6. Current strategy for esophageal varices in Japan

Abstract

Esophageal varices (enlarged, swollen and tortuous veins at the lower end of the esophagus) and life-threatening variceal bleeding may be caused by portal hypertension, liver cirrhosis (degenerative disease in which liver tissue is replaced by fibrous tissue and fatty deposits), schistosomiasis (infection with a parasite transmitted to humans by water contaminated by feces), or hepatic (liver) obstruction. In Japan the most frequently used treatment modality for both emergency and elective cases of esophageal varices is sclerotherapy. Other methods of treatment include tamponade (use of pressure to stop bleeding); endoscopic sclerotherapy (injection of a sclerosing, or hardening, agent directly into esophageal varices using an illuminated optic instrument to see into the esophagus); devascularization (interruption of the circulation of blood to the bleeding point); and shunts (surgical procedures which divert the blood supply away from the area of bleeding). In Japan shunt operations are performed on 20 percent of cases of bleeding esophageal varices. Treatment of esophageal varices prophylactically (before bleeding occurs) is commonly performed in Japan. Factors that influence the decision regarding either surgery or sclerotherapy include: patient age less than 65 would favor surgery; elective treatment favors surgery, and emergency treatment, sclerotherapy; nature of the underlying liver disease; liver function and distribution of collateral vessels. Abstract

We report two cases of squamous cell carcinoma of the esophagus following endoscopic injection sclerotherapy for esophageal varices. The interval between sclerotherapy and the development of carcinoma was 24 months in case 1 and 21 months in case 2. The sclerosant was 5% sodium morrhuate in case 1 (total dose, 10 ml) and 5% ethanolamine oleate in case 2 (45.5 ml). Although no recurrent variceal bleeding occurred after sclerotherapy, we could not perform any curative surgical treatment for esophageal cancer because of the advanced stage of the cancer and the severity of the accompanying liver dysfunction. It is difficult to determine the relationship between sclerotherapy and carcinoma: however, long-term surveillance is essential to avoid overlooking a neoplasm in the esophagus after endoscopic injection sclerotherapy. Abstract

The mechanism by which the mean blood clearance (CLtot, B) of indocyanine green (ICG) was reduced in cirrhotic patients was examined pharmacokinetically. It was demonstrated that the reduction in the CLtot, B of ICG in cirrhosis would be mainly due to the decrease in the hepatic uptake clearance and partly due to the increase in the efflux clearance from the liver to plasma. It is suggested that the decrease in the hepatic uptake clearance in cirrhosis mainly reflects the decrease in the intrinsic clearance of hepatic uptake rather than the decrease in hepatic blood flow.
