2. The effect of toroidicity on relaxed states at low aspect ratio

The experimentally measured single helical states in the low aspect ratio (R/a=2) RELAX device have been compared with the predictions of a cylindrical relaxation theory and a substantial deviation in terms of some macroscopic characteristics has been found, in particular for cases corresponding to a large reversal of the edge toroidal field. In this paper we consider the effect of the toroidal geometry by employing the VMEC and the RelaxFit equilibrium codes. We show that the inclusion of toroidicity produces significant corrections to the cylindrical states, especially for the deep reversed cases.

Ophelia syndrome is paraneoplastic limbic encephalitis (PLE) with Hodgkin lymphoma. Some Ophelia syndrome patients have been reported as testing positive for anti-metabotropic glutamate receptor 5 (mGluR5) antibodies. However, we experienced a case of anti-mGluR5 antibody-negative Ophelia syndrome. The type of onset, neurological symptoms, and imaging as well as electroencephalographic findings were like previous reports except for a normal cell count in cerebrospinal fluid (CSF). Unfortunately, a lymph node biopsy failed and could not diagnose the patient before death because steroid treatment for limbic encephalitis had shrunk lymph nodes. We believe it is essential to accumulate cases of this syndrome and clarify the association between PLE and Hodgkin lymphoma so chemotherapy can be initiated even if malignant lymphoma cannot be pathologically proven or when antibodies cannot be measured or are negative.

A recent study [1] about the scaling with the aspect ratio (i.e. the ratio between the major and minor radius of the torus) of the dominant mode in Single Helical (SH) Reverse Field Pinch (RFP) plasmas has shown, at intermediate aspect ratio, that the dominant toroidal mode number in the helical states can be interpreted as the result of a relaxation process. Reliable equilibrium reconstruction is indispensable for understanding and controlling hot magnetized plasmas to achieve fusion reactors. In axisymmetric systems, current and pressure profiles that satisfy the force balance conditions are given by the Grad–Shafranov (GS) equation. While many novel approaches have been developed to swiftly and robustly find an optimum solution of the GS equation, approaches based on a single solution search may not be adaptable if diagnostics fail to provide sufficient constraints. Here, we investigate the solution space of the GS equation when only basic edge magnetic measurements are available. By combining Gaussian process regression and Markov chain Monte Carlo sampling within the Bayesian framework, we treat each current element as an independent variable and evaluate the probability distribution that describes all possible solutions. We have applied this inference frame to the geometry of the PLATO tokamak and shown that the flux surface locations can be determined relatively well only from 16 pick-up coils, 4 flux loops and a diamagnetic loop. On the other hand, the toroidal current density is inferred with limited success, and the inferences of the safety factor and pressure profiles are difficult. The characterization of possible choices of equilibria realized by this inference framework will help optimize diagnostic setups for equilibrium reconstruction. [ABSTRACT FROM AUTHOR]

Superconducting (SC) tokamak JT-60SA plays an essential role in fusion research and development by supporting and complementing the ITER project, providing directions to the DEMO design activity and fostering next generation scientists and engineers. Since the short circuit incident at the terminal joints of equilibrium field coil #1 during the integrated commissioning (IC) in March 2021, both EU and JA implementing agencies (IAs) have examined how to ensure safe operation of JT-60SA by mitigating the risk of possible discharge occurrence inside the cryostat. Based on the experience of the global Paschen tests, the IAs have established a strategy of risk mitigation measures, which is a combination of (i) reinforcement of insulation, (ii) avoiding unnecessary voltage application to the coil systems and (iii) immediate de-energization of the coils when deteriorated vacuum conditions are detected. Thanks to the considerable efforts of the Integrated Project Team members, the IC restarted in May 2023. After confirmation of the SC state of the coil systems (TF, EF and CS), the coil energization test and the plasma operation phase 1 (OP-1) started. The first plasma was successfully achieved on 23 October 2023 with a limited value of voltage and current applied to the coils. The plasma configuration control was also confirmed with low plasma current and low auxiliary heating power conditions. Based on the IO–F4E–QST collaboration, activities of JT-60SA have been shared with the IO and provided an important lesson for ITER assembly and commissioning, and will provide an outstanding contribution to fusion research at large. After OP-1, maintenance & enhancement phase 1 (M/E-1) starts from January 2024, in which in-vessel components are installed, and heating and diagnostic systems are extensively upgraded to allow a high power heating experiment planned in OP-2. In order to make the best use of JT-60SA, a newly organized JT-60SA experiment team will refine the research plan for the future high heating power operation phase.

15. Distressing Belching with Chorea Induced by Caudate Infarction

Although belching is mostly associated with gastrointestinal disorders, it occasionally accompanies movement disorders such as Parkinsonism or dystonia. A woman in her 80s presented distressing belching and chorea of the right arm and leg from 3 years earlier. A brain MRI showed a left caudate infarction and atrophic change. Haloperidol significantly improved belching and chorea. Caudate infarction can cause distressing belching with chorea. It might be important to select the appropriate drug by referring to the accompanying involuntary movement to treat belching with movement disorders.

17. A Combinatorial Code for CPEB-Mediated c-myc Repression

During early embryonic development, the RNA-binding protein CPEB mediates cytoplasmic polyadenylation and translational activation through a combinatorial code defined by the cy-toplasmic polyadenylation element (CPE) present in maternal mRNAs. However, in non-neuronal somatic cells, CPEB accelerates deadenylation to repress translation of the target, including c-myc mRNA, through an ill-defined cis-regulatory mechanism. Using RNA mutagenesis and electrophoretic mobility shift assays, we demonstrated that a combination of tandemly arranged consensus (cCPE) and non-consensus (ncCPE) cytoplasmic polyadenylation elements (CPEs) constituted a combinatorial code for CPEB-mediated c-myc mRNA decay. CPEB binds to cCPEs with high affinity (Kd = ~250 nM), whereas it binds to ncCPEs with low affinity (Kd > ~900 nM). CPEB binding to a cCPE enhances CPEB binding to the proximal ncCPE. In contrast, while a cCPE did not activate mRNA degradation, an ncCPE was essential for the induction of degradation, and a combination of a cCPE and ncCPEs further promoted degradation. Based on these findings, we propose a model in which the high-affinity binding of CPEB to the cCPE accelerates the binding of the second CPEB to the ncCPEs, resulting in the recruitment of deadenylases, acceleration of deadenylation, and repression of c-myc mRNAs.

Neuronal intranuclear inclusion disease (NIID) is a neurodegenerative disorder that is caused by the abnormal expansion of non-coding trinucleotide GGC repeats in NOTCH2NLC. NIID is clinically characterized by a broad spectrum of clinical presentations. To date, the relationship between expanded repeat lengths and clinical phenotype in patients with NIID remains unclear. Thus, we aimed to clarify the genetic and clinical spectrum and their association in patients with NIID. For this purpose, we genetically analyzed Japanese patients with adult-onset NIID with characteristic clinical and neuroimaging findings. Trinucleotide repeat expansions of NOTCH2NLC were examined by repeat-primed and amplicon-length PCR. In addition, long-read sequencing was performed to determine repeat size and sequence. The expanded GGC repeats ranging from 94 to 361 in NOTCH2NLC were found in all 15 patients. Two patients carried biallelic repeat expansions. There were marked heterogenous clinical and imaging features in NIID patients. Patients presenting with cerebellar ataxia or urinary dysfunction had a significantly larger GGC repeat size than those without. This significant association disappeared when these parameters were compared with the total trinucleotide repeat number. ARWMC score was significantly higher in patients who had a non-glycine-type trinucleotide interruption within expanded poly-glycine motifs than in those with a pure poly-glycine expansion. These results suggested that the repeat length and sequence in NOTCH2NLC may partly modify some clinical and imaging features of NIID.

26. Evaluation of the applicability of weak shoulder and arm sparing signs in amyotrophic lateral sclerosis by multiple neurologists and neurology residents: A single‐center study.

- Abstract
Introduction/Aims: Amyotrophic lateral sclerosis (ALS) exhibits selective muscle weakness. The weak shoulder and arm sparing signs, assessed by a single experienced neurologist, have been reported to be superior to previous signs in sensitivity and specificity. However, it is unknown whether the same results are observed when assessed by multiple neurologists. Methods: Subjects were retrospectively identified from our department's inpatient database from 2014 to 2023. Medical Research Council (MRC) scores of the deltoid (Del), biceps brachii (BB), triceps brachii (TB), and first dorsal interosseous (FDI) muscles were evaluated. The weak shoulder sign was defined as positive when Del was weaker than BB and TB. The arm sparing sign was defined as positive when both Del and FDI were weaker than BB and TB. Sensitivity was analyzed in all ALS patients and in subgroups based on the region of symptom onset, presence or absence of upper motor neuron (UMN) signs, and the Japanese ALS Severity Classification. Results: Seventy‐one patients with ALS were identified. Eight neurologists and three neurology residents evaluated each patient's MRC scores. The weak shoulder and arm sparing signs were observed in 72% and 48% of patients, respectively, with no significant difference in sensitivity across patient subgroups. Discussion: The weak shoulder and arm sparing signs showed high and moderate sensitivity, respectively, consistent with a previous report, even when evaluated by multiple examiners. This expands the clinical utility and increases the reliability of these signs, potentially contributing to accurate ALS diagnosis when combined with other clinical features and objective assessments. This paper presents the development of an all-in-one probe to simultaneously measure all components of the generalized Ohm's law in reversed-field pinch plasmas and tokamaks. The polyhedral configuration of the Mach probe is achieved through the specific arrangement, angle, and depth of the collimator channel apertures drilled into the surface of a hollow boron nitride cylinder encasing it. This probe includes a central Mach probe to assess the ion velocity field in three dimensions. Initial tests at the RELAX and Madison Symmetric Torus machines have confirmed the probe's effectiveness, revealing an octahedron form similar to a tetrahedron. The probe seems to function correctly and is expected to facilitate the empirical validation of two-fluid equilibria at the periphery of toroidal plasmas. [ABSTRACT FROM AUTHOR]

Metformin causes metabolic encephalopathy in some patients with end-stage chronic kidney disease, resulting in impaired consciousness and parkinsonism. This encephalopathy has a very characteristic magnetic resonance imaging feature in lentiform nuclei known as the “lentiform fork sign”. However, the mechanism is unknown. Here, we report a case of metformin-induced encephalopathy with a novel observation of lactate accumulation in the lentiform nuclei on magnetic resonance spectroscopy without systemic lactic acidosis. Since metformin is an inhibitor of mitochondrial complex-I, this focal brain lactate accumulation implies that a part of the pathogenesis of metformin-induced encephalopathy is the focal vulnerability of mitochondria to metformin in the lentiform nuclei. When metformin causes encephalopathy, not only testing for serum lactic acidosis and performing routine magnetic resonance imaging but also evaluation of brain lactate accumulation by magnetic resonance spectroscopy should be required to elucidate the etiology.

40. Guillain-Barré Syndrome and Posterior Reversible Encephalopathy Syndrome following Spinal Surgery

Guillain-Barré syndrome (GBS) typically occurs after gastroenteritis and respiratory tract infection, but surgery has also been considered one of the triggers. Posterior reversible encephalopathy syndrome (PRES) is a rare complication of GBS. A normotensive female in her 70s presented ascending paralysis and frontal-parieto-occipital subcortical lesions with intermittent hypertension after spinal surgery. Nerve conduction studies revealed demyelinating polyneuropathy. The patient’s brain lesions disappeared with amelioration of hypertension. She was diagnosed with the demyelinating form of GBS and PRES caused by intermittent hypertension. Intravenous immunoglobulin G (IVIG) improved her symptoms without exacerbation of the PRES. Surgery can be a trigger of GBS, and GBS can cause PRES by hypertension and present as central nervous lesions. It is important to treat hypertension before using IVIG when PRES is suspected as a complication of GBS, since the encephalopathy can be exacerbated by IVIG. There may be more undiagnosed cases of the coexistence of GBS and PRES after surgery because surgery itself can also cause PRES. Proper control of blood pressure and confirmation of negative central nervous lesions are required to treat GBS patients with IVIG safely.

The Mn-matrix was defined by Mohan [20] in which he has shown a method of constructing (1,-1)-matrices and studied some of their properties. The (1,-1)-matrices were constructed and studied by Cohn [5],Wang [33], Ehrlich [8] and Ehrlich and Zeller[9]. But in this paper, while giving some resemblances of this matrix with Hadamard matrix, and by naming it as M-matrix, we show how to construct partially balanced incomplete block (PBIB) designs and some regular bipartite graphs by it. We have considered two types of these M- matrices. Also we will make a mention of certain applications of these M-matrices in signal and communication processing, and network systems and end with some open problems., Comment: 21 pages,3 figures

要旨 女子大学生を対象に、COVID-19 に関する知識と予防行動および情報源とe ヘルスリテラシーの程度を把握し、感染予防行動に影響を与える情報源を探索して感染症対策への示唆を得ることを目的にWeb 調査を実施した。結果、家族等身近な者から感染しやすく、無症状でも感染源となる知識は概ね有していことが示された。手指衛生については、アルコール消毒液に依存している傾向がみられた。マスクの着用、食事前の手指衛生は高率で実施されていたが、他の人と近い距離で会話や発声をしない、食事中の会話をさける行動については不十分であったことから、食事場面での感染対策の教育が一層重要であることが示された。また、情報源の数が多い者、eヘルスリテラシー得点の高い者で予防行動の実施率が高い傾向がみられ、e ヘルスリテラシー向上に寄与する教育の重要性が示唆された。さらに、家族、教員の関わりは予防行動の促進に有効である可能性が示された。, This study aimed to ascertain knowledge and preventive behavior regarding COVID-19 and the effect of information sources and e-health literacy among female university students and to search for information sources that influence their infection prevention behavior to obtain suggestions for infection control measures. For this purpose, a web survey was conducted among female university students. The survey results showed that, in terms of knowledge, the students generally had the knowledge that they could easily be infected by people close to them, such as their families, and that even asymptomatic cases could be a source of infection. Regarding preventive behavior, they tended to rely on alcohol sanitizers for hand hygiene. Additionally, wearing a mask and hand hygiene before meals were performed at a high rate, but other types of behavior, such as avoiding speaking or vocalizing in close proximity to other people or avoiding conversation during meals, were insufficient, which indicates the importance of education on infection control in the eating. Furthermore, those with more information sources and those with higher e-health literacy scores tended to be more likely to take preventive actions. It was suggested that the education in improving e-health literacy is importunt. Furthermore, the involvement of families and teachers may effectively promote preventive actions.

Laparoscopic myomectomy may become a cause to increase intraoperative hemorrhage. The purpose of this investigation was to study the relationship between clinical features, MR imaging findings and intraoperative hemorrhage during laparoscopic myomectomy. This study included 126 patients clinically and pathologically diagnosed as uterine leiomyoma between 2010 and 2015. These findings were statistically compared between significant intraoperative hemorrhage group (greater than 300 ml) and nonsignificant intraoperative hemorrhage group (less than 300 ml). Correlation between the maximum diameter of resected leiomyomas and the maximum diameter leiomyomas on MR imaging were also evaluated. Significant hemorrhage group showed larger maximum diameter of resected leiomyoma (p = 0.01), larger maximum diameter of leiomyoma on MR imaging (p = 0.01) and have cellular leiomyomas on MR imaging more frequently (p = 0.03) than nonsignificant intraoperative hemorrhage group.
