2. Epidemioclinical, Etiological and Therapeutic Study of Paraphimosis at the Centre de Santé de Référence (CSRéf) of Markala, about Five Cases

The aim was to study the epidemiological, clinical and therapeutic aspects of paraphimosis at the CSRéf of Markala. Patients and Methods: We conducted a prospective and descriptive study of five patients who had consulted for paraphimosis in the operating theatre of the Markala referral health centre from 1er January 2019 to 31 December 2021, i.e. a two-year period. Our study included all patients who consulted for paraphimosis at the CSRéf of Markala with a usable medical file. The criteria for non-inclusion were: Phimosis without other pathologies of the penis. The records were not usable, as this was a prospective study. Results: The frequency of paraphimosis was 2.1% out of 235 patients hospitalized and operated on at the CSRéf of Markala during the study period. Four of our patients had no particular medical or surgical history, but the oldest of our patients had already undergone a hernia repair and an appendectomy at the time ; the average age of our patients was 15.3 years, with extremes ranging from 3 years to 53 years . However, we note that there is no specific age of onset of paraphimosis The treatment received by all our patients was posthectomy. No major complication was noted in our patients, however we deplore a loss of sight in two of our patients for a long term follow-up in order to identify other unknown aspects in the management of paraphimosis. Conclusion: Paraphimosis is a true urological emergency that occurs in uncircumcised men when the foreskin is trapped behind the glans crown, which can lead to long-term strangulation of the glans and compromise its vascularity hence their immediate management paraphimosis.

3. Management of Macroscopic Hematurias in the Urology Department of Chu Point G

Introduction: Hematuria is the presence of erythrocytes in abnormal amounts in the urine during urination, it is macroscopic if the number of erythrocytes is greater than 500 000 red blood cells / min. considered as a banal sign, particularly the endemic countries of Bilharzia, it remains a frequent reason for consultation and a concern in urology. A total of 71 patients met our inclusion criteria; the mean age was 54.28 ± 18 with extremes ranging from 11 years to 85 years. The sex ratio is 2.22 in favors of men, 70% of our patients had consulted for total hematuria and 49.3% in outpatient. The duration of evolution of hematuria was greater than three months in 42% of cases, cystoscopy was performed in 38% of patients and the selected diagnoses were dominated by bladder tumors (67.9%). prostate adenoma (14%), adenocarcinoma of the prostate (7.2%). Saline bladder irrigation was performed in 63.4% of patients for 1 to 3 days in 53%. The discharge of our patients was done after diagnosis and specific treatment in 38.4% of cases and in 25% of cases after the management of hematuria. Conclusion: Macroscopic hematuria affects twice as many men as women, bladder tumors and prostatic disease alone are responsible for almost all etiologies. Bladder irrigation and specific treatment play an important role in its management.

Etude de la dynamique des populations d’Anopheles gambiae s.l et polymorphisme allélique du gène TEP1 en période de transmission du paludisme dans la commune rurale de Bancoumana, Mali. La lutte anti vectorielle est une stratégie importante de lutte contre le paludisme, elle permet l’interruption dans la chaîne de transmission du paludisme. Cette lutte inclut la connaissance approfondie du vecteur et les facteurs environnementaux qui ont une influence sur sa vie. L’objectif de cette étude est d’étudier la dynamique de la population adulte d’An.gambiae s.l. et d’évaluer le polymorphisme allélique du gène TEP1 impliqué dans la lyse parasitaire chez le vecteur. Cette étude était conduite dans une zone d’endémie palustre à Nanguilabougou et son hameau de culture Kouroubabougou dans la commune rurale de Bancoumana entre Juin 2014 et Novembre 2015 durant la saison de transmission du paludisme. Les moustiques étaient collectés à l’aide de pièges lumineux, conservés dans des tubes de 50 ml contenant de l’éthanol à 70%, ont été traités au laboratoire avec la PCR (extraction de l’ADN, identification des espèces et génotypage). En 2014 parmi les 1433 moustiques collectés, l’espèce An. coluzzii était prédominante 72,2%, suivi d’An. gambiae 21,1% et l’hybride An. coluzzi/gambiae 3,1%. La même situation était observée en 2015 sur 6612 moustiques collectés, An. coluzzii 72,4%, An. gambiae 16,7% et l’hybride 0,4%. Deux An. arabiensis ont été trouvés en 2015. Une forte présence du génotype R1/R1 chez l’espèce An. coluzzii 85,2% en 2014 et 66,2% en 2015 a été constatée. L’espèce An. gambiae était plus associée au génotype S1/S1 en 2014 et S1/R2 en 2015. Les autres formes alléliques étaient faiblement présentes.An. coluzzi était prédominante parmi les espèces d’An. gambiae s.l. Elle avait une fréquence élevée du gène TEP1 impliqué dans la lyse parasitaire chez les moustiques.

Introduction: Road traffic accidents (RTAs) are nowadays a real worldwide scourge because of the high number of victims. In Mali, we have little data related to the characteristics of deaths related to MVAs, hence the interest of this study which aimed to determine the sociodemographic, clinical and paraclinical characteristics of deaths involving a motorcycle received in our forensic medicine unit. Materials and method: This was a prospective, monocentric, cross-sectional study conducted from January 1, 2016 to December 31, 2016 in the forensic medicine unit of the CHU GT. Were included, all deaths involving a motorcycle received in our unit and accompanied by a requisition. Results: Of 458 MVA deaths, we identified 147 motorcycle deaths, a prevalence of 32.1%. The mean age was 36.5 years with extremes of 1 to 72 years and a sex ratio of 7.65. Students were the most represented, 90 cases (25.18%). The motorcycle mechanism was more frequent 90 cases (61.9%). The month of May and the hours from 19:00 to 05:00 were the most frequent times of occurrence of accidents respectively 37 cases (25.2%) and 76 cases (51.7%). Polytraumatic lesions were found on 83 bodies, i.e. 56.46%. Death was noted on arrival in 80% of cases and none of our bodies had undergone autopsy. Conclusion: Motorcycle accidents on public roads remain a major public health problem in our country.

Aims: The goal of our work was to clarify the epidemiologicals, clinicals, etiologicals and prognostics of upper gastrointestinal bleeding at Segou Regional Hospital. Patients and methods: This cross-sectional study, which took place from October 1, 2017 to September 31, 2018, involved patients hospitalized for digestive hemorrhage. Results: Upper gastrointestinal bleeding represented 9.7% of all hospitalizations. The mean age of the patients was 50, 94 ± 21, 6 years with a sex ratio of 1.6 in favor of men. Housewives and farmers were represented in 37.7% and 34% of cases. The main modes of disclosure were hematemesis and melena. Evolution was favorable with a mortality of around 34%. Conclusion: upper gastrointestinal bleeding is relatively common in our context. High mortality is linked to delayed treatment. [ABSTRACT FROM AUTHOR]

The normal resistance and the tangential conductivity of a single fracture with Gaussian or self-affine surfaces are systematically studied as functions of the nature of the materials in contact and of the geometrical parameters. Analytical formulas are provided in the lubrication limit for fractures with sinusoidal apertures; these formulas are used to substantiate empirical formulas for resistance and conductivity. Les liposarcomes primaires du médiastin (LPM) sont des tumeurs extrêmement rares. Nous rapportons un cas chez une patiente de 39 ans qui a été mal diagnostiquée et prise en charge initialement pour pleurésie puis secondairement pour tumeur broncho-pulmonaire. La tumeur occupait la cavité pleurale gauche et comprimait le parenchyme pulmonaire. Une thoracotomie exploratrice puis exérèse totale de la tumeur a été réalisée avec succès. La tumeur était d’origine médiastinale. Le résultat de l’examen anatomopathologique réalisé a conclu à un liposarcome bien différentié.Mots clés : Liposarcome, médiastin, cavité pleurale.Primary liposarcoma of the mediastinum are extremely rare tumors. Here in we describe a 39- year-old woman who was misdiagnosed and managed for pleural liquid then secondary for pulmonary tumeur. The huge tumor occupied the left pleural cavity, with the left lung totally compressed. the patient underwent an exploratory thoracotomy, followed by a successful tumor total removal. Result of pathologic examination showed that the tumor was well differentiated liposarcoma.Keys words: Liposarcoma, mediastinum, hemithorax.

19. Dérivations bilio-digestives dans la chirurgie palliative du cancer de la tête du pancréas : à propos de 45 cas

Le but de ce travail était d’évaluer l’importance de la dérivation bilio-digestive dans le traitement palliatif des cancers de la tête du pancréas. Il s’agissait d’une étude rétrospective sur une période de 11 ans et 6 mois (janvier 1999 à juin 2010) dans le service de chirurgie A du CHU du Point G. Les dossiers cliniques de 45 patients atteints de cancer de la tête du pancréas à des stades avancés ont été colligés. Ont été inclus dans l’étude les malades opérés pour tumeurs malignes de la tête du pancréas non résécables confirmées à l’histologie. Les malades non opérés ou ceux présentant des tumeurs d’autres localisations pancréatiques n’ont pas été inclus dans l’étude. Les procédures de dérivations bilio-digestives ont été conduites chez 45 malades pour tumeurs de la tête du pancréas. Elles ont consisté en une anastomose cholédoco-jéjunale dans 18 cas (40%), une cholécysto-jéjunostomie dans 15 cas (33,3%), une anastomose cholédoco-duodénale dans 12 cas (28%). Cette dérivation bilio-digestive a été associée à une gastro-entéro-anastomose dans 100% des cas. La morbidité était de 13,3% avec 3 cas de fistules bilio-digestives et 3 cas d’éviscération. La mortalité opératoire a été de 11,1%. Les dérivations bilio-digestives dans la prise en charge chirurgicale palliative des cancers de la tête du pancréas permettent d’améliorer la qualité de vie des patients.Mots clés : cancer, pancréas, dérivation bilio-digestive

20. LES COMPLICATIONS UROLOGIQUES LIEES A LA CHIRURGIE PELVIENNE AU CHU DU POINT-G, A PROPOSDE 23 CAS.

Introduction: Bladder and ureteral lesions are the most common urologic complications occurring during pelvic surgery with 1% and 0.5% to 3%, respectively. These lesions are rarely recognized intraoperatively and pose a major problem to urologists, gynecologists, and general surgeons. Objective: To study the factors favoring urologic complications following pelvic surgery at the University Hospital Point-G. Patients and Method: We conducted a retrospective study at the University Hospital Point-G between 2006 and 2015. It involved 23 patients with a whole urological lesion following pelvic surgery. These patients underwent a clinical and para-clinical examination (intravenous urography, methylene blue test) to confirm the urological lesion and to determine its management. Results: The average age of our patients was 32.00 years with extremes of 18 and 40 years old. The leakage of urine was found in 82.6% (19/23). Interventions that caused urologic injury were: caesarean section 52.2% (12/23), hysterectomy 30.4% (7/23), as well as a caesarean section and hysterectomy 17.4% (4/23). Methylene blue was performed in 19 patients, it was positive in 52.2% (12/23) and intravenous urography (IVU) in 7 patients. The lesions encountered were: retrotrigonal fistula vesico-vaginal 10 cases, vesico-vaginal fistula under trigonal 2 cases, uretero-vaginal fistula 7 cases, and bilateral ureteral ligation 4 cases. Fistulorraphy was performed in 52.2% followed by direct ureterovesical reimplantation. The postoperative outcome was satisfactory in 100% of cases with obtaining a good bladder tightness. The average duration of hospitalization was 12 days (+/- 4 days). Conclusion: Pelvic surgery results in urological injuries. Caesarean section and hysterectomy are contributing factors. Vesico-vaginal fistula or ureteral lesions are common. [ABSTRACT FROM AUTHOR]

Les causes de volvulus du colon sigmoïde sont variées et parmi elles la strangulation est des plus fréquentes dans notre contexte d’exercice. Les lésions vues tard permettent très rarement un traitement sans résection. La survenue d’un volvulus du colon sigmoïde autour d’un pédicule de testicule géant ectopique est une première que nous rapportons dans cette étude de cas.

36. Surgery of the pulmonary hydatid disease in Mali.

- Abstract
Introduction: The pulmonary hydatid disease has not been the subject of many studies in Mali.Objective: List the cases hydatid cyst of the lung operated on in Mali, in order to analyze their surgical aspects.Methods and patients: This is a retrospective study of consecutive and non-selected cases. From 1960 to 2000, eleven cases of the pulmonary hydatid disease were operated on in Mali. The earliest case dates back to 1968 and the most recent was in 2000. All of these patients were taken care of at the ‹ Point G Hospital › in Bamako.Results: Patients were aged between 4 and 42 years, with an estimated average at 20. The gender ratio (F/M) was 1.75. The circumstance leading to the discovery stems from 8 cases of cough. Seven patients had at least a sign of complicated cyst. Out of a total of 12 cysts, 9 were located to the right lung. The treatment of the cyst consisted in a kystectomy accompanied by an atypical resection of the parenchyma on 6 patients and a typical resection of the attacked lobe on 5 others. The operating effects were complicated in 5 cases. No mortality was reported. Anatomopathology confirmed pulmonary hydatidosis in all cases.Conclusion: Paradoxically, the frequency of pulmonary hydatidosis is low in Mali. Patients were consulted late. This explains in part the high level of lobectomy during surgical treatment. [Copyright &y& Elsevier]

38. Pyrolysis Kinetics of Byrsonima crassifolia Stone as Agro-Industrial Waste through Isoconversional Models.

This study is aimed at the analysis of the pyrolysis kinetics of Nanche stone BSC ( Byrsonima crassifolia) as an agro-industrial waste using non-isothermal thermogravimetric experiments by determination of triplet kinetics; apparent activation energy, pre-exponential factor, and reaction model, as well as thermodynamic parameters to gather the required fundamental information for the design, construction, and operation of a pilot-scale reactor for the pyrolysis this lignocellulosic residue. Results indicate a biomass of low moisture and ash content and a high volatile matter content (≥70%), making BCS a potential candidate for obtaining various bioenergy products. Average apparent activation energies obtained from different methods (KAS, FWO and SK) were consistent in value (~123.8 kJ/mol). The pre-exponential factor from the Kissinger method ranged from 10
5 to 1014 min-1 for the highest pyrolytic activity stage, indicating a high-temperature reactive system. The thermodynamic parameters revealed a small difference between EA and ∆H (5.2 kJ/mol), which favors the pyrolysis reaction and indicates the feasibility of the energetic process. According to the analysis of the reaction models (master plot method), the pyrolytic degradation was dominated by a decreasing reaction order as a function of the degree of conversion. Moreover, BCS has a relatively high calorific value (14.9 MJ/kg) and a relatively low average apparent activation energy (122.7 kJ/mol) from the Starink method, which makes this biomass very suitable to be exploited for value-added energy production.- Published

- Abstract
This study focused on the analysis of the pyrolytic behavior of four lignocellulosic biomasses: avocado stone (AS), Agave salmiana bagasse (AB), cocoa shell (CS), and α-cellulose (CEL). According to the triplet kinetics analysis, the order of pyrolytic decomposition was AS < AB < CEL < CS. The AS was dominated by a second-order reaction, while AB followed a 2D diffusion-Valensi model. On the other hand, the pyrolysis of CS starts with an nth-order reaction and ends random nucleation model, and CEL was dominated by one-dimensional diffusion and first-order reaction. Thermodynamic studies reveal that the difference between the activation energy versus enthalpy change was<6.5 kJ/mol for all biomasses, thus showing the ease of pyrolysis reaction of these biomasses. Furthermore, the AS and AB showed that the reactions are close to thermodynamic equilibrium and stability, whereas CS and CEL indicated high reactivity., (Copyright © 2021 Elsevier Ltd. Aims: The goal of our work was to clarify the epidemiologicals, clinicals, etiologicals and prognostics of upper gastrointestinal bleeding at Segou Regional Hospital., Patients and Methods: This cross-sectional study, which took place from October 1, 2017 to September 31, 2018, involved patients hospitalized for digestive hemorrhage., Results: Upper gastrointestinal bleeding represented 9.7% of all hospitalizations. The mean age of the patients was 50, 94 ± 21, 6 years with a sex ratio of 1.6 in favor of men. Housewives and farmers were represented in 37.7% and 34% of cases. The main modes of disclosure were hematemesis and melena. Evolution was favorable with a mortality of around 34%., Conclusion: upper gastrointestinal bleeding is relatively common in our context. The goal of this work was to study helicobacter pylori (HP) infection and gastric mucosa in cirrhotic patient., Method: The study was comparative (case-indicator) and transversal from October 2015 to June 2016 in the gastroenterology and hepatology center of CHU Gabriel Touré of Bamako. Hp was identified in pathologic center of CHU du point "G" de Bamako., Results: At the end of the study Hp infection frequency was 73.6% without significative differency with the comparative group (p= 0,200). The mean age was 46,3 ± 20,8 years. The sex ratio was 2,11. The home workers women were more represented (32.1%). Ascites and jaundice were the main signs respectively in 64.3% and 53.6% cases. The upper endoscopy found varices in 89.2% of cases. 42. [Surgery wound infection at surgery "A" department of the University Hospital Point G].

Aim: To determine the frequency of the surgery wound infection, its favoring factors, causative germs and its antibiotic treatment of choice in the surgery "A" department of the University Hospital Point G., Method: In a retrospective descriptive study over a 6-month period in the surgery "A" department of the University Hospital Point G, we enrolled all patients who underwent surgery necessitating at least 48 hours of hospital admission during our study period and in conformity with the Atlanta CDC criteria. Surgery patients with less than 48 hours postoperative hospital stay and those admitted to the hospital without surgery were not included.After the surgery, the nature and location of the infection have been clarified. In this work, ethical considerations have been respected and there is no conflict of interest., Results: Two hundred and sixty-five (n=265) patient files were collected including 24 cases of surgery wound infection (a hospital frequency of 9%. The average age was 41.41 years (y.o) old with extremes of 7 y.o and 102 y.o. The sex ratio was 0.9 in favor of women. The commonly studied pathologies were digestive surgery in 52.8%, gynecological surgery in 24.5%, etc… Patients were seen in regular outpatient surgery visits in 75.8% and surgical emergencies in 24.2%. Of the patients urgently admitted, 26.5% presented an ISO; for those received in ordinary consultation it was 3.48%. Fifteen (15) cases were classified ASA III and two (2) ASA II. Based on the Altemeier classification, surgery was clean in 66.8% of our patients, contaminated clean in 12.4%, contaminated in 12.1%, and dirty in 8.7% (including half resulting in surgery wound infection). In total, 79% of patients whose surgeries lasted more than two (2) hours presented a surgery wound infection. Postoperative infection of patients was superficial in 58.3% and deep in 33.3%. In 8.3% of cases, it was a space infection. At the cytobacteriological examination of the pus from the operative site, Escherichia coli (E. coli) was the most common germ with 58.3%. E. coli was sensitive to amoxicillin-clavulanic acid in 57.14%. Surgically, a re-intervention was performed in 20.8% of cases, a secondary suture in 12.5%, and a single dressing in 66.7%. The average postoperative stay was 6.5 days with extremes of 2 days and 69 days. Aim: This study was aimed to evaluate patients' satisfaction with tranvesical prostatectomy in the Urology department of University Hospital Gabriel Touré according to the (International Prostate Symptoms Score (IPSS) and the quality of life scores., Patients and Methods: We conducted a prospective descriptive study over a 12-month period including a consecutive cohort of patients who had transvesical prostatectomy. Each patient was evaluated before and six weeks after the surgery. We used the IPSS as an assessment tool, supplemented by the Quality of Life (SQ) questionnaire., Results: One hundred and thirteen (113) patients aged 71 ± 8 years old on average were evaluated. The mean duration of symptoms associated with benign prostatic hyperplasia was 19 months and 17% of patients had a mechanical complication. Aim: Our aim was to evaluate the diagnosis and therapeutic aspects of the amoebic liver abscesses in the surgery «A» department of the University hospital Point "G"., Patients and Methods: Were included in this study, patients admitted to the surgery «A» department for amoebic liver abscess confirmed by a positive amoebic serology and the chocolate appearance of bacteria-free pus. Bacterial liver abscesses were not included into this study., Results: Over a 10-year period, 52 cases of patients diagnosed with amoebic liver abscess were collected in the department. There were 41 men and 11 women, with a sex ratio of 3.7. The mean age was 37.8 years old with extremes of 15 and 66 years. The most represented professions were farmers (36.5%) followed by salesmen (26.7%) and students (11.5%). The average outpatient visit delay time was 18.5 days with the extremes of 5 and 34 days. The most frequent clinical signs were pain in the right hypochondrium (86.5%), fever (78.8%) and hepatomegaly (61.5%). Abdominal ultrasound showed a single located abscess in 44 patients (84.6%) and these abscesses were localized in the right hepatic lobe in 34 patients(65.4%). The average volume of the abscess was 366.5 cm
3 with the extremes of 36 cm3 and 1580 cm3 . Amoebic serology was positive in 38 patients (80.9%). Cytobacteriological analysis of pus in 37 patients (71%) was negative. Patients underwent an ultrasound assisted needle aspiration of pus in 65.4% he. Laparotomy and a laparoscopic approach were performed in 7.7% and in 5.8%, respectively. The clinical course was uneventfulin 94.2%. The mean hospital stay duration was 16.5 days with the extremes of 4 and 29 days. No death was recorded during hospitalization., Conclusion: Amoebic liver abscess is an uncommon pathology in a surgical setting. Abdominal ultrasound andechography guided liver puncture allowed the diagnosis. - Abstract
Introduction: Bladder and ureteral lesions are the most common urologic complications occurring during pelvic surgery with 1% and 0.5% to 3%, respectively. These lesions are rarely recognized intraoperatively and pose a major problem to urologists, gynecologists, and general surgeons., Objective: To study the factors favoring urologic complications following pelvic surgery at the University Hospital Point-G., Patients and Method: We conducted a retrospective study at the University Hospital Point-G between 2006 and 2015. It involved 23 patients with a whole urological lesion following pelvic surgery. These patients underwent a clinical and para-clinical examination (intravenous urography, methylene blue test) to confirm the urological lesion and to determine its management., Results: The average age of our patients was 32.00 years with extremes of 18 and 40 years old. The leakage of urine was found in 82.6% (19/23). Interventions that caused urologic injury were: caesarean section 52.2% (12/23), hysterectomy 30.4% (7/23), as well as a caesarean section and hysterectomy 17.4% (4/23). Methylene blue was performed in 19 patients, it was positive in 52.2% (12/23) and intravenous urography (IVU) in 7 patients. The lesions encountered were: retrotrigonal fistula vesico-vaginal 10 cases, vesico-vaginal fistula under trigonal 2 cases, uretero-vaginal fistula 7 cases, and bilateral ureteral ligation 4 cases. Fistulorraphy was performed in 52.2% followed by direct ureterovesical reimplantation. The postoperative outcome was satisfactory in 100% of cases with obtaining a good bladder tightness. The average duration of hospitalization was 12 days (+/- 4 days)., Conclusion: Pelvic surgery results in urological injuries. Caesarean section and hysterectomy are contributing factors. The aim of this study was to evaluate the surgical treatment of goiter in the «A» surgical department of the Point G University Hospital in Bamako., Patients and Methods: This was a retrospective and descriptive study in the «A» surgical department of the Point G University Hospital from January 2007 to August 2014. Included in the study were all the patients operated for goiter. The patients who underwent surgery for other thyroid diseases were not selected. Postoperatively patients were followed for one year. The sociodemographic, clinical, para-clinical, therapeutic aspects and the postoperative course were the study parameters. Data analysis was performed on SPSS 21. The Chi square test was used for statistical analysis. A p-value inferior to 0.05 was considered statistically significant., Results: A total of 378 patients' records were collected. Thyroidectomy represented 6.2% (378 cases) over 6027 operations. The mean age was 41.29 years with a standard deviation of 13.58. Extremes were 3 and 85 years. The sex ratio was 7.22 in favor of women. The frequent reason for consultation was due to antero-cervical tumor with 97.07% of cases (n = 367). We identified 118 cases (31.22%) of goiter in cooled hyperthyroidism patients, 211 cases (55.82%) of euthyroid goiter, 7 cases (1.85%) of the thyroid neoplasia and one case (0.3%) of hypothyroidism goiter. Pathological histology helped to highlight a micro-vesicular macro adenoma in 16.1% of cases (61 patients); adenocarcinoma in 0.7% of cases (3 patients) and papillary carcinoma in 1.1% of patients (4 patients). The patients classified as ASA I and ASA II represented the majority with respectively 248 cases (65.60%) and 120 cases (31.74%). The most performed surgical techniques were subtotal thyroidectomy (n=181; 49.7%), the isthmo-lobectomy (126; 33.3%), total thyroidectomy (n=11; 2.9%) and sternotomy in one case of endothoracic goiter. Intraoperative complications were marked by hemorrhage in 12.7% of cases (45 patients). The postoperative one month follow up was uneventful in 91.79% (n = 347) and transient recurrent nerve injury was found in 2% of cases (1 patient). In one year follow up the postoperative outcome was good in 94.17% (n = 356) of patients. The mean hospital stay was 4.18 days. Six deaths were recorded (1.7%)., Conclusion: Goiter surgery is relatively common in the surgery 'A' department of the Point G University hospital. - Abstract
Aim: to evaluate the contribution of laparoscopic surgery in the management of chronic abdominal pains of unspecified etiology., Methods: A retrospective descriptive study was conducted over a period of 49 months (March 2008 to March 2012), in the surgery A service of the Point G university hospital., Results: 52 patients suffering from chronic abdominal pain for 3 months have been listed. There were 36 women and 16 men, with a sex-ratio of 2.25. The pathologies found in laparoscopy were: 16 cases of adherences, 16 cases of gynaecological diseases, 6 cases of chronic appendicitis, 5 cases of chronic cholecystitis, 6 cases of abdominal tumors and 3 cases of intrauterine contraceptives in abdomen. Conversion to laparotomy was not made and mortality was null. No lesion was found in 11.54% of cases. The mean duration of hospitalization was 2 days. The duration of follow-up of the patients was 18 months with a minimum of 6 months and a maximum of 24 months. No painful symptomatology was found in 6 months in 88.46% of the cases. Persistent stomach pains were found in 3% of the cases. 8.54% of the patients had an improvement of symptomatology., Conclusion: Chronic abdominal pains put a diagnosis problem in surgery. Numerous studies have shown the influence of some bacteria colonizing the breeding sites on the development of the parasite in Anopheles malaria vectors. The aim of this study was to evaluate the impact of the breeding sites of Plasmodium falciparum on the gametocytes infectivity of Anopheles gambiae sl using the membrane feeding method. We carried out an experimental infestation study from September 2010 to January 2011 in a village located in the southern savanna of Mali. Cross sectional surveys were conducted to collect larvae and to select gametocyte carriers. Female offspring of L3 and L4 stage larvae were used for the experimental infestation. The gametocyte carriers were children aged within 4 to 12 years. Bacteria identified in breeding water of larvae and adult mosquitoes were: Escherichia coli, Salmonella spp, Klebsiella oxytoca, Pseudomonas spp, Staphylococcus spp, Shigella sp. The oocyst load of infected mosquitoes did not significantly vary according to the samples (F = 1.517, P = 0.230). Low infection rates (5.4% and 2.8%) were observed in the larval bedding and in mosquitoes containing K. oxytoca and Shigella. 49. [Study of factors influencing clinical outcomes of obstetric urogenital fistula surgery at the Point G University Hospital of Bamako, Mali].

The aim of this study was to identify factors influencing the clinical outcomes of surgery of obstetric urogenital genital fistula. This was a cross sectional study, which focused on factors influencing results in the treatment of urogenital fistula, from January 2011 to January 2012 at the urology department of the Point-G University Hospital. This study involved 115 patients suffering from urogenital fistula. Cure of urogenital fistula accounted for 17% of the bloc's activities. Closure of the fistula was achieved in 68.7% of cases. This result was influenced by certain factors. Including factors related to condition of the tissue peri-fistula (good trophic tissue, or fibrosis peri-fistula). Fistulas operated primarily peri-sinus tissue remodeling were closed in 71.42% of cases. Factors related to the patient and care: those carried out for the first time have a success rate of 68.42%, only 47.62% of success in patients who presented after five years. Factors related to clinicopathological seat: fistula vesicovaginal septum and cervical-vaginal urethrovaginal reported a success rate of 76.92%. Care-related factors include: the experience of the surgeon, surgeons of the urology service have a 76.19% success rate against 25% from other surgeons. The incision fistula was related to the clinicopathological seat, so the trigonal-vaginal fistulas and uretero addressed by the upper route have been successful in 85%. - Abstract
The goal of this study was to investigate internal hemorrhoid disease. This cross-sectional study was conducted at the digestive endoscopy center of the Gabriel Toure University Hospital from December 2011 to December 2012. Sixty cases of internal hemorrhoid disease were found - 36 men and 24 women and an average age of 47 ±14.4 years. Clinical signs were frequently: bleeding (66.7%), constipation (58.4%), anal pains (58.3%) and prolapsed hemorrhoids (18.3%). The rectoscopy found isolated internal hemorrhoids in 78.4% of cases and stage I of the illness in 81.7% of cases.
