal reflux. These episodes disappeared spontaneously at 6 months of age. At 12 months, episodes similar to the previous ones reappeared. A few months later, a cluster of several episodes manifest as impaired responsiveness and vomiting occurred. The patient initially performed long-term video-EEG monitoring (LTVEM) however, no seizures were captured. During a second hospitalization for LTVEM, a focal to bilateral clonic seizure was recorded. Brain MRI was normal. Next Generation Sequencing (NGS) panel for genes associated with epilepsy showed a de novo mutation of SCL2A1 gene. The CSF showed glucose of 41 mg/dL, and the CSF/serum glucose ratio was equal to 0.46. The ketogenic diet was started with optimal efficacy in seizure control. Meal-sensitivity in childhood onset focal seizures may be associated with GLUT-1 deficiency syndrome that can be confirmed by biochemical analysis on blood and CSF following diagnostic genetic study., Competing Interests: The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (© 2022 The Author(s).)

- Abstract
Aim: The aim of this study was to describe the characteristics of Posterior Reversible Encephalopathy Syndrome (PRES) in infants and young children (<6 years) and to compare them with the older pediatric population affected by PRES., Methods: we retrospectively reviewed records of 111 children (0-17 years) diagnosed with PRES from 2000 to 2018 in 6 referral pediatric hospitals in Italy. The clinical, radiological and EEG features, as well as intensive care unit (ICU) admission rate and outcome of children aged <6 years were compared to those of older children (6-17 years). Factors associated with ICU admission in the whole pediatric cohort with PRES were also evaluated., Results: Twenty-nine patients younger than 6 years (26%) were enrolled with a median age at onset of PRES of 4 years (range: 6 months-5 years). Epileptic seizures were the most frequent presentation at the disease onset (27/29 patients). Status epilepticus (SE) was observed in 21/29 patients: in detail, 11 developed convulsive SE and 10 presented nonconvulsive SE (NCSE). SE was more frequent in children <6 years compared with older children (72% vs 45%) as well as NCSE (35% vs 10%). Seventeen children aged <6 years required ICU admission. Prevalence of ICU admissions was higher within younger population compared to older (59% vs 37%). - Abstract
Sudden cardiac death in the young is a relatively uncommon but marked event usually related to congenital diseases or anomalies. Despite the prevalence of each condition being variable, most common causes include primary myocardial diseases and arrhythmic disorder, frequently with inheritance pattern. Sudden cardiac death is usually preceded by symptoms, thus making personal and family history fundamental for its prevention. Nevertheless, in more than 50% of cases, sudden cardiac death is the first manifestation of the disease. In this review, we describe the different causes of sudden cardiac death, their incidence, and currently used preventive strategies.

Aims: Several trials demonstrated the life saving role of implantable cardioverter-defibrillators (ICD) in primary prevention of sudden cardiac death (SCD). The aim was to evaluate the clinical characteristics and 4-year outcome of consecutive patients treated in clinical practice by prophylactic ICD implantation on the basis of class I recommendations and up-to-date ICD programming., Methods and Results: IRIDE multi-center, prospective and observational study enrolled 604 consecutive patients (mean age: 66 ± 10 years) treated by ICD between 01/01/2006 and 30/06/2010. Main characteristics were similarly distributed among the inclusion criteria of MADIT II (24%), SCD-HeFT (24%), COMPANION (26%) and MADIT-CRT (18%) trials, while a small number of patients met the MUSTT and MADIT (7%) inclusion criteria. Single-chamber ICDs were implanted in 168 (28%) patients, dual-chamber in 167 (28%) and biventricular in 269 (43%) patients. ATP programming was activated in 546 (90%) patients. Overall survival and rate of appropriate ICD intervention by ATP and/or shock at 12-24-36-48 months of follow-up were 94%, 89%, 80%, 75% and 16%, 28%, 37% and 50%, respectively. No difference in mortality rate between the groups who received or did not receive appropriate ICD interventions was demonstrated (p=ns)., Conclusions: The IRIDE study confirms the effectiveness in real world practice of ICD implantation in patients at risk of SCD. The life saving role of ICD therapy increases as the duration of follow-up is prolonged and the survival benefit is similar in patients who received or did not receive appropriate device treatment, thus suggesting a beneficial effect of up-to-date device programming., (Copyright © 2013 Elsevier Ireland Ltd. - Abstract
Background: Most witnessed out-of-hospital cardiac arrests (OHCA) do not receive bystander cardiopulmonary resuscitation (CPR). The incidence of laypersonsO' CPR could be increased by widespread training. We evaluated the effect of distribution of CPR educational material to high-school students in the area of Como, Italy., Methods and Results: From January 2008 to October 2009, we distributed 3200 resuscitation manikins to pupils (62% boys, mean age 16.5 ± 0.8 years) at 20 high schools in the area of Como. All students received a kit including a personal manikin and a 27 min educational digital video disc. Furthermore, they received 40 min school training in the fundamental maneuvers of CPR utilizing the manikin. Afterwards, they were encouraged to train friends and relatives at home (second tier), utilizing the kit. Eight months later, a questionnaire was randomly submitted to a sample of 600 students (19% of the distributed manikins). The 600 kits had been used to train 1058 from the second tier (mean, 1.77 persons per pupil; 95% confidence interval 1.62-1.93). Boys had a significantly lower multiplier effect than girls: 1.45 ± 1.01 vs. 2.26 ± 1.89 (P < 0.0001). Ninety-five percent of pupils considered themselves sufficiently trained in CPR and 62.3% declared their availability to effectively practice CPR if necessary. One pupil performed an effective CPR during her mother's OHCA., Conclusions: CPR training can be disseminated using personal manikins distributed to pupils. Most students declared themselves trained and willing to start bystander CPR if necessary. One successful CPR was effectively performed.

- Abstract
In patients with beta-thalassemia major cardiac events due to iron overload are the main cause of death. Moreover, a chronic hypercoagulable state has been described in these patients, which sometimes complicates with thromboembolic events, mainly cerebral ischemic accidents and deep venous thrombosis. We describe a case of cardiac hemochromatosis complicated with fatal massive multiorgan embolism due to a large left ventricular thrombus in a patient with beta-thalassemia major.

- Abstract
Atrial fibrillation is the most commonly encountered sustained arrhythmia. Echocardiography has augmented the knowledge about etiology and complications of atrial fibrillation. Transthoracic echocardiography allows rapid, safe and comprehensive assessment of cardiac structure and function, and is recommended for all subjects with atrial fibrillation. The use of transesophageal echocardiography has contributed to a better understanding of the thromboembolic risk in patients with atrial fibrillation, especially in the setting of electrical cardioversion. Several investigators have demonstrated the feasibility and safety profile of early cardioversion with short-term anticoagulation in patients with atrial fibrillation and a transesophageal echocardiography negative for atrial thrombi. More recently, transesophageal and intracardiac echocardiography have been employed in patients with atrial fibrillation to monitor percutaneous procedures such as pulmonary veins radiofrequency ablation or left atrial appendage obliteration. In this review the available echocardiographic imaging modalities and their specific role in the evaluation and management in atrial fibrillation are described.

A 50-year-old women with Charcot-Marie-Tooth hereditary neuropathy type 1A due to the PMP22 duplication on chromosome 17p11.2-12 developed a left bundle branch block and progressive dilatation of the left ventricle since age 40 years and recurrent heart failure since age 44 years. At age 50 years left ventricular hypertrabeculation/noncompaction was first recognized on transthoracic echocardiography.
